1.Application of DynaCT combined with 3D iGuide puncture technique to microwave ablation of lung cancer

Objective To investigate the feasibility and safety of DynaCT microwave ablation (MWA) guided by 3D iGuide puncture technology for lung cancer. Methods The clinical data of 19 patients with primary or metastatic lung cancer who underwent DynaCT MWA from June 2019 to December 2020 in our hospital were retrospectively analyzed, including 15 males and 4 females with an average age of 64.9±11.7 years. The technical success rates, adverse reactions and complications, postoperative hospital stay, and local therapeutic efficacy were recorded. Results Technical success rate was 100.0%. The mean time required to target and place the needle was 15.7±3.7 min and the mean ablation time was 5.7±1.6 min. Thirteen patients underwent biopsy synchronously before the ablation, and 10 (76.9%) patients had positive pathological results. The main adverse reactions were pain (7/19, 36.8%), post-ablation syndrome (4/19, 21.1%) and cough (2/19, 10.5%). The minor complications were pneumothorax (6/19, 31.6%), hemorrhage (5/19, 26.3%), pleural effusion (2/19, 10.5%) and cavity (1/19, 5.3%). Three patients had moderate pneumothorax and received closed thoracic drainage. The median hospitalization time after ablation was 2.0 (2.0, 3.0) d, and no patient died during the perioperative period. The initial complete ablation rate was 89.5% (17 patients) and the incomplete ablation rate was 10.5% (2 patients) at 1-month follow-up, and no local progression was observed. Conclusion DynaCT MWA of lung cancer under the guidance of 3D iGuide system is safe and feasible with a high short-term local control rate, but the long-term efficacy remains to be further observed.
2.Preliminary study on the expression and function of bone morphogenetics protein 7 in ESCC

Objective :
To analyze the expression and immune infiltration levels of the BMP7 gene ( BMP7) in e- sophageal squamous cell carcinoma(ESCC) .
Methods :
Initially,in 274 cases of ESCC and 242 cases of normal tissues,the level of BMP7 was verified by immunohistochemistry ,and the relationship between the expression difference and the survival cycle and clinical pathological characteristics of patients with ESCC was explored,and BMP7 overexpression plasmid transfection of ESCC cells was established,and the effect of BMP7 on the biological behavior of ESCC cells was examined by CCK-8,Clone,and Transwell.
Results :
BMP7 expression in normal e- sophageal tissues was higher than that of ESCC(P<0. 001) ,the expression level of BMP7 was correlated with the degree of differentiation of patients(P = 0. 006) and TNM staging(P <0. 001) ,and the survival of patients with high expression of BMP7 exceeded that of patients with low BMP7 (P = 0. 041) ,and the experiments of CCK-8 and Clone showed that the proliferation effect of cells in the overexpressed BMP7 group was lower than that of the control group.Transwell experiments confirmed that the cell invasion migration capacity of the overexpressed BMP7 group was less than that of the control one.The immune infiltration results showed that BMP7 was positively correlated with macrophages(P = 0. 008) and negatively correlated with γ-δT cells(P = 0. 028) .
Conclusion
BMP7 is low in ESCC and associated with poor prognosis and immune infiltration levels in patients.
3.Relationship of proprioception, cutaneous sensitivity and strength to postural stability among older adults

Objective To explore the impacts of proprioception, cutaneous sensitivity and strength on dynamic and static postural stability.Methods From June to November, 2020, a total of 164 elderly participated in this study. Independent parameters included proprioception of knee flexion/extension and ankle dorsi/plantar flexion with proprioception measurement; the cutaneous sensitivity of great toe, first and fifth metatarsals, arch, and heel with monofilament; and strength of ankle dorsi/plantar flexion and hip abduction with isokinetic test. The Berg Balance Scale (BBS) and center of pressure (COP)-based postural stability tests were conducted to represent dynamic and static postural stability. Exploratory factor and multivariable linear regression analyses were used to explore the relationship of each generated factor to postural stability outcomes.Results Proprioception was significantly correlated to the score of BBS (r=-0.449, P<0.001) and the root mean square (RMS) of COP in mediolateral direction (r=0.254, P=0.004). Cutaneous sensitivity was significantly correlated to the RMS of COP in the anteroposterior direction (r=0.281, P=0.002). Strength was significantly correlated to the score of BBS (r=0.493, P<0.001).Conclusion Proprioception and strength are related to dynamic postural stability, while proprioception and cutaneous sensitivity are associated with static postural stability.
4.The safety and efficacy of microwave ablation in the treatment of lung cancer: A clinical analysis of single center

Objective To evaluate the safety and efficacy of microwave ablation (MWA) in the treatment of lung tumors. Methods The clinical data of 31 patients with lung neoplasms treated with MWA from January 2019 to August 2020 in a single center were retrospectively analyzed. There were 17 males and 14 females at an age of 63.4±10.4 years. The characteristics of the lesions, technical success rate, technical efficiency, local progression rate, adverse reactions and complications were recorded in detail. Results There were 39 target lesions with an average diameter of 20.2±10.6 mm. A total of 36 MWA procedures were completed. The initial technical success rate was 84.6% (33/39), and the technical efficiency was 92.3% (36/39). The median postprocedure hospital stay was 2.0 (2.0, 3.0) d. A total of 12.9% (4/31) of the patients had local progression, and the local control rate was 87.1%. The main adverse reactions were pain (12/36, 33.3%), cough (6/36, 16.7%), post-ablation syndrome (6/36, 16.7%) and pleural effusion (3/36, 8.3%). The main complications were pneumothorax (11/36, 30.6%), hemorrhage (8/36, 22.2%), cavitation (2/36, 5.6%) and pulmonary infection (1/36, 2.8%). The median follow-up time was 13.0 (8.0, 18.0) months. No patient died during the follow-up. Conclusion MWA is safe and effective in the treatment of lung tumors with controllable complications. Successive researches with large sample, and medium and long-term follow-ups are needed to explore the significance of combined therapies.

Objective To use World Health Organization Family International Classifications (WHO-FICs) to explore the framework and approaches of development and research of guidelines of rehabilitation at levels of policies, community and services.Methods The important documents and tools of rehabilitation at international level, including United Nations Convention on the Rights of Persons with Disabilities, WHO World Report on Disability, Community-based Rehabilitation Guideline, Rehabilitation in Health Service System, and International Classification of Diseases (ICD), International Classification of Functioning, Disability and Health (ICF), and International Classification of Health Intervention (ICHI) of WHO-FICs, had been discussed.Results The framework, classifications, diagnosis and description of diseases and functioning, coding, intervention and functioning evaluation based on ICD-11, ICF and ICHI-β-2 had been established for development and implementation of rehabilitation guidelines and Cochrane rehabilitation.Conclusion The framework and systematic approaches of ontology, classification, terminology, coding, diagnosis and description of diseases and functioning, interventions and evaluations for the development and implementation of rehabilitation guidelines had been developed.
6.Strategy of surgical management for pulmonary atresia with intact ventricular septum and mid-term follow-up

To evaluate the surgical strategy and follow-up for pulmonary atresia with intact ventricular septum (PA/IVS) in our heart center. Methods From January 2008 to December 2018, 151 patients with PA/IVS were divided into two groups: a one-stage surgery group (26 patients), including 17 males and 9 females at an average age of 14.7±13.2 months, and a staged surgery group (125 patients) including 72 males and 53 females at an average age of 6.4±6.3 months. The clinical effectiveness of the two groups were analyzed. Results All patients were followed up for 1-11 years. Eighteen patients died and 19 patients were lost to follow-up. The 1-year, 5-year and 10-year survival rate was 90.2%, 87.0%, and 85.2%, respectively. Two patients died in the one-stage surgery group. Twelve patients died after initial surgery, and 4 patients died after final operation in the staged surgery group. The Z value of tricuspid valve (P=0.013) and severe right ventricular dysplasia (P=0.025) were the risk factors of postoperative death in the patients with PA/IVS. Furthermore, 58 patients completed final operation, and the total number of the final operation (including one-stage radical surgery) accounted for 55.6% (84/151). Five patients accepted the re-operation intervention in the medium-term follow-up. The rest of the patients recovered well. Only 2 patients were classified as grade Ⅲ in cardiac function, and the rest patients were classified as gradeⅠ-Ⅱ. Conclusion According to the degree of right ventricular hypoplasia, the age at operation and the presence or absence of coronary artery malformation, the individualized surgical strategy could significantly improve the success rate of PA/IVS, and early completion of right ventricular decompression operation is conducive to improve the chance for biventricular repair.
7.Anatomic correction for congenitally corrected transposition of the great arteries and 10-year follow-up: A retrospective cohort study

Objective To evaluate the effects of anatomic correction for congenitally corrected transposition of the great arteries (ccTGA) and 10-year follow-up. Methods From January 2008 to December 2018, 48 patients with ccTGA who underwent anatomic correction were reviewed. There were 29 males and 19 females with age of 39.2 (3-91) months. The cohort was divided into two groups: a biventricular anatomic correction group (39 patients) and a 1.5 ventricular anatomic correction group (9 patients). They were followed for in-hospital mortality, late mortality, long-term survival, freedom from reoperation, and heart function. Results There were 3 early deaths and 2 early re-intervention in the biventricular anatomic correction group, but no death and only one re-intervention in the 1.5 ventricular anatomic correction group. Compared with the biventricular anatomic correction group, the operation time, tracheal intubation and ICU time were significantly reduced or shortened in the 1.5 ventricular anatomic correction group (P<0.05). The patients were followed up for 0.5-10.4 years. Four patients were lost. Two patients died in the biventricular anatomic correction group, and two patients received re-intervention. The 1-year, 5-year and 10-year survival rate was 88.2%, 84.0%, and 84.0%, respectively. There was no death or intervention in the 1.5 ventricular anatomic correction group. The quality of life of the other patients in the medium-term follow-up was satisfactory. Only 2 patients were classified as grade Ⅲ in cardiac function, and the other patients were classified as grade Ⅰ-Ⅱ. Conclusion According to the different anatomic characteristics of ccTGA, the individualized strategy of anatomic correction can achieve satisfactory surgical results, and the medium-term quality of life was good. Especially, 1.5 ventricular anatomic correction may obtain better therapeutic effects because of its lower operative mortality and less postoperative complications.
8.Effect of off-pump pulmonary valvotomy as the initial surgery for pulmonary atresia with intact ventricular septum: A single-center clinical analysis

Objective To evaluate the effect of off-pump pulmonary valvulotomy for the patients with pulmonary atresia with intact ventricular septum (PA/IVS). Methods The clinical data of 61 PA/IVS patients who underwent off-pump right ventricular decompression surgery in our hospital from January 2013 to September 2019 were retrospectively analyzed, including 37 males and 24 females, with an average age of 29.7 (2.0-86.0) d and weight of 4.1 (2.5-6.9) kg. Thirty-nine patients received off-pump pulmonary valvulotomy (an open-view valvulotomy group) and 22 patients received balloon valvuloplasty through the right ventricle pulmonary valve (a hybrid therapy group). The postoperative mortality, early re-intervention, and completion of final operation of the two groups were compared. Results There were 2 deaths in the study with a mortality rate of 3.3% (2/61), and the mortality rate of the two groups was not significantly different (2.6% vs.4.5%, P=0.68). The rate of early re-intervention in the two groups was 5.3% and 19.0%, respectively (P=0.09). There was no statistical difference in intubation time (56.0±25.9 h vs. 62.0±28.9 h, P=0.41), ICU retention time (4.7±2.9 d vs. 5.5±2.2 d, P=0.23) and postoperative hospital stay time (3.9±0.9 d vs. 4.3±1.1 d, P=0.38) between the two groups. The follow-up time was 45.3 (4.0-84.0) months. There were 5 patients lost to follow-up. During the follow-up period, in the open-view valvulotomy group, 17 patients did not need further operation, 13 patients completed the final operation. In the hybrid therapy group, 7 patients did not need further operation, 8 patients completed the final operation. Heart function classification of all patients was in New York Heart Association class Ⅰ-Ⅱ. Conclusion Compared with the hybrid therapy, off-pump pulmonary valvulotomy for PA/IVS also has the advantages of simple operation, short operation time and high survival rate, and it may be easier to be promoted in clinical application because of its more economic benefits and relatively lower re-intervention rate.

近年来 DNA修复途径成为癌症治疗的热门靶点, 聚ADP核糖聚合酶[poly(ADP)-ribosepolymerase,PARP]作为DNA修 复的关键酶得到了广泛研究,目前上市的PARP抑制剂(PARP inhibitor,PARPi)药物,在乳腺及卵巢等肿瘤中取得了革命性的突 破。结直肠癌(CRC)具有异质性,其发生与发展是一个多途径、多基因参与及多步骤的过程,其发病率与病死率在中国逐年上 升。研究发现,PARP1与CRC的发生、发展及治疗密切相关,本文从PARP1的分子结构及生物学功能、PARPi的作用机制、PARP1 在CRC组织中的表达情况、PARP1与结直肠癌干细胞的关系、PARPi与CRC治疗的关系等5个方面对PARPi在CRC中作用的研 究进展作一系统综述,为临床治疗CRC寻找新的治疗策略。

以程序性死亡因子 1(PD-1)及其配体 1(PD-L1)为代表的B7/CD28家族在肿瘤免疫治疗中显示出极大的应用潜力,但仍 有很多PD-L1检测阴性患者无法从中受益。作为B7家族成员,人内源性逆转录病毒-H长末端重复关联蛋白2 (HHLA2)分子结 构与PD-L1等其他B7家族成员有一定的同源性和相似性, 对T细胞有共刺激和共抑制作用,在多数实体肿瘤中呈高表达,且在某 些实体肿瘤中表达比PD-L1更广泛;其通过与受体穿膜和免疫球蛋白结构域2(TMIGD2)结合可促进肿瘤免疫逃逸,导致肿瘤的 进展和转移。基于此,HHLA2/TMIGD2有望成为肿瘤免疫治疗新的通路和靶点之一。本文就HHLA2及其受体TMIGD2的分子 结构、生物学功能及其在实体肿瘤中作用的研究进展作一综述。
