Objective:To investigate ⅰ)the therapeutic effects of different doses of recombinant human prourokinase(rh-proUK)on patients with ST segment elevation myocardial infarction(STEMI)undergoing percutaneous coronary inter-vention(PCI),and ⅱ)its impact on peripheral blood P-selectin(CD62p)and Adropin levels. Methods:This randomized control study enrolled 132 STEMI patients undergoing PCI in Weinan Second Hospital between June 2020 and June 2023.Patients were randomly divided into control group(n=44,routine medication),low dose group(n=44,10 mg rh-proUK therapy)and high dose group(n=44,20 mg rh-proUK therapy).Therapeutic effective rate, myocardial perfu-sion immediate after operation, cardiac function, peripheral blood CD62p and Adropin levels, incidence of major adverse cardiovascular events(MACE)within 1 month after operation were compared among three groups. Results:Compared to patients in control group, those in high dose group had significantly higher total effective rate(95.5%vs.54.6%),pro-portions of TIMI myocardial perfusion grade(TMPG)grade 3(86.4%vs.25.0%)and ST segment resolution above 30%(∑STR≥30%)(88.6%vs.59.1%),stroke volume(SV)[(76.81±24.47)ml vs.(61.89±14.84)ml]and cardiac output(CO)[(6.48±1.45)L/min vs.(5.48±1.98)L/min],and significantly lower corrected TIMI frame count(CT-FC)[(31.80±6.32)frames vs.(52.39±7.14)frames],CD62p[(70.52±9.54)%vs.(82.42±12.44)%],Adropin[(82.48±9.55)pg/ml vs.(94.48±10.53)pg/ml]and total incidence of MACE(9.1%vs.38.6%)(P<0.05 or<0.01).Compared to those in low dose group, patients in high dose group had significantly higher total effective rate,pro-portions of TMPG grade 3 and ∑STR≥30%,SV and CO,and significantly lower CTFC,CD62p,Adropin and total inci-dence of MACE(P<0.05 or<0.01).Conclusion:The therapeutic effect of 20 mg rh-proUK on STEMI patients under-going PCI is significantly better than that of 10 mg,which could improve cardiac function, reduce CD62p,Adropin levels, and incidence of major adverse cardiovascular events.

Objective:To observe the effects of auricular point compression and exercise therapy on glycolipid metabolism, oxidative stress and sleep quality in type 2 diabetes mellitus(T2DM)patients with poor glycemic control. Methods:This study was a single-center randomized controlled study,86 T2DM patients with poor glycemic control who were treated in our hospital from June 2022 to June 2024 were divided into control group(received conventional treatment,n=43)and observation group(received auricular point compression and exercise therapy,n=43)by using the random number table method. The clinical efficacy, blood lipid indexes, blood glucose indexes, oxidative stress indexes and sleep quality were compared between the two groups. Results:The total clinical effective rate,high-density lipoprotein cholesterol(HDL-C),superoxide dismutase(SOD)and Glutathione peroxidase(GSH-Px)of the observation group after treatment were higher than those of the control group, and the fasting blood glucose(FBG),glycated hemoglobin(HbA1c),Homeostatic Model Assessment of Insulin Resistance(HOMA-IR),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),triglycerides(TG),reactive oxygen species(ROS),malondialdehyde(MDA)and Pittsburgh sleep Quality Index(PSQI)score were lower than those of the control group(P<0.05).Conclusion:Auricular point compression and exercise therapy can regulate glycolipid metabolism in T2DM patients with poor glycemic control, improve the degree of oxidative stress and sleep quality.

Objective To investigate the differences in demographic characteristics, reproductive health status, and the distribution of pregnancy-related diseases between couples conceived via assisted reproductive technology(ART)and naturally conceived couples, and to analyze the impact of ART treatment on the incidence of preterm birth(PTB)in singleton and twin and multiple pregnancies. Methods We conducted a retrospective analysis of the maternal and infant cohort data of Jing'an District from 2013 to 2020.Based on the conception method, the subjects were categorized into two groups:the ART group and the natural conception group.Chi-square test was applied to compare baseline characteristics and disease distributions differences between the two groups, and logistic regression models were used to evaluate the association between ART and the PTB risks.A causal mediation model was used to evaluate the mediating effect of twin and multiple pregnancy in the relationship between ART and PTB.Results A total of 117 717 parturients were included,6 265 in the ART group and 111 452 in the natural conception group.Compared with the natural conception group, couples in the ART group were significantly older and had a higher prevalence of reproductive system diseases. The incidences of diabetes and hypertensive disorders during pregnancy in ART parturient were 13.76%and 9.99%,respectively,which were significantly higher than 7.88%and 4.75%in the natural conception group(both P<0.001).The overall PTB rate in the ART group was 14.81%,higher than 5.35%in the natural conceptions group(P<0.001).The PTB rate in ART for singleton pregnancies in the ART group was 6.40%,higher than 4.83%in the natural conception group(P<0.001),while the PTB rate in ART for twin and multiple pregnancies in the ART group was 53.97%,lower than 60.42%in the natural conception group(P<0.05).Mediation analysis showed that 97.99%of the effect of ART on PTB was mediated by twin and multiple pregnancy, with ART increasing the PTB risk by 3.44 times through multiple pregnancy. Conclusion The overall PTB rate of ART recipients is higher than that of natural recipients, but ART does not increase the PTB risk in singleton and twin and multiple pregnancies.Twin and multiple pregnancy is the key mediating factor contributing to PTB in ART-conceived recipients.Compared with naturally conceived couples,ART conception couples own more advanced maternal age,and have higher risks of suffering gestational diabetes, gestational hypertension, and PTB.

Chronic kidney disease(CKD)is a chronic disease characterized by renal structural damage and dysfunction. At present, there is still a lack of effective therapeutic drugs and prevention and treatment methods for CKD in clinical practice.More and more studies have shown that necroptosis,as a new type of programmed cell death, plays a vital role in the onset and progression of CKD.Targeting key molecules in the necroptosis pathway, such as RIPK1,RIPK3 and MLKL,the development of small molecule inhibitors has become an emerging strategy for the treatment of CKD,and has shown significant potential to pro-tect the kidneys and alleviate renal fibrosis in a variety of in vitro and in vivo models. Therefore, this article summarizes the re-search progress of the mechanism of necroptosis in recent years, and focuses on the potential role of necroptosis in the pathogene-sis of CKD and the therapeutic potential of targeting this path-way,providing a new perspective and research direction for the prevention and treatment of CKD in the future.
