1.Incidence and Mortality Trends of Lung Cancer in Jin-shan District of Shanghai from 2002 to 2020

[Purpose]To analyze the trends of incidence and mortality of lung cancer in Jinshan District of Shanghai from 2002 to 2020.[Methods]The data of lung cancer was obtained from the Shanghai Cancer Registry and the population data was obtained from the Jinshan District Bureau of Public Security.The crude rate and age-standardized rate of incidence and mortality(ASIR and ASMR),and the average annual percentage change(AAPC)of lung cancer were calculated.[Results]From 2002 to 2020,8 982 new cases and 6 081 deaths of lung cancer were reported.The ASIR of lung cancer increased from 36.55/105 to 56.72/105 with an AAPC of 3.34%(P<0.05).The ASIR was also increased in male and female,with higher increase in female(AAPC=8.56%)than that in male(AAPC=0.50%).The ASMR decreased from 32.25/105 to 23.06/105 with an AAPC of-2.08%(P<0.05).The ASMR was decreased both in male and female,with higher decline in male(AAPC=-2.71%)than that in female(AAPC=-0.43%).The disease risk in male was higher than that in female,while the difference in ASIR between male and female was decreasing.The inci-dence and mortality increased with age.[Conclusion]From 2002 to 2020,the incidence of lung cancer in Jinshan District was increasing,but the age-standardized mortality was decreasing.The incidence and mortality of lung cancer varied among different ages and sexes,so targeted preven-tion and control measures should be implemented.
2.Expert consensus on intraoperative repositioning for patients with spine fracture and dislocation (version 2025)

Spine fracture and dislocation are common traumatic spinal conditions that often require surgical intervention due to compromised spinal stability. Surgical approaches include anterior, posterior, and combined anterior-posterior spinal procedures. According to the specific surgical requirements, patients may be placed in the prone position or repositioned between prone and supine positions during surgery. Intraoperative repositioning has become an essential step in patient positioning. However, during repositioning, patients with spinal fracture and dislocation are at increased risk for complications such as hemodynamic instability, nerve injury, and pressure injuries to the skin and soft tissue. Notably, due to the instability of the spinal cord, even minor manipulations can further exacerbate the damage, potentially leading to severe outcomes like paraplegia. Although the current clinical guidelines provide instructive recommendations for standard position, there remains no specific protocols for intraoperative repositioning in patients with spine fracture and dislocation. With a concern for the lack of clinical studies on positioning techniques, risk prevention, and operational norms for special patients, no applicable guidelines or standards are available. A consensus was required to provide clinical reference, meet the requirements of surgical treatment, and minimize the safety risks of patients caused by improper placement of positions. Professional Committee of Operating Room Nursing of Shaanxi Nursing Association organized experts in nursing management and operating room nursing from major hospitals across China to formulate Expert consensus on intraoperative repositioning for patients with spinal fracture and dislocation ( version 2025). The consensus provides 11 recommendations covering pre-repositioning preparation, intraoperative maneuvers, and post-repositioning observation, aiming to provide references for clinical standardization of the intraoperative repositioning process and protection of patients′ safety.
3.Efficacy and safety of anlotinib in neoadjuvant treatment of newly diagnosed locally advanced thyroid cancer

Objective:To evaluate the efficacy and safety of anlotinib neoadjuvant therapy for newly diagnosed locally advanced thyroid cancer (LATC).Methods:Twenty-four newly diagnosed LATC patients (10 males and 14 females, age (47.1±3.3) years) admitted to Affiliated Hospital of Integrated Traditional Chinese and Western Medicine of Nanjing University of Chinese Medicine were prospectively included from January 2023 to April 2024. Patients were given anlotinib neoadjuvant therapy (12mg/d, 2 weeks of medication, 1 week of discontinuation), and the efficacy of the treatment was evaluated by CT and multi-disciplinary treatment at the end of each treatment cycle. Patients assessed as suitable for surgery would be scheduled for surgery, while those who were not suitable for surgery would continue to receive neoadjuvant therapy and periodic evaluations. The primary endpoints were objective response rate (ORR) and disease control rate (DCR), and the R0/1 resection rate and adverse events (AE) after neoadjuvant therapy were observed. Paired- t test was used to analyze the differences between groups, and the Clopper-Person accurate method was used to calculate the bilateral 95% CI of ORR and other indicators. Results:Twenty-four patients received 2(2, 3) cycles of neoadjuvant therapy with anlotinib, of which 23 underwent surgery after anlotinib therapy. After neoadjuvant therapy, the mean maximum diameter of target lesions decreased by 23.5%(95% CI: 2.8%-44.3%) compared with baseline ( t=9.22, P<0.001). The ORR and DCR were 37.5%(95% CI: 18.8%-59.4%) and 100%(95% CI: 85.8%-100%), respectively. About 91.7%(95% CI: 73.0%-99.0%) of patients eventually underwent R0/1 resection. Hand and foot skin reactions, hypertension, oral mucositis, and leukopenia were common AE; grade 4 and 5 AE were not observed. Conclusion:Anlotinib can be safely used as neoadjuvant therapy for newly diagnosed LATC patients with good antitumor effects, providing better surgical opportunities for R0/1 resection.
4.Effect of quality improvement based on the ASK model on quality of life and emotion in patients with acute myocardial infarction

Objective:To investigate effect of quality improvement based on the Attitude-Skill-Knowledge(ASK)model on quality of life and emotion in patients with acute myocardial infarction(AMI).Methods:This random-ized controlled study enrolled 220 AMI patients admitted in Liyang People's Hospital between October 2020 and Oc-tober 2022.They were divided into control group(n=110,routine nursing)and intervention group(n=110,addi-tional quality improvement intervention based on ASK model).After 2-month intervention,scores of Patient Health Questionaire-9(PHQ-9),Beck Anxiety Inventory(BAI)and General Quality of life Inventory(GQOLI-74),cardiac function and satisfaction for nursing were compared between two groups.Results:After 2-month intervention,compared with patients in control group,those in intervention group had significant higher score of GQOLI-74[(372.82±23.07)points vs.(306.48±15.53)points],significant lower scores of PHQ-9[(9.58±2.41)points vs.(15.36±3.51)points]and BAI[(8.61±0.24)points vs.(16.81±3.54)points],levels of N-terminal pro brain natriuretic peptide(NT-proBNP)[(5.03±1.15)pg/ml vs.(13.11±1.22)pg/ml],cardiac troponin I(cTnⅠ)[(2.90±0.51)pg/ml vs.(4.76±1.32)pg/ml]and creatine kinase isoenzyme MB(CK-MB)[(8.70±2.31)ng/ml vs.(13.63±2.63)ng/ml](P<0.001 all).Total satisfaction for nursing in intervention group was significantly higher than that of control group(94.55%vs.83.64%,P=0.009).Conclusion:Quality improvement based on Attitude-Skill-Knowledge model could improve cardiac function and quality of life as well as negative emotion in AMI patients with high satisfaction.
5.Design and clinical application of a novel fixation device used for percutaneous transhepatic cholanginal drainage tube

Objective To explore the clinical application effect of a novel fixation device used for percutaneous transhepatic cholangial drainage(PTCD)tube in patients after receiving PTCD.Methods A novel type of fixed device for PTCD tube was independently designed and produced by authors.One hundred patients,who underwent PTCD(retention time of the tube at least one month)at the Department of Interventional Radiology,Ruijin Hospital,Shanghai Jiao Jong University School of Medicint from September 2024 to April 2025,were collected for this study.According to the order of hospitalization dates,the patients were divided into an observation group(n=50)and a control group(n=50).The patients in the observation group used a novel PTCD tube fixation device as secondary fixation,while the patients in the control group used conventional medical silk tape for cross type secondary fixation.The postoperative 2-hour pain scores,readiness for hospital discharge,comfort level of fixed device during PTCD tube retention for one month,drainage tube dropping-off rate,drainage obstruction rate,and incidence of peripheral dermatitis were compared between the two groups.Results The postoperative 2-hour pain scores and the comfort level after one-month retention of PTCD tube in the observation group were obviously lower than those in the control group,the differences were statistically significant(both P<0.05).The PTCD tube dropping-off rate,poor drainage rate,and incidence of dermatitis in the observation group during tube retention period were lower than those in the control group(all P<0.05).The readiness for hospital discharge of both groups was at a medium-to-low level,although the score of the observation group was higher than that of the control group,the difference was not statistically significant(P>0.05).Conclusion The use of this novel PTCD tube fixation device can effectively increase the comfort level of patients during the period of carrying a tube.The modular ice pack component can alleviate postoperative pain,reduce the incidences of drainage tube dropping-off,drainage obstruction and peripheral dermatitis during tube retention period.Therefore,this novel PTCD drainage tube fixation device is worthy of clinical promotion and use.
6.Underlying target of bullatine A in treating rheumatoid arthritis based on LiP-SMap drug target proteomics

Aim To identify the underlying target of bullatine A(BA)against rheumatoid arthritis(RA)u-sing limited proteolysis-small molecule mapping(LiP-SMap)drug target proteomics and to provide a scientif-ic basis for clinical application of Aconiti brachypodi Radix in the treatment of RA.Methods LiP-SMap drug target proteomics was employed to perform bioin-formatics analysis for comparing and validating the dif-ferential protein expression after BA intervention.A collagen-induced arthritis(CIA)model was estab-lished in DBA/1 mice using bovine type Ⅱ collagen.The mice were then divided into the CIA model group,methotrexate-positive control group(MTX group),and BA groups(10 mg·kg-1 and 20 mg·kg-1)based on their clinical scores.After drug intervention,the thera-peutic efficacy against RA was assessed by joint index scores and foot thickness measurements.Histopatholog-ical changes in the arthritic joints of CIA mice were e-valuated using hematoxylin and eosin(HE)staining.Enzyme-linked immunosorbent assay(ELISA)was employed to detect inflammatory cytokines interleukin-17(IL-17)and total IgG and IgG3 anti-collagen-spe-cific antibodies levels from the serum of CIA mice.Flow cytometry was used to detect the expression levels of intracellular Th17 cells(IL-17+CD4+T cells)and Th1 cells(IFN-γ+CD4+T cells).Fluorescent quanti-tative PCR was performed to detect the expression of genes related to differential proteins.Results The proteomic analysis identified Serpinb1a as a protein with strong binding affinity to BA,and KEGG enrich-ment analysis indicated IL-17 signaling pathway was a crucial pathway of BA in against RA.BA treatment significantly reduced clinical scores and foot thickness,improved local arthritis symptoms in CIA mice,and al-leviated inflammatory cell infiltration into arthritic joints(P<0.05).Differential protein validation re-sults showed that BA had strong affinity with Serpinb1a(-5.92 kJ·mol-1)and downregulated the expres-sion of Serpinb1a mRNA.Furthermore,the administra-tion of BA markedly reduced serum IL-17 A levels from CIA mice,inhibited the expression of intracellular IL-17 A and IFN-γ cytokines in splenic CD4+T cells(P<0.05),and significantly downregulated the transcrip-tional expression of IL-17F(P<0.05).Conclusion BA exhibits therapeutic effects on collagen-induced arthritis,and its mechanism of action may involve the regulation of Serpinb1a and the IL-17 signaling path-way.
7.Efficacy and safety of biologics in plaque psoriasis patients with thrombocytopenia: a single-center retrospective cohort study

Objective:To summarize the clinical characteristics of plaque psoriasis patients with thrombocytopenia, and to evaluate the efficacy and safety of biologics in such patients.Methods:A single-center retrospective cohort study was conducted. Clinical data were collected from plaque psoriasis patients with thrombocytopenia at the Department of Dermatology, Peking University Third Hospital between January 2017 and October 2024. Comparative analysis was conducted on clinical data, such as platelet counts, before and after the use of biologics, and the efficacy and safety of biologics were evaluated.Results:Eleven patients (10 males, 1 female; age range: 33 - 72 years) had thrombocytopenia prior to biologic therapy. Thrombocytopenia was caused by hypersplenism secondary to liver cirrhosis in 7 patients, and the causes of cirrhosis including prior medications for psoriasis (5 cases) and viral hepatitis (2 cases) ; 3 patients were diagnosed with primary immune thrombocytopenia (ITP), and 1 patient with aplastic anemia. All the 11 patients achieved a Psoriasis Area and Severity Index (PASI) 90 response after biologic therapy. Only one patient experienced a transient episode of further decrease in platelet counts, which was considered potentially related to anti-tuberculosis drugs. The Wilcoxon signed-rank test showed no significant decrease in platelet counts after biologic therapy in the 11 patients (pre-treatment platelet counts M [ Q1, Q3]: 77 [55, 87] × 10 9/L, post-treatment platelet counts: 84 [65, 114] × 10 9/L, P = 0.083) ; notably, 3 patients with ITP showed an upward trend in platelet counts after treatment with interleukin (IL) -17A or IL-23 inhibitors. Conclusions:Thrombocytopenia may not be a contraindication for biologic therapy in patients with plaque psoriasis. Plaque psoriasis patients with ITP may obtain dual benefits from the use of IL-17A inhibitors or IL-23 inhibitors.
8.Chinese version of the Short Self-Regulation Questionnaire and its reliability and validity in breast cancer patients

Objective:To translate the Short Self-Regulation Questionnaire (SSRQ) into Chinese, and test its reliability and validity in breast cancer patients.Methods:Using the Brislin translation model, cross-cultural adaptation was conducted by combining expert consultation and a pre-survey on SSRQ. From August to December 2023, 324 breast cancer patients from Department of Breast Surgery of Peking University First Hospital were selected by convenience sampling to test the reliability and validity of SSRQ.Results:A total of 324 questionnaires were distributed and 307 valid questionnaires were recovered, with an effective recovery rate of 94.75% (307/324). The final questionnaire consisted of 30 items. The content validity index at the item level ranged from 0.80 to 1.00, and the content validity index at the scale level was 0.97. The predictive validity result showed a good model fit. The Cronbach's α coefficient for the questionnaire was 0.90, the folded reliability coefficient was 0.85, and the retest reliability coefficient was 0.79.Conclusions:SSRQ has good reliability and validity in breast cancer patients and has value for clinical promotion.
9.Establishment of a LC-MS/MS method for detecting gamma-aminobutyric acid in plasma and its clinical application

Objective To establish a stable liquid chromatography-tandem mass spectrometry(LC-MS/MS)method for detecting gamma-aminobutyric acid(GABA)in plasma,and to evaluate the value of GABA detection in the diagnosis of sleep disorders.Methods GABA was detected using a UPLC Xevo TQs system.The method was pre-validated and its performance was verified to establish a reference range for healthy individuals.The difference in plasma GABA levels between apparently healthy individuals and patients with sleep disorders was compared.Results We employed deuterated compounds as isotopic internal standards and utilized an Amide chromatographic column for separation.The mobile phase was 0.050%formic acid in water and 90%acetonitrile in water containing 0.175%formic acid and 5 mmol/L ammonium acetate with gradient elution in the column temperature of 35℃.The linear range for the detection of GABA by LC-MS/MS was 0.05-10.00 μmol/L,with a lower limit of quantification of 0.02 μmol/L,the inter-day CV<3.00%and intra assay CV<4.00%,respectively,and the recovery rate was 101.06%-109.02%.The reference ranges for plasma GABA were established by analyzing 300 healthy controls stratified by age:18-34 years(0.08-0.15 μmol/L),35-49 years(0.10-0.20 μmol/L),and≥50 years(0.12-0.23 μmol/L).Then plasma GABA was used as a biomarker for auxiliary diagnosis of sleep disorders in analyzing 221 patients and 300 healthy controls,which revealed that AUC values were 0.510(P=0.850),0.686(P=0.002),and 0.890(P<0.001)in the groups of 18-34 years,35-49 years,and≥50 years,respectively,with optimal cut-off values of 0.09,0.10 and 0.11 μmol/L.Conclusion A reliable LC-MS/MS method for detecting GABA has been established,which can detect plasma GABA levels sensitively and accurately and can be used in assisting the clinical diagnosis of sleep disorders.
10.Topological Properties of Brain Structure Network in Obese Adolescents Based on Graph Theory Analysis

Purpose To investigate the change of network topology attribute of brain structure in obese adolescents using graph theory analysis.Materials and Methods A total of 86 obese adolescents in Children's Hospital of Nanjing Medical University from January 2023 to April 2024 were prospectively collected,including 60 patients in the simple obesity group(OB group),26 patients in the obese group with metabolic syndrome(MS group).Meanwhile,24 healthy volunteers were recruited as the control group.Diffusion tensor imaging scan of the head,anthropological characteristics examination and blood test were performed.The differences of global and local topology properties among the three groups were compared.Global or node attributes with statistical differences were correlated with clinical indicators.Results There were significant differences in body height,body weight,body mass index,waist circumference,diastolic blood pressure,systolic blood pressure and high density lipoprotein cholesterol among the three groups(χ2/F=3.208-5.085,all P<0.05).Compared with the control group,there were significant differences in total body fat,triglyceride,total cholesterol,alanine amino-transferase,aspartate aminotransferase and fasting insulin between the OB group and the MS group(all P<0.05).No statistically significant differences were observed in the small world attribute,shortest path length and global efficiency among the three groups(all P>0.05).A statistically significant difference was observed in the clustering coefficient and local efficiency among the three groups(F=2.534,3.012,both P<0.05),with the values of clustering coefficient and local efficiency of OB group were significantly lower than those of the control group(P<0.05).The node attribute differences occurred in the left frontal lobe,right insula,right anterior central gyrus,left anterior cingulate gyrus,left anterior cuneus and bilateral occipital lobes.The connection value between module Association and Limbic in control group was significantly lower smaller than those of the OB group(P=0.033).A negative correlation was observed between local efficiency and age(r=-0.239),body weight(r=-0.254),body mass index(r=-0.230)and waist circumference(r=-0.263,all P<0.05).The modularity was negatively correlated with fasting insulin(r=-0.325,P<0.05).Conclusion Adolescent obesity leads to the abnormal topological properties of structural network in some brain regions,which may be associated with the abnormal cognitive function and could provide neuroimaging evidence for clinical intervention.
