1.Endovascular Treatment for Acute Posterior Circulation Tandem Lesions: Insights From the BASILAR and PERSIST Registries

Background:
and Purpose Limited evidence exists on the effectiveness of endovascular treatment (EVT) for acute posterior circulation tandem lesion (PCTL). This study aimed to explore the role of extracranial vertebral artery (VA) stenting in patients with PCTL stroke undergoing EVT.
Methods:
Individual patient data were pooled from the BASILAR (EVT for Acute Basilar Artery Occlusion Study) and PERSIST (Posterior Circulation Ischemic Stroke) registries. Patients with PCTLs who underwent EVT were included in the present cohort and divided into the stenting and nonstenting groups based on the placement of extracranial VA stents. The primary efficacy outcome was the modified Rankin Scale (mRS) scores at 90 days and 1 year. Safety outcomes included 24-hour symptomatic intracranial hemorrhage (sICH) and all-cause mortality at 90 days and 1 year post-surgery.
Results:
A combined dataset of 1,320 patients with posterior circulation artery occlusion, including 263 (19.9%) with tandem lesions, of whom 217 (median age, 65 years; 82.9% male) met the inclusion criteria for the analysis. The stenting group had 84 (38.7%) patients, while the non-stenting group had 133 (61.3%). After adjustment for the potential confounders, extracranial VA stenting was associated with favorable shifts in mRS scores at both 90 days (adjusted common odds ratio [OR], 2.30; 95% confidence interval [CI], 1.23–4.28; P<0.01) and 1 year (adjusted OR [aOR], 2.04; 95% CI [1.05–3.97]; P=0.04), along with lower rate of mortality at both 90 days (aOR, 0.45; 95% CI [0.21–0.93]; P=0.01) and 1 year (aOR, 0.36; 95% CI [0.16–0.79]; P=0.01), with no significant difference in sICH incidence (aOR, 0.35; 95% CI [0.06–1.98]; P=0.24).
Conclusion
Extracranial VA stenting during EVT may improve functional outcomes and reduce mortality in patients with PCTL strokes.

Objective To investigate the recovery of plasma metabolism in asymptomatic and mild patients of coronavirus disease 2019(COVID-19)one year after recovery.Methods A total of 174 participants were recruited from the communities in Wuhan,including 80 healthy volunteers and the COVID-19 patients who had recovered for one year.According to the disease severity,the recovered COVID-19 patients were grouped as asymptomatic patients(n=80)and mild patients(n=14).The liquid chromatography mass spectrometry platform was employed to study the metabolomic characteristics of the plasma from all the participants.Results The plasma metabolites in asymptomatic patients and mild patients remained abnormal compared with those in healthy volunteers.Among the differential metabolites in asymptomatic patients and mild patients,some metabolites showed a downward trend only in mild patients,such as phosphatidylethanolamine[20∶3(5Z,8Z,11Z)/P-18∶0],sphingomyelin(d18∶1/24∶0),and cholesteryl(15∶0).The metabolic pathway involving the differential metabolites in mild patients was mainly glycerophospholipid metabolism.Conclusions Even one year after recovery,the mild COVID-19 patients still exhibit metabolic abnormalities.Hence,these patients may experience an extended period of time for recovery.

Metastatic Crohn's disease (MCD) represents one of the rare cutaneous extraintestinal manifestations in patients with Crohn's disease. The genital area, particularly the vulva in females, is the most commonly affected site in MCD. However, clinical cases of metastatic vulval Crohn's disease (MVCD) are relatively scarce, and the symptoms often lack specificity, making differential diagnosis challenging. This article aims to summarize the clinical characteristics, diagnostic approaches, and treatment modalities of MVCD, thereby providing a reference for the clinical management of this condition.
4.Study on underscreening among cervical cancer in Wuxiang County,Shanxi Province

Objective To analyze the prevalence and risk factors of underscreening among cervical cancer screening participants in Wuxiang County,Shanxi Province in 2019,providing evidence-based support for optimizing mobilization strategies.Methods Data from cervical cancer screening programs conducted between 2019 and 2024 in Wuxiang County were retrospectively collected.The follow-up screening behaviors of women screened in 2019 were analyzed,and factors associated with underscreening were identified.Results A total of 3759 women underwent cervical cancer screening in 2019.Among them,492 women(13.09%)with abnormal primary screening results requiring follow-up in 12 months,yet only 43(8.74%)completed;2154 women(57.30%)with negative liquid-based cytology testing(LCT)results needed re-screening after 3 years,701(32.54%)completed;1113 women(29.61%)with negative HPV/combined results needed re-screening after 5 years,734(65.95%)completed.Overall,2299 women(60.69%)exhibited underscreening.Multivariate analysis showed that underscreening was more likely among community residents than rural residents(OR=2.309,P=0.018),older women(OR=1.065,P<0.001),those in organized screening compared to opportunistic screening(OR=3.789,P<0.001),those undergoing LCT(OR=4.607,P<0.001)or combined screening instead of human papillomavirus testing(OR=3.624,P<0.001),and those with abnormal screening results(OR=6.859,P<0.001).Conclusion Substantial proportions of cervical cancer screening participants demonstrate poor adherence to guideline-recommended screening intervals,and particularly need to focus on older women and those with abnormal screening results.Implementation of electronic screening record systems and emphasizing knowledge of periodical screening in health education could enhance compliance with"70%screening coverage"target for cervical cancer prevention.
5.Training methods and effects of individualized eyebrow shape design before eyebrow transplantation

Objective:To explore the training method for personalized eyebrow shape design before eyebrow transplantation surgery and to evaluate its effectiveness.Methods:Thirty physicians with similar foundational knowledge in facial aesthetic design and equivalent educational backgrounds were prospectively selected from the Second Hospital of Lanzhou University and Beijing Biliansheng Medical Beauty Clinic from January to December 2023. All the physicians were right-handed. Based on prior experience in eyebrow transplantation surgery, they were divided into two groups: an observation group [ n=20; 10 males, 10 females; aged (27.8±3.9) years] without experience in personalized eyebrow transplantation surgery, and a control group [ n=10; 5 males, 5 females; aged (29.6±4.0) years] with at least 2 years of experience in personalized eyebrow transplantation surgery. All the physicians received 2 hours of theoretical instruction on personalized eyebrow shape design and 8 hours of simulated operation training. Differences in personalized eyebrow design scores between the two groups were compared before and after training. After the assessment, physicians in the observation group performed eyebrow shape design on 20 eyebrow transplantation patients with randomly assigned face shapes, and patient satisfaction was evaluated. Results:Before training, personalized eyebrow design scores were 1.16±0.29 for the observation group and 3.75±0.22 for the control group, showing a statistically significant difference ( P<0.001). After operation training, the scores were 4.51±0.11 for the observation group and 4.89±0.08 for the control group, with no statistically significant difference ( P>0.05). The difference between pre- and post-training scores within the observation group was statistically significant ( P<0.05). Physicians in the observation group designed eyebrow shapes for male and female patients with various face shapes. Immediately after surgery, 80% (16/20) of patients were very satisfied, and 20% (4/20) were satisfied. Conclusion:After training of combined theoretical lectures, simulated practice, and hands-on patient design, physicians in the observation group demonstrate improved ability in personalized eyebrow shape design and achieve high patient satisfaction.
6.Relationship Between Semi-Quantitative Parameters SUVmax and TBR of 18F-FDG PET/CT and Disease Activity of Takayasu Arteritis

Purpose To explore the relationship between semi-quantitative parameters maximum standard uptake value(SUVmax)and target-background ratio(TBR)of 18F-FDG PET/CT imaging and the activity of Takayasu arteritis.Materials and Methods A total of 110 patients with Takayasu arteritis from February 2016 to December 2022 in the First Affiliated Hospital of Zhengzhou University who underwent 18F-FDG PET/CT images were retrospectively reviewed.All Takayasu arteritis patients were divided into active(92 cases)and inactive groups(18 cases)according to Kerr score.The region of interest technique was used to measure the SUVmax of the arterial wall,and compares the SUVmax with the average SUV in the center of the inferior vena cava and the maximum SUV measured in the right lobe of the liver to obtain TBRblood pool and TBRliver.The diagnostic efficacy of SUVmax,TBRblood pool and TBRliver in Takayasu arteritis activity was compared using the receiver operating characteristic curve.The relationship between PET/CT parameters and Kerr score,erythrocyte sedimentation rate and C-reactive protein was evaluated.Results SUVmax,TBRblood pool and TBRliver in the active group were higher than those in the inactive group[3.70(3.03,5.18)vs.1.45(1.38,1.53),2.50(1.93,3.88)vs.0.91(0.79,1.16)and 1.37(1.00,1.96)vs.0.44(0.39,0.63);Z=5.510-5.900,all P<0.001],respectively.A SUVmax level of 2.65 was shown to be the best predictive cut-off value for Takayasu arteritis disease activity(sensitivity 83.7%,specificity 100%and area under the curve 0.91).A TBRblood pool level of 1.63 was shown to be the best predictive cut-off value for Takayasu arteritis disease activity(sensitivity 83.7%,specificity 94.4%and area under the curve 0.93).A TBRliver level of 0.81 was shown to be the best predictive cut-off value for Takayasu arteritis disease activity(sensitivity 87.0%,specificity 100%and area under the curve 0.94).SUVmax,TBRblood pool,TBRliver were all positively correlated with the Kerr score(r=0.88,0.89.0.90,all P<0.001),among which the correlation coefficient between TBRliver and the Kerr score was the highest.Conclusion SUVmax,TBRblood pool and TBRliver can evaluate disease activity in Takayasu arteritis patients,with TBRliver being the best.

Taxifolin (TAX) is a natural compound known for its liver protection effect, but the mechanism remains unknown. Phosphorylated proteomics analyses discovered that the phosphorylation level of NDRG1 at T328 was a key event of TAX-improved liver fibrosis. We established models with NDRG1 knockout (KO) in vivo and in vitro, demonstrating that NDRG1 KO attenuated the development of hepatocyte injury, and combining NDRG1 KO and TAX administration did not result in a reduction in protection against liver injury. Cellular thermal shift assay and surface plasma resonance analysis showed that TAX directly binds to NDRG1 rather than its upstream kinase, subsequently demonstrating that TAX regulated phosphorylation of NDRG1 at T328 through binding to its C289 site. NDRG1 T328A (phosphorylated mutation) and T328E (mimic phosphorylation) in vivo and in vitro confirmed that pNDRG1T328 exacerbates hepatocyte injury along with DNA damage, inflammatory response, and apoptosis, thereby contributing to hepatic stellate cells (HSCs) activation. In contrast, TAX can inhibit the above pathological abnormalities and block hepatocyte injury-triggered HSCs activation and fibrosis. Overall, TAX is a potent liver protection drug primarily targeting NDRG1 and inhibiting pNDRG1T328 in hepatocytes.
