ObjectiveTo investigate the associations of multiple glycolipid metabolic indicators and their cumulative abnormality burden with incident colorectal cancer risk in a general population-based prospective cohort, and to examine the mediating role of glycolipid metabolic abnormalities in the relationships between smoking, alcohol consumption, physical activity, and colorectal cancer incidence. MethodsA total of 17 897 eligible participants recruited from the Taizhou Cohort between 2011 and 2014 were included. Cox proportional hazards regression models were used to assess the associations of conventional and derived glycolipid indicators, as well as a glycolipid abnormality index constructed from total cholesterol, triglycerides, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, fasting plasma glucose, and insulin, with incident colorectal cancer risk. Restricted cubic spline models were applied to evaluate dose-response relationships for major indicators. Receiver operating characteristic curves were generated to compare predictive performance across indicators. Mediation analyses were conducted to assess the mediating effects of the glycolipid abnormality index on the associations of smoking, alcohol consumption, and physical activity with colorectal cancer incidence. ResultsDuring a median follow-up of 11.2 years, 102 incident colorectal cancer cases were identified, with an incidence density of 51.2 per 100 000 person-years. After adjustment for potential confounders, Cox proportional hazards regression analyses showed that decreased high-density lipoprotein cholesterol was associated with a 1.74-fold higher risk of colorectal cancer (HR=1.74, 95%CI: 1.03‒2.95), elevated low-density lipoprotein cholesterol was associated with a 1.78-fold higher risk (HR=1.78, 95%CI: 1.05‒3.02), and abnormal fasting plasma glucose was associated with a 1.86-fold higher risk (HR=1.86, 95%CI: 1.15‒3.02). Triglycerides and fasting plasma glucose showed an approximately linear increasing association with colorectal cancer risk in multivariable restricted cubic spline models. The glycolipid abnormality index showed a clear gradient association with colorectal cancer risk; participants with three or more abnormal indicators had a 3.08-fold higher risk than those without abnormalities (HR=3.08, 95%CI: 1.60‒5.92). The area under the curve was 0.795, higher than that of individual glycolipid indicators and other combined indices, and gender-stratified analyses showed generally consistent patterns. Glycolipid metabolic abnormalities partially mediated the associations of smoking, alcohol consumption, and physical activity with colorectal cancer incidence, with mediation proportions of 8.33%, 9.45%, and 9.04%, respectively. ConclusionGlycolipid metabolic abnormalities are associated with an increased risk of incident colorectal cancer. The glycolipid abnormality index shows an increasing relationship with colorectal cancer risk and demonstrates better discrimination, and it partially mediates the associations of smoking, alcohol consumption, and physical activity with colorectal cancer incidence in the overall population.

ObjectiveTo observe the clinical effectiveness and safety of Sanjie Waifu Formula （散结外敷方） application therapy for benign thyroid nodules of qi constraint and phlegm obstruction syndrome. MethodsEighty patients with benign thyroid nodules of qi constraint and phlegm obstruction syndrome were randomly divided into 40 cases each in a treatment group and a control group. The control group was given health education and follow-up observation， while the treatment group applied Sanjie Waifu Formula externally on the basis of the control group，6~8 h each time ， once a day. Both groups took 30 days as a course， and were observed for 3 consecutive courses. The main effectiveness indexes of both groups were compared before and after treatment， including the volume of thyroid nodules， the maximum diameter of the largest nodule， the scores of traditional Chinese medicine （TCM） syndrome， and the secondary effectiveness indexes of serum thyroid function including free triiodothyronine （FT₃）， free thyroxine （FT₄）， thyrotropin （TSH） levels were also observed. The effectiveness of the TCM syndrome of both groups was compared after treatment. Meanwhile， the safety indexes were observed before and after treatment， such as the blood routine， liver and kidney function indexes， and the occurrence of adverse reactions during the study was recorded. ResultsCompared with in group before treatment， the volume of thyroid nodules， the maximum diameter of the largest nodule， and the TCM symptom scores were lowered in both groups after treatment （P＜0.05 or P＜0.01）. Comparing between groups after treatment， the difference in thyroid nodule volume was not statistically significant （P＞0.05）， but the maximum diameter of the largest nodule and the TCM symptom scores of the treatment group were lower than those of the control group （P＜0.01）. The total effective rate of TCM syndrome in the treatment group was 77.50% （31/40）， which was significantly higher than that of the control group with 52.5% （21/40， P＜0.01）. Thyroid function， blood routine and liver and kidney function indexes of both groups before and after treatment were compared between and within groups， and the differences were not statistically significant （P＞0.05）， and they were all within the normal range. During the treatment period， no allergic reactions such as skin itching， red rash and blisters occurred in the treatment group. ConclusionThe external application of Sanjie Waifu Formula can improve the clinical symptoms of patients with benign thyroid nodules， reduce volume and the maximum diameter of nodules， and has no significant effects on thyroid function and show good safety.

Objective: To delineate the clinical features, inheritance pattern, and genotype-phenotype correlation of a Chinese patient with a 17q25.3 duplication. Methods: Whole exome sequencing(WES), chromosomal microarray analysis (CMA), chromosomal karyotyping and fluorescence in situ hybridization(FISH) were employed for the analysis of the proband and his family members. Results: A 5.7 Mb duplication at 17q25.3→qter was identified by WES and CMA in the 4-year-old boy with multiple congenital anomalies, which was classified as a clinically pathogenic variant. This duplication was confirmed by FISH, and was inherited from his unaffected mother who carried a balanced translocation. Further study revealed that his grandmother also carried the balanced translocation but had gestated three healthy children and had no abortion history. His uncle also carried the balanced translocation, while his aunt was normal. Conclusion Above results have enriched the clinical phenotypes of 17q25.3 duplication. Genetic counseling was provided for the family.P4HB, ACTG1, BAIAP2 and TBCD genes may underlie the clinical features for the 17q25.3 duplication.

The aim of the study was to prepare enrofloxacin nanosuspension injection and evaluate its pharmacokinetics after giving a single intramuscular injection.The high pressure homogeneous technique was used to prepare enrofloxacin nanosuspension injection and preliminary evaluation of the quality was done.The high performance liquid chromatography (HPLC) method was used to determinate content of enrofloxacin in pig plasma.And the pharmacokinetic characteristics of enrofloxacin nanosuspension injection were compared with Baytril injection.The content of enrofloxacin in this preparation is 97.9％.The average particle size of enrofloxacin nanosuspension injection was (613.21±5.78) nm,PDI was (0.22±0.02) and the potential was-2.02 mV.Maximal plasma concentrations were (0.32±0.12) and (0.67 ± 0.09) mg/L after i.m administration with enrofloxacin nanosuspension injection and Baytril injection.The peak times were (2.88 ±0.96) and (0.79±0.26) hours,respectively.Mean elimination half-lifes were (5.99± 1.37) and (4.49 ± 1.25) hours,respectively.Areas under concentration-time curve were (4.63±1.30) and (4.40±0.45) mg/L · h,respectively.Mean residence times were (9.59±2.34) and (5.41±1.10) hours,respectively.The relative bioavailability of enrofloxacin nanosuspension injection was 105.2％.The preparation method of high pressure homogeneous was simple and good reproducibility.Enrofloxacin nanosuspension injection was characterized by non-sedimentation,easy-redispersion,relatively stable.Comparing with Baytril injection,enrofloxacin nanosuspension injection had a certain slowrelease effect,showing slower elimination than enrofloxacin injeetion.

The experiment was done to explore the effect of sodium Tanshinone Ⅱ_A, sulfonat (STS) on myocardial reperfusion injury. Myocardial ischemia was induced with the occlusion of left anterior descending coronary artery lasting 30 minutes, and postischemic reperfusion persisted 30 minutes also. Fifty rabbits were randomly divided in to five groups: operative control group (group Ⅰ, n=10), myocardial ischemia group (group Ⅱ, n=10), myocardial ischemia and reperfusion group (group Ⅲ, n=10), receiving intravenous STS 5mg?kg~(_1) 5 mins before the reperfusion (group Ⅳ, n=10) and receiving intravenous STS 5mg?kg~(_1) 10 mins before the ischemia and 5 mins before the reperfusion respectively (group Ⅴ, n=10).
