1.Effects of different forms of shaker exercises on hyoid muscles activation in healthy adults

BACKGROUND:Shaker exercise can enhance the contraction force of the hyoid muscles,which is effective in the treatment of oropharyngeal dysphagia.Shaker exercise should be performed through the main force of the anterior cervical region to train the hyoid muscles.However,patients often experience physical fatigue,leading to compensatory abdominal force exertion.Moreover,the effect of different forms of shaker exercises on the activation of the hyoid muscles in healthy adults is currently unknown.OBJECTIVE:To investigate the effects of shaker exercises with varying force on the activation of the hyoid muscles in healthy adults.METHODS:Twenty healthy adults,including 10 males and 10 females with a mean age of(26.8±3.4)years were recruited as participants for the study.They performed isometric movements of the shaker exercise in the anterior neck and abdominal force modes in sequence.Simultaneously,real-time surface electromyography data were collected from the suprahyoid and infrahyoid muscles and rectus abdominis muscle.At the end of the experiment,participants were asked to rate the physical exertion levels for each type of force using the Borg Rating of Perceived Exertion Scale at the end of the training.Following a series of processing of the original electromyography signals,the peak amplitude and/or average amplitude values of the suprahyoid and infrahyoid muscles and rectus abdominis in the 60-second movement window were calculated and compared,and the differences in the subjective fatigue scores corresponding to the two force modes were compared.RESULTS AND CONCLUSION:The average amplitude of the rectus abdominis muscle in the abdominal force mode was found to be higher than that in the neck force mode(P<0.05).However,there were no significant differences in the mean and peak amplitudes between the suprahyoid and infrahyoid muscles in the two force modes(P>0.05).There was also no significant difference in the Borg Rating of Perceived Exertion Scale scores in the two force modes(P>0.05).These findings suggest that shaker exercises with different force modes(anterior neck and abdomen)have minimal impact on the hyoid muscles and similar feeling of fatigue in healthy adults.
2.Positive pressure oxygen therapy combined with drugs to improve pulmonary circulation can improve the treatment efficiency of severe viral pneumonia at high altitude areas

Objective To observe the efficacy of positive pressure oxygen therapy combined with drugs to improve pulmonary circulation in the treatment of severe viral pneumonia in high-altitude areas.Methods A two-way cohort study was conducted.Patients with severe viral pneumonia and those with common viral pneumonia complicated with underlying heart and lung diseases admitted to department of intensive care unit of Xizang Autonomous Region People's Hospital were selected as the research subjects.Patients who received conventional treatment in the early stage were assigned to the control group,while those who received conventional treatment plus active positive pressure oxygen therapy combined with drugs to improve pulmonary circulation in the later stage were assigned to the study group.The treatment effective rates of the two groups were observed(including the time for viral nucleic acid to turn negative,hospital stay,and 28-day follow-up mortality)and changes in cardiopulmonary function indicators[pulmonary artery pressure,tricuspid annular plane systolic excursion(TAPSE),left ventricular stroke volume(SV),and lung ultrasounol score(LUS)]before and after treatment were also observed,and the Kaplan-Meier curve was drawn to analyze the 28-day cumulative survival rate of the two groups.Results There was no statistically significant difference in the time for viral nucleic acid to turn negative and hospital stay between the two groups.Compared with the control group,the 28-day mortality in the study group was significantly lower[6.5%(2/31)vs.25.0%(13/52),P<0.05].Compared with before treatment,pulmonary artery pressure gradually decreased,TAPSE significantly increased,and left ventricular SV significantly increased after treatment in the study group,and the differences were statistically significant compared with 10 days after treatment[pulmonary artery pressure(mmHg,1 mmHg≈0.133 kPa):28.84±8.71 vs.34.68±10.76,TAPSE(cm):2.37±0.11 vs.2.03±0.41,SV(mL):68.68±7.17 vs.59.61±6.73,all P<0.01].Pulmonary lesions,especially bilateral pulmonary exudative lesions,significantly improved compared with before treatment,atelectasis improved significantly,and LUS significantly decreased(14.77±5.33 vs.20.32±5.63,P<0.01).Kaplan-Meier curve analysis showed that the 28-day cumulative survival rate in the study group was significantly higher than that in the control group(Log-Rank test:χ2=4.510,P=0.034).Conclusion Active use of positive pressure ventilation and early administration of drugs to improve pulmonary circulation in patients in high-altitude areas can significantly reduce pulmonary artery pressure and significantly improve left and right heart function and pulmonary exudative lesions.These improvements may reduce the mortality rate of viral pneumonia and viral infections complicated with underlying heart and lung diseases in high-altitude areas.

Dipsaci Radix is a commonly used yang tonifying medicine in clinical practice.Ancient books record that its preparation methods are diverse,mainly concentrated in the Ming and Qing dynasties,including wine soaking,wine washing,wine baking,wine stir frying,stir frying,wine mixing,and salt water stir frying.Wine roasting can promote blood circulation,dispel cold stagnation,and has been used throughout history;salt roasting has been seen in modern times,which can induce Chinese materia medica to descend and enhance liver and kidney tonifying effects;at present,it is mainly used for slicing raw materials,but there are also processed products such as wine fried products,salt fried products,stir fried slices,and charcoal slices.This article reviewed the herbal monographs,TCM ancient books,processing standards and modern literature,and combed the related elaboration of the processing history and modern processing research of Dipsaci Radix in the literature,so as to provide references for the processing mechanism,method research,clinical application and resource development and utilization of Dipsaci Radix.

7.Evaluation of Molecular Residual Disease by a Fixed Panel in Resectable Colorectal Cancer

Purpose:
Molecular residual disease (MRD) is a promising biomarker in colorectal cancer (CRC) for prognosis and guiding treatment, while the whole-exome sequencing (WES) based tumor-informed assay is standard for evaluating MRD based on circulating tumor DNA (ctDNA). In this study, we assessed the feasibility of a fixed-panel for evaluating MRD in CRC.
Materials and Methods:
Seventy-five patients with resectable stage I-III CRC were enrolled. Tumor tissues obtained by surgery, and preoperative and postoperative day 7 blood samples were collected. The ctDNA was evaluated using the tumor-agnostic and tumor-informed fixed assays, as well as the WES-based and panel-based personalized assays in randomly selected patients.
Results:
The tumor-informed fixed assay had a higher preoperative positive rate than the tumor-agnostic assay (73.3% vs. 57.3%). The preoperative ctDNA status failed to predict disease-free survival (DFS) in either of the fixed assays, while the tumor-informed fixed assay-determined postoperative ctDNA positivity was significantly associated with worse DFS (hazard ratio [HR], 20.74; 95% confidence interval [CI], 7.19 to 59.83; p < 0.001), which was an independent predictor by multivariable analysis (HR, 28.57; 95% CI, 7.10 to 114.9; p < 0.001). Sub-cohort analysis indicated the WES-based personalized assay had the highest preoperative positive rate (95.1%). The two personalized assays and the tumor-informed fixed assay demonstrated same results in postoperative landmark (HR, 26.34; 95% CI, 6.01 to 115.57; p < 0.001), outperforming the tumor-agnostic fixed panel (HR, 3.04; 95% CI, 0.94 to 9.89; p=0.052).
Conclusion
Our study confirmed the prognostic value of the ctDNA positivity at postoperative day 7 by the tumor-informed fixed panel. The tumor-informed fixed panel may be a cost-effective method to evaluate MRD, which warrants further studies in future.

Novel coronavirus Omicron variant infection can cause severe illness and even death in certain populations. Omicron variant infection may lead to systemic inflammatory response, coagulation disorder, multi-organ dysfunction and other pathophysiological changes, which are different from other Novel coronavirus variants to a certain extent, so therapeutic strategies should not be the same. The National Medical Center for Major Public Health Events invited experts in fields of infectious diseases, respiratory medicine, intensive care, pediatrics and fever clinic to develop this quick guideline based on the current best evidence and extensive clinical practices. 9.Prognostic Factors in Patients with Advanced Lung Adenocarcinoma Treated with Icotinib

Objective To investigate the relationship between the treatment of EGFR-TKI icotinib and the prognosis of advanced lung adenocarcinoma patients with EGFR mutation. Methods Patients with advanced lung adenocarcinoma who had EGFR19 and 21 gene mutations and were treated with EGFR-TKI icotinib were enrolled. The relationships of clinical features, EGFR gene mutation subtypes, and different sites with patients'prognosis were analyzed. Results A total of 101 patients with advanced lung adenocarcinoma were included in this study, including 58 cases (57.4%) of EGFR gene exon 19 deletion mutation (EGFR Del19) and 43 cases (42.6%) of EGFR gene exon 21 point mutation (EGFR L858R). The objective response rate was 63.4%. The mPFS and mOS were 13 months and 27 months, respectively. In addition, the mPFS and mOS of EGFR Del19 and EGFR19 mutation 746-750 were higher than those of EGFR L858R and other EGFR mutations, respectively. 10.Prevention of Nurse-patient Disputes under the "4P" Medical Model

At present, the medical mode has changed from bio-psycho-social medical model to "4 P" medical mode. Therefore, it is necessary to fully consider the individual needs of patients, treat patients equally, adheres to the principle of faith and compliance, and adhere to the humanistic nursing concept. To prevent nurse-patient disputes, we should start from three aspects. Firstly, we should reform the current model of nurse education and training, reform the content of education and attach importance to moral sentiment education. Secondly, we should strengthen the cooperation between doctors and nurses, improve the level of health education, and build a harmonious nursing environment and a social atmosphere of respecting medical staff. Finally, we call for strengthening medical legislation and protecting the legitimate rights and interests of nurses with legal weapons.
