OBJECTIVE To evaluate the efficacy, safety, and cost-effectiveness of brivaracetam (BRV) as adjunctive therapy in patients with drug-resistant focal epilepsy.METHODS PubMed, Embase, Cochrane Library, Web of Science, CNKI, Wanfang, and official health technology assessment (HTA) agency websites were searched from inception to January 31, 2026 for HTA reports, systematic reviews/meta-analyses, and pharmacoeconomic studies on BRV. After literature screening and quality assessment, a descriptive analysis was performed.RESULTS A total of 21 publications were included: 13 systematic reviews/meta-analyses, 7 pharmacoeconomic studies (including one study that simultaneously conducted a systematic review/meta-analysis), and 2 HTA reports. Regarding efficacy, BRV significantly improved the 50% responder rate [risk ratio (RR)=1.81] and seizure-freedom rate (RR=5.89) versus placebo. Among third-generation anti-seizure medications (ASMs), BRV ranked 1st-2nd for seizure freedom, while its 50% responder rate was at a moderate level. The optimal adult dose for seizure freedom was 100 mg/d. Regarding safety, BRV was well tolerated, overall adverse event rates did not differ from placebo, though withdrawal due to adverse event rates were higher than with placebo. Tolerability ranked 1st-2nd among the third-generation ASMs. Psychiatric/behavioral adverse event rates were lower with BRV (6.4%) than with levetiracetam (14.1%). Regarding cost-effectiveness, BRV showed pharmacoeconomic advantages in countries with lower drug pricing (Australia, Finland) and only in high-risk subgroups in Japan, but did not show such advantages at current prices in the US or Italy. BRV has been included in China’s national reimbursement drug list in 2025, and the economic conclusions drawn during its initial market launch are outdated and require re-evaluation.CONCLUSIONS BRV has proven efficacy and good safety, especially suitable for patients intolerant to levetiracetam. However, due to pricing and medical insurance policies, the cost-effectiveness of BRV after its inclusion in China’s national reimbursement drug list still needs to be re-evaluated.

ObjectiveTo investigate the mechanism by which Xianglian Huazhuo prescription (XLHZ) ameliorates chronic atrophic gastritis (CAG) through regulating glycolysis via the microRNA-34b-5p (miR-34b-5p)/6-phosphofructo-2-kinase/fructose-2,6-bisphosphatase 1 (PFKFB1) signaling pathway. MethodsBioinformatic screening was performed to identify the target genes of miR-34b-5p. In the animal experiments, a mouse model of CAG was established through a combination of N-methyl-N′-nitro-N-nitrosoguanidine (MNNG), ranitidine, and irregular feeding. The mice were randomized into the following groups: normal, model, Morodan group (2 g·kg-1), and high-, medium-, and low-dose XLHZ groups(7.2,3.8,1.9 g·kg-1). In the cell experiments, GES-1 cells were treated with MNNG for the modeling of CAG and grouped as follows: GES-1, MNNG+GES-1, MNNG+GES-1+XLHZ(10%), MNNG+GES-1+miR-34b-5p mimics, MNNG+GES-1+miR-34b-5p inhibitor, MNNG+GES-1+PFKFB1 inhibitor, and MNNG+GES-1+PFKFB1 mimics+XLHZ(10%). The general conditions of mice were observed. Real-time PCR was adopted to measure the expression levels of miRNA-34b-5p and PFKFB1 mRNA in the gastric mucosa of mice in each group. Dual luciferase reporter gene was used to verify the targeting interaction between miR-34b-5p and PFKFB1 in GES-1 cells exposed to MNNG. The cell-counting kit-8 (CCK-8) was used to screen the drug-containing serum concentration. Real-time PCR was employed to verify the transfection efficiency. Fluorescent probes were used to detect the expression and nuclear translocation of hypoxia-inducible factor-1α (HIF-1α) in each group of cells. The protein levels of HIF-1α, hexokinase 2 (HK2), PFKFB1, phosphofructokinase 1 (PFK1), and lactate dehydrogenase (LDHA) in the gastric mucosa of mice were determined by Western blot. The levels of glucose consumption and lactate in the serum and cells were measured by biochemical methods. Enzyme-linked immunosorbent assay (ELISA) was adopted to measure the content of adenosine triphosphate (ATP) in the gastric mucosa tissue and cells. ResultsThe animal experiments showed that compared with the normal group, the model group exhibited symptoms including mental fatigue, lethargy, sparse and dull fur, a tendency to curl up, sluggish responses, reduced food and water intake, and slow body weight gain. In addition, the model group showed decreased expression of miRNA-34b-5p (P<0.01), increased mRNA level of PFKFB1 (P<0.01), upregulated protein levels of HIF-1α, HK2, PFKFB1, PFK1, and LDHA (P<0.01), and increased glucose consumption, lactate content, and ATP content (P<0.01). Compared with the model group, the XLHZ group and Morodan group showed improved mental state, gradually improved fur, increased activity, food and water intake, and body weight. HE showed that each treatment group exhibited well arranged and increased gastric mucosal glands, reduced inflammatory cells, and recovered gastric pits. Transmission electron microscopy showed that the size of mitochondria in each treatment group returned to normal, with no obvious irregular shape, complete and clear cristae, and uniform matrix. Furthermore, each treatment group showed increased expression of miRNA-34b-5p (P<0.05), reduced mRNA level of PFKFB1 (P<0.05), downregulated protein levels of HIF-1α, HK2, PFKFB1, PFK1, and LDHA (P<0.05), and decreased glucose consumption, lactic acid, and ATP (P<0.05). The cell experiments showed that the expression and nuclear translocation of HIF-1α were reduced in all groups after treatment (P<0.05). The cell transfection efficiency was satisfactory (P<0.05). miR-34b-5p exhibited an effect comparable to that of XLHZ, effectively inhibiting glycolysis, whereas PFKFB1 promoted glycolysis and reversed the protective effect of XLHZ in CAG mice (P<0.05). ConclusionXLHZ could regulate the miR-34b-5p/PFKFB1 axis to inhibit glycolysis, thereby ameliorating MNNG-induced gastric mucosal atrophy.

T-cell acute lymphoblastic leukemia (T-ALL) is a highly aggressive hematologic malignancy with a poor prognosis, despite advancements in treatment. Many patients struggle with relapse or refractory disease. Investigating the role of the super-enhancer (SE) regulated gene ubiquitin-specific protease 20 (USP20) in T-ALL could enhance targeted therapies and improve clinical outcomes. Analysis of histone H3 lysine 27 acetylation (H3K27ac) chromatin immunoprecipitation sequencing (ChIP-seq) data from six T-ALL cell lines and seven pediatric samples identified USP20 as an SE-regulated driver gene. Utilizing the Cancer Cell Line Encyclopedia (CCLE) and BloodSpot databases, it was found that USP20 is specifically highly expressed in T-ALL. Knocking down USP20 with short hairpin RNA (shRNA) increased apoptosis and inhibited proliferation in T-ALL cells. In vivo studies showed that USP20 knockdown reduced tumor growth and improved survival. The USP20 inhibitor GSK2643943A demonstrated similar anti-tumor effects. Mass spectrometry, RNA-Seq, and immunoprecipitation revealed that USP20 interacted with hypoxia-inducible factor 1 subunit alpha (HIF1A) and stabilized it by deubiquitination. Cleavage under targets and tagmentation (CUT&Tag) results indicated that USP20 co-localized with HIF1A, jointly modulating target genes in T-ALL. This study identifies USP20 as a therapeutic target in T-ALL and suggests GSK2643943A as a potential treatment strategy.

BACKGROUND:
Significant brain volume deviation is an essential phenotype in children with neurodevelopmental delay (NDD), but its genetic basis has not been fully characterized. This study attempted to analyze the genetic factors associated with significant whole-brain deviation volume (WBDV).
METHODS:
We established a reference curve based on 4222 subjects ranging in age from the first postnatal day to 18 years. We recruited only NDD patients without acquired etiologies or positive genetic results. Cranial magnetic resonance imaging (MRI) and clinical exome sequencing (2742 genes) data were acquired. A genetic burden test was performed, and the results were compared between patients with and without significant WBDV. Literature review analyses and BrainSpan analysis based on the human brain developmental transcriptome were performed to detect the potential role of genetic risk factors in human brain development.
RESULTS:
We recruited a total of 253 NDD patients. Among them, 26 had significantly decreased WBDV (<-2 standard deviations [SDs]), and 14 had significantly increased WBDV (>+2 SDs). NDD patients with significant WBDV had higher rates of motor development delay (49.8% [106/213] vs . 75.0% [30/40], P = 0.003) than patients without significant WBDV. Genetic burden analyses found 30 genes with an increased allele frequency of rare variants in patients with significant WBDV. Analyses of the literature further demonstrated that these genes were not randomly identified: burden genes were more related to the brain development than background genes ( P = 1.656e -9 ). In seven human brain regions related to motor development, we observed burden genes had higher expression before 37-week gestational age than postnatal stages. Functional analyses found that burden genes were enriched in embryonic brain development, with positive regulation of synaptic growth at the neuromuscular junction, positive regulation of deoxyribonucleic acid templated transcription, and response to hormone, and these genes were shown to be expressed in neural progenitors. Based on single cell sequencing analyses, we found TUBB2B gene had elevated expression levels in neural progenitor cells, interneuron, and excitatory neuron and SOX15 had high expression in interneuron and excitatory neuron.
CONCLUSION
Idiopathic NDD patients with significant brain volume changes detected by MRI had an increased prevalence of motor development delay, which could be explained by the genetic differences characterized herein.

Aim To observe the effect of total flavonoid from Mori folium(TFMF) on renal interstitial fibrosis in type 1 diabetic mice and its possible mechanism. Methods Diabetic mice were induced by intraperitoneal injection of streptozotocin(STZ) dissolved in 0.01 mol·L-1 citrate buffer(pH 4.5) at 150 mg·kg-1 body weight after 12 h of food deprivation.Forty model mice were divided randomly into four groups: model group, and low-(0.25 g·kg-1), moderate-(0.5 g·kg-1), high-dose groups(1 g·kg-1) fed with TFMF once daily. In addition, eight normal mice were used as normal group. After 12 weeks, the fasting blood glucose(FBG), serum creatinine(Cr), blood urea nitrogen(BUN) and microalbuminuria(mAlb) were measured.Masson staining, Sirius red staining and collagen type Ⅳ immunohistochemical staining were used to detect the expression of collagen protein in the cortex, while laminin staining to assess the degree of glomerular and renal tubular basement membrane thickening. The protein expressions related to epithelial-mesenchymal transition and PI3K/Akt/mTOR in the renal cortex of mice were detected by Western blot. Results The moderate and high dose of TFMF could significantly decrease the levels of FBG, Cr, BUN and mAlb in diabetic mice, meanwhile decreasing the expression of α-SMA protein by inhibiting the activation of PI3K/Akt/mTOR signaling pathway, which led to the amelioration of the pathological alterations of renal tissue. Conclusions The moderate and high dose of TFMF can reduce the level of renal interstitial fibrosis in type 1 diabetic mice, and its mechanism may be related to the inhibition of activation of PI3K/Akt/mTOR signaling pathway.

Journal of Biomedical Engineering 2015;32(3):656-661
Intravascular ultrasound (IVUS) is widely used in coronary artery examination. Ultrasonic elastography combined with IVUS is very conspicuous in identifying plaque component and in detecting plaque vulnerability degree. In this study, a simulation model of the blood vessel based on finite element analysis (FEA) was established. The vessel walls generally have radial changes caused by different intravascular pressure. The signals at lower pressures were used as the pre-deformation data and the signals at higher pressure were used as the post-deformation data. Displacement distribution was constructed using the time-domain cross-correlation method, and then strain images. By comparison of elastograms under different pressures, we obtained the optimal pressure step. Furthermore, on the basis of the obtained optimize pressure step, the simulation results showed that this method could effectively distinguish characteristics between different component plaques, and could guide the later experiments and clinical applications.
