Diabetic cataract, a prevalent ocular complication of diabetes mellitus, arises from a complex interplay of pathological mechanisms, with oxidative stress and glycation stress playing central roles. Autophagy, a critical cellular self-protection mechanism, sustains intracellular homeostasis by selectively degrading damaged organelles and misfolded proteins, thereby counteracting the detrimental effects of oxidative and glycation stress under hyperglycemic conditions. Emerging evidence indicates a synergistic interaction between glycation stress and oxidative stress, which may exacerbate autophagic dysfunction and accelerate the onset and progression of diabetic cataract. However, the precise molecular mechanisms underlying this relationship remain incompletely understood. This review systematically examines the regulatory role of autophagy inthe pathogenesis of diabetic cataract, with a particular focus on how autophagic impairment influences disease progression under the combined effects of glycation and oxidative stress. By elucidating these mechanisms, the paper aims to provide novel insights into molecular diagnostic approaches and targeted therapeutic strategies for diabetic cataract.

Objective To observe the value of artificial intelligence iterative reconstruction(AIIR)for improving image quality of 80 kVp low-dose CT enterography(CTE)for Crohn disease(CD).Methods Totally 59 patients with CD who underwent 80 kVp low-dose CTE were retrospectively enrolled,and CTE images were reconstructed with hybrid iterative reconstruction(HIR)(HIR group)and AIIR(AIIR group),respectively.Then subjective and objective scores of image quality were evaluated and compared between groups,and the value of AIIR was analyzed.Results Compared with HIR group,AIIR group clearly displayed the intestinal wall,intestinal lumen,mesenteric vessels and peri-intestinal soft tissue,and displayed typical CD signs more obviously,with higher subjective scores of imaging quality(all P＜0.001).The standard deviation(SD)was lower,while signal-to-noise ratio(SNR)of intestinal walls without disease,SNR and contrast-to-noise ratio(CNR)of diseased intestinal walls in AIIR group were all higher than those in HIR group(all P＜0.001).The effective dose of 80 kVp CTE examination was(2.82±0.54)mSv.Conclusion AIIR was helpful for improving image quality of 80 kVp low-dose CTE for CD.

Obesity, as a chronic recurrent disease, has become a major public health challenge in China. To implement the requirements of the Healthy China Initiative (2019—2030), under domestic guidelines or consensus statements on overweight and obesity, and in alignment with the latest scientific advances globally, the Quality control protocol for adult overweight and obesity screening in health management (examination) institutions (2025 edition) was developed. This protocol was drafted by the Health Management Center of Shanghai Changzheng Hospital and formulated through multiple rounds of deliberation by experts in China’s health examination quality control field. The protocol establishes unified standards for screening facilities, personnel qualifications, and measurement or testing procedures. It defines specific screening items, outlines a standardized screening pathway, and sets requirements for the final medical review, ensuring the scientific validity, effectiveness, and safety of the screening process. The implementation of this protocol will enhance the consistency of weight management practices for adults across health examination institutions and strengthen the quality control of overweight and obesity screening programs.

OBJECTIVE To investigate the improvement effects and potential mechanism of ulinastatin(UTI)on cisplatin(CP)-induced myocardial injury.METHODS Rabbits were used as study subjects and randomly divided into blank group(normal saline 2 mL/d,for consecutive 7 d),CP group(150 mg/m2,on the 1st and 4th day),UTI low-dose/high-dose group(UTIL/UTIH group,UTL 25 000 or 50 000 IU/kg,once a day,for consecutive 7 d),c-Jun N-terminal kinase(JNK)inhibitor group(JNKi group,SP600125 15 mg/kg,on the 1st and 4th day+CP 150 mg/m2,on the 1st and 4th day+normal saline 2 mL/d,on the other 5 days).Except for the blank group,rabbits in the other groups were injected with relevant medicine and/or normal saline via a marginal ear vein on one side.The serum level of cardiac troponin Ⅰ(cTnⅠ)in rabbits on the 1st and 8th days of the detection experiment,as well as the levels of superoxide dismutase(SOD),glutathione(GSH)and malondialdehyde(MDA)in myocardial tissue on the 8th day of the experiment,were all measured in each group.The pathological changes and myocardial cell apoptosis in myocardial tissue were observed,and the protein expressions of B-cell lymphoma-2(Bcl-2),Bcl-2 associated X protein(Bax),JNK,phosphorylated JNK(P-JNK),mitochondrial fission factor(Mff),and dynamin-related protein 1(Drp1)in myocardial tissue were all determined.RESULTS Compared with the blank group,the CP group showed significant myocardial injury.The cell apoptotic rate,the levels of cTnⅠ in serum and MDA in myocardial tissue,as well as protein expressions of Bax,JNK,p-JNK,Mff,and Drp1,were all significantly increased or up-regulated,SOD level and protein expression of Bcl-2 significantly reduced or down+regulated(P＜0.05).Compared with the CP group,the myocardial injury in rabbits from each drug treatment group showed improvement.The cell apoptosis index,the levels of cTnⅠ in serum and MDA in myocardial tissue,as well as protein expressions of Bax,JNK,p-JNK,Mff(except for UTIL group),and Drp1,were all significantly reduced or down-regulated,while SOD,GSH level and protein expression of Bcl-2 significantly increased or up-regulated(P＜0.05);moreover,the improvement in some indicators was significantly better in the UTIH group and the JNKi group compared to the UTIL group(P＜0.05).CONCLUSIONS UTI may ameliorate CP-induced myocardial injury,the potential mechanism of which may be associated with antagonizing oxidative stress and inhibiting the JNK/Mff signaling pathway.

Objective To investigate the effects of knowledge-attitude-practice(KAP)model based nursing interventions on positional care in patients after vitrectomy combined with intraocular filling.Methods A total of 62 patients who underwent vitrectomy combined with intraocular silicone filling,between February 2022 and November 2023,in the Department of Ophthalmology of a Grade ⅢA hospital in Tongling were selected as the study subjects.The patients were randomly divided into a control group(n=32)and an intervention group(n=32).The control group received routine nursing care,while the intervention group received KAP-model based nursing care.The two groups were compared in terms of the time of positional maintenance,the average time per day for maintaining a head-down face-down position in the first 3 days after surgery,general comfort questionnaire(GCQ),postural compliance,numerical rating scale(NRS)for pains,self-rating anxiety scale(SAS)and incidence of postoperative complications.Results In comparison with the control group,the intervention group had a longer positional maintenance time and longer average time per day for maintaining a head-down face-down position in the first 3 days after surgery(13.25±1.25 vs 15.86±1.36,20.87±1.04 vs 17.56±1.29,both P<0.001).The overall postural compliance rate in the intervention group was significantly higher(96.88％vs 76.67％,P<0.001).GCQ scores of the intervention group were higher(87.96±5.15 vs 70.24±4.89,P<0.001).NRS scores in the intervention group were significantly lower(2.41±0.26 vs 3.34±0.34,P<0.001).SAS scores in the intervention group were significantly lower(37.26±4.21 vs 44.37±4.71,P<0.001).The total incidence of postoperative complications in the intervention group was lower(3.12％vs 23.33％,P<0.05).Conclusion After vitrectomy combined with intraocular silicone filling,the implementation KAP-model based nursing care interventions prolongs the time of positional maintenance in the patients,improves positional comfort and compliance,reduces pains and relieve anxiety,as well as reduces the incidence of postoperative complications.

Objective To investigate the expression of FBXO43in various cancers and its relationship with immune cell infiltration and prognosis using bioinformatics.Methods Gene expression data for 33 cancers were obtained from The Cancer Genome Atlas(TCGA)and Genotype-Tissue Expression(GTEx)databases to analyze FBXO43expression.Clinical and survival data were sourced from the TCGA database.Gene Expression Profiling Interactive Analysis(GEPIA)was used to assess the correlation between FBXO43expression and the clinical stage.Kaplan-Meier survival analysis was used to evaluate the relationship between FBXO43expression and prognosis.R lan-guage was used to analyze the associations between FBXO43expression and clinical stages,immune cell infiltration,immune checkpoint genes,tumor mutation burden(TMB),microsatellite instability(MSI),and mismatch repair(MMR)genes.The potential biological mech-anisms of FBXO43were explored using gene set enrichment analysis(GSEA).Moreover,qRT-PCR was performed to measure FBXO43 expression in normal gastric mucosal cells(GES-1),gastric cancer cells(HGC-27,MGC-803,and MKN-45),normal liver cells(LO-2),and liver cancer cells(SMMC-7721,HEPG2,HuH7,and MHCC97-H).Results Combined TCGA and GTEx database statistics revealed higher FBXO43expression in 26 types of cancer tissues,including adrenocortical carcinoma(ACC),bladder urothelial carcinoma,breast invasive carcinoma,cervical squamous cell carcinoma and endocervical adenocarcinoma,cholangiocarcinoma,colon adenocarcinoma,and diffuse large B-cell lymphoma,than in normal tissues(P<0.05).However,FBXO43expression was lower in three types of cancer tissues:kidney chromophobe(KICH),testicular germ cell tumor(TGCT),and thyroid carcinoma(P<0.05).GEPIA data analysis showed that FBXO43expression positively correlated with the clinical stages of ACC,KICH,kidney renal clear cell carcinoma(KIRC),and kidney renal papillary cell carcinoma(KIRP)and negatively correlated with the clinical stages of ovarian serous cystadenocarcinoma(OV)and TGCT(P<0.05).Kaplan-Meier survival analysis indicated that abnormal FBXO43expression was associated with the prognosis of various cancers(P<0.05).Specifically,high FBXO43expression was a risk factor for ACC,KICH,KIRC,KIRP,low-grade glioma,liver hepato-cellular carcinoma,mesothelioma,and sarcoma,but protective in thymoma(THYM).The XCELL algorithm found that FBXO43expres-sion was negatively correlated with immune scores in nine types of cancer tissues,including glioblastoma multiforme,KIRP,acute myeloid leukemia,lung squamous cell carcinoma,and OV,and closely related to immune cell infiltration levels in 24 types of cancer tissues,espe-cially showing a significant negative correlation with most immune cells in SARC(P<0.01).Correlation analysis revealed a significant association between FBXO43and TMB,MSI,and MMR in all cancers.GSEA analysis indicated that FBXO43is involved in the cell cycle and immune-related functions in various tumors.qRT-PCR results showed that FBXO43expression was upregulated in liver cancer cells and downregulated in gastric cancer cells(all P<0.05).Conclusion Abnormal FBXO43expression is closely associated with the occur-rence and progression of multiple cancers.Thus,FBXO43may serve as a novel marker of immune cell infiltration and prognosis,thereby offering new directions for targeted cancer therapy.

ObjectiveTo investigate the value of baseline red cell distribution width （RDW） and alkaline phosphatase （ALP） level after ursodeoxycholic acid （UDCA） treatment for one month in predicting the response to UDCA treatment in patients with primary biliary cholangitis （PBC）. MethodsA retrospective analysis was performed for the data of 127 patients with PBC who were diagnosed in Department of Hepatology， The Third People’s Hospital of Jiangsu University， from January 2015 to July 2022， with data collected at baseline， after one month of treatment， and after one year of follow-up. Based on the Paris-I criteria， the patients were divided into good response group and poor response group， and the two groups were analyzed in terms of clinical and laboratory features and their association with response to UDCA. The Logistic regression method was used to investigate the independent risk factors for response to UDCA treatment. The area under the ROC curve （AUC） was used to determine the optimal cut-off values of related indicators； the patients were divided into two groups based on such values， and the two groups were compared in terms of baseline indicators and response. The independent-samples t test was used for comparison of normally distributed continuous data between two groups， and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups； the chi-square test was used for comparison of categorical data between two groups. ResultsCompared with the good response group， the poor response group had significantly higher levels of total bilirubin， aspartate aminotransferase/alanine aminotransferase， ALP， RDW， and RDW-CV at baseline and a significantly higher level of ALP after one month of UDCA treatment （Z=-4.792， -3.697， -2.399， -4.102， -3.220， and -4.236， all P<0.05）. Compared with the good response group， the poor response group had significantly lower levels of albumin， hemoglobin， lymphocytes， hematocrit， and body mass index at baseline （Z=-3.592， -3.603， -2.602， -3.829， -2.432， all P<0.05）， as well as significantly lower levels of prealbumin， albumin/globulin ratio， apolipoprotein A， and free triiodothyronine at baseline （t=4.530， 3.402， 3.485， and 3.639， all P<0.001）. Compared with the poor response group， the good response group had a significantly lower proportion of patients with liver cirrhosis， gallstones/cholecystitis， or anemia （χ²=20.815， 3.892， and 12.283， all P<0.05）. Baseline RDW （odds ratio ［OR］=1.157， 95% confidence interval ［CI］： 1.028‍ — ‍1.301， P=0.015） and ALP level after one month of treatment （OR=1.012， 95%CI： 1.005‍ — ‍1.020， P=0.002） were independent risk factors for response to UDCA， with an AUC of 0.713 and 0.720， respectively. The patients with baseline RDW≥upper limit of normal （ULN） and ALP≥2.2×ULN after one month of UDCA treatment had a lower UDCA response rate （42.6% vs 8.2%， χ²=20.813， P<0.001）. ConclusionPatients with baseline RDW≥ULN and ALP≥2.2×ULN after one month of UDCA treatment tend to have a low biochemical response rate to UDCA.

Objective:To investigate the hemodynamic characteristics of transverse sinus with sigmoid sinus wall dehiscence (SSWD) of pulsatile tinnitus (PT) based on 4D flow MRI.Methods:Retrospective analysis was performed on all patients admitted to Beijing Friendship Hospital, Capital Medical University from January 2019 to January 2021 for dehiscent sigmoid plate pulsatile tinnitus. A total of 26 patients (sides) who met the criteria and underwent 4D flow MRI were included. A total of 26 subjects (46 sides), matched 1∶1 according to gender and age, were included in the normal healthy control group. Nonparametric rank sum test, Student′s t test, and ANOVA were performed by SPSS 19.0 software. Binary Logistic regression was applied to the data with statistical significance. Results:There were more patients with dominant drainage on the affected side in PT group than in control group (73.1% vs. 42.3%). The incidence of transverse with a focal intraluminal filling defect and tapered stenosis was higher than that in control group (21.7% vs. 69.2%; 17.4% vs. 42.3%). Average through-plane velocity and maximum through-plane velocity in PT group were higher than those in control group [(33.75±13.88) cm/s vs. (15.84±7.21) cm/s; (93.19±33.55) cm/s vs. (40.40±14.40) cm/s]. The middle part and proximal end of Flow avg (ml/s) in PT group were larger than those in control group [4.69 (2.87; 5.62) ml/s vs. 2.76 (1.67; 4.99) ml/s; 3.41 (2.16; 5.47) ml/s vs. 2.67 (1.68; 4.41) ml/s]. In control group, the velocity of transverse sinus changed relatively gently, while in PT group, the velocity of proximal sinus increased significantly. Binary Logistic regression showed that SSWD PT was independently correlated with proximal maximum flow velocity [ OR=1.086(1.029-1.146), P=0.003]. Conclusion:4D flow MRI showed that the dominant drainage and higher velocity at the proximal end of the transverse sinus might be an important hemodynamic characteristics of dehiscent sigmoid plate pulsatile tinnitus.
