1.Evaluation of the efficacy of linaclotide combined with PEG in the bowel preparation for colonoscopy of patients with chronic constipation

Objective To explore the application of linaclotide combined with compound polyethylene glycol electrolyte powder(PEG)in bowel preparation for colonoscopy of patients with chronic constipation.Methods A toltal of 190 patients with chronic constipation who underwent electronic colonoscopy in Beijing NO.6 Hospital from January to October 2023 were selected as the objects.The patients were divide into linaclotide group(n=63),3L PEG group(n=64),and 4L PEG group(n=63)by random number table method.The application effects of three groups were compared.Results Boston bowel preparation scale score was higher than 3L PEG and 4L PEG groups,there were significant difference among groups(P<0.05).The qualified rate of patients in linaclotide group was better than that of 3L PEG group and 4L PEG group,and the qualified rate of patients in 4L PEG group was better than that of 3L PEG group,there were significant difference among groups(P<0.05).Conclusion Linaclotide combined with PEG can not only improve the intestinal cleaning quality of patients with constipation,but also do not increase the adverse reactions of patients.

Objective To compare the clinical efficacy between arthroscopic posteromedial dual-portal valvotomy combined with cyst wall resection and open surgery in the treatment of popliteal cysts.Methods A total of 64 patients with popliteal cysts who were treated in the Kunshan Second People's Hospital from May 2018 to May 2023 were enrolled and randomly assigned to two groups.The combined group(n=32)underwent arthroscopic posteromedial dual-portal valvotomy combined with cyst wall resection,and the traditional group(n=32)underwent open surgery.Surgery-related parameters,pain,inflammatory levels,knee joint function,quality of life,postoperative complications and recurrence rate were compared between the two groups.Results The combined group had less intraoperative blood loss and shorter incision length,operation time,and hospital stay than the traditional group(P<0.05).The Rauschning-Lindgren grade was improved at 6 months after surgery in both groups(P<0.05),but there was no significant difference in the Rauschning-Lindgren grade at 6 months after surgery between the two groups(P>0.05).VAS scores at 6,24 and 48 h postoperatively,lower limb alignment angles,and the levels of interleukin(IL)-6,tumor necrosis factor-α,IL-8,and C reactive protein in the combined group were lower than those in the traditional group(P<0.05).The range of motion of the knee,Lysholm knee scores,and QOL scores in all dimensions in the combined group were higher than those in the traditional group(P<0.05).The incidence of postoperative complications in the combined group was lower than that in the traditional group(P<0.05).Conclusion Arthroscopic posteromedial dual-portal valvotomy combined with cyst wall resection is superior to open surgery in the treatment of popliteal cysts,with less intraoperative blood loss,shorter operation time and hospital stay,lower pain levels,better knee joint function and higher quality of life.

Objective To investigate the relationship between tooth loss and the occurrence of esophageal cancer in a natural village in Wenfeng District, Anyang City, Henan Province. Methods A prospective cohort study was conducted to observe the occurrence of tooth loss and esophageal cancer among the asymptomatic residents of the natural village for 16 years from January 2008 to July 2024. Data were analyzed by chi-square test, binary logistic regression, and restricted cubic spline. Results Among the total population of 711 cases, 136 cases were lost to follow-up and 575 cases were included in the final statistics, including 45 cases with esophageal cancer. Significant statistical difference was found between esophageal cancer patients with and without tooth loss (P<0.05). Objective:To characterize the histopathological subtypes and their clinicopathological parameters of gender and onset age by common, rare and sparse primary esophageal malignant tumors (PEMT).Methods:A total of 272 437 patients with PEMT were enrolled in this study, and all of the patients were received radical surgery. The clinicopathological information of the patients was obtained from the database established by the State Key Laboratory of Esophageal Cancer Prevention & Treatment from September 1973 to December 2020, which included the clinical treatment, pathological diagnosis and follow-up information of esophagus and gastric cardia cancers. All patients were diagnosed and classified by the criteria of esophageal tumor histopathological diagnosis and classification (2019) of the World Health Organization (WHO). The esophageal tumors, which were not included in the WHO classification, were analyzed separately according to the postoperative pathological diagnosis. The χ 2 test was performed by the SPSS 25.0 software on count data, and the test standard α=0.05. Results:A total of 32 histopathological types were identified in the enrolled PEMT patients, of which 10 subtypes were not included in the WHO classification. According to the frequency, PEMT were divided into common (esophageal squamous cell carcinoma, ESCC, accounting for 97.1%), rare (esophageal adenocarcinoma, EAC, accounting for 2.3%) and sparse (mainly esophageal small cell carcinoma, malignant melanoma, etc., accounting for 0.6%). All the common, rare, and sparse types occurred predominantly in male patients, and the gender difference of rare type was most significant (EAC, male∶ female, 2.67∶1), followed with common type (ESCC, male∶ female, 1.78∶1) and sparse type (male∶ female, 1.71∶1). The common type (ESCC) mainly occurred in the middle thoracic segment (65.2%), while the rare type (EAC) mainly occurred in the lower thoracic segment (56.8%). Among the sparse type, malignant melanoma and malignant fibrous histiocytoma were both predominantly located in the lower thoracic segment (51.7%, 66.7%), and the others were mainly in the middle thoracic segment.Conclusion:ESCC is the most common type among the 32 histopathological types of PEMT, followed by EAC as the rare type, and esophageal small cell carcinoma and malignant melanoma as the major sparse type, and all of which are mainly occur in male patients. The common type of ESCC mainly occur in the middle thoracic segment, while the rare type of EAC mainly in the lower thoracic segment. The mainly sparse type of malignant melanoma and malignant fibrous histiocytoma predominately occur in the lower thoracic segment, and the remaining sparse types mainly occur in the middle thoracic segment.
7.Experimental study of magnetic compression technique for anastomosis reconstruction of esophagus

Objective To investigate the feasibility of magnamosis rings designed based on magnetic compression technique in esophageal anastomosis reconstruction. Methods According to the anatomical characteristics of esophagus in SD rats, the esophageal magnamosis rings were designed. SD rats were used as animal models (n=10, 5 males and 5 females) to complete the magnetic anastomosis reconstruction of the cervical esophagus using magnamosis rings, and the operation time, animal survival, postoperative complications, magnetic rings excretion time were recorded. Two weeks after operation, the rats were killed, and the esophageal anastomotic specimens were obtained. The blasting pressure of the anastomotic site was measured and the formation of the anastomotic site was observed with naked eyes. Results Esophageal magnamosis was successfully performed in 10 SD rats, and the median operation time was 11 (8-13) min. All rats survived without anastomotic leakage, anastomotic stenosis, or magnetic rings incarceration. The magnetic rings were discharged after 8 (5-10) days and the burst pressure was higher than 300 mm Hg. Visual observation showed that the anastomotic muscle healed well and the mucosa was smooth. Conclusion The magnetic compression technique can be used for anastomosis reconstruction of esophagus, which has the advantages of simple operation and reliable anastomosis effect, and has clinical application prospect.

[Objective] To compare the antipyretic and anti-inflammatory actions and acute toxicity between Chuanxinlian Soft Capsules (CSC) and Chuanxinlian Tablets. adrenergic receptor is the third subtype of ?-adrenergic receptors. The genetic structure and pharmacological property of ? 3-adrenergic receptor are markedly distinguished from ? 1-and ? 2-adrenergic receptor subtypes. Recently studies show that myocardial ? 3-adrenergic receptor mediates negative inotropic effect through Gi-protein/NO/cGMP pathway, the expression of ? 3-adrenergic receptor and negative inotropic effect mediated by ? 3-adrenergic receptor are increased in heart failure. However, because of the low expression of ? 3-adrenergic receptor in the heart, the actual pathophysiological significance of ? 3-adrenergic receptor remains unknown.
