1.Effect of hyperbaric oxygen combined with Xingnaojing injection on 53 patients with delayed encephalopathy caused by acute carbon monoxide poisoning

Objective:To observe the effects of hyperbaric oxygen combined with Xingnaojing injection on serum nerve injury,serum factors,and cognitive function in patients with acute carbon monoxide poisoning delayed encephalopathy(ACMPDE).Methods:A total of 106 cases with ACMPDE who were admitted to the First People’s Hospital of Nanning from July 2018 to July 2020 were selected as the research subjects and divided into control group and research group according to the random number table method,with 53 cases in each group. The control group was given hyperbaric oxygen treatment for 20 days,while the research group was given Xingnaojing injection on the basis of the treatment in the control group for 20 days. The levels of hypersensitive C-reactive protein(hs-CRP),interleukin-6(IL-6),interleukin-10(IL-10),tumor necrosis factor - α(TNF - α),neuron specific enolase(NSE),5-hydroxytryptamine(5-HT),angiopoietin-2(Ang-2),inducible nitric oxide synthase(iNOS),malondialdehyde(MDA),superoxide dismutase(SOD)in the two groups were detected before and after treatment. The Montreal Cognitive Assessment Scale(MoCA),Mini-Mental State Examination(MMSE)and 36-Item Short Form Health Survey(SF-36)were used to evaluate the cognitive function and quality of life of the two groups of patients and to compare the clinical efficacy of the two groups after treatment.Results:The levels of hs CRP,TNF - α and IL-6 in the research group were lower than those in the control group( P < 0.01),the level of IL-10 in the research group was higher than that in the control group( P < 0.01),the level of 5-HT in the research group was higher than that in the control group( P < 0.01),the levels of NSE,Ang-2,and iNOS in the research group were lower than those in the control group( P < 0.01),the level of SOD in the research group was higher than that in the control group( P < 0.01),and the level of MDA in the research group was lower than that in the control group( P < 0.01).The MMSE,MOCA and SF-36 scores of the research group were higher than those of the control group( P < 0.01). After treatment,the total effective rate of the research group(98.11%)was significantly higher than that of the control group(84.91%),and the difference was statistically significant(χ 2 = 4.371, P = 0.037). Conclusion:Hyperbaric oxygen combined with Xingnaojing injection can improve the level of serum factors in patients with ACMPDE,inhibit inflammatory response,alleviate nerve injury,reduce oxidative stress response,and improve the cognitive function and the quality of life of patients,which is worthy of clinical application.

3.Multivariate analysis of long-term outcomes of storage symptom improvement in elderly patients with benign prostatic hyperplasia after GreenLight laser vaporization

Objective To investigate the relationship between preoperative urodynamic parameters and the improvement of overactive bladder (OAB)symptoms after GreenLight laser vaporization,and to explore prognostic factors for improvement of OAB symptoms in the elderly.Methods A retrospective study was conducted in 100 benign prostatic hyperplasia (BPH)patients undergoing GreenLight laser vaporization at the Department of Urology of Beijing Hospital from July 2015 to March 2017.All patients completed a preoperative urodynamic examination and received GreenLight laser vaporization.Clinical data including age,prostate-specific antigen (PSA),prostate volume,international prognostic scoring system(IPSS),overactive bladder symptom score (OABSS),quality of life(QOL)and urodynamic parameters were collected.The related factors for improvement of OAB symptoms after GreenLight laser vaporization were analyzed by a binary Logistic regression analysis.Results All patients underwent surgery successfully and completed a 12-month follow-up.Both urinary storage and voiding symptoms improved at 3 and 12 months after GreenLight laser vaporization(P<0.05).The scores of IPSS,IPSS storage (IPSS-S),IPSS voiding (IPSS-V),OABSS and QOL and nighttime voiding frequency decreased and urinary storage and voiding symptoms improved at 3 and 12 months after GreenLight laser vaporization,compared with pre-surgery data(P<0.05).The success rates of storage symptom improvement at 3 and 12 months after GreenLight laser vaporization were 62.0 % (62/100) and 68.0 % (68/100) evaluated by IPSS-S and 68.0 % (68/100) and 75.0% (75/100)by OABSS,respectively.Multiple Logistic regression analysis showed that age,detrusor contractility,residual urine volume and nighttime voiding frequency were independent influencing factors for prognosis(OR =35.714,0.352,0.110 and 0.040,P =0.000,0.027,0.018 and 0.002).Conclusions GreenLight laser vaporization is an effective method in treating BPH with OAB.Age is an independent unfavorable factor and the residual urine volume,nighttime voiding frequency and detrusor contraction are independent influencing factors for prognosis.Enough attention should be paid to these related parameters before surgery.

In recent years,we have progressed in the treatment of coronary heart disease by a large margin,which improved the prognostic condition of coronary heart disease patients,conspicuously.Whereas,we have still harbored discordant reservations about what fuels in the theraputic stragety of the left main and/or three vessel disease which causes acute myocardial infarction and the sudden cardiac death,for the time being,internationally.EuroSCORE and SYNTAX Score are both widely accepted in probing in the theraputic stragetic credits,internationally.Hence,we are about to review the twin credits system on the optional directive meaning of the strategy of myocardial revasscularization in the left main and/or three vessel patients.
