Objective To investigate correlations of dynamic changes in serum C-reactive pro-tein(CRP),procalcitonin(PCT),and neutrophil-to-lymphocyte ratio(NLR)before and after symptomatic treatment with prognosis of neonates with neonatal respiratory distress syndrome(NRDS)of different etiologies.Methods A total of 110 premature infants were selected as study subjects and divided into neonatal infection group(group A)and fetal intrauterine distress group(group B)based on different causes of NRDS.Additionally,30 neonates with NRDS caused solely by prematurity were selected as control group.Serum CRP and PCT levels and NLR were compared among the three groups before and after treatment.Results There were statistically significant differences in oxygen therapy duration,ventilation duration,the proportion of infants requiring re-intubation,hospital stay,and the number of apnea episodes among three groups(P＜0.006).Before treatment,there was no sta-tistically significant difference in serum CRP levels between group A and group B(P＞0.05).Ser-um CRP levels in group B were higher than those in the control group.Moreover,serum PCT levels was lower than the group A,and NLR in the group B were higher than those in the group A and the control group.CRP,PCT levels,and NLR in the group A were higher than those in the control group(P＜0.05 or P＜0.006).After treatment,serum CRP levels in all three groups decreased compared to before treatment.Serum PCT levels in the group A decreased compared to before treat-ment.NLR in both group A and group B decreased compared to before treatment,with statistically significant differences(P＜0.006).There were statistically significant differences in serum CRP and PCT levels between the group A and the control group after treatment(P＜0.006).After treat-ment,there was a statistically significant difference in serum CRP levels between the group B and the control group(P＜0.006),but no statistically significant differences in PCT levels and NLR(P＞0.05).Compared with group A,there was a statistically significant difference in PCT levels in the group B(P＜0.006).Multivariate Logistic regression results showed that CRP,PCT,and NLR were independent influencing factors for the prognosis of infants in the group A.The receiver operat-ing characteristic curve analysis results showed that the areas under the curve(AUC)after treatment for serum CRP,PCT,and NLR alone in predicting prognosis in the group A were 0.789,0.738,and 0.758,respectively,and the AUC for combined prediction was 0.934.In the group B,the AUC for serum CRP,PCT,and NLR alone in predicting prognosis after treatment were 0.719,0.772,and 0.768,respectively,and the AUC for combined prediction was 0.886.The sensitivity and specificity of combined prediction in both groups were higher than those of each indicator alone,and the predictive value of their combined detection for the prognosis of premature infants in group A was higher than that in the group B.Conclusion There are differences in PCT levels and NLR before treatment among neonates with NRDS of different etiologies.After symptomatic treatment,CRP,PCT levels and NLR decrease in all three groups,indicating a good prognosis.

Objective:To explore the clinical effect of using great toe fibular flaps of both feet on reconstruction of pulp defects of two neighbouring digits.Methods:A total of 14 digit pulp defects in 7 cases were repaired in Zhoukou Huaihai Hospital using great toe fibular flaps of both feet from August 2020 to January 2023. Of the 7 cases, there were 4 males and 3 females, with an average of 28 years old, ranging from 19 to 45 years old. Meanwhile, there were 4 cases in left hand and 3 cases in right hand. There were 3 cases of digit pulp defects in index and middle fingers, 2 in middle and ring fingers, and 2 in thumb and index fingers. The area of soft tissue defect in 1.2 cm×1.5 cm-3.0 cm×2.5 cm, and flap was 1.5 cm ×1.8 cm-3.2 cm×2.8 cm. Furthermore, 1 case underwent emergency surgery and 5 were repaired in elective surgery. The donor site of the flap was closed directly, and an intermediate-thickness skin graft was prepared from the medial plantar area for transfer in the case of high suture tension at the wound edge. After surgery, patients received postoperative by outpatient clinic and WeChat to observe the appearance, sensation, functional recovery and flap contracture of digits, as well as the movement of the great toes of both feet.Results:After the surgery, all flaps in the 7 cases survived smoothly and the donor sites healed. All patients entered scheduled follow-ups postoperatively for 6 months to 2 years, with an average of 9 months. The flap showed an aesthetic appearance and excellent sensation, with a TPD of 3-6 mm, and satisfactory digit function. The donor site of the great toe fibular flap left linear scars only, without abnormality in range of motion and gait in walking. In addition, there were 5 in excellent and 2 in good according to the Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association.Conclusion:Application of great toe fibular flaps of both feet is an ideal option for the simultaneous repair of pulp defects of two neighbouring digits, which can achieve good reconstructive results.

Objective To understand the types, proportions, and radiation dose levels of pediatric cardiac interventional procedures. Methods A retrospective analysis was conducted using the basic information and radiation dose data from all cases of pediatric cardiac interventional procedures performed at two tertiary medical institutions in Beijing from July 2023 to August 2024. The data included air kerma-area product, reference point air kerma, and fluoroscopy time. Descriptive statistical analysis was conducted to evaluate the basic information and dosimetric indicators of the procedures. Non-parametric tests were employed to examine the differences between variables, and Spearman’s correlation coefficient was utilized to investigate the relationships among various factors. All cases were categorized into the diagnostic group and the therapeutic group. In the diagnostic group, the majority of procedures involved catheter examinations and angiography for identifying the type of disease. In the therapeutic group, the primary procedures included patent ductus arteriosus, atrial septal defect, ventricular septal defect, patent foramen ovale, pulmonary stenosis, radiofrequency catheter ablation, major aortopulmonary collateral arteries, complex congenital heart diseases, and other interventional procedures. Results The survey comprised 1008 cardiac interventional procedures involving 476 males and 532 females. The ages of the participants ranged from 0 to 15 years and the body weights from 4.5 to 105 kg. Among these participants, 92 and 916 were classified in the diagnostic and therapeutic groups, respectively. The proportions of interventional procedures were in the order of atrial septal defect (36.9%), radiofrequency catheter ablation (22.3%), patent ductus arteriosus (15.1%), complex congenital heart disease (4.8%), patent foramen ovale (3.1%), pulmonary stenosis (3.1%), ventricular septal defect (2.2%), other procedures (2.0%), and major aortopulmonary collateral arteries (1.5%). The radiation doses of different interventional procedures were statistically different (P < 0.05), with air kerma-area product ranging from 0.02 to 658.4 Gy·cm², reference point air kerma from 0.1 to 2524 mGy, and fluoroscopy time from 0.2 to 62 min. Conclusion In pediatric cardiac interventional procedures, there is considerable variability in radiation doses for the same type of procedure. Additionally, radiation doses for identical procedures vary between different medical institutions, and a few cases carry a risk of deterministic effects.

Objective　 : To obtain the peak skin dose (PSD) of patients with interventional cardiology procedures and toevaluate the risk of deterministic effects. Methods　 : Gafchromic XR RV3 films were used in a Level A tertiary hospital inBeijing to measure the PSD of patients who underwent interventional cardiology procedures. The measurement focused onfour common types of procedures, including coronary angiography, percutaneous transluminal coronary angioplasty,cathet-er radiofrequency ablation, and congenital heart disease. The films were scanned by EPSON EXPRESSION 10000XL andanalyzed by FILM QA ProTM 2014 software. Results　 : PSD was measured in 59 patients with interventional cardiologypro-cedures, including 23 with coronary angiography, 21 with percutaneous transluminal coronary angioplasty, 9 with catheterradiofrequency ablation, and 6 with congenital heart disease. The seven patients with PSD ≥ 2 Gy all underwentpercu-taneous transluminal coronary angioplasty, one with PSD > 3 Gy and six with PSD < 2 Gy. Conclusion　 The PSD of somepatients with interventional cardiology surgery exceeded the dose threshold of deterministic effects recommended by theICRP 118. There is a risk of deterministic effects in interventional cardiology surgery, especially in patients withpercu-taneous transluminal coronary angioplasty.

Objective:To analyze the status quo, hotspots and fronts of emergency nursing training research at home and abroad in the past ten years, and to provide reference and ideas for the efficient development of emergency nursing training in China.Methods:CiteSpace 6.2.R2 software was used to visually analyze the Chinese and English literatures on emergency nursing training included in CNKI and Web of Science core databases from January 1, 2013 to June 1, 2023.Results:A total of 1 177 Chinese literatures and 1 163 English literatures were included. The number of foreign articles in this field increased year by year, while the number of domestic articles showed a downward trend since 2018. There were many stable core author groups and core institution groups in foreign countries, while there was less cooperation among domestic authors and institutions. The common research hotspots and frontiers at home and abroad focused on broadening the training audience of emergency nursing, innovating the training methods of emergency nursing, strengthening the evaluation of the effect of emergency nursing training, and paying attention to the training experience and needs of nurses. Foreign researches also focused on specialized nurses, interprofessional education and nurses′mental health, etc, and the research direction was diversified.Conclusions:The development stages of emergency nursing training researches at home and abroad are different, and the research hotspots are different. In the future, we should learn from foreign research, strengthen interdisciplinary cooperation, improve the depth and breadth of research, and strengthen the cooperation between authors, institutions and countries to promote the high-quality development of emergency nursing training research in China.

Objective:To explore the clinical efficacy of free superficial circumflex iliac artery perforator flap (SCIAPF) in reconstruction of soft tissue defects of hand and foot in children.Methods:From August 2021 to August 2023, free SCIAPFs were used to reconstruct soft tissue defects of hands and feet in 8 children at the Department of Hand and Foot Surgery, Zhoukou Huaihai Hospital. There were 6 boys and 2 girls aged between 2 and 7 years old. The sites of soft tissue defect were: 1 of metacarpophalangeal joint in the functional area of left thumb, 2 of dorsal right hand, 1 of dorsal left foot, 2 of medial malleolus of right foot, and 2 of right forefoot, and all were accompanied with tendon and bone exposure. The size of soft tissue defects ranged from 2.5 cm×3.0 cm to 6.0 cm×3.5 cm. After emergency debridement, the children received flap transfers, the flaps sized from 3.0 cm×3.5 cm to 7.0 cm×4.0 cm. Donor sites were closed by running intradermal suture. All children were included in the scheduled postoperative follow-up by visit of outpatient clinic, WeChat interviews and home visits to observe the appearance, colour, texture and sensation of the flaps, the functional recovery of the affected hands and feet, as well as the healing of donor sites.Results:After surgery, all 8 flaps survived beside 1 had a partial necrosis at the edge and healed with a scab after dressing change. Flap donor sites healed in one stage. Scheduled postoperative follow-up lasted from 7 to 22 months, with an average of 10 months. The flaps presented a slightly bloated appearance, soft texture with a restored protective sensation and satisfactory functional recovery of hands and feet. Linear scars were left at the donor sites. According to the Evaluation Trial Standards of Upper Limb Partial Functional of Hand Surgery of Chinese Medical Association, 1 child was of excellent and 2 children of good in hand function of the 3 children with hand injury. The Maryland Foot Score revealed 3 children were of excellent and 2 of good in foot function of the 5 children with foot injury.Conclusion:Free SCIAPF is an ideal option for reconstruction of soft tissue defects of hand and foot in children, with concealed donor site, convenient flap harvesting, fewer complications and satisfactory reconstructive effect.

With the rapid development of pet hospitals and the increasing application of radiological diagnosis in China, the protection of staff and assistants in these hospitals is worthy of attention. Based on the progress in international management and research with respect to radiological diagnosis and protection in animal hospitals, this paper presents a brief review of the current situation, existing problems and management regulations of animal hospitals in China. It summarizes typical issues that may be encountered in radiation protection by government departments, animal hospitals, radiation professionals and assistants in animal hospitals. It also puts forward the key points of radiation protection in animal hospitals in China, which can provide scientific suggestion to assist government in decision-making and standardizing and improving the radiation protection in animal hospitals.

Objective:To determine the peak skin dose (PSD) to patients from neuro-interventional procedures and evaluate the risk of the deterministic effect.Methods:Gafchromic XR RV3 films were used in a level A tertiary hospital in Beijing to measure the patients′ PSD from neuro-interventional procedures, mainly three common types of procedures, including vascular embolization, vascular angioplasty and vascular angiography. The films were scanned by Epson Expression 10000XL, read by ImageJ software, and analyzed by Film QA Pro?2014 software.Results:PSD was measured in 23 embolizations, 14 stentings and 12 arteriography. There were 20 patients whose PSD were equal or greater than 2 Gy, including 15 in vascular embolization and 5 in angioplasty. The PSDs to patients in cerebral arteriography were all below 2 Gy. The PSDs to some of the patients were higher than the threshold for deterministic effect recommended by ICRP Publication 118.Conclusions:There is a risk of deterministic effect in neurointerventional procedures. It is suggested that the patients be followed up to observe their radiation injury as well as to know in time the subsequent diagnosis and treatment.

Objective:To observe the inhibitory effect of gambogic acid on the proliferation of liver cancer cells HepG2 and explore its mechanism.Methods:The vitro cultured liver cancer cells HepG2 were divided into the control group, low dose gambogic acid group, medium dose gambogic acid group and high dose gambogic acid group according to random number table method, 5 multiple holes in each group. The control group was cultured in normal medium and 0.5% DMSO was added as the solvent control. The 0.1, 1 and 10 μmol/L of gambogic acid were added to low, medium and high dose group of gambogic acid for intervention. MTT method was used to detect the inhibition rate of cell proliferation, flow cytometry was used to detect the apoptosis rate, Hoechst nuclear staining was used to observe the nucleus apoptosis, and Western blot was used to detect the expression of Bcl-2 and Bax protein.Results:Compared with the control group, the HepG2 cells proliferation rate (68.00% ± 3.55%, 51.93% ± 4.36%, 47.16% ± 4.73% vs. 99.87% ± 4.53%) in low, medium and high dose gambogic acid group significantly decreased ( P<0.01); apoptosis rate (23.00% ± 1.22%, 40.09% ± 4.65%, 70.32% ± 4.99% vs. 4.33% ± 0.57%) significantly elevated ( P<0.01); Bcl-2 experssion (0.73 ± 0.10, 0.25 ± 0.10, 0.19 ± 0.08 vs. 0.97 ± 0.11) significantly decreased ( P<0.01); and Bax expression (0.39 ± 0.14, 0.88 ± 0.15, 0.85 ± 0.13 vs. 0.22 ± 0.08) significantly elevated ( P<0.01); Bax/Bcl-2 (0.34 ± 0.10, 1.87 ± 0.29, 1.63 ± 0.23 vs. 0.13 ± 0.06) significantly increased ( P<0.01). Hoechst staining showed that the apoptosis rate of HepG2 cells increased in a dose-dependent manner with the increasing concentration of gambogic acid. Conclusions:Gambogic acid could inhhibit HepG2 cells proliferation by regulating the expression of Bax and Bcl-2.

Objective:To establish a new model for the prediction of severe outcomes of COVID-19 patients and provide more comprehensive, accurate and timely indicators for the early identification of severe COVID-19 patients.Methods:Based on the patients’ admission detection indicators, mild or severe status of COVID-19, and dynamic changes in admission indicators (the differences between indicators of two measurements) and other input variables, XGBoost method was applied to establish a prediction model to evaluate the risk of severe outcomes of the COVID-19 patients after admission. Follow up was done for the selected patients from admission to discharge, and their outcomes were observed to evaluate the predicted results of this model.Results:In the training set of 100 COVID-19 patients, six predictors with higher scores were screened and a prediction model was established. The high-risk range of the predictor variables was calculated as: blood oxygen saturation <94 %, peripheral white blood cells count >8.0×10 9, change in systolic blood pressure <-2.5 mmHg, heart rate >90 beats/min, multiple small patchy shadows, age >30 years, and change in heart rate <12.5 beats/min. The prediction sensitivity of the model based on the training set was 61.7 %, and the missed diagnosis rate was 38.3 %. The prediction sensitivity of the model based on the test set was 75.0 %, and the missed diagnosis rate was 25.0 %. Conclusions:Compared with the traditional prediction (i.e. using indicators from the first test at admission and the critical admission conditions to assess whether patients are in mild or severe status), the new model’s prediction additionally takes into account of the baseline physiological indicators and dynamic changes of COVID-19 patients, so it can predict the risk of severe outcomes in COVID-19 patients more comprehensively and accurately to reduce the missed diagnosis of severe COVID-19.
