By reconstructing the integrated Chinese and western medicine diagnostic and treatment system, the "Two Reconstructions" theoretical framework establishes a standardized pathway of "classification-staging-syndrome differentiation", which improves the accuracy of disease identification and strengthens the capacity for full-course intervention; in addition, by reconstructing the modern materia medica system, it innovatively integrates the traditional properties and efficacy of Chinese herbal medicinals with modern pharmacological mechanisms, forming a "state-target co-regulation" precise medication model, and builds a dose-effect theoretical system for prescriptions and medicinals, thereby enhancing both the targeting accuracy and dosage precision of therapeutic interventions. The "Two Reconstructions" theorecitcal framework is a key strategy for enhancing clinical efficacy. It can precisely identify "states" and "targets" for directed intervention, shift the focus of prevention and treatment earlier to enable full-cycle management, establish standardized paradigms for reproducible and evaluable efficacy, and expand the scope of clinical practice to address conditions without typical syndromes and critical illnesses. As a systematic pathway for innovation in TCM, this theoretical framework provides valuable insights and references for promoting the high-quality development of integrative Chinese and western medicine.

Objective:To analyze the correlation between thymus dose-volume parameters and lymphopenia in patients with early-stage breast cancer (BC) receiving adjuvant radiotherapy (RT).Methods:Medical records of 54 patients with early-stage BC who received postoperative adjuvant RT in the Second Affiliated Hospital of Soochow University from January to December 2019 were retrospectively analyzed. Absolute lymphocyte counts (ALC) were collected at 1 month before (baseline) and weekly during RT. Lymphopenia was graded based according to the common terminology criteria for adverse events version 5.0 and nadir/baseline ALC was calculated. The thymus was delineated according to anatomical boundaries in the original RT planning system. Dosimetric parameters were obtained from the dose volume histograms. Stepwise multiple linear regression analysis was used to explore the factors associated with nadir/baseline ALC. The cutoff values of dosimetric parameters for predicting ≥grade 3 lymphopenia were obtained using the receiver operating characteristic (ROC) curve. Results:The proportion of 54 patients experiencing ≥ grade 3 lymphopenia was 38.9%. The median value of thymus volume, mean dose, V 5 Gy, V 10 Gy were 14.02 cm 3, 4.95 Gy, 36.18%, and 6.61%, respectively. Stepwise multiple linear regression analysis revealed that baseline ALC ( P=0.005), quadrant location ( P=0.005) and mean thymus dose ( P<0.001) were significantly associated with nadir/baseline ALC. ROC curve analysis indicated that the cutoff values of thymus mean dose, V 5 Gy and V 10 Gy for predicting ≥ grade 3 lymphopenia were 6.12 Gy, 35.2%, and 7.4%, respectively. Conclusions:Lymphopenia in early-stage BC patients is significantly correlated with high dosimetric parameters of the thymus during postoperative adjuvant RT. Thymus may be considered as an organ at risk during RT.

Objective To explore the correlation between single-leg standing posture control and ankle plantar flexor muscle stability,so as to provide a new theoretical basis for improving the ability of human posture control. Methods A total of 20 healthy male college students were randomly selected as the experimental subjects. The iBalance tester and training system was used to test the trajectory data of the center of pressure(COP)of the foot standing on one leg.The CON-TREX MJ multi-joint isokinetic test and training system was used to test the moment amplitude data during the contraction of ankle plantar flexor muscles.Univariate repeated measures variance analysis was used to analyze the standard deviation data of ankle plantar flexor moment amplitude between groups. The Pearson correlation coefficient was used to study the correlation. Results The greater the intensity of the muscle stabilization task performed by the ankle plantar flexor muscle, the greater the standard deviation of the moment amplitude. The C90 area was positively correlated with the coefficient of variation(CV)of the 10%maximum voluntary contraction(MVC)moment of ankleplantar flexor muscle(r=0.761,P<0.05)during single-leg standing without interference. The C90 area was positively correlated with the CV(r=0.632,P<0.05)of the 30%MVC moment of ankle plantar flexor muscle during single-leg standing. When the proprioception was interfered during single-leg standing, the C90 area was positively correlated with the CV(r=0.583,P<0.05)of the 20%MVC moment amplitude of ankleplantar flexor muscle. Conclusions With the increasing difficulty of muscle strength stabilization performed by the ankle plantar flexor muscles, muscle stability decreases. There is a positive correlation between ankle plantar flexor strength stability and single-leg standing posture control.Compared with the case without interference, under visual and proprioceptive interference,an additional information input is reduced or disturbed, and it is more difficult to maintain body balance, and the ankle plantar flexor muscle needs a higher muscle stability in the force mode to participate in the posture control of the human body during single-leg standing.
