1.Metabolic characteristics of lymphomas with Chinese experts grade Ⅰ recommendation for examination of 18F-FDG PET/CT guideline

Objective To observe the metabolic characteristics of lymphomas with Chinese experts grade Ⅰrecommendation for examination of 18F-FDG PET/CT guideline.Methods Totally 115 patients with lymphoma who underwent baseline 18F-FDG PET/CT were retrospectively enrolled and divided into grade Ⅰ A group(n=69,lymphomas with international experts grade Ⅰ recommendation,including Hodgkin lymphoma[HL]and diffuse large big cell lymphoma[DLBCL]),grade Ⅰ B group(n=26,lymphomas with Chinese experts grade Ⅰ recommendation,including peripheral T-cell lymphoma[PTCL],primary mediastinal large B-cell lymphoma[PMBL],Burkitt lymphoma[BL],high-grade follicular lymphoma[FL]and lymphoblastic leukemia/lymphoma)and grade Ⅱ group(n=20,lymphomas with international experts grade Ⅱ recommendation,including marginal zone lymphoma[MZL],mantle cell lymphoma[MCL],low-grade FL and non-gastric mucosa-associated lymphoma tissue lymphoma[MALToma]).PET/CT parameters were compared among 3 groups and between each 2 groups.Results Significant differences of the maximum standard uptake value(SUVmax),the mean standard uptake value,peak standard uptake value,the maximum standard uptake value normalized by lean body mass,the mean standard uptake value normalized by lean body mass,peak standard uptake value normalized by lean body mass,lesion/liver SUVmax and lesions/mediastinum SUVmax were found among 3 groups(all P<0.05),also between grade Ⅱ group and each of the other 2 groups(both P<0.05).Conclusion Lymphomas with Chinese experts grade Ⅰ recommendation for examination of 18F-FDG PET/CT guideline presented high metabolism on 18F-FDG PET/CT,similar to HL and DLBCL but higher than MZL,MCL,low-grade FL and non-gastric MALToma.

3.Effect of balloon occlusion combined with intra-sac injection of thrombin in the treat-ment of ruptured abdominal aortic aneurysm

Objective:To investigate the safety and effectiveness of balloon occlusion and intra-sac thrombin injection in the endovascular repair of ruptured abdominal aortic aneurysm.Methods:From October 2019 to October 2022,the clinical data of 16 patients with rAAA treated with balloon occlusion technique and intra-sac thrombin injection combined with EVAR were retrospectively analyzed,including 13 males and 3 females,aged 42-85 years,with a median age of 70.5 years.The time of preoperative first aid(from hospital arrival to operation start),average operation time,stay in intensive care unit(ICU),average hospitalization time,success rate of surgical treatment,perioperative(30 d)mortality rate,incidence of complications,the maximum diameter and volume change of the aneurysm were observed and recorded.Results:Among the 16 patients with ruptured abdominal aortic aneurysm,the technical success rate was 100.0%(16/16).One patient died of multiple organ dysfunction 6 hours af-ter operation.The success rate of surgical treatment was 93.8%(15/16).The preoperative first aid time was(53.3±6.2)min,the average operation time was(89.9±17.1)min,the stay in the intensive care unit(ICU)was(1.7±0.8)d,and the average hospitalization time was(7.8±1.3)d.The intrao-perative balloon occlusion time was(32.4±4.1)min.The postoperative renal function of all the pa-tients had no significant deterioration compared with that preoperative.Abdominal compartment syndrome(ACS)occurred in 1 patient after operation,which improved after CT puncture and drainage.The medi-an follow-up time was 36 months.During the follow-up period,1 patient died of acute myocardial infarc-tion 2 years after operation,and the remaining 14 patients survived.Among the 14 follow-up patients,1 type Ⅱ endoleak occurred,and no other types of endoleak occurred.By the end of the follow-up,the maximum diameter of the aneurysm sac in 14 patients was significantly lower than that before operation[(44.6±8.0)mm vs.(66.0±15.5)mm,P<0.001],and in 12 patients with CTA,the volume of the aneurysm sac was significantly shrunk than that before operation[(311.7±170.3)mm3 vs.(168.6±68.1)mm3,P<0.05].Conclusion:Balloon occlusion during endovascular repair is safe and effective in the treatment of ruptured abdominal aortic aneurysm;intraoperative thrombin injection of the aneurysm sac can significantly reduce the incidence of intraoperative and postoperative abdominal compartment syndrome and endoleak and,to a certain extent,improve the success rate of treatment.
4.Correlation between intrahepatic triglyceride content quantified by proton magnetic resonance spectroscopy and glucose metabolism

Objective:To investigate the correlation between intrahepatic triglyceride content (IHTC) and glucose metabolism in patients with non-alcoholic fatty liver disease (NAFLD) diagnosed by proton magnetic resonance spectroscopy ( 1H-MRS). Methods:A total of 239 subjects without diabetes mellitus were previously enrolled and underwent 1H-MRS scans. Anthropometric indexes including height, weight, waist and blood pressure, and laboratory findings as plasma glucose (PG), insulin (INS), C-peptide (CP), liver enzymes [alanine aminotransferase (ALT), aspartate transaminase (AST), γ-glutamyl transpeptidase (GGT)] and lipid profiles were collected. According to IHTC levels, participants were divided into three groups: the non-NAFLD group (IHTC<5.56%), the mild NAFLD group (IHTC 5.56%-<33%), and the moderate and severe NAFLD group (IHTC ≥ 33%). The clinical characteristics of each group were analyzed, and the correlation between IHTC and glucose metabolism were assessed. Results:Compared with those in the non-NAFLD group, male proportion, waist, 120 min postprandial PG (PG120), CP, liver enzymes and total cholesterol (TC) levels were greater in the NAFLD group, whereas insulin sensitivity index-Cederholm (ISI-Cederholm) and high density lipoprotein cholesterol (HDL-C) levels were lower in the NAFLD groups. Subjects in the moderate and severe NAFLD group had higher levels of 120 min postprandial INS (INS120) and Stumvoll indexes, and lower ISI-Cederholm than those in the mild NAFLD group [80.37 (57.68, 112.70) mU/L vs.110.50(71.78, 172.80)mU/L, 1453(1178, 1798)vs.1737(1325, 2380), 358(297, 446) vs.441(318, 594), 2.27(2.01, 2.53) vs.2.06(1.81, 2.39), respectively, all P<0.05]. Correlation analyses showed that IHTC was significantly positively correlated with waist hip ratio (WHR), PG120, INS120, HOMA insulin resistance (HOMA-IR), Stumvoll 1st-insulin secretion, Stumvoll 2nd-insulin secretion, ALT, AST, GGT and TC ( r=0.197, 0.274, 0.334, 0.162, 0.199, 0.211, 0.406, 0.361, 0.215, and 0.196, respectively, all P<0.05), and negatively correlated with ISI-Cederholm and HDL-C ( r=-0.334, and-0.237, respectively, all P<0.05). Furthermore, a multiple linear stepwise regression analysis indicated that ISI-Cederholm (Standardized β =-0.298, P<0.001) and Stumvoll 1st insulin secretion (Standardized β = 0.164, P = 0.024) were independent factors of IHTC. Conclusions:Peripheral insulin resistance occurs in the early stage of NAFLD and becomes worse with the progression of the disease. IHTC was independently associated with insulin sensitivity and first-phase insulin secretion.
5.Investigation on association of activating transcription factor 6 Ala145Pro variant with glucose and lipid metabolism in Chinese

Objective To investigate the association of activating transcription factor 6(ATF6)gene Ala145Pro(GCG→CCG)variant with glucose and lipid metabolism in Chinese.Methods The genotypes were determined by PCR-RFLP in 689 Chinese in Shanghai.Among them,361 subjects showed normal glucose regulation,250 cases were newly-diagnosed diabetic patients without taking any drug and 78 cases were probands of early-onset type 2 diabetes pedigrees.The following phenotypes were measured:body height and weight to calculate BMI;waist,hip and femoral circumference to calculate waist-to-hip ratio and waist-to-femoral ratio;blood pressure;plasma glucose levels obtained at 0 and 120 minute during 75 g oral glucose tolerance test;fasting serum lipid profile including total cholesterol,triglyceride,high density lipoprotein-cholesterol and low density lipoprotein-cholesterol;body fat percentage and distribution.Results(1)The frequency of C allele is significantly lower in probands from early-onset type 2 diabetes patients compared with subjects with normal glucose regulation(P=0.035).(2)In subjects with normal glucose regulation,the CC+CG genotype had a significantly lower level of high density lipoprotein cholesterol as compared with GG genotype(P=0.014).(3)In type 2 diabetic patients,the CC+CG genotype had a significantly higher level of low density lipoprotein-cholesterol as compared with GG genotype(P=0.041).Conclusion These findings suggest that variant of ATF6 plays a role in glucose and lipid metabolism in Chinese.
