1.Correlation of fat fraction and T2 value of multifidus muscle with Pfirrmann grade of L4-5 intervertebral disc in patients with lumbar muscle strain

Objective To observe the correlation between fat fraction(FF)and T2 value of multifidus muscle and Pfirrmann grade of L4-5 intervertebral disc in patients with lumbar muscle strain.Methods Totally 200 L4_5 intervertebral discs in 200 patients with lumbar muscle strain were retrospectively enrolled,including 6 cases of Pfirrmann grade Ⅰ,61 of grade Ⅱ,52 of grade Ⅲ,56 of grade Ⅳ and 25 cases of grade V.Based on modified DIXON quantification(mDIXON-Quant)and T2 mapping,FF and T2 value of multifidus muscle were calculated and compared among intervertebral discs with different Pfirrmann grades,and the correlations between the above parameters and Pfirrmann grade of intervertebral disc were analyzed.Results Significant differences of left,right and bilateral multifidus muscles FF,as well as of T2 values in left and bilateral multifidus muscles were found among L4-5 intervertebral discs with different Pfirrmann grades.Multifidus muscle FF was moderately positively correlated with Pfirrmann grade of L4-5 intervertebral disc(r=0.538,P<0.05),while T2 value was weakly positively correlated with Pfirrmann grade of L4-5 intervertebral disc(r=0.202,P<0.05).Conclusion FF of multifidus muscle was moderately positively correlated with Pfirrmann grade of L4-5 intervertebral disc,while T2 value of multifidus muscle was weakly positively correlated with Pfirrmann grade of L4-5 intervertebral disc in patients with lumbar muscle strain.
2.Value of early maternal serological indicators in predicting delivery of full-term small-for-gestational-age infants

Objective To investigate the value of early maternal serological indicators in predic-ting the delivery of full-term small-for-gestational-age(SGA)infants.Methods The clinical data of pregnant women who delivered at Beijing Tongzhou District Maternal and Child Health Hospital from August 2023 to August 2024 were analyzed.A total of 180 pregnant women who delivered full-term SGA infants were included in case group,and 180 pregnant women who delivered full-term appropri-ate-for-gestational-age(AGA)infants during the same period were included in control group.The levels of serum pregnancy-associated plasma protein A(PAPP-A),placenta growth factor(PLGF),and soluble fms-like tyrosine kinase-1(sFlt-1)in the first trimester(11 to 13 weeks+6 days of ges-tation)of the two groups were compared.Receiver operating characteristic(ROC)curves were used to analyze the predictive value of early maternal serological indicators for the delivery of full-term SGA infants.Results The levels of serum PAPP-A and PLGF in the first trimester of the case group were lower than those in the control group,while the level of sFlt-1 was higher(P<0.05).The area un-der the ROC curve(AUC)for the combined detection of serum PAPP-A,PLGF,and sFlt-1 levels in the first trimester in predicting the delivery of full-term SGA infants was 0.823,which was higher than the AUCs for detection of each indicator(0.738,0.720,0.724)(P<0.05).Conclusion The detection of early maternal PAPP-A,PLGF,and sFlt-1 levels can reflect the fetal growth status in utero.The combined detection of these three indicators has a higher predictive value for the delivery of full-term SGA infants by pregnant women,which is helpful for clinicians to take intervention measures in advance.
3.Application research on a discharge preparation service plan for patients undergoing transcatheter heart valve surgery

Objective To explore the application effects of a discharge planning service program based on King's Goal Attainment Theory in patients undergoing transcatheter heart valve surgery.Methods Using convenience sampling method,80 patients who underwent transcatheter heart valve surgery at a tertiary comprehensive hospital in Zhejiang Province from August 2023 to January 2024 were selected as the research subjects.Among them,40 patients from November 2023 to January 2024 were selected as an experimental group.A discharge preparation service plan for patients undergoing transcatheter heart valve surgery based on King's Goal Attainment Theory was adopted.Totally 40 patients who underwent transcatheter heart valve surgery from August to October 2023 were selected as a control group and underwent routine measures for discharge preparation.The differences in the scores of the discharge preparation scale,discharge guidance quality scale,self-management scale for patients after mechanical heart valve replacement surgery,and unplanned readmission rate were compared between 2 groups before and after intervention.Results During the application of the plan,a total of 2 cases were excluded,and 38 cases were included in the experimental group and 40 cases were included in the control group.After intervention,the experimental group had higher scores in discharge readiness,discharge guidance quality,and self-management ability than the control group,and the differences were statistically significant(P<0.001).The unplanned readmission rate within 30 days in the experimental group(2.63%)was lower than that in the control group(7.50%),and the difference was not statistically significant(P>0.05).Conclusion The discharge preparation service plan for patients undergoing transcatheter heart valve surgery based on King's Goal Attainment Theory can improve the discharge preparation,discharge guidance quality,and self-management ability of patients undergoing transcatheter heart valve surgery,and has a positive effect on improving their quality of life.
4.Analysis of drug application and management of cancer pain in hospice patients in Zhejiang Province

Objective:To investigate the management of cancer pain and the clinical practices of hospice care across 11 cities in Zhejiang Province.Methods:From May 22 to 29, 2023, the Zhejiang Provincial Health Commission conducted a survey to assess the current status of hospice care practitioners regarding cancer pain management, the practices employed by medical staff in managing cancer pain, and the understanding of medical personnel concerning self-controlled analgesia for cancer pain treatment in Zhejiang Province.Results:A total of 505 questionnaires were collected from 198 hospitals across 11 cities in the province.Among the medical staff in secondary and tertiary medical institutions, 85.71%(198 out of 231)participated in the management of cancer pain in patients.Oral analgesics emerged as the most commonly used treatment for pain outbreaks, accounting for 38.53%(89 out of 231)of cases.Additionally, 37.66%(87 out of 231)of medical personnel were involved in the development of self-controlled analgesia devices within their institutions.Conclusions:In the management of cancer pain within hospice care, it is essential to enhance the theoretical training of medical staff, ensure the availability of basic analgesic medications, and establish standardized management protocols for the entire process as promptly as possible.
5.Development of a training content framework for hospice volunteers based on competency model

Objective:To develop a training content framework for hospice volunteers based on the competency model.Methods:This was a qualitative study. A draft of the training content framwork for hospice volunteers was first formed according to results of literature review and qualitative interviews based on the competency model. From December 2023 to March 2024, two rounds of Delphi consultation with 16 experts were conducted to develop the final version of the system, and the weights of each indicator were determined with analytic hierarchy process.Results:Among the 14 consulting experts who completed two rounds of consultation, including 10 females, all held intermediate or above professional titles, and their working years ranged from 10 to 34 years, covering fields of hospice care, nursing management, clinical nursing and nursing education. In two rounds of consultation the expert positive coefficient was 14/16 and 14/14, the expert authority coefficient was 0.882 and 0.893, and the Kendall coordination coefficient of all indicators was 0.109 and 0.161 ( P<0.01), the coefficient of variation was 0-0.31 and 0-0.19, respectively. The final version of the system consisted of 3 first-level indicators, 7 second-level indicators, 20 third-level indicators, and 55 specific training contents. The three first-level indicators were knowledge, skill, role and value, and the weight coefficients were 0.335, 0.313 and 0.352, respectively. Each judgment matrix was consistent. Conclusions:A training content system for hospice care volunteers has been developed in this study, which may be used for medical institutions and organizations to train hospice care volunteers.

10.Research progress on nurses' knowledge, attitude and practice and associated factors concerning patient-controlled analgesia for cancer pain

Pain is one of the most common and unendurable symptoms in cancer patients and a major factor affecting their quality of life.Patient-controlled analgesia(PCA) is an important palliative measure in additional to conventional pharmacological control of pain.Nurses play the primary role in the management of PCA for cancer pain, and their knowledge, attitude and practice about PCA for cancer pain directly affect the effectiveness of cancer pain management.This article summarizes and analyzes the current status of nurses' knowledge, attitude and practice and associated influencing factors, aiming to improve nursing management of PCA, reinforce specialized nursing training, propose recommendations for an expert consensus on PCA for cancer pain and provide a reference for nursing practice in PCA for cancer pain.
