1.Comparison of public awareness of tuberculosis control information in Inner Mongolia between the initial stage and 17 years after the implementation of DOTS strategy

ObjectiveTo provide evidence for strengthening tuberculosis control in Inner Mongolia by analyzing changes and influencing factors in public awareness of tuberculosis prevention and control information among residents between the early stage and 17 years after the implementation of the directly observed treatment, short-course (DOTS) strategy. MethodsBased on the "National Public Knowledge, Belief and Behavior Questionnaire on Tuberculosis Prevention and Control" designed by the Chinese Center for Disease Control and Prevention, a questionnaire survey was conducted among residents in Inner Mongolia using a multi-stage stratified random sampling method. Chi-square test was used for univariate analysis, and binary Logistic regression was employed to explore the influencing factors of public awareness of tuberculosis prevention and control information. ResultsThe overall awareness rate of core information on tuberculosis prevention and control among the public was 67.7% in 2006, and it decreased to 49.2% in 2023 (P<0.05). Multivariate Logistic regression analysis showed, compared with the groups aged 15-29, illiterate and semi-illiterate, those with public medical care, and those with three or fewer family members, the awareness rate of the groups aged 60 and above (OR=0.689), those with primary and junior high school education (OR=0.856), and those with self-paid medical care (OR=0.468) was significantly lower in 2006. The awareness rate was relatively high in groups with more than 3 family members (OR=1.236) (P<0.05). Compared with the groups aged 15-29, illiterate and semi-illiterate, married, Han ethnicity, employed, and those receiving public medical care, the awareness rate was higher among the groups aged 30-59 (OR=1.976), 60 and above (OR=2.224), those with high school education and above (OR=2.801), and single (OR=2.244) in 2023. However, the awareness rates of ethnic minorities (OR=0.737), the unemployed (OR=0.557), and self-funded medical care (OR=0.497) groups were significantly lower (P<0.05). ConclusionPublic awareness of TB prevention knowledge in Inner Mongolia remains suboptimal. Strengthening diversified health education campaigns, addressing social stigma, and improving healthcare access are critical to enhance regional TB control outcomes.

Debates persist regarding the efficacy and safety of azvudine, particularly its real-world outcomes. This study involved patients aged ≥60 years who were admitted to 25 hospitals in mainland China with confirmed SARS-CoV-2 infection between December 1, 2022, and February 28, 2023. Efficacy outcomes were all-cause mortality during hospitalization, the proportion of patients discharged with recovery, time to nucleic acid-negative conversion (T NANC), time to symptom improvement (T SI), and time of hospital stay (T HS). Safety was also assessed. Among the 5884 participants identified, 1999 received azvudine, and 1999 matched controls were included after exclusion and propensity score matching. Azvudine recipients exhibited lower all-cause mortality compared with controls in the overall population (13.3% vs. 17.1%, RR, 0.78; 95% CI, 0.67-0.90; P = 0.001) and in the severe subgroup (25.7% vs. 33.7%; RR, 0.76; 95% CI, 0.66-0.88; P < 0.001). A higher proportion of patients discharged with recovery, and a shorter T NANC were associated with azvudine recipients, especially in the severe subgroup. The incidence of adverse events in azvudine recipients was comparable to that in the control group (2.3% vs. 1.7%, P = 0.170). In conclusion, azvudine showed efficacy and safety in older patients hospitalized with COVID-19 during the SARS-CoV-2 omicron wave in China.

Peptide receptor radionuclide therapy (PRRT) with radiolabeled SSTR2 agonists is a treatment option that is highly effective in controlling metastatic and progressive neuroendocrine tumors (NETs). Previous studies have shown that an SSTR2 agonist combined with albumin binding moiety Evans blue (denoted as 177Lu-EB-TATE) is characterized by a higher tumor uptake and residence time in preclinical models and in patients with metastatic NETs. This study aimed to enhance the in vivo stability, pharmacokinetics, and pharmacodynamics of 177Lu-EB-TATE by replacing the maleimide-thiol group with a polyethylene glycol chain, resulting in a novel EB conjugated SSTR2-targeting radiopharmaceutical, 177Lu-LNC1010, for PRRT. In preclinical studies, 177Lu-LNC1010 exhibited good stability and SSTR2-binding affinity in AR42J tumor cells and enhanced uptake and prolonged retention in AR42J tumor xenografts. Thereafter, we presented the first-in-human dose escalation study of 177Lu-LNC1010 in patients with advanced/metastatic NETs. 177Lu-LNC1010 was well-tolerated by all patients, with minor adverse effects, and exhibited significant uptake and prolonged retention in tumor lesions, with higher tumor radiation doses than those of 177Lu-EB-TATE. Preliminary PRRT efficacy results showed an 83% disease control rate and a 42% overall response rate after two 177Lu-LNC1010 treatment cycles. These encouraging findings warrant further investigations through multicenter, prospective, and randomized controlled trials.
4.Analysis on effective components and biological activity of volatile oil in anti-URTI formula

Objective:To analyze the extraction, chemical composition, antioxidant, and anti-inflammatory activity of the TCM formula essential oil for the treatment of upper respiratory tract infections (URTI); To provide a scientific basis for its further development.Methods:The formula essential oil was extracted using the steam distillation method and analyzed for chemical composition by gas chromatography-mass spectrometry (GC-MS). The DPPH, ABTS scavenging ability, and hydroxyl radical scavenging ability of volatile oils were measured. The effect of the essential oil on the viability of RAW264.7 cells was assessed using the CCK-8 assay. ELISA and Western blot methods were used to determine the effects of volatile oil on LPS induced inflammatory cytokines IL-6 and TNF-α.Results:The average extraction rate of the formula essential oil was 1.12%, with a density of 0.973 2 g/ml. Twelve main chemical components were identified, with 1,8-cineole (42.9%) and patchoulol (19.9%) being the predominant constituents. The essential oil exhibited DPPH and ABTS radical scavenging capacities of 52% and 59%, respectively, and a hydroxyl radical scavenging capacity exceeding 70%. Essential oil could reduce the levels of IL-6 and TNF-α ( P<0.05). Conclusion:TCM formula essential oil for the treatment of URTI contains multiple bioactive components and demonstrates significant antioxidant and anti-inflammatory effects.
5.Thrombus aspiration combined with different thrombolysis regimens for the treatment of acute intermediate-high-risk pulmonary embolism:a comparative study

Objective To compare the clinical efficacy of pulmonary artery thrombus aspiration plus catheter-directed thrombolysis and pulmonary artery thrombus aspiration plus peripheral venous thrombolysis in treating acute intermediate-high-risk pulmonary embolism.Methods The clinical data of patients with acute intermediate-high-risk pulmonary embolism,who received pulmonary artery thrombosis aspiration combined with catheter-directed thrombolysis or peripheral venous thrombolysis at the Affiliated Hospital of Nantong University and the Qidong Municipal People's Hospital from January 2017 to May 2024,were retrospectively analyzed.Based on the propensity score matching(PSM)method,the clinical efficacy of the two treatments was compared.The observation indicators included technical success rate,clinical success rate,changes in cardiopulmonary function,and incidence of complications.Results After propensity score matching,the pulmonary artery thrombosis aspiration combined with catheter-directed thrombolysis group(group A)and the pulmonary artery thrombosis aspiration combined with peripheral venous thrombolysis group(group B)had 40 patients each.In group A and group B,the technical success rate was 100%and 100%respectively,the clinical success rate was 97.5%and 87.5%respectively,and the clinical success rate was 97.5%and 87.5%respectively(P=0.201);the postoperative one-week mean oxygen partial pressure was(98.4±16.4)mmHg and(95.5±14.7)mmHg respectively(P<0.001);the postoperative one-week mean pulmonary artery pressure was(26.3±10.5)mmHg and(28.5±11.8)mmHg respectively(P=0.005);the mean reduction value of B-type natriuretic peptide(BNP)level was(767.2±1 005.5)ng/L and(831.0±1 371.8)ng/L respectively(P<0.001).No statistically significant difference in the incidence of surgery-related complications existed between the two groups(P>0.05).Conclusion In treating patients with acute intermediate-high-risk pulmonary embolism,pulmonary artery thrombosis aspiration combined with catheter-directed thrombolysis is superior to pulmonary artery thrombosis aspiration combined with peripheral venous thrombolysis in better short-term efficacy.
6.Prevalence and rational use of polypharmacy in elderly patients in psychiatric hospitals: a review and discussion

In the context of aging population, the issue of polypharmacy among elderly patients with mental disorders has become increasingly prominent.Cognitive decline and depressive symptoms render these patients more vulnerable to medication-related risks, while poorly managed physical illnesses further complicate their treatment.To address these challenges, this paper proposes a series of management strategies that emphasize the critical role of pharmacists in conducting medication reviews.A comprehensive assessment of drug risks, benefits, and patient adherence is essential.The proposed strategies not only require careful consideration of patients' clinical needs and individual preferences but also highlight the importance of multidisciplinary team collaboration to reach a consensus on medication therapy.The use of clinical decision support systems as an auxiliary tool is recommended to enhance the scientific rigor of medication decision-making.Furthermore, pharmacists can optimize medication regimens through scientifically validated methods and promote patient or family involvement in self-management to improve acceptance and adherence to treatment adjustments.
7.Efficacy and influencing factors of antiviral therapy for hepatitis B e antigen-positive chronic hepatitis B in children

Objective:To investigate the efficacy of antiviral therapy and influencing factors of hepatitis B surface antigen(HBsAg) negative conversion for hepatitis B e antigen(HBeAg)-positive chronic hepatitis B(CHB) in children.Methods:The clinical data of 38 children with CHB who received antiviral treatment in Children's Hospital Affiliated to Xi'an Jiaotong University from January 2019 to August 2024 were collected.All patients were treated with interferon alpha monotherapy or combined with nucleoside analogues for 48 weeks.The patients were divided into HBsAg negative group and HBsAg non-negative group according to the therapeutic results at 48 weeks.Multivariate Logistic regression were used to identify influencing factors of HBsAg negative conversion at 48 weeks.The receiver operator characteristic(ROC)curve was used to analyze the predictive value of each factor to HBsAg negative conversion.Results:The alanine aminotransferase normalization rate,hepatitis B virus DNA negative rate,HBeAg negative rate and HBsAg negative rate were 76.3%,94.7%,39.5% and 47.4%,respectively at 48 weeks.There were 18 cases in HBsAg negative group and 20 cases in HBsAg non-negative group.There were statistical significant differences in age and HBsAg decline level at 12 and 24 weeks of antiviral treatment between HBsAg negative group and HBsAg non-negative group( P<0.05).Multivariate Logistic regression analysis showed that age and HBsAg decline level at 12 and 24 weeks of antiviral treatment were independent predictors of HBsAg negative conversion at 48 weeks( OR=0.664,95% CI 0.473-0.932, P=0.018; OR=8.719,95% CI 1.920-39.604, P=0.005; OR=6.182,95% CI 2.083-18.347, P=0.001).The area under the curve of age and HBsAg decline level at 12 and 24 weeks were 0.737(95% CI 0.576-0.899, P=0.012),0.847(95% CI 0.725-0.969, P<0.001)and 0.939(95% CI 0.811-0.991, P<0.001),respectively.When the age was less than 4.625 years,the sensitivity,specificity,positive predictive value and negative predictive value of HBsAg negative conversion at 48 weeks were 83.3%,65.0%,68.2% and 81.3%,respectively.A decrease in HBsAg level of >1.07 lg IU/mL at 12 weeks of treatment had a sensitivity,specificity,positive predictive value,and negative predictive value of 72.2%,90.0%,86.7%,and 78.3%,respectively,for predicting HBsAg seroclearance at 48 weeks.A reduction in HBsAg of >1.92 lg IU/mL at 24 weeks of treatment showed a sensitivity,specificity,positive predictive value,and negative predictive value of 83.3%,90.0%,88.2%,and 85.7%,respectively,in predicting HBsAg seroclearance at 48 weeks. Conclusion:The children with CHB have a higher rate of HBsAg negative conversion after antiviral therapy at 48 weeks.Age and HBsAg decline level at 12 and 24 weeks of antiviral treatment can serve as early predictors for HBsAg negative conversion in children with CHB.

Objective:To introduce the research and innovations around the application of aquatic rehabilitation techniques in the treatment of lumbago, and to summarize specific methods and applications in aeromedicine. Literature resource and selection Literature on the prevention and treatment of lumbago using aquatic rehabilitation techniques as well as on the rehabilitation of lumbago in pilots was retrieved and selected. Literature quotation Sixty-one references were cited. Literature synthesis Aquatic rehabilitation techniques can be classified into shower therapy, immersion therapy, and aquatic exercise therapy according to treatment regimens. Among them, immersion therapy and aquatic exercise therapy are commonly used in the treatment of lumbago. Aquatic rehabilitation techniques can effectively alleviate patients′ lumbago, improve their functional disorders, and enhance their quality of life. Prior to aquatic therapy, patients with lumbago need to have their motor function assessed to tailor the treatment protocol based on diagnostic findings and specific therapeutic objectives. Aquatic rehabilitation techniques, when used to prevent and treat lumbago in pilots under convalescence, are not only more effective than land-based training, but also more accessible to pilots due to their comfort and compliance. Many of our military rehabilitation and convalescent centers in China have integrated modern aquatic rehabilitation techniques into rehabilitation treatment by drawing on their rich water resources so as to improve the physical and mental health, social adaptability and military adaptability of pilots. Conclusions:When used for the prevention and treatment of lumbago, aquatic rehabilitation techniques are highly effective, safe and enjoyable, so they should be made more accessible to pilots during convalescence.
9.Effect and mechanism of CXCL8 on the paclitaxel sensitivity of cervical cancer cells

Objective The objective of this study was to explore the effect of knocking down CXCL8 on the efficacy of pacli-taxel chemotherapy in cervical squamous cell carcinoma and to investigate its potential mechanism of action.Methods The Hela cell model was used to specifically inhibit CXCL8 gene expression through lentivirus-mediated RNA interference(RNAi)technology.The optimal transfection conditions were HitransG P and a multiplicity of infection(MOI)of 100.The CCK-8 assay was used to screen the optimal intervention concentration of puromycin as 1.5μg/mL.The LV-CXCL8-RNAi(3 targets)and negative control lentivirus were transfected into cells under optimal transfection conditions and set up a blank control group.The qRT-PCR assay was used to select the sh-CXCL8-13 group lentivirus as the intervention sequence and virus for subsequent experiments.The experiments were divided into the blank control group,negative control group,sh-CXCL8 group,paclitaxel group,and sh-CXCL8+paclitaxel group.The prolifer-ative activity and invasive ability of cervical cancer cells were assessed by CCK-8 and cell invasion assays.The expression of CXCL8,Bcl2,Bax,and β-actin were detected by qRT-PCR and Western blot.Results Compared with the other four groups,the proliferative and invasive ability of Hela cells was significantly reduced in the sh-CXCL8+paclitaxel group,and the difference was statistically sig-nificant(P<0.01).The qRT-PCR results showed that the expression of CXCL8,Bcl2,PIK3CB,and Akt1 genes was significantly re-duced,and the expression of Bax gene was significantly increased in the sh-CXCL8+paclitaxel group.The difference between the groups was statistically significant(P<0.001).The results of Western blot showed that the expression of CXCL8,PIK3CB,and p-Akt1 proteins was reduced in the sh-CXCL8+paclitaxel group(P<0.05).Conclusion Knocking down CXCL8 can reduce the prolif-erative and invasive capacity of Hela cells,possibly by affecting the PI3K/Akt pathway to affect the drug sensitivity of Hela cells to paclitaxel.
10.Effects of Inonotus obliquus polysaccharides on main organs and hormones in male mice infected with Neospora caninum

The aim of this study is to determine the effects of Inonotus obliquus polysaccharides on the main organs and hormones of male mice infected with Neosporidium.The animal model of neosporidiosis in BALB/c mice was established.After intragastric administration,the pathological changes of brain,liver and spleen were observed by histopathology,the worm load in brain,liver and spleen was detected by qPCR,and the levels of FSH,LH and T4 in serum of male mice were detected by ELISA.The results showed that compared with the blank control group,granular spherical lesions could be seen in the brain tissue of mice in the model group,with patchy necrosis of hepatocytes,splenic hemorrhage,red pulp hyperemia and a large number of red blood cell infil-tration.Inonotus obliquus polysaccharides can improve the injury of brain,liver,spleen and other organs and tissues.The worm load in the brain of the Inonotus obliquus polysaccharide group was significantly lower than that of the model group on the 14th day and 21st day(P<0.01),and that of the liver and spleen on the 21st day was significantly lower than that of the model group(P<0.01).The level of FSH in the model group and Inonotus obliquus polysaccharide group was signif-icantly lower than that in the blank control group(P<0.05 or P<0.01),while that in the Inono-tus obliquus polysaccharide group was significantly higher than that in the model group(P<0.05 or P<0.01).The level of LH in the model group was significantly higher than that in the blank control group(P<0.01),while that in the Inonotus obliquus polysaccharide group was significant-ly higher than that in the blank control group on the 7th day and 42nd day(P<0.05).There was significant difference in T4 level between the model group and the blank group(P<0.05 or P<0.01),while the Inonotus obliquus polysaccharide group was similar to the blank control group on the 21st and 35th day(P>0.05).Inonotus obliquus polysaccharides can improve the damage of Neosporidium on brain,liver and spleen of male rats,reduce the insect load in brain,liver and spleen,regulate the levels of FSH,LH and T4 hormones,and maintain the stability of reproductive system.
