Purpose To investigate the predictive value of MRI-based peritumoral edema(PE)of lymph node(LN)metastasis burden in patient with invasive breast cancer.Materials and Methods A total of 213 patients in Sichuan Cancer Hospital with invasive breast cancer confirmed by pathology after surgery were enrolled retrospectively from September 2017 to February 2019.Based on the status and number of LN metastases proved by axillary surgery pathology, all patients were divided into high-burden LN metastasis group(n=47 cases, total number of metastatic LN greater than 2)and low-burden LN metastasis group(n=166 cases, total number of metastatic LN less than or equal to 2).T2WI features, including PE type and PE degree, were analyzed. On DCE-MRI,MRI features of breast cancer were analyzed based on BI-RADS.The predictive value of T2WI features and MRI features for LN metastasis burden was assessed by univariate analysis, and the statistically significant features identified by univariate analysis were further subjected to multivariate Logistic regression analysis to establish a prediction model. The model's performance for LN metastasis burden prediction was evaluated via receiver operating characteristic curve, and the predictive metrics were calculated based on the Youden index. Results The rate of high-burden LN metastasis was 22.1%(47/213).The results of univariate analysis showed that PE degree(OR=18.70,P<0.001),PE type(OR=16.00,P<0.001),tumor maximum diameter(OR=1.40,P=0.025),and tumor minimum diameter(OR=2.01,P=0.003)were predictive features for high-burden LN metastasis. The multivariate regression analysis showed that the peritumoral edema features, including PE degree(OR=8.02,P<0.001)and PE type(OR=5.53,P=0.001)were independent predictive features for high-burden LN metastasis. The area under the curve of the final predictive model was 0.842.The diagnostic sensitivity was 0.766,the specificity was 0.861,the positive predictive value was 0.610,and the negative predictive value was 0.929.Conclusion The PE on preoperative MRI have good predictive performance for LN metastasis burden prediction in patient with invasive breast cancer, particularly for low-burden LN metastasis.

Objective:To develop a self-locked DNAzyme nanoprobe-based fluorescence amplification strategy for wash-free and ultrasensitive detection of breast cancer-derived small extracellular vesicles (sEV).Method:A DNAzyme self-locked probe was designed to recognize the epithelial cell adhesion molecule (EpCAM) specifically expressed on breast cancer-derived sEVs. Upon binding to EpCAM, the DNAzyme-lock structure was opened, restoring the DNAzyme cleavage activity. The activated DNAzyme then cyclically cleaved the RNA site on the substrate strand. Fluorescently labeled substrate strands were used to detect sEVs at varying concentrations, and the detection limit and linear range were determined. Results:The DNAzyme self-locked probe successfully identified breast cancer-derived sEVs and generated a fluorescent signal through cyclic cleavage. The proposed method achieved wash-free detection of sEVs, with the fluorescence intensity showing a strong linear correlation with sEV concentration ( R2=0.98). The linear detection range was 1.0×10 2-1.0×1.0 7 particles/μl, with a detection limit of 59 particles/μl. Conclusion:This study established a wash-free and highly sensitive strategy for quantifying breast cancer-derived sEVs, which provides a promising technical approach for the early diagnosis of cancer.

AIM:C57BL/6J mice were transplanted with fecal microbiota from a patient with maturity-onset diabetes of the young(MODY)or a healthy control. This study aimed to investigate the effects of fecal microbiota trans-plantation(FMT)on glucose and lipid metabolism, intestinal flora composition, and short-chain fatty acids(SCFAs)me-tabolism in mice, thereby providing an experimental foundation for understanding the role of gut microbiota in MODY.METHODS:A total of 36 male C57BL/6 mice were randomly allocated into three groups, including:control(CON)group(receiving PBS),normal individual-FMT(NF)group(intervened with normal human fecal microbiota)and MODY patient-FMT(MF)group(intervened with MODY patient fecal microbiota),and the mice were performed fecal microbio-ta,transplantation through oral gavage for 8 weeks.Glucose tolerance was assessed using an oral glucose tolerance test(OGTT),while insulin sensitivity was evaluated through an intraperitoneal insulin tolerance test(ITT).Blood biochemi-cal indicators were measured using ELISA,the composition of the gut microbiota was analyzed using 16S rRNA sequenc-ing,and the concentration of fecal short-chain fatty acids(SCFAs)was determined by gas chromatography-mass spec-trometry(GC-MS).RESULTS:The mice in MF group showed impaired glucose tolerance and reduced insulin sensitivi-ty.The FMT altered the composition of gut microbiota in mice, leading to significant differences in microbial diversity among groups.Specifically, the distribution of 18 bacterial species varied significantly among the three groups, with a no-table reduction in SCFA-producing bacteria observed in the MF group.Analyses of fecal SCFAs revealed that acetic acid levels were significantly lower in the MF group(P<0.05).CONCLUSION:The FMT reduces production of SCFAs by altering gut microbiota composition in mice, which was likely related to impaired glucose tolerance and decreased insulin sensitivity.

ObjectiveTo observe the effects of Hirudo, Notoginseng Radix et Rhizoma, and drug pair on renal pathological morphology and protein phosphatase 2A (PP2A)/adenylate activated protein kinase (AMPK)/mammalian target of rapamycin (mTOR) signal pathway in rats with chronic renal failure (CRF). MethodThe 55 male SD rats were randomly divided into a normal group (n=11) and a modeling group (n=44). The normal group was fed conventionally, and the modeling group was given 0.25 g·kg-1·d-1 adenine by gavage for 28 days to replicate the CRF model. After successful modeling, rats were randomly divided into model group, Hirudo group (3 g·kg-1·d-1), Notoginseng Radix et Rhizoma group (3 g·kg-1·d-1), and Hirudo + Notoginseng Radix et Rhizoma group (3 g·kg-1·d-1), with 9 rats in each group. The normal group and model group were given a constant volume of normal saline by intragastric administration for 30 days. At the end of the experiment, the levels of serum creatinine (SCr) and urea nitrogen (BUN) in all groups were measured. The renal pathological morphology changes were observed by hematoxylin-eosin (HE) staining, Masson staining, and electron microscopy. The mRNA expressions of PP2A, AMPK, and mTOR were detected by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). The protein expression levels of PP2A, AMPK, phosphorylation(p)-AMPK, mTOR, and p-mTOR in renal tissue were detected by Western blot. ResultCompared with the normal group, the renal pathological structure changes were obvious, and the levels of SCr and BUN were significantly increased. The mRNA expression of PP2A, protein expression of PP2A, and p-mTOR/mTOR expression were significantly increased, and the p-AMPK/AMPK was significantly decreased in the model group (P<0.05). Compared with the model group, the renal pathological morphology changes were significantly improved, and the levels of SCr and BUN were significantly decreased. The mRNA expression of PP2A, protein expression of PP2A, and p-mTOR/mTOR expression in the renal tissue were significantly decreased, and the p-AMPK/AMPK was significantly increased (P<0.05) in all groups after drug intervention. In addition, the effect in the Hirudo+Notoginseng Radix et Rhizoma group was better. The mRNA expression levels of AMPK and mTOR in the renal tissue were not significantly different among the normal group, model group, and other groups. ConclusionThe efficacy of Hirudo and Notoginseng Radix et Rhizoma pairs in improving renal fibrosis in rats with CRF is significantly better than that of the single drug, and its improvement on renal fibrosis in rats with CRF may be related to the regulation of PP2A/AMPK/mTOR signaling pathway.
