Objective:To analyze the efficacy of phantom-less quantitative computer tomography (PL-QCT) for the diagnosis of osteoporosis in patients with lumbar degenerative diseases. Methods:From October 2021 to October 2023, a total of 1 248 patients with lumbar degenerative disease who did not receive anti-osteoporosis treatment in the Department of Spine Surgery, Tianjin Hospital were retrospectively analyzed. There were 520 males and 728 females, aged 62.31±9.37 years (range, 40-87 years), height 1.66±0.08 m (range, 1.43-1.89 m), weight 69.04±8.27 kg (range, 49-93 kg). The mean body mass index was 26.11±3.67 kg/m 2 (range, 14.40-37.11 kg/m 2). Dual-energy X-ray absorptiometry (DXA) and PL-QCT were used to diagnose osteoporosis, and the detection rates of the two diagnostic methods were compared. The receiver operating characteristic (ROC) curve of PL-QCT for the diagnosis of osteoporosis was drawn, the area under the curve (AUC) and 95% confidence interval (CI), sensitivity and specificity were calculated. Results:Among 1 248 patients with lumbar degenerative diseases, 626 (50.2%) were diagnosed as osteoporosis by PL-QCT, 423(33.9%) were diagnosed by spine DXA, 488(39.1%) were diagnosed by hip DXA and 539 patients(43.2%) were diagnosed by dual-site DXA. The detection rate of osteoporosis of PL-QCT was higher than that of spine DXA (χ 2=193.557, P<0.001), hip DXA (χ 2=322.201, P<0.001) and dual-site DXA (χ 2=94.683, P<0.001), and the difference was statistically significant. Taking the diagnostic results of spinal DXA for osteoporosis as a reference, the ROC curve of PL-QCT for the diagnosis of osteoporosis showed a sensitivity of 79%, a specificity of 81%, and an AUC and 95% CI of 0.82(0.79, 0.85). Taking the diagnostic results of hip DXA for osteoporosis as a reference, the ROC curve of PL-QCT for the diagnosis of osteoporosis showed a sensitivity of 85%, a specificity of 55%, and an AUC and 95% CI of 0.75(0.71, 0.78). Taking the diagnostic results of two-site DXA for osteoporosis as a reference, the ROC curve of PL-QCT for the diagnosis of osteoporosis showed a sensitivity of 72%, a specificity of 75%, and an AUC and 95% CI of 0.81(0.78, 0.83). Conclusion:Compared with DXA, PL-QCT has a higher detection rate of osteoporosis in patients with degenerative lumbar spine disease and good diagnostic efficacy.

Objective:To evaluate the value of HALP index on postoperative survival of patients with distal cholangiocarcinoma. Methods:A retrospective analysis was performed on 165 patients with distal cholangiocarcinoma who underwent surgery at Beijing Chaoyang Hospital, Capital Medical University from Jan 2011 to Dec 2022. Hemoglobin, albumin, lymphocyte count, and platelet count were recorded within one week before surgery, and hemoglobin×albumin×lymphocyte count/platelet count (HALP) was calculated. The receiver operating characteristic curve was drawn to determine the optimal cutoff value for predicting postoperative survival. According to the cutoff value, the patients were divided into high HALP group and low HALP group, and the prognosis of different groups was compared. Multivariate COX regression analysis was used to construct a prediction model for variables with statistically significant differences. Results:The median overall survival (OS) for the entire cohort was 29 months, with cumulative 1-, 3-, and 5-year survival rates of 78.2%, 38.8%, and 30.4%, respectively. The low-HALP group (median OS: 22 months) had significantly inferior survival compared to the high-HALP group (median OS: 37 months), with 1-, 3-, and 5-year survival rates of 70.1%,31.4%,22.8% vs. 89.7%,45.3%,38.1% ( χ2=7.695, P=0.006). Conclusion:The HALP index calculated from the patient's preoperative indicators can be used to predict the overall survival of patients with distal cholangiocarcinoma.

Chinese Journal of Hepatobiliary Surgery 2025;31(7):524-528
Objective:To analyze the predictive value of the preoperative lymphocyte-to-monocyte ratio (LMR) for the postoperative prognosis of distal cholangiocarcinoma. Methods:Clinical data of 197 patients with distal cholangiocarcinoma undergoing radical surgery at Beijing Chaoyang Hospital Affiliated to Capital Medical University from January 2011 to December 2023 were retrospectively analyzed, including 118 males and 79 females, aged (65.1±10.2) years. Patients were randomly divided into the training set ( n=137) and the test set ( n=60) in a ratio of 7∶3. The training set is used to construct the nomogram, and the test set is used to evaluate the nomogram. Multivariate analysis of the influencing factors of long-term survival after surgery for distal cholangiocarcinoma was conducted using Lasso regression and Cox regression. And based on the results of multiple factors, a nomogram for predicting the postoperative prognosis of distal cholangiocarcinoma was constructed. The receiver operating characteristic (ROC) curve, conformance index, calibration curve and decision curve were used to analyze and evaluate the predictive efficacy of the model. Results:Lasso regression screening showed that smoking history, preoperative LMR, preoperative total bilirubin, preoperative carbohydrate antigen (CA) 19-9, intraoperative blood loss, tumor differentiation degree, T staging of tumor, portal vein system invasion, nerve invasion and lymph node metastasis were risk factors affecting the long-term survival after surgery (all P<0.05). Multivariate Cox regression analysis showed that preoperative LMR, CA19-9, T staging of tumor, portal vein system invasion and lymph node metastasis were the influencing factors for long-term survival after surgery for distal cholangiocarcinoma. Based on the above factors, a nomogram for predicting the postoperative prognosis of distal cholangiocarcinoma was constructed. The areas under the ROC curves of this nomogram for predicting 3-year postoperative survival in the training set and the test set were 0.806 (95% CI: 0.719-0.893) and 0.811 (95% CI: 0.696-0.927), respectively. The consistency indices of the training set and the test set were 0.730 (95% CI: 0.678-0.783) and 0.714 (95% CI: 0.637-0.790), respectively. The calibration curves of the model in the training set and test set shows that the model fits well in both sets. The decision curve analysis (DCA) shows that the model has good clinical predictive efficacy in both sets. When the threshold range in the training set is between 10% and 84% and in the test set is between 18% and 82%, the model can bring benefits in predicting postoperative prognosis. Conclusion:The preoperative LMR level is one of the influencing factors for the postoperative prognosis of patients after surgery for distal cholangiocarcinoma. LMR-based model can effectively predict postoperative prognosis of patients with distal cholangiocarcinoma.

Chinese Journal of Trauma 2025;41(4):325-338
With the rapid development of competitive sports, the incidence of anterior cruciate ligament (ACL) injury is on the rise. Such injuries may shorten athletes′ career and lead to other long-term adverse consequences. Although athletes generally recover well after ACL reconstruction, many still struggle to return to their pre-injury performance levels. Advances in the understanding of ACL anatomy and injury mechanisms, along with the evolution of surgical techniques and rehabilitation methods, have provided more individualized and tailored options for athletes following ACL injuries. However, there is currently no consensus in China regarding surgical and rehabilitation strategies for competitive athletes aiming to return to sports after ACL injuries. To this end, the Sports Medicine Committee of the Chinese Research Hospital Association and the Editorial Board of the Chinese Journal of Trauma jointly formulated the Expert consensus on surgical treatment and rehabilitation for competitive sports athletes returning to sports after anterior cruciate ligament injury ( version 2025), and presented 14 recommendations covering surgical indications, preoperative rehabilitation, surgical timing, surgical strategies and postoperative rehabilitation strategies, aiming to improve the surgical treatment and rehabilitation system for ACL injuries in competitive athletes and facilitate their return to high-level sports performance after injury.

Objective:To identify the bacteria isolated from sepsis patients in a tertiary hospital in Shanghai and analyze their antimicrobial resistance features. Methods:This study included 439 patients with clinically diagnosed sepsis who underwent microbiological culture in a tertiary hospital in Shanghai from July 2021 to October 2024. Results of microbiological culture and antimicrobial susceptibility testing were retrospectively collected and analyzed. Differences between groups were analyzed using Chi-square test and Fisher′s exact test. Results:The positive rate of microbiological culture was 49.0% (215/439). The positive rate of blood culture was 24.1% (93/386) and 100 strains were isolated from the samples, including 57 Gram-negative bacteria (57.0%). The predominant isolates in blood samples were Escherichia coli, Klebsiella pneumoniae, and coagulase-negative staphylococci. The positive rate of bacterial culture from bronchoalveolar lavage fluid samples was 84.1% (37/44), with Klebsiella pneumoniae, Acinetobacter baumannii, and Pseudomonas aeruginosa being the predominant strains. The positive rate of bacterial culture from urine samples was 35.6% (127/357), with Escherichia coli, Klebsiella pneumoniae, and Enterococcus faecium being the most common. Escherichia coli, Klebsiella pneumoniae, Enterococcus faecium, coagulase-negative staphylococci, Pseudomonas aeruginosa, and Acinetobacter baumannii exhibited high resistance rates to fluoroquinolones [46.8% (29/62)-97.0% (32/33)]. The resistance rates of Acinetobacter baumannii to most commonly used antibiotics were >80.0%. The resistance rates of Escherichia coli and Klebsiella pneumoniae to the third-generation cephalosporins ranged from 41.8% (28/67) to 66.0% (31/47). Carbapenem resistance was observed in 38.1% (24/63)-40.3% (25/62) of Klebsiella pneumoniae isolates, and most of the isolates from bronchoalveolar lavage fluid samples showed a higher resistance rate than those from blood or urine samples ( P<0.05). Conclusions:The positive rate of bacterial culture is nearly 50% in this study, with Gram-negative bacteria being the most common. Six major pathogenic bacteria exhibit high resistance rates to fluoroquinolones. Klebsiella pneumoniae isolates have high resistance rates to the third-generation cephalosporins and carbapenems, with significant differences in the resistance rate between isolates from different samples, and it should be cautious to choose the third-generation cephalosporins and carbapenems in clinical practice.

On the basis of that the fluorescence wavelength of copper nanoclusters(CuNCs)could cover the entire visible region, multicolor fluorescent CuNCs/starch composites were prepared and applied in fingermark development. With L-glutathione as the reducing agent and protective ligand, blue emissive and orange emissive CuNCs solutions were obtained in alkaline solutions at 90℃and 25℃,respectively.With the aggregation-induced emission effect induced by ethanol as a poor solvent, the fluorescence of orange emissive CuNCs with a higher intensity was achieved in an ethanol-water solution. With ascorbic acid as the reducing agent and 3-mercaptopropionic acid as the protective agent, green emissive CuNCs solution was prepared in an acid solution.Particle morphologies, chemical compositions and optical properties of these three CuNCs above were investigated using physical characterization and spectroscopic analysis, indicating that well-dispersed CuNCs had excellent photoluminescent properties. These CuNCs solutions were combined with starch to form composite powders by simply drying. The influences of the type of CuNCs and the ratio of CuNCs to starch on the emission wavelength and fluorescence intensity of the products were studied. The obtained CuNCs/starch composites could emit blue, green and orange fluorescence under 365 nm ultraviolet light, respectively, which were suitable for fingermark development.Minutiae and partial level-3 features of latent fingermarks could be effectively developed.High-quality fluorescence fingermark images would be captured using appropriate optical filters to eliminate background interference of various substrates.

Due to the poor ionization efficiency and the weak mass spectrometry(MS)intensity of weakly polar substances, direct analysis using the traditional electrospray ionization mass spectrometry(ESI-MS)is a big challenge. In this study,a novel rapid on-site detection method of four prohibited sex hormones in cosmetics was proposed using online derivatization strategy coupled with a miniature mass spectrometer. The target substances in the samples were extracted by a custom-made polyaniline/multi-walled carbon nanotube solid-phase microextraction(SPME)probe. The stirring speed was 200 r/min,the extraction temperature was 40℃,and the extraction time was 2 min.A pulled dual-channel θ borosilicate glass capillary emitter was used as the nano-ESI ion source. The SPME probe was inserted into the channel containing methanol in theθborosilicate glass capillary. When the spray voltage was applied, the four sex hormones were desorbed and formed spray microdroplets, which then collided with the hydroxylamine microdroplets generated from the other channel. The microdroplets of reaction product entered into the miniature mass spectrometer for direct analysis. The limits of detection(LOD)and limits of quantification(LOQ)for the four sex hormones were 10-20 ng/mL and 20-50 ng/mL,respectively.The recoveries were from 84.6%to 107.8%with the relative standard deviations(RSD)from 4.1%to 11.6%.Compared to detection without derivatization, the MS signals of the four target substances were increased by 3 to 15 times. This method was simple, rapid, highly efficient and sensitive, and suitable for on-site rapid analysis of weakly polar sex hormones in cosmetics.
