1.Advances in treatment of immune-related adverse reactions and hyperprogressive diseases associated with antitumor immunotherapy with PD-1/PD-L1 monoclonal antibody drugs

There have been breakthroughs in the development and clinical application of immuno-therapeutic agents in recent years.As the clinical targets of second-generation immune checkpoint inhibitors(ICIs),programmed death-1(PD-1)and its ligand(programmed death-ligand 1,PD-L1)have become one of the hot spots in drug discovery.The Food and Drug Administration of the USA and National Medical Products Administration in China have approved a variety of PD-1/PD-L1 monoclonal antibody drugs(such as nivolumab,pembrolizumab and atezolizumab),which are marketed for the treatment of small-cell lung cancer,rectal cancer,colon cancer,and melanoma,among other diseases.However,in the clinical application of PD-1/PD-L1 monoclonal antibody drugs,it is found that they can cause different degrees of immune-related adverse reactions in the lung,liver,kidney,gastrointestinal system,endocrine system and skin,and even the emergence of hyperprogressive disease.Effective monitoring and management of clinical applications of PD-1/PD-L1 monoclonal antibody drugs and rational use of some drugs can improve the immunotherapy process and reduce the effects of adverse reactions and hyperprogressive diseases in patients under immunotherapy.
2.Investigation on the incidence of chronic diseases among the seafarers of Yantai port and analysis of related factors

Objective To investigate the health status of the seafarers in Yantai port,and to analyze the influential factors of chronic diseases,so as to provide reliable evidence for the development of related prevention and treatment measures.Methods Medical check-up data of 10 200 seafarers from our hospital were random selected for the study,and the collected data were classified and analyzed.Results The morbidity rate of chronic diseases among the seafarers in Yantai port was 34.62%.Statistical analyses indicated that hypertension,hyperlipidemia,coronary heart disease,fatty liver,diabetes,cholecystitis,depression and other chronic diseases were the main disorders that affected the health of the seafarers.Conclusions Greater efforts should be made to strengthen health education and mental consultation,enhance the 3-level prevention and health care,and have regular health examinations,with the aim to achieve early detection,early diagnosis and early treatment,so that the health of the seafarers could be better assured.
3.Clinical observation on the efficacy of acupuncture for relieving the depressed liver and regulating qi in the treatment of depression among seafarers

Objective To observe the therapeutic efficacy of acupuncture for relieving the depressed liver and regulating qi in the treatment of depression among seafarers.Methods A randomized controlled trial (RCT) was applied in the study.Sixty seafarers with depression were randomly assigned as the acupuncture group and the western medicine group.After 2 treatment courses,observations and comparisons were made between the 2 groups.Results Following treatment,the SDS scores(the acupuncture group:38.50 ± 8.72,the western medicine group:45.75 ± 9.42) and HAMD scores of both groups all decreased significantly,and significant differences could be seen,as compared with those before treatment(P < 0.01).There was statistical significance,when comparisons were made between the 2 groups (P < 0.01).The therapeutic efficacy of the acupuncture group was obviously superior to that of the western medicine group.Conclusions The acupuncture for relieving the depressed liver and regulating qi could noticeably improve symptoms of depression among seafarers,and its therapeutic efficacy was better than that of the western medicine.

6.Efficacy and safety of compound azintamide on dyspepsia symptoms in a multicentre self-controlled trial

Objective To investigate the efficacy and safety of compound azintamide on dyspepsia symptoms. Methods One hundred and eighty dyspepsia patients were divided into two groups according to dyspepsia symptom related with gastrointestinal disease ( group A) or biliary system disease (group B),whose dyspepsia symptom were not improved by the Domperidone 10 mg tid for 2 weeks. Two tablets of compound azintamide were administered orally following a meal, tid for 2 weeks. The changes of symptoms score of upper abdominal distention, upper abdominal pain or discomfort anorexia and effective rate as well as adverse events were recorded. Results Compound azintamide greatly improved the symptoms of upper abdominal distention, upper abdominal pain or discomfort and anorexia. All symptoms scores were significantly decreased after 2 weeks of compound azintamide (P < 0.01). The effective rate of each symptom and total symptoms score were more than 84.9% and 92.5%. One patient reported mild rash at the fourteenth days, which disappeared 3 days later. Conclusion Compound azintamide showed effective and safety in treatment of patient with dyspepsia symptoms when Domperidone therapy is not satisfactory.
