Objective This study aimed to investigate the antimicrobial resistance profiles of bacterial strains isolated from pediatric intensive care units(PICU)in China for better antimicrobial therapy.Methods Clinical isolates were collected from 17 institutions,including tertiary care children's hospitals and pediatric department of tertiary general hospitals in China from January 1,2020 to December 31,2022.Antimicrobial susceptibility testing was carried out according to a unified protocol using Kirby-Bauer method or automated systems.Results were interpreted according to the breakpoints released by the Clinical and Laboratory Standards Institute(CLSI)in 2020.Results A total of 10 688 isolates were collected,including gram-positive organisms(39.2%)and gram-negative organisms(60.8%).The top three organisms were S.aureus(13.6%,1 453/10 688),A.baumannii(10.0%,1 067/10 688),and coagulase-negative Staphylococcus(9.9%,1 058/10 688).Multi-drug resistant organisms(MDROs)were very common in children.The prevalence of methicillin-resistant Staphylococcus aureus(MRSA),carbapenem-resistant Enterobacterales(CRE),carbapenem-resistant E.coli,carbapenem-resistant K.pneumoniae(CRKP),carbapenem-resistant A.baumannii(CRAB),and carbapenem-resistant P.aeruginosa(CRPA)was 41.1%,19.4%,8.8%,30.9%,67.4%,and 28.8%,respectively.Overall,more than 50%of Enterobacteriales isolates were resistant to cephalosporins,while nearly 25%of Enterobacteriales isolates were resistant to carbapenems.MDROs were highly resistant to commonly used antibiotics.More than 80%of CRE and CRAB strains were resistant to all beta-lactam antibiotics.CRE and CRAB showed low resistance rates to tigecycline and polymyxin.CRPA showed lower resistance rates to piperacillin,beta-lactamase inhibitor combinations than the resistance rates to third and fourth generation cephalosporins.All of the Staphylococcus and Enterococcus isolates were susceptible to vancomycin and tigecycline.None of PRSP strains isolated from meningitis and nonmeningitis samples were resistant to rifampicin,vancomycin,or linezolid.The prevalence of β-lactamase-negative ampicillin-resistant(BLNAR)strains was 43.3%in Haemophilus influenzae.Conclusions MDROs were prevalent in PICU.It is necessary to establish an effective multidisciplinary team(MDT)to control the antimicrobial resistance.

3.Effects of a nursing care plan decision-making support system in the training management of newly hired nurses

Objective:To build a nursing care plan decision-making support system based on the hospital's nursing information system and evaluate its effects in newly hired nurses.Methods:This study adopted a non-synchronous pre-post control design. Totally 181 newly hired nurses recruited by Beijing Tiantan Hospital, Capital Medical University, were selected by convenience sampling. The control group consisted of 85 new nurses hired in August 2021, while the intervention group consisted of 96 new nurses hired in August 2022. The control group received traditional training methods, while the intervention group received training through the nursing care plan decision-making support system in addition to the traditional methods. The nursing decision-making ability and the quality of specialty nursing care were compared between the two groups.Results:Finally, 81 nurses were included in the control group, and 93 nurses were included in the intervention group. The nursing decision-making ability score in the intervention group was (80.15±4.59), which was significantly higher than the (78.62±5.13) in the control group ( t=-2.080, P=0.039). The intervention group also had a significantly higher qualified rate of specialty nursing care compared to the control group (χ 2=6.626, P=0.036) . Conclusions:The application of the nursing care plan decision-making support system in the standardized training of newly hired nurses can improve their nursing decision-making abilities and the quality of specialty nursing care.
4.Analysis and prediction of albinism co-phenotypes based on co-occurrence network

Objective To analyze the phenotypic characteristics of albinism and their correlations. Methods Literatures related to albinism in PubMed database before June 10, 2023 were obtained, and phenotypic entities were extracted from their abstract texts to construct a phenotypic co-occurrence network. GePhi and VOSviewer software were used to analyze the overall characteristics and phenotypic clustering of the network. The Apriori algorithm was used to mine the association rules between phenotypes. The AA index was used for link prediction, predicting possible combinations of albino phenotypes. Results The phenotypic co-occurrence network of albinism had a small-world characteristic, and nystagmus, hypopigmentation of skin and foveal dysplasia of retina were the main phenotypes. Phenotypic abnormalities of albinism were mainly divided into five categories, including visual system abnormalities, immune system abnormalities, skin and hair system abnormalities, nervous system abnormalities, and respiratory as well as digestive system abnormalities. A variety of ocular abnormalities were associated with nystagmus. The combination of abnormal phenotypes that may occur in albinism included pneumonia with hypopigmentation of the skin, foveal dysplasia of the retina and atrophy of the optic nerve. Conclusion Predicting and analyzing the phenotypic characteristics of albinism and the associative patterns among phenotypes, as well as the potential co-occurrence of phenotypes in albinism patients can provide valuable references for identifying research directions for albinism and for the recognition, diagnosis, and prediction of albinism development.
5.Establishment and application of an electronic transfer system for emergency surgery patients

Objective? To explore the effects of the electronic transfer system on emergency surgery patients. Methods? The electronic transfer system for emergency surgery patients were established. Totally 98 patients who were transferred from the emergency service into the Operation Room using the electronic surgery system were included into the observation group from January to December 2017, while 88 patients who were transferred from the emergency service into the Operation Room using oral and printed transfer records were included into the control group form January to December 2016. The success rate of transfer, patient's and nurse's satisfaction were compared between the two groups. Results? The success rate of transfer using the electronic transfer system for emergency surgery patients was 95.92%, while that of the control group was 77.27% (χ2=14.343,P< 0.05). Patient's satisfaction of the observation group was 92.86%, while that of the control group was 59.09% (χ2=29.743,P< 0.05). The nurse's satisfaction of the observation group was 95.92%, while that of the control group was 87.50%(χ2=4.432,P<0.05). Conclusions? The establishment of the electronic transfer system for emergency surgery patients further clarifies the content of transfer, enhances nurses' responsibility and shortens the time of transfer, which plays a preventative and control role in risk management and safety management in emergency surgery patients.
6.Identification and pedigree analysis of ABO subgroup B303.

OBJECTIVETo explore the molecular mechanism of a case of ABO discrepancies based on the results of blood group serology.
METHODSFive cases of the two-generation pedigrees were analyzed. ABO genotypes were determined using serological tests. DNA sequence analysis was performed on exon 6, exon 7 and intron 3 of the 5 cases to confirm the genotypes of a proband with B subgroup and 4 family members.
RESULTSThere were 3 cases of subgroup AB3 and 1 case of subgroup B3 among the 5 family members. The genotypes were identified as A102/B303 and O02/B303, respectively. B303 differed from B101 by intron 3 point mutation (intron3 + 5G>A).
CONCLUSIONThe point mutation of intron 3 (intron 3+5G>A) is specific in B303.
7.Differential expression of ADD3 splicing isoforms between colorectal cancer and normal mucosa tissues

AIM:To investigate the relationship between the expression of adducin 3 (ADD3) and its splicing isoforms and colorectal cancer (CRC).METHODS:The expression of ADD3, ADD3-Ia and ADD3-Ib in 50 pair of CRC tissues , 20 pairs of colorectal polyp tissues , and 2 CRC cell lines SW480 and SW620 before and after oxaliplatin or fluoroura-cil intervention were detected by real-time PCR.The cell activity was determined by MTT assay , the cell migration ability was evaluated by wound-healing assay , and the cell invasion ability was measured by Transwell assay .RESULTS:The expres-sion levels of ADD3 and ADD3-Ib were decreased in the CRC tissues as compared with the normal mucous (P<0.01), and ADD3-Ia/Ib ratio was increased in the CRC tissues (P<0.01).The expression level of ADD3-Ia was higher in T3-4 group than that in T1-2 group (P<0.05).Reduced expression of ADD3, ADD3-Ia and ADD3-Ib in colorectal polyps was observed compared with the normal tissues (P<0.01).Compared with the SW480 cells, the expression levels of ADD3-Ia and ADD3-Ib were lower (P<0.05) and the ADD3-Ia/Ib ratio was higher (P <0.01) in the SW620 cells.After treated with oxalipla-tin or fluorouracil, the cell activity, migration and invasion in the SW620 and SW480 cells were weakened accompanied by the increases in the expression levels of ADD 3, ADD3-Ia and ADD3-Ib to various certain extents .CONCLUSION:In CRC there is a tendency that ADD3-Ib reduction leads to ADD3 decrease, accompanied by an increased ADD3-Ia/Ib ratio.The expression changes of ADD 3 and its splicing isoforms in the CRC may be relevant to its invasion ability .
8.Relationship between plasma D-dimer level and diagnosis of pulmonary embolism

Objective:To explore the relationship among clinical manifestations ,SⅠ QⅢ TⅢ feature of ECG ,plasma level of D‐dimer (DD) and diagnosis of pulmonary embolism (PE) .Methods :Clinical data of 212 inpatients ,who received pulmonary CT angiography (CTA) in our hospital from Jun 2012 to May 2014 ,were retrospectively ana‐lyzed .According to pulmonary CTA results ,patients were divided into PE group (n=56) and non‐PE group (n=156) .Basic hospitalization data ,including clinical manifestations ,ECG features and plasma DD level ,were collect‐ed and compared between two groups .Results:Compared with non‐PE group ,there were significant rise in percent‐ages of dyspnea (44.87% vs .75% ) and prolonged bedridden time (3.85% vs .14.29% ) ,significant reduction in percentage of no clinical manifestations (38.46% vs .3.57% ) in PE group , P<0.01 all .Percentage of ECG SⅠQⅢTⅢ feature in PE group was significantly higher than that of non‐PE group (50% vs .23.08% ) , P<0.01. Compared with non‐PE group ,percentage of plasma DD>10μg/ml significantly rose (19.23% vs .32.14% ) in PE group ,P<0.05 .Conclusion:Patients with dyspnea and/or prolonged bedridden time ,that cannot be explained by other car‐diopulmonary diseases ,and SⅠ QⅢ TⅢ feature of ECG ;plasma DD level significant rising (> 10 μg/ml) should be considered to be PE .
