1.Epidemiological characteristics and spatial-temporal clustering of varicella in Changchun City from 2020 to 2024

Objective:
To investigate the epidemiological characteristics and spatial-temporal clustering of varicella in Changchun City from 2020 to 2024, so as to provide the evidence for formulating local varicella prevention and control measures.
Methods:
The individual case data of varicella were collected through the Surveillance and Reporting Management System of the Chinese Disease Prevention and Control Information System in Changchun City from 2020 to 2024. Descriptive epidemiological methods were used to analyze the population ,regional, and temporal distribution. Spatial autocorrelation and spatio-temporal scanning analyses were used to identify the spatial-temporal clustering characteristics.
Results:
A total of 8 850 varicella cases were reported in Changchun City from 2020 to 2024, with an average annual incidence of 19.64/105. There were 4 929 male cases and 3 921 female cases, with a male-to-female ratio of 1.26∶1. The age was mainly 0-<20 years (6 649 cases, 75.13%), and students were the predominant occupation (6 036 cases, 68.20%). The top three counties (cities, districts) with the highest number of cases were Chaoyang District (1 944 cases), Gongzhuling City (1 054 cases) and Nanguan District (987 cases), accounting for 45.03%. The peak incidence periods were from April to June and from October to December, with 2 166 and 4 226 cases, accounting for 24.47% and 47.75%, respectively. Spatial autocorrelation analysis showed that spatial clustering existed from 2020 to 2024. The high-high clustering areas were mainly some townships (streets) in Chaoyang District, Nanguan District, Changchun New District and Jingyue District. Spatio-temporal scanning analysis identified 6 high-risk clustering areas. The class Ⅰ clustering area was Nanhu Street in Chaoyang District, with the clustering period from September 2020 to February 2022.
Conclusions
Varicella cases in Changchun City were mainly males and students aged 0-<20 years from 2020 to 2024. The peak incidence was mainly in winter. Chaoyang District was a high-risk area, with obvious spatial-temporal clustering.
2.Analysis of health effects of occupational exposure to Dimethylformamide

Objective:To analyze the abnormal results of health surveillance for workers exposed to dimethylformamide (DMF), and to provide reference for the formulation of relevant policies and standards.Methods:In April 2024, 11, 224 workers who participated in occupational health examinations in Jiangsu Province in 2023 were selected as the research subjects. Among them, 5, 615 people exposed to DMF were taken as the exposure group, and 5, 609 people not exposed to hepatotoxic chemicals were taken as the control group. By inquiring about related symptoms and combining with the occupational health examination of workers, the abnormal rates of blood pressure, blood routine, urine routine, electrocardiogram, liver function and B-ultrasonography of the survey subjects were statistically analyzed.Results:Compared with the control group, the abnormal detection rates of indicators such as blood pressure, blood routine, urine routine, and electrocardiogram were higher, and the difference was statistically significant P<0.05). The abnormal rates of various liver function indicators in the DMF exposure group were all higher than those in the control group. Among them, the abnormal rates of alanine aminotransferase, total protein, total bilirubin, and liver B-ultrasound in the exposure group were all higher than those in the control group, and the differences were statistically significant ( χ2=34.88, 42.49, 43.07, 55.28, P<0.001). Compared with the control group, the detected values of alanine aminotransferase, total protein and total bilirubin in the DMF exposure group were higher, and the difference was statistically significant ( F=5.367, 29.543, 37.766, P<0.05). In the DMF exposure group, the abnormal rates of liver function, electrocardiogram and liver B-ultrasound in men were much higher than those in women, and the differences were statistically significant ( χ2=185.05, 10.06, 141.94, P<0.001, P=0.002, P<0.001). Grouped by length of service, the abnormal detection rate of liver function was similar in each length of service segment, and the difference was not statistically significant ( χ2=0.34, P>0.05). The abnormal detection rate of electrocardiogram increased with the increase of length of service, among which it was the highest in the 10-20 years of service, and the difference was statistically significant ( χ2=7.26, P=0.026). The abnormal rate detected by liver B-ultrasound significantly increased with the increase of working years, and was the highest in the section of ≥20 years of working years. The difference was statistically significant ( χ2=44.15, P<0.001) . Conclusion:Occupational exposure to DMF can affect the health of workers, especially increasing the detection rate of abnormal liver function and liver B-ultrasound. It is very important to strengthen the occupational health monitoring of personnel exposed to DMF, improve the working environment, and pay attention to the chronic liver damage caused by DMF occupational exposure to workers.
3.Analysis of hotspots and trends in traditional Chinese medicine treatment of neurogenic bladder based on bibliometrics and knowledge graph

Objective:To analyze the current research hotspots and trends of traditional Chinese medicine (TCM) treatment for neurogenic bladder (NB).Methods:The Chinese and English articles on TCM treatment of neurogenic bladder were searched in CNKI, Wanfang Database, PubMed, and Web of Science from the inception to May 31, 2024, using the terms "neurogenic bladder" "intervention" "treatment" "clinical" "Chinese medicine" "electroacupuncture" "acupuncture", and "moxibustion". VOSviewer and Citespace bibliometric software were used to analyze the publication trend, authors, research institutions, source journals and keywords of these articles.Results:A total of 776 Chinese articles and 253 English articles on the diagnosis and treatment of NB by traditional Chinese medicine were retrieved, the number of publications was increasing every year. Most Chinese papers came from Shandong University of Traditional Clinese Medicine, and most English papers came from Sun Yat-sen University. Some authors and institutions had formed networks of cooperation. Most papers were published in the journal of Traditional Chineses Medicine Clinical Research (in Chinese) and Neural Regeneration Research (in English). This study generated 244 Chinese core key words with 14 clustering networks, and 233 English core key words with 10 clustering networks. The main symptoms of NB are uroschesis and urinary incontinence. NB are primarily caused by spinal cord injury, diabetes mellitus and stroke. The main treatment methods of TCM for NB are electroacupuncture, acupuncture and percutaneous acupoint electrical stimulation. The research on NB mechanisms focuses on the apoptosis, regeneration and plasticity of spinal neurons, the activation of the bladder autophagy signaling pathway, the expression of proteins related to the contractile function of the forced muscles. Conclusion:The research quantity and quality of traditional Chinese medicine in diagnosis and treatment NB have increased in recent years, and the mechanism and treatment of NB are the research hotspots; however, the extension and depth of researches are limited, and the institutional cooperations are insufficiente.
4.Effects of Moxibustion at "Guanyuan (CV 4)" on Oxidative Stress and Autophagy-Related Gene Expression of Skin Tissue in Photoaging Model Rats

ObjectiveTo explore the potential mechanism of moxibustion at Guanyuan (CV 4) in delaying skin photoaging. MethodsThirty-two male Wistar rats were randomly divided into four groups, namely blank group, model group, vitamin E group, and moxibustion group, with 8 rats in each group. Except for the blank group, dorsal skin of rats were exposed to ultraviolet (UV) radiation to establish a skin photoaging model. One week after modeling, the moxibustion group received moxibustion at "Guanyuan (CV 4)" once a day, five days per week; the vitamin E group received vitamin E (25 mg/kg·d) once a day by gavage, five days per week; the blank group, model group, and moxibustion group received an equivalent volume of normal saline via gavage; the intervention lasted for 7 weeks. After 7 weeks, dorsal skin tissues were collected to analyze the following indicators, such as skin tissue moisture content, histomorphological changes using hematoxylin-eosin (HE) staining, Collagen Ⅰ and collagen Ⅲ content using ELISA. Malondialdehyde (MDA), glutathione peroxidase (GSH-Px), superoxide dismutase (SOD), hydrogen peroxide (H2O2), and catalase (CAT) activity in skin tissue were dectected. Western Blot was used to determin autophagy-related proteins, including microtubule-associated protein 1A/1B-light chain 3 (LC3), polyubiquitin-binding protein (p62), and autophagy-specific gene (Beclin-1); LC3, p62, and Beclin-1 mRNA expression was detected via qRT-PCR, and autophagosome formation was observed using transmission electron microscopy (TEM). ResultsHE staining showed that the epidermal structure in the blank group was orderly and evenly thick, while the model group exhibited uneven epidermal thickness. In the moxibustion group, the epidermis was well-structured, smooth, and uniform, with densely arranged dermal layers; the epidermis in the vitamin E group was thicker than that in the model group. Compared with the blank group, the model group exhibited decreased skin moisture content and reduced level of Collagen Ⅰ and collagen Ⅲ, reduced SOD, CAT, and GSH-Px activity in skin tissue, increased H2O2 and MDA activity, elevated p62 protein and mRNA expression, reduced LC3 and Beclin-1 protein and mRNA expression (P<0.05 or P<0.01). Compared with the model group, the moxibustion group showed significant improvement in all these indicators (P<0.05 or P<0.01); whereas the vitamin E group did not show a statistically significant difference in Collagen Ⅰ and collagen Ⅲ levels (P>0.05). TEM results showed that, compared with the blank group, the model group had atrophic skin cells, extensive mitochondrial vacuolization, and degraded cellular structures; the moxibustion group exhibited crescent- or cup-shaped autophagosomes with a significantly increased number of autophagosomes per unit area, whereas the vitamin E group showed less improvement than the moxibustion group. ConclusionMoxibustion at "Guanyuan (CV 4)" may alleviate skin photoaging by regulating oxidative stress imba-lance, modulating cellular autophagy, and promoting collagen synthesis, thereby slowing the aging process of the skin.
5.Effect of different doses of agomelatine on liver function in patients with depressive disorders: a real-world study

BackgroundAgomelatine, a novel antidepressant with dual efficacy in mood improvement and sleep regulation, has been widely utilized in clinical treatment of depressive disorders. The association between agomelatine and hepatic dysfunction has garnered increasing attention, yet there remains limited research on its long-term effect of liver function in real clinical scenarios. ObjectiveTo investigate the effect of different doses of agomelatine on liver function in patients with depressive disorders in real clinical scenarios, and to ascertain its safety profile and efficacy differences. MethodsA retrospective study was conducted, enrolling 200 patients diagnosed with depressive disorders according to the International Classification of Diseases, tenth edition (ICD-10), who received agomelatine treatment at the Department of Psychiatry of Chaohu Hospital Affiliated to Anhui Medical University from January 2019 to December 2024. Longitudinal follow-up was performed based on real-world data. Patients were divided into a low-dose group (25 mg/d) (n=121) and a high-dose group (50 mg/d) (n=79) based on their agomelatine dosage. Follow-up assessments were conducted at baseline, the 2nd, 6th, 14th, and 26th weeks after treatment initiation. Liver function indicators, including alanine aminotransferase (ALT), aspartate aminotransferase (AST), and total bilirubin (TBIL), were measured using a fully automated biochemical analyzer. Clinical symptoms were evaluated using the Hamilton Depression Scale-24 item (HAMD-24) and the Hamilton Anxiety Scale (HAMA). ResultsNo statistically significant time effect, intergroup effect, or time-by-group interaction effect was observed for ALT, AST, or TBIL in either the low-dose or high-dose group (P>0.05). The time effects for both HAMD-24 and HAMA scores in the two groups were statistically significant (Ftime=430.573, 395.737, P<0.01). From the end of the 2nd week of treatment onward, the scores at each follow-up time point were significantly lower than those at the baseline period (P<0.01). ConclusionBoth low-dose and high-dose agomelatine may have no significant effect on liver function in patients with depressive disorders, with no difference in liver function impairment was observed between dosage groups. Low-dose and high-dose agomelatine may be equally effective in alleviating depressive and anxiety symptoms in patients with depressive disorders. 6.Effect of enteral nutrition formula containing slow-release starch on blood glucose variability and prognosis in patients with severe acute pancreatitis

Objective To explore the impact of enteral nutrition formula containing slow-release starch on blood glucose variability and prognosis in patients with severe acute pancreatitis(SAP).Methods A total of 204 SAP patients were enrolled and randomly divided into control group and ob-servation group using a random number table method,with 102 patients in each group.The control group received early enteral nutrition support with a standard enteral nutrition formula,while the ob-servation group received early enteral nutrition support with an enteral nutrition formula containing slow-release starch.Blood glucose variability indicators[largest amplitude of glycemic excursions(LAGE),standard deviation of blood glucose(SDBG),blood glucose coefficient of variation(BGCV),mean blood glucose(MBG),mean amplitude of glycemic excursions(MAGE),and time in range(TIR)]were compared between the two groups after treatment,along with clinical indicators during hospitalization,inflammatory markers[procalcitonin,C-reactive protein,interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)],and nutritional indicators(albumin,prealbumin)levels.Kaplan-Meier analysis was conducted to assess the prognosis of the two groups,and multivariate Cox regression analysis was performed to identify factors influencing prognosis.Results After treat-ment,the observation group exhibited lower levels of MBG,LAGE,SDBG,BGCV,MAGE,and a higher TIR compared with the control group(P<0.05).During hospitalization,the observation group had a shorter duration of enteral nutritiontherapy,lower insulin usage,and lower incidence rates of multiple organ failure and infectious pancreatic necrosis compared with the control group(P<0.05).After treatment,the observation group had lower levels of procalcitonin,C-reactive protein,IL-6,and TNF-α compared with the control group(P<0.05);however,there were no statistically significant differences in albumin and prealbumin levels between the two groups(P>0.05).Kaplan-Meier analysis showed that the cumulative mortality rate in the observation group was 12.75%,which was lower than the 17.65%in the control group(Log-rank x2=4.361,P=0.037).Multivariate Cox regression analysis revealed that TIR after treatment(HR=0.920;95%CI,0.869 to 0.974)was an independent protective factor for prognosis in SAPpatients(P<0.05),while infectious pancreatic necrosis(HR=4.269;95%CI,1.922 to 9.482)was an inde-pendent risk factor for prognosis in SAP patients(P<0.05).Conclusion Enteral nutrition formu-la containing slow-release starch helps stabilize blood glucose variability,control inflammatory mark-er levels,improve nutritional status,and prognosis in SAP patients.Both TIR and infectious pancre-atic necrosis are closely related to the prognosis of SAP patients.
7.Epidemiological and clinical characteristics of pertussis identified through active surveillance

Objective:To analyze the epidemiological and clinical characteristics of pertussis cases identified through active surveillance.Methods:Active surveillance for pertussis was conducted in three sentinel hospitals in Yiwu, Zhejiang Province, and Yongcheng, Henan Province. The study population included cases that met the surveillance case definition and sought medical care at outpatient/emergency departments or were hospitalized between June 1, 2021, and May 31, 2022. Samples were collected for bacterial culture and PCR detection. Case information and clinical data were collected. Differences in rates were assessed using the chi-square test or Fisher's exact probability test, and the differences in cough time were compared using the Mann-Whitney U test. Results:Among 1 423 cases of pertussis surveillance, the positive rate of pertussis was 28.11% (400/1 423), with a median age of 5 years (interquartile range: 2, 8). The positive rate in Yongcheng, Henan Province, and Yiwu, Zhejiang Province were 39.27% (216/550) and 21.08% (184/873), respectively; the positive rate of pertussis was highest in July 2021, and the highest positive rate of pertussis was among those aged 10-14. The positive rate of pertussis in hospitalized cases was higher than in outpatient/emergency cases (26.68%) ( χ2=4.16, P=0.041). Among the 400 laboratory test-positive cases, the highest proportion of atypical symptom cases was in adults aged 20-59 (43.33%, 13/30). The specificity rates of apnea and worsening nocturnal cough in monitored cases under 3 months of age were 100.00% and 73.81%, respectively. Among monitored cases aged 3 months to 9 years, the proportions of symptoms including worsening nighttime cough (63.00%) and night sweats (4.59%) in test-positive cases were significantly higher than those in the test-negative group (47.77% and 0.56%, respectively), with statistically significant differences (both P<0.05). The specificity rates of worsened nighttime coughing and night sweats were 52.23% and 99.44%, respectively. Conclusions:The active surveillance results for pertussis showed that the 10-14 age group exhibited the highest positivity rate. Active surveillance enhanced the detection rate of pertussis. Among laboratory-confirmed cases, the proportion of atypical symptoms was the highest in adults, suggesting that laboratory testing should be combined to diagnose programs of pertussis. For infants under 3 months, worsening nighttime cough and apnea increase the diagnostic specificity, while for individuals aged 3 to 9 years old, worsening nighttime cough and night sweats increase the diagnostic specificity.
8.Establishment and validation of predictive model for postoperative pulmonary complications in patients undergoing robot-assisted laparoscopic urological surgery

Objective:To construct and validate a predictive model for postoperative pulmonary complications (PPCs) in patients undergoing robot-assisted laparoscopic urological surgery.Methods:This retrospective study included the medical records of 932 patients who underwent robot-assisted laparoscopic urological surgery at the First Affiliated Hospital of Zhejiang University School of Medicine from January 2020 to February 2022. The patients were divided into a training group ( n=559) and a validation group ( n=373) at a 6∶4 ratio. Logistic regression analysis was used to determine the independent risk factors for PPCs, and a nomogram prediction model was constructed based on these factors. The performance of the model was evaluated using the receiver operating characteristic curve and calibration curve, and the clinical benefit was assessed using the clinical decision curve analysis. Results:The independent risk factors for PPCs included advanced age (>60 yr), smoking history, respiratory tract infection within 1 month, preoperative low SpO 2 (<96%), and prolonged length of postoperative hospital stay ( P<0.05), and the body mass index (18.5-<28.0 kg/m 2) was a protective factor. The nomogram prediction model developed based on the aforementioned 6 influencing factors had an area under the receiver operating characteristic curve of 0.81 (95% confidence interval 0.76-0.86) in training group and 0.80 (95% confidence interval 0.75-0.86) in validation group. The calibration curve indicated a good consistency between the predicted and actual occurrence curves, and the clinical decision curve analysis showed good accuracy and net benefit of the prediction model. Conclusions:The predictive model for PPCs is successfully constructed based on age, low body mass index, smoking history, history of respiratory tract infection within 1 month, preoperative low SpO 2 and prolonged length of postoperative hospital stay and has good predictive performance in patients undergoing robot-assisted laparoscopic urological surgery.

10.Vertebroplasty combined with microwave ablation for treating vertebral compression fractures caused by metastatic cancer

Objective To observe the efficacy of percutaneous vertebroplasty(PVP)combined with microwave ablation(MWA)for treating vertebral compression fractures(VCF)caused by metastatic cancer.Methods Totally 112 patients with VCF caused by metastatic cancer were retrospectively enrolled,including 77 cases(105 vertebrae)underwent MWA+PVP(group A)and 35 cases(68 vertebrae)who underwent PVP alone(group B).The success rates of the treatments were recorded,and complications were observed.Visual analogue scale(VAS)score of pain,daily morphine consumption(DMC)and Oswestry disability index(ODI)of patients were compared within and between groups after treatments,and local tumor recurrence rates after treatment were observed.Results The success rates of MWA and PVP were both 100%.After treatments,bone cement leakage occurred in 12 cases(12/77,15.58%)in group A and 19 cases(19/35,54.29%)in group B,in group A was lower than in group B(P<0.001).No other complications occurred.One week and 1,3 and 6 months after treatments,VAS scores of pain,DMC and ODI in group A and B were all lower than those before treatments(all P<0.001).Three and 6 months after treatments,VAS scores of pain in group A were lower than those in group B(both P<0.01),while 6 months after treatments,DMC and ODI in group A were lower than those in group B(both P<0.01).No significant difference of VAS scores of pain,DMC nor ODI was found between groups at the other time points(all P>0.05).Six months after treatments,local tumor recurrence rate in lesions in group A(17.14%,18/105)was significantly lower than that in group B(32.35%,22/68)(P=0.020).Conclusion For treating VCF caused by metastatic cancer,PVP combined with MWA could reduce incidence of complications,prolong pain relief time and bring better short-term local tumor control effect.
