Obesity, as a chronic recurrent disease, has become a major public health challenge in China. To implement the requirements of the Healthy China Initiative (2019—2030), under domestic guidelines or consensus statements on overweight and obesity, and in alignment with the latest scientific advances globally, the Quality control protocol for adult overweight and obesity screening in health management (examination) institutions (2025 edition) was developed. This protocol was drafted by the Health Management Center of Shanghai Changzheng Hospital and formulated through multiple rounds of deliberation by experts in China’s health examination quality control field. The protocol establishes unified standards for screening facilities, personnel qualifications, and measurement or testing procedures. It defines specific screening items, outlines a standardized screening pathway, and sets requirements for the final medical review, ensuring the scientific validity, effectiveness, and safety of the screening process. The implementation of this protocol will enhance the consistency of weight management practices for adults across health examination institutions and strengthen the quality control of overweight and obesity screening programs.

Peri-implant keratinized mucosa (PIKM) augmentation refers to surgical procedures aimed at increasing the width of PIKM. Consensus reports emphasize the necessity of maintaining a minimum width of PIKM to ensure long-term peri-implant health. Currently, several surgical techniques have been validated for their effectiveness in increasing PIKM. However, the selection and application of PIKM augmentation methods may present challenges for dental practitioners due to heterogeneity in surgical techniques, variations in clinical scenarios, and anatomical differences. Therefore, clear guidelines and considerations for PIKM augmentation are needed. This expert consensus focuses on the commonly employed surgical techniques for PIKM augmentation and the factors influencing their selection at second-stage surgery. It aims to establish a standardized framework for assessing, planning, and executing PIKM augmentation procedures, with the goal of offering evidence-based guidance to enhance the predictability and success of PIKM augmentation.

3.Clinical Analysis of Torque Teno Virus Infection after Hematopoi-etic Stem Cell Transplantation in Children

Objective:To investigate the incidence,clinical characteristics,and complications of Torque teno virus(TTV)in children after hematopoietic stem cell transplantation(HSCT).Methods:A total of 40 children with hematological diseases who underwent HSCT were selected,and metagenomic next-generation sequencing(mNGS)technology was used to detect the gene sequences of pathogenic microorganisms in the blood.Combined with clinical data,the characteristics of TTV infection were analyzed.Results:Among the 40 pediatric patients post-HSCT,the TTV positive rate was 42.5%(17/40).There were no statistically significant differences between the TTV-positive group and the TTV-negative group in sex,age,white blood cell count(WBC),red blood cell count(RBC),hemoglobin,platelet count,neutrophil count,lymphocyte count,and high-sensitivity C-reactive protein(all P>0.05).The incidence of TTV infection was significantly higher in children who underwent haploidentical HSCT and in those with bone marrow stem cells(BMSC)as the transplant source(P<0.05).However,there were no significant differences in the TTV infection rate among patients with different disease types,different HLA matching statuses,or different engraftment times of neutrophils and platelets(all P>0.05).Among 17 children infected with TTV,13(76.5%)had co-infections with other viruses,mainly including cytomegalovirus(58.8%,10/17),human polyomavirus(41.2%,7/17),and Epstein-Barr virus(17.6%,3/17).In children with TTV infection,the most common complications were sepsis(82.4%),graft-versus-host disease(GVHD)(70.6%),pulmonary infection(41.2%),and hemorrhagic cystitis(17.6%).The incidence of GVHD in the TTV-positive group was significantly higher than that in the TTV-negative group(P<0.05).Conclusion:TTV infection is common in children undergoing HSCT,and it is prone to be complicated with cytomegalovirus infection and GVHD,which has an important influence on the clinical outcomes.
4.Advances in role and mechanism of traditional Chinese medicine active ingredients in regulating balance of Th1/Th2 and Th17/Treg immune responses in asthma patients.

Asthma is a chronic inflammatory disease involving multiple inflammatory cells and cytokines. Its pathogenesis is complex, involving various cells and cytokines. Traditional Chinese medicine(TCM) theory suggests that the pathogenesis of asthma is closely related to the dysfunction of internal organs such as the lungs, spleen, and kidneys. In contrast, modern immunological studies have revealed the central role of T helper 1(Th1)/T helper 2(Th2) and T helper 17(Th17)/regulatory T(Treg) cellular immune imbalance in the pathogenesis of asthma. Th1/Th2 imbalance is manifested as hyperfunction of Th2 cells, which promotes the synthesis of immunoglobulin E(IgE) and the activation of eosinophil granulocytes, leading to airway hyperresponsiveness and inflammation.Meanwhile, Th17/Treg imbalance exacerbates the inflammatory response in the airways, further contributing to asthma pathology.Currently, therapeutic strategies for asthma are actively exploring potential targets for regulating the balance of Th1/Th2 and Th17/Treg immune responses. These targets include cytokines, transcription factors, key proteins, and non-coding RNAs. Precisely regulating the expression and function of these targets can effectively modulate the activation and differentiation of immune cells. In recent years,traditional Chinese medicine active ingredients have shown unique potential and prospects in the field of asthma treatment. Based on this, the present study systematically summarizes the efficacy and specific mechanisms of TCM active ingredients in treating asthma by regulating Th1/Th2 and Th17/Treg immune balance through literature review and analysis. These active ingredients, including flavonoids, terpenoids, polysaccharides, alkaloids, and phenolic acids, exert their effects through various mechanisms, such as inhibiting the activation of inflammatory cells, reducing the release of cytokines, and promoting the normal differentiation of immune cells. This study aims to provide a solid foundation for the widespread application and in-depth development of TCM in asthma treatment and to offer new ideas for clinical research and drug development of asthma.

5.Advances in pathogenesis of asthma airway remodeling and intervention mechanism of traditional Chinese medicine.

Asthma, a chronic inflammatory airway disease with a high global prevalence, has a complex pathogenesis, in which airway remodeling plays a key role in the chronicity of the disease. Airway remodeling involves a series of pathophysiological changes, including airway epithelial damage, proliferation of mucous glands and goblet cells, subepithelial fibrosis, proliferation and migration of airway smooth muscle cells, and epithelial-mesenchymal transition. These complex pathological changes significantly increase airway resistance and responsiveness, forming an important pathological basis for refractory asthma. Currently, the regulatory mechanisms of airway remodeling focus on signaling pathways and regulatory targets. The signaling pathways include phosphatidylinositol 3-kinase(PI3K)/protein kinase B(Akt), nuclear factor-κB(NF-κB), transforming growth factor-β1(TGF-β1)/Smads, and mitogen-activated protein kinase(MAPK). The regulatory targets include microRNAs(miRNAs), competing endogenous RNAs(ceRNAs), long non-coding RNAs(lncRNAs), and circular RNAs(circRNAs). Key proteins involved in these processes include TGF-β1, silencing information regulator 2-related enzyme 1(SIRT1), chitinase 3-like protein 1(YKL-40), and adenosine deaminase-metalloproteinase 33(ADAM33). In recent years, the potential of traditional Chinese medicine in the treatment of asthma has become increasingly evident. Its active ingredients, extracts, and complexes can inhibit airway remodeling in asthma through multiple pathways, demonstrating a variety of effects, including anti-inflammatory actions, inhibition of smooth muscle cell proliferation and migration, regulation of epithelial-mesenchymal transition, attenuation of fibrosis and basement membrane thickening, reduction of mucus secretion, inhibition of vascular remodeling, modulation of immune imbalance, and antioxidative stress. This paper aims to provide an in-depth analysis of the pathogenesis and therapeutic targets of asthma, offering theoretical support and innovative strategies for clinical research and drug development in the treatment of asthma.

6.Analysis of the Influencing Factors and Short-Term Prognosis of Early Onset Coronary Heart Disease in Women in Wansheng District of Chongqing

Objective:To explore the incidence status,influencing factors and short-term prognosis characteristics of early onset coronary heart disease in women in Wansheng District of Chongqing,and to provide scientific basis for formulating regional prevention and treatment strategies.Methods:This study was a single-center retrospective study,100 coronary heart disease in women from January 2022 to December 2023 at Chongqing Wansheng Economic and Technological Development Zone People's Hospital were prospective selected,and they were divided into early onset group of 40 cases(≤ 65 years old)and late onset group of 60 cases(>65 years old)based on their age of onset.Another 60 healthy women who underwent physical examinations during the same period to exclude coronary heart disease were selected as the control group.Univariate factor and multiple factor logistic regression analysis were used to identify independent risk factors for early onset coronary heart disease in women.Draw receiver operating characteristic(ROC)curve for the subjects,the efficacy of risk factors in predicting early onset coronary heart disease based on the area under the curve(AUC)of ROC curve were evaluated.Patients were followed up for 1 year to observe the occurrence of major adverse cardiovascular events(MACE).Result:Among 100 fcoronary heart disease in women,the early onset group accounted for 40.00%(40/100).Univariate analysis showed that age,hyperlipidemia history,smoking history,hypertension history,family history,diabetes history,total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C)were related to the early onset coronary heart disease.Multivariate analysis showed that,hyperlipidemia history(OR=4.124,95%CI:2.343-7.217),smoking history(OR=3.564),hypertension(OR=3.253),family history(OR=2.981),diabetes history(OR=2.873)were independent risk factors.ROC curve analysis results showed that joint evaluation had the best predictive value,with AUC of 0.829,which was higher than the AUC of individual evaluation for each factor.The incidence of MACE in the early onset group(45.00%)was significantly higher than that in the late onset group(P<0.05).Conclusion:Early onset coronary heart disease in women in Wansheng District of Chongqing is related to the hyperlipidemia history,smoking,hypertension history,family history and diabetes history.The incidence of MACE in early-onset patients followed up for 1 year is higher than that in late-onset patients.

Objective To investigate the improvement of interleukin-1 receptor associated kinase(IRAK1)/nuclear factor κB(NF-κB)signaling pathway regulated by"srperficiality-origin matching acupoints"electroacupuncture on intestinal damage in diabetic rats and lipid metabolism.Methods SD rats were randomly separated into control group,diabetes group,diabetes+electroacupuncture group,diabetes+pcDNA group,diabetes+pcDNA-IRAK1 group,diabetes+electroacupuncture+pcDNA group,and diabetes+electroacupuncture+pcDNA-IRAK1 group,18 rats in each group.Except for the control group,rats in other groups were fed with high glucose and high fat diet and intraperitoneal injection of streptozotocin(STZ)to construct the model of diabetes rats.After successful modeling,rats in each group were treated every other day for 8 consecutive weeks.The weight,fasting blood glucose,serum triglycerides and total cholesterol levels of rats were detected.16S rRNA amplicon sequencing was applied to detect the relative abundance of rat gut microbiota;the intestinal permeability and water content of rats were tested.Immunohistochemistry was applied to detect the average optical densities of zonula occludens-1(ZO-1)proteins in small intestine tissue.Western blot was applied to detect the expression of IRAK1,phosphorylated nuclear factor inhibitory protein(p-IκBα),and phosphorylated nuclear factor B p65(p-NF-κB p65)proteins in rat small intestine tissue.Results Compared with the control group,the levels of fasting blood glucose,triglyceride,total cholesterol,relative abundances of Firmicutes,Proteobacteria,Actinomycetes,Allobaculum,fluorescein isothiocyanate(FITC)-glucan concentration,intestinal water content and the expression of IRAK1,p-IκBα,p-NF-κB p65 proteins in the diabetes group increased significantly(P<0.05).Weight,the relative abundance of Bacteroidetes,Lactobacillus,Romboutsia,Lachnospiraceae,Ruminocoaceae,the average optical densities of occludin and ZO-1 proteins decreased significantly(P<0.05).Compared with the diabetes group,the levels of fasting blood glucose,triglyceride,total cholesterol,relative abundances of Firmicutes,Proteobacteria,Actinomycetes,Allobaculum,FITC-glucan concentration,intestinal water content and the expression of IRAK1,p-IκBα,p-NF-κB p65 proteins in the diabetes+electroacupuncture group decreased significantly(P<0.05).Weight,the relative abundance of Bacteroidetes,Lactobacillus,Romboutsia,Lachnospiraceae,Ruminocoaceae,the average optical densities of occludin and ZO-1 proteins increased significantly(P<0.05),the change trend of corresponding indicators in the diabetes+pcDNA-IRAK1 group was opposite to the above(P<0.05).Compared with the diabetes+electroacupuncture group and the diabetes+electroacupuncture+pcDNA group,the levels of fasting blood glucose,triglyceride,total cholesterol,relative abundances of Firmicutes,Proteobacteria,Actinomycetes,Allobaculum,FITC-glucan concentration,intestinal water content and the expression of IRAK1,p-IκBα,p-NF-κB p65 proteins in the diabetes+electroacupuncture+pcDNA-IRAK1 group increased significantly(P<0.05).Weight,the relative abundance of Bacteroidetes,Lactobacillus,Romboutsia,Lachnospiraceae,Ruminocoaceae,the average optical densities of occludin and ZO-1 proteins decreased significantly(P<0.05).Conclusion"Srperficiality-origin matching acupoints"electroacupuncture may improve intestinal flora imbalance and intestinal barrier damage in diabetes rats by inhibiting IRAK1/NF-κB signaling pathway,and improve diabetic glucose and lipid metabolism.
9.Outcomes of transcatheter transseptal mitral valve-in-valve replacement using Edward's SAPIEN 3 in high surgical risk patients-a multicenter study in China

Objective To evaluate the safety and efficacy of valve-in-valve transcatheter mitral valve replacement(ViV-TMVR)in patients with bioprosthetic valve degeneration who are at high surgical risk.Methods This study is a multi-center,retrospective cohort analysis of 20 consecutive patients who underwent transseptal ViV-TMVR using the Edwards SAPIEN 3 transcatheter heart valve(THV).The primary endpoints include technical success and procedural success,both defined according to the Mitral Valve Academic Research Consortium(MVARC)criteria,as well as mortality and functional change assessed based on New York Heart Association(NYHA)classification at 30-days and six months post-procedure.Clinical follow-up assessments are conducted at 30-days and six months.Results From February 2021 to October 2022,a total of 20 patients with symptoms of bioprosthetic valve degeneration were enrolled across nine sites in China.The patients had a mean age of(73.5±5.5)years,with 85.0%being females and 70.0%classified as NYHA class Ⅲ/Ⅳ.The study achieved a 100.0%technical success rate and a 90.0%procedural success rate finally.All patients remained alive during the 30-day follow-up period.However,six months post-intervention,two patients(10.0%)were re-hospitalized due to heart failure,and sadly,one of them(5.0%)died.None of the patients reported any adverse events related to ViV-TMVR during the follow-up period.Notably,there was a significant improvement in NYHA class compared to baseline(P=0.0004)at six-month follow-ups.Conclusions The transseptal ViV-TMVR technique proved to be highly successful and was associated with significant improvement in NYHA class function.These findings strongly suggest that it serves as a safe and efficient treatment alternative for high-risk patients suffering from bioprosthetic valve degeneration.

Objective To investigate the improvement of interleukin-1 receptor associated kinase(IRAK1)/nuclear factor κB(NF-κB)signaling pathway regulated by"srperficiality-origin matching acupoints"electroacupuncture on intestinal damage in diabetic rats and lipid metabolism.Methods SD rats were randomly separated into control group,diabetes group,diabetes+electroacupuncture group,diabetes+pcDNA group,diabetes+pcDNA-IRAK1 group,diabetes+electroacupuncture+pcDNA group,and diabetes+electroacupuncture+pcDNA-IRAK1 group,18 rats in each group.Except for the control group,rats in other groups were fed with high glucose and high fat diet and intraperitoneal injection of streptozotocin(STZ)to construct the model of diabetes rats.After successful modeling,rats in each group were treated every other day for 8 consecutive weeks.The weight,fasting blood glucose,serum triglycerides and total cholesterol levels of rats were detected.16S rRNA amplicon sequencing was applied to detect the relative abundance of rat gut microbiota;the intestinal permeability and water content of rats were tested.Immunohistochemistry was applied to detect the average optical densities of zonula occludens-1(ZO-1)proteins in small intestine tissue.Western blot was applied to detect the expression of IRAK1,phosphorylated nuclear factor inhibitory protein(p-IκBα),and phosphorylated nuclear factor B p65(p-NF-κB p65)proteins in rat small intestine tissue.Results Compared with the control group,the levels of fasting blood glucose,triglyceride,total cholesterol,relative abundances of Firmicutes,Proteobacteria,Actinomycetes,Allobaculum,fluorescein isothiocyanate(FITC)-glucan concentration,intestinal water content and the expression of IRAK1,p-IκBα,p-NF-κB p65 proteins in the diabetes group increased significantly(P<0.05).Weight,the relative abundance of Bacteroidetes,Lactobacillus,Romboutsia,Lachnospiraceae,Ruminocoaceae,the average optical densities of occludin and ZO-1 proteins decreased significantly(P<0.05).Compared with the diabetes group,the levels of fasting blood glucose,triglyceride,total cholesterol,relative abundances of Firmicutes,Proteobacteria,Actinomycetes,Allobaculum,FITC-glucan concentration,intestinal water content and the expression of IRAK1,p-IκBα,p-NF-κB p65 proteins in the diabetes+electroacupuncture group decreased significantly(P<0.05).Weight,the relative abundance of Bacteroidetes,Lactobacillus,Romboutsia,Lachnospiraceae,Ruminocoaceae,the average optical densities of occludin and ZO-1 proteins increased significantly(P<0.05),the change trend of corresponding indicators in the diabetes+pcDNA-IRAK1 group was opposite to the above(P<0.05).Compared with the diabetes+electroacupuncture group and the diabetes+electroacupuncture+pcDNA group,the levels of fasting blood glucose,triglyceride,total cholesterol,relative abundances of Firmicutes,Proteobacteria,Actinomycetes,Allobaculum,FITC-glucan concentration,intestinal water content and the expression of IRAK1,p-IκBα,p-NF-κB p65 proteins in the diabetes+electroacupuncture+pcDNA-IRAK1 group increased significantly(P<0.05).Weight,the relative abundance of Bacteroidetes,Lactobacillus,Romboutsia,Lachnospiraceae,Ruminocoaceae,the average optical densities of occludin and ZO-1 proteins decreased significantly(P<0.05).Conclusion"Srperficiality-origin matching acupoints"electroacupuncture may improve intestinal flora imbalance and intestinal barrier damage in diabetes rats by inhibiting IRAK1/NF-κB signaling pathway,and improve diabetic glucose and lipid metabolism.
