1.Level and related factors of latent tuberculosis infection in junior and senior high school freshmen in Lanzhou from 2023 to 2024

Objective:
To investigate the current status of latent tuberculosis infection (LTBI) among freshmen in junior and senior high schools in Lanzhou, so as to provide scientific basis for improving the tuberculosis prevention and control strategy in schools.
Methods:
The screening results of 74 516 freshmen in senior and boarding junior high schools in Lanzhou during 2023 and 2024 were collected. The Chi square test and multivariate Logistic regression model were applied to analyze LTBI level, strongly positive risk for tuberculin skin test (TST) and related factors of the freshmen.
Results:
During 2023 and 2024, the screening rate of tuberculosis among freshmen in senior and boarding junior high schools in Lanzhou was 93.45%, of which the positive rate for TST was 5.71%, the infection rate for LTBI was 3.80%, and the strongly positive rate for TST was 1.24%. There were statistically significant differences in the screening rate of tuberculosis among freshmen in different years, grades, regions, school types and districts ( χ 2=5.34, 2 463.88, 3 516.13, 132.34, 4 436.56, all P <0.05). Multivariate Logistic regression analysis showed that senior high schools ( OR =1.62, 2.18) and urban areas ( OR =2.08, 3.07 ) were all related factors for LTBI and strong positivity for TST among freshmen; schools located in Xigu District, Honggu District, Yongdeng County, Yuzhong County, and Lanzhou New Area ( OR =3.57, 5.67, 9.12, 3.70, 3.64) were related factors of strong positivity for TST among freshmen (all P <0.05).
Conclusions
The LTBI level among freshmen in senior and boarding junior high schools in Lanzhou is relatively low. Grades and regions are related factors for LTBI and strong positivity for TST.
2.Research progress on the mechanism of traditional Chinese medicine intervening in osteoarthritis by modulating the inflammatory microenvironment

The inflammatory microenvironment is closely associated with the initiation and progression of osteoarthritis (OA), specifically manifesting as macrophage activation, dysregulation of inflammatory cytokines, and redox imbalance. Following an overview of the pathological characteristics of the OA inflammatory microenvironment, this paper reviews the research progress on the mechanism of traditional Chinese medicine (TCM) intervening in OA by modulating the inflammatory microenvironment. It has been found that TCM monomers/active ingredients (such as total alkaloids from Strychnos nux-vomica , quercetin, triptolide, etc.), herb pairs (e.g. Angelica pubescens - Gentiana macrophylla , Carthami Flos-Lycopodii Herba), and TCM formulas (such as Zhuanggu jianxi formula, Duhuo jisheng decoction and Rongjin niantong formula, etc.) can inhibit macrophage activation, reduce the release of proinflammatory cytokines and the generation of reactive oxygen species by inhibiting multiple signaling pathways, including nuclear factor-κB, Wnt/ β -catenin, and mitogen-activated protein kinase, thereby alleviating the articular inflammatory microenvironment, restoring local joint homeostasis, and slowing the progression of OA.
3.Effects of different storage temperatures and durations on the activity of coagulation factor Ⅷ and Ⅸ in whole blood

Objective: To investigate the effects of different storage temperatures and durations on the activities of coagulation factor Ⅷ (Factor Ⅷ, FⅧ) and coagulation factor Ⅸ (Factor Ⅸ, FⅨ) after whole blood collection, so as to provide data support for the optimal storage conditions. Methods: A total of 16 mL of whole blood was collected from each of the 20 healthy volunteers at our blood center and aliquoted into 8 sodium citrate anticoagulant tubes. Two tubes were immediately centrifuged for the measurement of FⅧ and FⅨ activity levels. The remaining 6 tubes of whole blood were respectively stored under room temperature and low-temperature conditions. At 2, 4, and 6 h, the whole blood samples were centrifuged and analyzed for FⅧ and FⅨ activity levels. The mean values of the two immediately tested tubes were used as the control group, while other tubes were designated as the experimental groups for comparison. Statistical analysis was performed using SPSS 26.0. Results: The activity of FⅧ in whole blood remained stable after 4 hours of storage at both room temperature and low temperature (116.53±25.95 vs 125.22±27.33, 109.77±23.23 vs 125.22±27.33) (P>0.05 for both). However, by 6 hours, FⅧ activity showed a statistically significant decline compared to the control group (108.65±22.92 vs 125.22±27.33, 100.46±20.19 vs 125.22±27.33) (P<0.05 for both), though the room temperature group results were closer to the control values. The activity of FⅨ in whole blood remained stable after 6 hours of storage under both conditions (97.14±19.48 vs 96.76±19.67, 97.10±17.45 vs 96.76±19.6) (P>0.05 for all comparisons). Conclusion: For whole blood samples after collection, storage at either room temperature or low temperature for up to 4 hours does not compromise the accuracy of test results. When stored for 6 hours, FⅨ activity remains stable, whereas FⅧ activity decreases significantly. Notably, FⅧ activity demonstrates better stability at room temperature than under low-temperature conditions within the 6-hour storage.
4.Correlation between APTT mixing test results and inhibitor titers in patients with hemophilia A inhibitor

Objective: To study the correlation between activated partial thromboplastin time (APTT) mixing test results and the inhibitor titers in hemophilia A inhibitor-positive patients. Methods: In this cross-sectional study, 41 patients with severe hemophilia A and inhibitors (and negative for lupus anticoagulant) were included from the hemophilia clinic of Shandong Blood Center from February 2022 to February 2024. All patients underwent APTT mixing test. The Rosner's index (RI, including the immediate RI and the RI after 2-hour water bath incubation [water bath 2h RI]), the time-dependent difference (Δ value), and the corrected percentage were calculated based on results of APTT mixing test. The median (interquartile range) of the corresponding indexes were calculated, and the ROC curves for identification of high inhibitor titers using the four indexes (the immediate RI, the water bath 2h RI, the Δ value, and the corrected percentage) were plotted, The correlations between APTT mixing test and inhibitor titers for coagulation factor Ⅷ (Factor Ⅷ, FⅧ) were investigated. Results: The median (lower quartile, upper quartile) of immediate RI, water bath 2h RI, Δ-value and corrected percentage for FⅧ inhibitor positive patients were 11.0 (5.4, 29.3)%, 45.0 (25.7, 75.0)%, 26.2 (7.6, 41.8) s, and 82.2 (58.5, 91.6)%, respectively. The median (lower quartile, upper quartile) of the immediate RI, water bath 2h RI, Δ-value and corrected percentage were 25.2 (13.0, 37.5)%, 64.1 (44.6, 72.6)%, 38.0 (14.3, 38.3) s, and 66.5 (50.1, 82.1)% for the high-titer inhibitor group, and 5.2 (4.2, 9.4)%, 17.9 (8.8, 28.0)%, 13.0 (7.6, 25.4) s, and 92.3 (88.0, 94.3)% for the low-titer inhibitor group. The AUCs of the ROC curves for discrimination between high and low titer inhibitor were: 0.9105 for immediate RI, 0.9118 for water bath 2h RI, 0.8873 for correcter percentage, and 0.6532 for Δ-value. Conclusion: High-titer inhibitors can be highly suspected in hemophiliac patients with an immediate RI >10% and a water bath 2h RI >45%, and the presence of low-titer inhibitors is suspected in patients with a 4-second < immediate RI <10% and a 13% < water bath 2h RI <45%.
5.A Case of Hereditary Angioedema

A 43-year-old male patient presented with recurrent edema in different anatomical sites for over 10 years, with facial edema worsening 1 day prior to admission. He had been repeatedly admitted to dermatology, general surgery, and emergency departments of external hospitals due to " acute abdomen" and " laryngeal edema, " resistant to antihistamines and glucocorticoid therapy. Physical examination revealed non-pitting swelling of the right upper eyelid, bilateral cheeks, and lips asymmetrically. On the night of admission, he developed acute laryngeal edema with dyspnea, which was promptly treated, leading to clinical stabilization. Laboratory screening during the attack revealed decreased serum complement C4 levels, along with reduced functional activity and concentration of C1 esterase inhibitor, confirming a diagnosis of type 1 hereditary angioedema. The patient received lanadelumab for prophylaxis and achieved satisfactory clinical outcomes. He remains under long-term follow-up.
6.Outcomes of transcatheter transseptal mitral valve-in-valve replacement using Edward's SAPIEN 3 in high surgical risk patients-a multicenter study in China

Objective To evaluate the safety and efficacy of valve-in-valve transcatheter mitral valve replacement(ViV-TMVR)in patients with bioprosthetic valve degeneration who are at high surgical risk.Methods This study is a multi-center,retrospective cohort analysis of 20 consecutive patients who underwent transseptal ViV-TMVR using the Edwards SAPIEN 3 transcatheter heart valve(THV).The primary endpoints include technical success and procedural success,both defined according to the Mitral Valve Academic Research Consortium(MVARC)criteria,as well as mortality and functional change assessed based on New York Heart Association(NYHA)classification at 30-days and six months post-procedure.Clinical follow-up assessments are conducted at 30-days and six months.Results From February 2021 to October 2022,a total of 20 patients with symptoms of bioprosthetic valve degeneration were enrolled across nine sites in China.The patients had a mean age of(73.5±5.5)years,with 85.0%being females and 70.0%classified as NYHA class Ⅲ/Ⅳ.The study achieved a 100.0%technical success rate and a 90.0%procedural success rate finally.All patients remained alive during the 30-day follow-up period.However,six months post-intervention,two patients(10.0%)were re-hospitalized due to heart failure,and sadly,one of them(5.0%)died.None of the patients reported any adverse events related to ViV-TMVR during the follow-up period.Notably,there was a significant improvement in NYHA class compared to baseline(P=0.0004)at six-month follow-ups.Conclusions The transseptal ViV-TMVR technique proved to be highly successful and was associated with significant improvement in NYHA class function.These findings strongly suggest that it serves as a safe and efficient treatment alternative for high-risk patients suffering from bioprosthetic valve degeneration.
7.Analysis of risk factors and countermeasures for prolonged hospital stay in children with bronchopneumonia

Objective To explore the risk factors and intervention countermeasures for prolonged hospital stay in children with bronchopneumonia.Methods A retrospective analysis was conducted on the clinical data of 152 children with bronchopneumonia admitted to Zhoushan Women and Children Hospital from April 2022 to June 2024.The children were divided into prolonged group(hospital stay ≥ 10 days,53 cases)and non-prolonged group(hospital stay<10d,99 cases)based on whether their hospital stay was prolonged.Multivariate Logistic regression analysis was used to determine the independent risk factors for prolonged hospital stay in children with bronchopneumonia.Pearson correlation analysis was used to determine the correlation between independent risk factors and prolonged hospital stay of the children.Results Univariate analysis showed that age,disease severity,duration of fever after admission,duration from onset to hospitalization,mycoplasma antibody,sputum culture,pneumonia and other related vaccination,and basic diseases were all influencing factors for prolonged hospital stay in children with bronchopneumonia(P<0.05).Multivariate Logistic regression analysis showed that age,disease severity,and duration of fever after admission were all independent influencing factors for prolonged hospital stay in children with bronchopneumonia(P<0.05).Age was negatively correlated with the prolongation of hospital stay(r=-0.285,P<0.001).The disease severity and duration of fever after admission were positively correlated with the prolongation of hospital stay(r=0.312,0.458,P<0.001).Conclusion Young age,severe condition and long duration of fever after admission are all independent risk factors for prolonged hospital stay in children with bronchopneumonia.
8.Evaluation of Hemolytic Performance of Cellulose Hemostatic Products

Objective:To compare the differences in the evaluation of hemolysis performance of cellulose hemostatic materials using different detection methods and test media,and to explore a m ore reasonable testing plan for such products.Methods:Hemolysis tests were conducted on cellulose hemostatic materials using the absorbance measurement hemolysis method and hemoglobin concentration measurement hemolysis method in accordance with YY/T 1651.1-2019 standard.We compared the changes in hemolysis rate,pH value,and osmotic pressure under different experimental media.Results:Under the same experimental method,compared to SC,the hemolysis results using PBS as the extraction medium are smaller,and the changes in pH and osmotic pressure are closer to the normal range of human body changes.Conclusions:The changes in pH and osmotic pressure may be one of the reasons for the high hemolysis rate of cellulose hemostatic materials.Choosing PBS with buffering effect as the leaching medium may be more suitable for evaluating the hemolysis performance of cellulose hemostatic materials.
9.Oroxylin A induces apoptosis in Ishikawa cell line of endometrial cancer via PI3K/AKT signaling pathway

Aim To investigate the effect of oroxylin A(OA)on apoptosis in Ishikawa cell line of endometrial cancer and the underlying mechanism through the phosphatidylinositol-3 kinase/protein kinase B(PI3K/AKT)signaling pathway.Methods Ishikawa cells were treated with different concentrations of OA(0,4,8,10,12,and 20 μmol·L-1)for 24 h-72 h,the cell viability was detected by CCK-8 assay,apoptosis was detected by flow cytometry,and the protein ex-pression levels of B-cell lymphoma-2(Bcl-2),Bcl-2-associated X protein(Bax),PI3K/AKT,recombinant cytochrome P450 1B1(CYP1B1),and catechol-O-methyltransferase(COMT)were detected by Western blot technique.Results OA inhibited the prolifera-tion of Ishikawa cells in a concentration-and time-de-pendent manner.Compared with the blank control group,the expression of Bax protein increased signifi-cantly,while the expression of Bcl-2 protein decreased significantly with the increase of OA concentration.The expression of COMT protein increased significant-ly,while the expression of CYP1B1 protein decreased significantly.PI3K/AKT:IGF-1(PI3 K agonist)sup-plementation reversed the effect,the expression of COMT protein significantly decreased,and the expres-sion of CYP1B1 protein significantly increased.Con-clusions OA exerts anti-tumor effects in Ishikawa cells of endometrial cancer,which may be related to cell apoptosis mediated by the inhibition of the PI3K/AKT signaling pathway.
10.Changes in Serum NFL,S100 β,and CRP Levels in Patients with Cognitive Impairment after Cardiac Surgery and Their Correlation with Prognosis

Objective:To analyze the changes in serum levels of neurofilament light chain(NFL),binding protein S100 β(S100 β),and C-reactive protein(CRP)in patients with cognitive impairment after cardiac surgery,and to analyze the correlation between these indicators and patient prognosis.Methods:124 patients with heart disease who underwent surgical treatment from January 2022 to January 2025 were selected.One week after surgery,the patients were evaluated using the Montreal Cognitive Assessment Scale(MoCA).60 patients with MoCA scores<26 were divided into the cognitive impairment group,and 64 patients with MoCA scores ≥ 26 were divided into the non cognitive impairment group.Compare the general information of two groups,including changes in serum NFL,S100 β,and CRP levels before and after surgery.Using logistic regression to analyze the influencing factors of cognitive impairment after cardiac surgery.Following a 4-month postoperative follow-up,the cognitive impairment group was assessed using the MoCA score.Patients with a MoCA score<26 were categorized as having a poor prognosis,while the remaining patients were classified as having a good prognosis.The NfL,S100β,and CRP levels in both groups were compared,and the ROC curve was used to evaluate the predictive value of serum NFL,S100β,and CRP for poor prognosis in patients with postoperative cognitive impairment.Results:There was no significant difference in gender,BMI,education level,comorbidities(hypertension,diabetes),disease type,ASA classification,intraoperative blood loss,intraoperative fluid replacement,and cardiopulmonary bypass time between cognitive impairment group and non-cognitive impairment group(P>0.05).The cognitive impairment group had a higher incidence of age,anesthesia maintenance time during surgery,no analgesia after operation and agitation during awakening than the non-cognitive impairment group(P<0.05);There was no significant difference in the levels of serum NFL,S100β,and CRP between the two groups of patients before surgery(P>0.05).On postoperative day 3,the levels of serum NFL,S100β,and CRP increased in both groups,with the cognitive impairment group being higher than the non cognitive impairment group(P<0.05);Using cognitive impairment(occurrence=1,no occurrence=0)as the dependent variable,logistic regression analysis showed that age,intraoperative anesthesia maintenance time,agitation during the recovery period,postoperative 3-day NFL,S100 β,CRP were independent influencing factors of cognitive impairment after cardiac surgery(P<0.05);The serum levels of NFL,S100β,and CRP in the good prognosis group and poor prognosis group were higher than those before surgery,and the NFL,S100 β,and CRP levels in the poor prognosis group were higher than those in the good prognosis group before and after surgery(P<0.05);Using poor prognosis as positive samples and good prognosis as negative samples,a ROC curve was plotted.The results showed that the sensitivity and specificity of the combined prediction of three indicators before and after surgery for poor prognosis of cognitive impairment in cardiac surgery were higher than those of a single indicator(P<0.05).Conclusion:The serum levels of NFL,S100β,and CRP are significantly elevated in patients with cognitive impairment after cardiac surgery,and the early postoperative elevation of NFL,S100β,and CRP is a contributing factor to the occurrence of cognitive impairment.Preoperative and early postoperative detection of these indicators can predict the prognosis of patients with cognitive impairment after cardiac surgery.
