1.Pulmonary mucoepidermoid carcinoma:a clinicopathological study of 23 patients

Objective:Primary pulmonary mucoepidermoid carcinoma(PMEC)is a rare malignant lung tumor that accounts for approxim-ately 0.1%-0.2%of all primary pulmonary neoplasms.Due to the non-specific clinical symptoms and epidemiological features,PMEC poses diagnostic challenges.Methods:Tissue blocks from 23 archived PMECs were collected from The First Affiliated Hospital of Soochow Uni-versity(November 2012 to December 2023).To establish definitive diagnoses,comprehensive histopathological evaluation,including histo-morphological analysis,immunohistochemistry(IHC),fluorescence in situ hybridization(FISH),and periodic acid-Schiff(PAS)staining were performed.Results:The tumors consisted of varying proportions of mucin-secreting cells(mucous cells),intermediate cells,and epidermoid cells.Immunophenotypically,CK7 was predominantly expressed in the mucous cells,whereas CK5/6,p40,and p63 were expressed in the epidermoid and intermediate cells.The Ki-67 proliferation index ranged from 5%to 60%.All tumors were negative for TTF-1 and Napsin A.Five of the tumors were positive for PD-L1(clone 22C3),with a tumor percentage score of 3%-20%.All 11 tumors tested for ALK(clone D5F3)were negative.IHC for c-Met was performed on two tumors and both were weakly positive(+).Mastermind-like transcriptional co-activator 2(MAML2)gene rearrangement was detected in 34.8%(8/23)of the tumors.Mucous cells were PAS positive.Kaplan-Meier surviv-al analysis revealed a significantly poorer prognosis for patients with lymph node metastasis,distant metastasis,advanced TNM stage(Ⅲ+Ⅳ),poor differentiation,or MAML2 gene rearrangement negativity.Univariate analysis identified poor histological differentiation,lymph node metastasis,distant metastasis,and advanced TNM stage as the major prognostic risk factors.Multivariate analysis confirmed poor differentiation and distant metastasis as independent risk factors for adverse outcomes.Conclusions:PMEC is an aggressive tumor with low incidence and non-specific clinical manifestations,leading to frequent misdiagnosis.Clinicians should maintain a high index of suspicion and ensure a thorough differential diagnosis.

Attention impairment in temporal lobe epilepsy (TLE) leads to other advanced cognitive impairments, such as memory and emotional recognition. Moreover, the severity of attention impairment determine the differences in strategies for preventing sudden unexpected death in epilepsy (SUDEP). Factors such as abnormal brain electrical discharges, sleep, and antiepileptic drugs are all associated with attention impairment. Neuroelectrophysiological observations have shown that abnormal electrical discharges interfere with the synchronization of cortical oscillations, and preventing the spread of abnormal epileptic discharges to subcortical activation structures helps reduce attention disorders. Neuroimaging reveals damage to the gray matter and white matter of the brain related to attention and poor connectivity of the whole-brain network. Therefore, early identification of attention impairment in TLE patients has become even more important. Treatment of attention impairment includes supplementing attention-related neurotransmitters, cognitive-behavioral therapy, surgical procedures, and neuromodulation techniques. This article mainly reviews the characteristics of attention impairment in TLE patients from the perspectives of clinical behavior, pathophysiology, and treatment, aiming to provide new insights for the early identification and management of attention impairment in TLE patients.
3.Research Progress on the Treatment of Corneal Neovascularization with Small Molecule Extracts of Tradi-tional Chinese Medicine

Corneal neovascularization(CNV)is a pathological condition characterized by the invasion of new blood vessels into the normally avascular corneal area from the corneal periphery,leading to severe vision loss and potentially blindness.Currently,surgical,physical,and pharmacological therapies are the main clinical approaches for treating CNV.Surgical treatment aims to remove abnormal vascular tissue or perform corneal trans-plantation to inhibit angiogenesis;however,it carries a risk of postoperative rejection.Physical therapy involves the direct application of non-invasive modalities,such as laser treatment,to the neovascularized area to suppress vascular growth.Nevertheless,this approach may cause damage to surrounding healthy tissues.Pharmacotherapy has recently become a research hotspot in CNV treatment due to its convenient administration.Clinically,the drugs used for CNV treatment mainly include anti-inflammatory agents,anti-VEGF drugs,and immunosuppressants,which inhibit CNV progression by targeting angiogenesis-related signaling pathways.However,these drugs often lead to drug resistance and toxic side effects.Therefore,there is an urgent need to develop more effective and safer therapeutic agents for CNV.This article reviews the current clinical treatment status of CNV and highlights recent advances in the use of small molecule extracts from traditional Chinese medicine for CNV therapy,aiming to provide potential candidate drugs and a scientific theoretical basis for clinical management of CNV.

7.Progress of Clinical Studies on the Use of Sodium Valproate in Adult Patients with Epilepsy Combined with Nonalcoholic Fatty Liver Disease

Epilepsy patients are prone to obesity and metabolic syndrome(MetS),which is a higher risk than the general population.Nonalcoholic fatty liver disease(NAFLD)is a manifestation of a metabolic syndrome,characterized by hepatic steatosis,which can be exacerbated to some extent in patients with epilepsy.Sodium valproate(VPA),a commonly used clinical antiepileptic drug,leads to triglyceride accumulation in liver cells and induces a risk of NAFLD.In adults with epilepsy combined with NAFLD,treatment with VPA can make the liver,which is already affected by steatosis,more sensitive to VPA-induced liver injury and affect VPA drug metabolism.Both VPA and NAFLD induce hepatic steatosis,and can exacerbate each other's induced hepatic steatosis synergistically during epilepsy treatment.In this paper,we reviewed the progress of clinical studies on the use of VPA in adult patients with epilepsy combined with NAFLD to provide references for effective treatment and future studies of VPA.
8.Immunohistochemical expression of TRPS1 in synovial sarcoma and its significance

Purpose To investigate the expression of TRPS1 in synovial sarcoma(SS)and its significance.Methods Twenty-one cases of synovial sarcoma diagnosed by SS1 8(18q11)(SYT)fluorescence in situ hybridization(FISH)or SS18-SSX immunohistochemistry were collected.The expression of TRPS1 was detected by immunohistochemistry EnVision two-step staining,and the relevant literature was reviewed.Results There were 21 cases of synovial sarcoma,including 9 females and 12 males,aged 17-79 years,with a median age of 50 years and an average age of 47.1 years.The tumors occurred in the lung(3 cases),legs(3 cases),kidney(3 cases),feet(2 cases),psoas major muscle(2 cases),rectum(1 case),tho-racic cavity(1 case),ankle canal(1 case),inguinal area(1 case),knee(1 case),palm(1 case),nasal cavity(1 case),and forearm(1 case).The maximum diameter of the tumor was 1.2-13 cm.Grossly,the tumor section was grayish-white,grayish-yellowish solid,and the lesion with hemorrhage was dark red.Microscopically,there were monophasic fibrous SS(16/21,76.2%),poorly differential SS(small cell)(3/21,14.3%),and biphasic SS(2/21,9.5%).Immunophenotypi-cally,tumor cells expressed SS18-SSX(18/18,100%),TLE1(12/12,100%),BCL2(18/18,100%),partially expressed EMA(10/16,62.5%),CK(12/18,66.7%),CD99(4/10,40.0%),SMA(3/16,18.6%),S-100(3/19,15.8%),and desmin(0/21).The proliferation index of Ki67 ranged from 3%-80%,with an average of 39.9%.FISH test showed that all 11 cases were positive for SS18(18q11)split-ting.All 21 cases of SS expressed TRPS1 to varying degrees and intensities(21/21,100%),of which 19 cases(19/21,90.5%)had a positive range of>50%.There were 19 cases(19/21,90.5%)with a positive intensity of 2+or above.All the 21 patients underwent surgical resection of the tumor,and 20 patients received follow-up visits.3 patients treated with postop-erative combination of radiotherapy and chemotherapy,18 pa-tients treated with postoperative chemotherapy,and 7 patients showed different degrees of recurrence or metastasis,and 9 pa-tients who received follow-up visits died(45%,9/20).Con-clusion Although TRPS1 is regarded as a highly sensitive and specific marker of breast tumor origin,it is also highly expressed in synovial sarcoma,and it is necessary to be vigilant at its pit-falls in the pathological diagnosis.
9.Analysis of hot topics and frontiers on blood pressure monitoring in research of chronic kidney disease at home and abroad,2011-2022

Objective To analyse the hot topics(CKD)frontiers,and of blood pressure monitoring in research of chronic kidney disease(CKD)at home and abroad,and provide references for future research in this field.Methods Articles on blood pressure monitoring in published in Web of Science,China Knowledge Network Infrastructure(CNKI),Wanfang(Wanfang Data)and China Science and Technology Journal Database(VIP)from 2011 to 2022 were searched,and CiteSpace 5.8.R3 visual analysis software was employed to analyse the number of articles involved,country,institution,keyword co-occurrence.The analysis was performed on the number of published papers,countries,institutions,keyword co-occurrence atlas,high frequency subject terms,keyword emergence and emergence of literatures.Results ① A total of 504 articles in English and 72 articles in Chinese were extracted from the literature search.Annual distribution of the number of articles generally showed a continuous upward trend,in which 2 peaks of articles were formed in 2016 and 2018;the main country of issuance was the United States,and the main institution of issuance was the Aristotle University of Thessaloniki,Greece;② In the analysis of keyword co-occurrence,8 high-frequency keywords with a word frequency greater than or equal to 30 were identified.It indicated that the research hotspots mainly focused on the classification of hypertension,the characteristics of blood pressure circadian rhythm,the management of cardiovascular disease and the prediction of death and prognosis in the blood pressure monitoring of CKD;③ Further testing of the emergent terms and emergent literatures yielded 23 strongest emergent terms and 11 emergent literatures,which went through three stages of development,namely,early,intermediate and latest.It was found that the research gradually shifted from the application of blood pressure monitoring in the assessment and diagnosis of kidney disease to the treatment,management and prognostic assessment of hypertension in CKD Conclusion The importance of blood pressure monitoring in the management of hypertension in CKD has received increasing attention from researchers,and future researches should focus on using different blood pressure monitoring schemes to enhance the assessment of cardiovascular risks and the individualised management of hypertension.
10.Evaluation of information needs scales for breast cancer patients

Objective:To evaluate the reliability and validity of the information needs assessment scales for breast cancer patients, so as to recommend an appropriate information needs assessment scale for breast cancer patients.Methods:Researches on the information needs assessment tools for breast cancer patients were retrieved by computer in PubMed, Embase, PsycINFO, EBSCOhost, WOS Core Collection, China National Knowledge Infrastructure (CNKI) , WanFang Data, and China Biomedical Literature Database. The inclusion criteria included that the research content was the breast cancer patient information needs scale/tumor patient information needs scale and the original research sample included breast cancer patients, and outcome indicators included reliability and validity. Exclusion criteria included assessment of information needs of family members/caregivers of breast cancer patients, assessment of unmet information needs, languages other than Chinese and English, comments, reviews, qualitative studies, systematic reviews, and Meta-analyses. The measurement characteristics of the scale were evaluated using the measurement characteristics evaluation criteria in the Consensus-based Standards for the selection of health Measurement Instruments (COSMIN) evaluation manual.Results:A total of 1 293 articles were retrieved, and 13 articles were finally included. The 13 scales had no hypothesis test of construct validity, cross-cultural validity, criterion validity and reactivity index. 11 of the scales were recommended as Grade B, and 2 were recommended as Grade C. Combined with the methodological quality evaluation results and measurement characteristics evaluation results, and the consistency between the research and development population and the user population, the Psychological Needs of Cancer Patient Scale and the Comprehensive Needs Assessment Tool in cancer (CNAT) are the optimal scales.Conclusions:There are currently no recommended scales according to the COSMIN guidelines. Considering the methodological quality, the evaluation results of measurement characteristics, and the consistency between the research and development population and the user population, the Psychological Needs of Cancer Patient Scale and the CNAT are the optimal scales.
