Background/Aims:
Lenvatinib resistance remains a critical barrier in advanced hepatocellular carcinoma (HCC) therapy. However, the underlying mechanisms and strategies for reversing resistance remain incompletely understood.
Methods:
Integrated transcriptomics of lenvatinib-resistant patient tumors and an acquired-resistance murine model identified a novel macrophage subpopulation. Functional validation employed CRISPR-SAM screening, conditioned medium (CM) assays, subcutaneous/orthotopic xenografts, patient-derived organoids (PDOs), and patient-derived xenografts (PDXs). Mechanistic studies included ChIP-qPCR, co-immunoprecipitation, and pharmacologic targeting. Clinical relevance was assessed in a retrospective cohort.
Results:
Resistant HCC exhibited significant enrichment of a COLEC12high TAM subset , which correlated with poor survival and treatment response. These TAMs secreted neuregulin-1 (NRG1) , activating HER2/HER3-AKT signaling in tumor cells to drive cancer stemness and lenvatinib resistance. Mechanistically, in TAMs COLEC12 sequestered STAT1 in the cytoplasm, preventing its phosphorylation, and thereby derepressing STAT3-mediated NRG1 transcription. Depletion of NRG1 reversed the stemness phenotypes and resensitized tumors to lenvatinib both in vitro and in vivo. Clinically, high NRG1 expression predicted an inferior lenvatinib response and shorter survival. Crucially, the bispecific anti-HER2/HER3 antibody zenocutuzumab restored lenvatinib efficacy in PDOs, PDXs, and murine models.
Conclusions
Our work establishes the COLEC12high TAM/NRG1 axis as a master regulator of therapeutic resistance and identifies NRG1 as a predictive biomarker, providing a clinically actionable strategy to overcome lenvatinib resistance in HCC.

BACKGROUND:The highest incidence of spinal fracture is in the thoracolumbar segment, and its symptoms are back pain, posterior convexity deformity, activity limitation,or with spinal cord nerve injury causing lower limb pain, numbness, and even paraplegia and other complications. The finite element method is a digital computer modeling technique, which can simulate the physical model and carry out force analysis realistically.OBJECTIVE:To review the application of finite element method in thoracolumbar spine fractures.METHODS:We searched the Chinese and English literature databases PubMed,Web of Science, and CNKI for relevant literature on the application of the finite element analysis method in spinal thoracolumbar fracture published before March 2024.The search terms in Chinese and English were:finite element analysis methods, biomechanical phenomena, stress analysis, thoracolumbar fractures, spinal fractures.Finally,55 papers were included.RESULTS AND CONCLUSION:(1)The exploration of thoracolumbar fractures caused by different etiologies(osteoporotic, traumatic, and pathological)through the finite element method is conducive to a deeper understanding of the biomechanics of various types of thoracolumbar fractures, and to improve the individualized and fine-tuned treatment of thoracolumbar fractures.(2)The finite element analysis of a single sample or a small number of samples has the chance, and a larger number of samples are required for the future finite element analysis to reduce the chance caused by the sample.(3)The rigid structure of bones alone cannot meet the biomechanical working conditions of the integrity of the physical object, and future finite element models need to incorporate all the structures of the physical object(e.g.,soft tissues, such as muscles and ligaments)as far as possible.(4)The finite element method has been used in more studies on osteoporotic and traumatic thoracolumbar spine fractures, which will need to be more in-depth in the future, and less in the field of pathologic thoracolumbar fractures, which has a wider scope for future research.

Objective:To explore a factor analysis method for job training needs based on "P4 medicine", and to provide evidence-based support for optimizing continuing education systems for medical staff in military rest homes and sanatoriums. Methods:A cross-sectional study was conducted with 95 medical staff from military rest homes and sanatoriums within the support system who participated in job training at Xinqiao Hospital. A structured questionnaire was developed based on the "P4 medicine" framework. After two rounds of Delphi expert consultation, 46 core indicators were selected. A principal component analysis was used to extract common factors, and a model of hierarchical needs was constructed by combining varimax rotation and entropy weight methods. Results:Four common factors were extracted, accounting for 81.564% cumulative variance. The first factor "dynamic updating of geriatric medical knowledge" (31.83%) covered clinical core competencies in managing geriatric comorbidities and recognizing critical values. The second factor "emergency responsiveness and personalized care" (26.05%) focused on military medical regulations, emergency treatment protocols, and tailored interventions. The third factor "multidisciplinary collaboration and leadership development" (12.41%) emphasized team reorganization in combat-ready scenarios. The fourth factor "integration of intelligent technologies" (11.29%) reflected data-driven decision-making needs. Entropy weight analysis highlighted dynamic medical knowledge updates (e.g., emergency skills with a weight of 0.050) and AI applications with a weight of 0.019 among the top 10% high-weight indicators. Conclusions:The "P4 medicine" effectively delineates the multidimensional training needs of grassroots medical personnel. Its four-dimensional structure (predictive, normative, collaborative, and innovative) provides a theoretical framework for curriculum design. We recommend constructing tiered training modules prioritizing technology-enabled mechanisms to enhance the precision and sustainability of geriatric healthcare services.

Objective:To evaluate the effects of a reduced-positioning restrictions nursing program on postoperative recovery, sleep quality, self-care ability, and hip joint function in patients undergoing total hip arthroplasty (THA), providing a reference for postoperative positioning management. Methods:A randomized controlled trial was conducted, and 140 THA patients admitted to Zhejiang Provincial People′s Hospital from January 2022 to June 2023 were selected using convenience sampling. They were divided into a control group (70 cases) and an observation group (70 cases) using a random number table. The control group received conventional rehabilitation care, while the observation group received a reduced-positioning restrictions nursing program in addition to the conventional care. The incidence of dislocation at 3 months postoperatively, the activities of daily living, the hip joint function recovery at 1 month and 3 months postoperatively, the sleep quality at 1 month postoperatively were compared between the two groups. Results:Finally 63 cases in the control group and 64 cases in the observation group completed the study. The control group included 25 males and 38 females, with an average age of (65.44 ± 10.97) years, while the observation group included 28 males and 36 females, with an average age of (65.13 ± 12.18) years. At 3 months postoperatively, there was no significant difference in the dislocation rate between the control group and the observation group ( P>0.05). At 1 month and 3 months postoperatively, the scores of activities of daily living in the observation group were (77.89 ± 6.71) and (93.52 ± 6.59) points respectively, which were higher than those in the control group [(69.68 ± 5.53) and (87.38 ± 7.72) points], the differences were statistically significant ( t=5.38, 6.96, both P<0.05), the time, interaction and inter-group effects were all statistically significant ( F=33.93, 10.81, 876.91, all P<0.05). At 1 month postoperatively, the excellent and good rate of hip joint function score grade in the observation group was 73.44% (47/64), which was higher than 41.27% (26/63) in the control group, the difference was statistically significant ( Z=-3.67, P<0.05), there was no statistically significant difference between the two groups at 3 months postoperatively ( P>0.05). At 1 month after the operation, the excellent and good rate of sleep quality score grades in the observation group was 81.25% (52/64), which was higher than 57.14% (36/63) in the control group, and the difference was statistically significant ( Z=-3.00, P<0.05). Conclusions:Incorporating a reduced-positioning restrictions nursing program into the perioperative management of THA does not increase the risk of prosthesis dislocation. This approach enhances patient comfort, improves sleep quality, increases self-care ability, and supports the recovery of hip joint function.
