1.Effect of Co-treatment Method of Stagnation of Phlegm and Blood Stasis (Danlou Tablet) on Vascular Endothelial Function in Patients with Atherosclerosis

ObjectiveTo clarify the protective effect of Danlou tablet, a representative traditional Chinese medicine of the stagnation of phlegm and blood stasis co-treatment method, on vascular endothelial function in patients with atherosclerosis (AS). MethodsA randomized controlled trial was conducted. From September 2023 to November 2023, a total of 72 patients who were diagnosed at Guang'anmen Hospital, China Academy of Chinese Medical Sciences and met the inclusion and exclusion criteria for carotid atherosclerosis (CAS) combined with coronary atherosclerotic heart disease (CHD) and stable angina pectoris (SAP) were enrolled. The patients were randomly divided into a control group (receiving conventional Western medicine treatment) and an observation group (receiving Danlou tablet combined with conventional Western medicine treatment), with 36 cases in each group. The intervention lasted for 12 weeks. The frequency of angina pectoris attacks was recorded to evaluate the clinical efficacy of Danlou tablet. Peripheral blood samples were collected from patients, and the expression levels of serum endothelial injury markers before and after treatment were detected by enzyme-linked immunosorbent assay (ELISA). The nitrate reductase method was employed to evaluate the protective effect of Danlou tablet on vascular function. The expression levels of serum inflammatory factors and lipoproteins were determined by ELISA and an automatic biochemical analyzer (dynamic timed scatter turbidimetry and enzymatic method) to assess the anti-inflammatory and lipid-regulating effects of Danlou tablet. ResultsIn terms of angina pectoris attacks, compared with that in the control group, the frequency of attacks in the observation group was reduced (P<0.05). In terms of endothelial injury markers, compared with the levels before treatment within the same group, the levels of endothelin-1 (ET-1), intercellular adhesion molecule-1 (ICAM-1), and vascular cell adhesion molecule-1 (VCAM-1) in the peripheral blood of the observation group were decreased (P<0.05), while the levels of nitric oxide (NO) and vascular endothelial growth factor (VEGF) were increased (P<0.05). Compared with the control group after treatment, the differences in ET-1, NO, ICAM-1, and VCAM-1 were significant (P<0.05). In terms of serum inflammatory factors, after treatment, the interleukin-6 (IL-6) level in the observation group was decreased significantly (P<0.05). Compared with the control group after treatment, the IL-6 level in the observation group was decreased significantly (P<0.01). In terms of serum lipoproteins, after treatment, the level of low-density lipoprotein cholesterol (LDL-C) in the observation group was decreased (P<0.05). After treatment, the level of high-density lipoprotein cholesterol (HDL-C) in the observation group was significantly higher than that in the control group (P<0.05). In terms of safety evaluation, no serious adverse events occurred in either group during the intervention period. ConclusionDanlou tablet applied to patients with CAS combined with CHD can improve endothelial function, reduce inflammatory indicators, alleviate symptoms, improve the quality of life of patients, and demonstrate good safety.

3.Effects of Qizhi Zhoufei Granules on Endoplasmic Reticulum Stress in Chronic Obstructive Pulmonary Disease Rats

Objective To investigate the effects of Qizhi Zhoufei Granules on endoplasmic reticulum stress in rats with chronic obstructive pulmonary disease(COPD);To explore its mechanism.Methods COPD rat model was induced by lipopolysaccharide tracheal instillation and smoking.Totally 60 Wistar rats were divided into control group,model group,Bufei Huoxue Capsules group and Qizhi Zhoufei Granules low-,medium-and high-dosage groups using random number table method,with 10 rats in each group.Drug gavage intervention was carried out for the treatment group since the 29th day of modeling,and normal saline was given to the control group and model group for 28 d.Lung function tests were performed,HE staining was used to detect morphology of lung tissue,TUNEL staining was used to detect the degree of apoptosis in lung tissue,RT-qPCR and Western blot were used to detect the endoplasmic reticulum stress and apoptosis related molecular mRNA and protein expression in lung tissue.Results Compared with the control group,the lung function indexes of peak inspiratory flow(PIF),peak expiratory flow(PEF)and minute volume(MV)significantly decreased,and frequency of breathing(F)significantly increased in the model group(P<0.05);the structural damage of the lung tissue was obvious,the lung injury score and apoptosis rate significantly increased(P<0.05),the expressions of glucose regulated protein 78(GRP78),protein kinase R-like ER kinase(PERK),C/EBP-homologous protein(CHOP),Caspase-3 and Caspase-9 mRNA were increased(P<0.05),the protein expressions of GRP78,p-PERK,activating transcription factor 4(ATF4),CHOP,Caspase-3 and Caspase-9 were significantly increased(P<0.05).Compared with the model group,PIF,PEF and MV significantly increased in Qizhi Zhoufei Granules medium-and high-dosage groups and Bufei Huoxue Capsules group,and F significantly decreased(P<0.05);the damage in lung tissue was improved,and the lung injury score and cell apoptosis rate significantly decreased(P<0.05),the mRNA expressions of GRP78,PERK,CHOP,Caspase-3 and Caspase-9 in lung tissue decreased(P<0.05),and the protein expressions of GRP78,p-PERK,ATF4,CHOP,Caspase-3 and Caspase-9 decreased(P<0.05).Conclusion Qizhi Zhoufei Granules can prevent cell apoptosis and excessive damage by inhibiting the expression of endoplasmic reticulum stress related factors in COPD rats,thereby promoting unfolded protein response and improving endoplasmic reticulum folding ability,constraining endoplasmic reticulum stress state,and assisting in its regulation.
4.Exploring the causal relationship between gut microbiota and gout: a Mendelian randomization study

Objective:Using Mendelian randomization analysis to investigate the unidirectional causal effects of gut microbiota on gout and serum uric acid levels.Methods:The Mendelian randomization analysis was conducted using summary statistics from genome-wide association studies (GWAS). The gut microbiota was used as the exposure factor, with gout and serum uric acid levels as the outcomes, utilizing the MiBioGen Consortium, FinnGen GWAS, and CKDGen Consortium meta-analysis databases. The analysis was performed using inverse variance weighted (IVW) method, MR-Egger, and weighted median (WM) approach. Additionally, sensitivity analysis was conducted by excluding heterogeneity and horizontal pleiotropy. Additionally, 5 microbiota taxa were associated with serum uric acid levels: phylum Actinobacteria [ OR(95% CI)=0.963(0.925, 0.992), P=0.027], family ⅩⅢ [ OR(95% CI)=0.965(0.932, 1.008), P=0.035], genus Escherichia Shigella [ OR(95% CI)=1.047(1.005,1.089), P=0.034], genus Lachnospiraceae FCS020 group [ OR(95% CI)=0.974(0.941, 1.003), P=0.028], and genus Lachnospiraceae NC2004 group [ OR(95% CI)=0.966(0.943, 0.995), P=0.018]. No abnormalities in SNPs were found in the sensitivity analysis. Conclusion:An increase in the levels of class Verrucomicrobiae, family Verrucomicrobiaceae, genus Akkermansia, and genus Escherichia Shigella is associated with an increased risk of gout or serum uric acid levels, while an increase in the levels of class Melainabacteria, family Streptococcaceae, unknown family, phylum Actinobacteria, and family ⅩⅢ is associated with a decreased risk of gout or serum uric acid levels.
5.Effects of Qizhi Zhoufei Granules on Endoplasmic Reticulum Stress in Chronic Obstructive Pulmonary Disease Rats

No abnormalities in SNPs were found in the sensitivity analysis. Conclusion:An increase in the levels of class Verrucomicrobiae, family Verrucomicrobiaceae, genus Akkermansia, and genus Escherichia Shigella is associated with an increased risk of gout or serum uric acid levels, while an increase in the levels of class Melainabacteria, family Streptococcaceae, unknown family, phylum Actinobacteria, and family ⅩⅢ is associated with a decreased risk of gout or serum uric acid levels.
7.Impact of delivery order on short-term clinical outcomes of preterm twins born before 32 weeks of gestation

Objective:To investigate the impact of delivery orders on the short-term clinical outcomes of preterm twins born before 32 weeks of gestation.Methods:A retrospective analysis was conducted on the clinical data of preterm twins born before 32 weeks of gestation who were admitted to the Neonatology Department of Women and Children's Hospital of Chongqing Medical University from January 2018 to December 2022. The twins were divided into two groups based on the order of delivery: the first-born group and the second-born group. The study analyzed the impact of delivery orders on resuscitation support measures and outcomes during the transition period in the delivery room, respiratory support modes and treatments in the neonatal intensive care unit (NICU), severe complications (hemodynamically significant patent ductus arteriosus, neonatal necrotizing entercolitis≥stage Ⅱ, early- and late-onset sepsis, retinopathy of prematurity, and intracranial hemorrhage), and mortality. Subgroup analyses were also conducted based on chorionicity and mode of delivery. Statistical analyses were performed using paired t-tests, rank-sum tests, and McNemar's tests (paired Chi-square tests). Results:(1) A total of 296 cases (148 pairs) of preterm twins born before 32 weeks of gestation were included in the study. The proportion of 1-minute Apgar scores≤7 was higher in the second-born group compared to the first-born group [27.7% (41/148) vs. 17.6% (26/148), χ2=5.94, P=0.015]; however, there were no statistically significant differences between the two groups in terms of the proportion of umbilical artery blood gas pH<7.2, 5-minute Apgar scores, intubation, use of pulmonary surfactant (PS) in the delivery room, chest compressions, birth weight, and/or adrenaline use. There were also no statistically significant differences in 1-minute Apgar scores≤7 between the first-born and second-born groups when analyzed by chorionicity and mode of delivery. (2) There was no statistically significant difference in the overall incidence of neonatal respiratory distress syndrome (NRDS) between the first-born and second-born groups, but the incidence of NRDS stages Ⅲ-Ⅳ was higher in the second-born group compared to the first-born group [27.0% (40/148) vs. 16.9% (25/148), χ2=5.94, P=0.015]. There were no statistically significant differences between the two groups in terms of the proportion of single and multiple doses of PS use, the proportion and total duration of mechanical ventilation, and the incidence of bronchopulmonary dysplasia. Among dichorionic twins, the incidence of respiratory failure and NRDS stages Ⅲ-Ⅳ was higher in the second-born group compared to the first-born group [87.4% (104/119) vs. 79.8% (95/119), paired Chi-square test, P=0.035; 27.7% (33/119) vs. 17.6% (21/119), χ2=4.03, P=0.045]. The incidence of NRDS stages Ⅲ-Ⅳ was higher in the second-born group compared to the first-born group during vaginal delivery and cesarean section [51.6% (16/31) vs. 22.6% (7/31), paired Chi-square test, P=0.012; 20.5% (24/117) vs. 15.4% (18/117), χ2=56.14, P<0.001]. (3) There were no statistically significant differences in the mortality and the incidence of complications between the first-born and second-born groups. When analyzed by different chorionicity and delivery methods, the differences in the mortality and incidence of the complications between the different birth order groups were also not statistically significant. Conclusions:Among preterm twins born before 32 weeks of gestation, the proportion of second-born infants with a 1-minute Apgar score≤7 and the incidence of NRDS stages Ⅲ-Ⅳ were higher. However, with active resuscitation, management during the transition period in the delivery room, and respiratory support treatment, the birth order did not significantly affect the short-term incidence of severe complications and mortality in the second-born infants.
8.Advances of OTSC in managing anastomotic leak and fistulas after colorectal cancer surgery

Anastomotic leak(AL)is one of the severe complications following colorectal cancer surgery,signifi-cantly impacting the patient prognosis and potentially endangering their life.Current treatments for AL involve colos-tomy diversion surgery or long-term total parenteral nutrition with abdominal drainage,supplemented by other con-servative measures.However,the overall effectiveness of these treatments remains unsatisfactory.In recent years,endoscopic techniques have emerged as a promising therapeutic option for the treatment of AL.Among these,a no-vel rake-shaped closure system,known as the over-the-scope clip(OTSC),has been recognized by clinicians for its simple operation,few complications,high technical success rate and high clinical success rate.

Objective To investigate the characteristics of body fluid metabolic profile in patients with kidney stones based on surface-enhanced Raman spectroscopy,and to explore its application value and provide a reference for the screening of patients with kidney stones.Methods A total of 25 patients with kidney stones and 25 healthy controls were involved.Urine and blood samples were collected,whose spectra were measured with surface-enhanced Raman spectroscopy.The mean and difference spectra were plotted with origin software.The normalized data were processed with principal component analysis combined with linear discriminant analysis(PCA-LDA).Finally,the performance of the PCA-LDA method was evaluated with the receiver operating characteristic(ROC)curve.Results The levels of phosphatidylinositol,phenylalanine,palmitic acid/fatty acids,etc.in the urine of patients with kidney stones are higher than those in healthy controls,while the levels of components such as uracil and glycogen are lower.The content of methyl bands in the plasma of patients with kidney stones is higher than that of healthy controls,while the contents of glycogen,phosphatidylinositol,protein-tyrosine,phenylalanine,palmitic acid/fatty acid,hydroxyproline/tyrosine,and lipids are lower than those of healthy controls.Conclusion Surface-enhanced Raman spectroscopy can identify the changes in various metabolites in patients with kidney stones,and the combination of PCA-LDA and ROC analysis is helpful for the screening of patients.
10.Diagnostic value of endorectal ultrasound in the rectal neuroendocrine neoplasm

Objective:To investigate the endorectal ultrasound findings in rectal neuroendocrine neoplasms (R-NEN) and to compare the diagnostic performance of ERUS and MRI for T staging of R-NENs.Methods:The imaging features of 77 confirmed R-NEN cases with different pathological grades in the Sixth Affiliated Hospital, Sun Yat-sen University from August 2015 to August 2021 were retrospectively analyzed and the abilities of ERUS and MRI for T staging of R-NENs were compared.Results:A total of 77 R-NEN patients underwent preoperative ERUS examinations and detected lesions in 62 patients with a detection rate of 80.52%. Among them, 30 cases underwent simultaneous MRI examinations, and detected lesions in 25 cases with a detection rate of 83.33%, without statistical difference between MRI and ERUS ( P>0.05). R-NEN exhibited hypoechoic nodules with rich blood flow in the submucosa on ERUS. Grade G1 and G2 tumors generally had sizes less than 10 mm, clear boundaries, and regular shapes, while G3 was typically large, irregular, poorly defined, and more likely to invade the musculi propria and serous layer. G3 demonstrated a more profound infiltration level, a less defined border, and a larger diameter than G1 and G2, with statistically significant differences (all P<0.05). For T staging, the accuracy of ERUS was 86.67%, and that of MRI was 94.67%, with no statistical difference ( P>0.05). Conclusions:ERUS is effective for detecting R-NEN lesions and useful for tumor grading with comparable performance to MRI, and should be recommended for preoperative evaluation of neuroendocrine tumors.
