1.A scope review of the application of Clinical Decision Support Systems involving nurses in depression management

Objective To review the research on the application of clinical decision support system(CDSS)involving nurses in depression both domestically and internationally.Methods 10 Chinese and English databases including PubMed,Web of Science,Scopus,CINAHL,PsycINFO,Embase,Cochrane Library,CNKI,Wanfang Database,and VIP were searched until March,2024.The eligible literature of the application of CDSS involving nurses in depression was selected to analyze the basic characteristics of studies,the functions,characteristics,feasibility,and effectiveness of CDSS.Results Finally,16 English articles involving 13 CDSS were included.This study found that nurses participating in CDSS can improve the degree of depression in patients,but its application in depression management is mainly for screening,and few CDSS can propose personalized depression management programs based on the psychological and social factors of patients.Conclusion The CDSS involving nurses can promote the screening of depression and improve depression in patients,but it fails to propose a personalized depression management program.Future research can develop CDSS based on electronic medical record systems to facilitate nurses'screening for depression and further develop an intelligent management module for CDSS to provide intelligent and personalized management solutions for patients with depression.
2.Clinical observation on the effect of intensive early fluid resuscitation in 30 cases of severe acute pancreatitis

Objective To investigate the treatment method and effect of intensive early fluid resuscitation on the patients with severe acute pancreatitis ( SAP) in acute reaction stage .Methods Fifty-eight patients with SAP treated in the hospital from Aug .2008 to Mar.2010 were randomly divided into the treatment group (n=30) and the control group (n=28).The patients in the former group were given intensive early fluid resuscitation , either with hydroxyethylstarch and an increased amount of colloidal fluid or occasionally with a certain amount of plasma and albumin .Physiological saline and/or balanced salt solution were used as crystal fluid , with a ratio of 1∶2-1∶1.The patients in the latter group were supplemented or treated with an effective circulating blood volume and electrolyte im -balance correction .They were daily infused with a certain amount of plasma and albumin .Analyses were made on such reactions as re-spiratory failure , renal insufficiency and cardiac dysfunction in the acute reaction stage in patients with SAP .Results With regard to the incidence of complications during the acute reaction stage for the patients of the 2 groups, the incidence of respiratory failure , renal insufficiency and cardiac dysfunction in the patients of the treatment group was significantly lower than that in the patients of the control group, with statistical significance(P<0.05).With regard to the amount of daily fluid infusion for the groups during the acute reaction stage, the amount of infusion for the treatment group was significantly lower than that for the control group , with statistical significance (P<0.05).Conclusion Effective and active early fluid resuscitation during the acute reaction stage in SAP patients could decrease the incidence of complications .The supplementation of Hydroxyethylstarch and the increase in the ratio of colloidal fluid could produce better resuscitative effects on patients with SAP .
3.Diagnostic value of echocardiography in univentricle

Objective To explore the value of transthoracic echocardiography in diagnosis of univentricle and analyze the sonogram typing. Methods The results of 66 patients with univentricle were reviewed retrospectively,and analayzed their typing connected with the reports in the literature. Results There were 3 ultrasonic types in 66 cases:①Type A(single left ventricle) 19 cases,single ventricle with left ventricular shape,residual cavity in front of it. ②Type B(single right ventricle) 38 cases, single ventricle with right ventricular form,and residual cavity in the rear.③Type C (solitary single-ventricle) 9 cases,there was only one ventricle. Thirty-one of them were treated surgically, 5 cases without operation had MRI or cardiac catheterization examination and the remaining 26 patients were only observed by echocardiography,the positive rate of diagnosis in type was 100%, the results were compared with cardiac catheterization or MRI examination and the operation: 1 cases of mixed type total anomalous pulmonary venous connection was misdiagnosed as heart-type total anomalous pulmonary venous drainage. But 1 case of descending aorta limitations narrow complicated patent ductus arteriosus(PDA), PDA was missed. The rest were completely correct diagnosis. Conclusions The transthoracic echocardiography can be used to evaluate types and all containing malformations of univentricle,and offers reliable information for operation.
