1.Fatigue gait recognition of athletes based on fish swarm algorithm

BACKGROUND:Gait movements are one of the important characteristics exhibited by athletes during exercise,reflecting their physical condition and athletic ability.In a state of fatigue,athletes may exhibit abnormal gait movements,such as reduced stride and body shaking,which can cause harm to their bodies.OBJECTIVE:To promote technological progress in the field of sports science by applying advanced algorithms and data analysis techniques to the training practice of athletes,so as to further improve the recognition accuracy of gait movements under sports fatigue.METHODS:A gait recognition method for athletes in fatigue state was based on fish swarm algorithm.By utilizing the normalized autocorrelation function and the principle of motion energy distribution,a single cycle gait energy map of athletes was obtained.Singular value decomposition was used to transform the image to highlight visual differences,generating a gait energy map of athletes.A convolutional neural network was used to construct a gait action recognition model,and the parameters of the model were solved using the fish swarm algorithm to improve the accuracy and efficiency of fatigue gait action recognition.RESULTS AND CONCLUSION:(1)The fish swarm algorithm had a small loss value in gait action recognition,and could accurately and quickly identify the gait actions of athletes,and dynamically monitor their physical fatigue.(2)The research on fatigue gait recognition of athletes based on fish swarm algorithm could effectively identify the gait movements of athletes in fatigue state and achieve accurate capture of subtle gait changes.(3)The system stability of this method is good,which can reduce the volatility of experimental test results and improve recognition efficiency,can more effectively manage sports fatigue and prevent sports injuries.In addition,when the gait characteristics of normal people change significantly,the system can give an early warning,indicating that the individual may be in a state of fatigue and need to rest or adjust the intensity of activity.

3.Application of Component Separation Technique in the Laparoscopic Treatment of Gastric Stromal Tumor Around the Cardia

Objective To explore the feasibility and safety of component separation technique (CST) in the laparoscopic treatment of gastric stromal tumor (GST) around the cardia.Methods Clinical data and follow-up results of 20 patients who underwent laparoscopic resection with CST in the treatment of GST around the cardia from January 2020 to June 2023 were retrospectively analyzed.Results All the 20 patients underwent surgery successfully without conversion to open surgery.The operative time was (90.5±18.9) min,the intraoperative blood loss was (22.0±14.7) ml,the postoperative ambulation time was (1.2±0.4) d,the first flatus time was (2.1±0.8) d,and the postoperative hospital stay was (5.7±1.2) d.Postoperative pathological results showed very low risk in 1 case,low risk in 14 cases,intermediate risk in 4 cases,and high risk in 1 case.The maximum tumor diameter was (3.6±0.9) cm.Postoperatively,1 patient experienced gastric emptying dysfunction and 1 had pleural effusion,both of whom were cured after active conservative treatment.There were no cardiac stenosis,gastric leakage,gastric bleeding,abdominal bleeding,incision infection,or other complications.All the patients were followed up for 6-36 months ( mean,15 months).They were able to eat normally,without choking,acid reflux,heartburn,or reflux symptoms.The CT scanning showed no tumor recurrence or metastasis.Conclusion Application of CST in the laparoscopic treatment of GST around the cardia is safe and feasible.

5.Influencing factors and pregnancy outcomes of unsuccessful cell-free DNA testing in maternal perinatal blood

Objective:To explore the possible factors leading to failure of cell-free DNA (cfDNA) testing in maternal peripheral blood and analyze the pregnancy outcomes of this group of pregnant women.Methods:This retrospective study involved 5 195 women who underwent cfDNA testing in Peking University Third Hospital from April 2017 to April 2019. Based on the first cfDNA testing results, clinical characteristics of the pregnant women with successful (success group, n=5 107) and failed (failure group, n=88) cfDNA testing were compared using Mann-Whitney U test and Chi-square test. Multivariate logistic regression was used to analyze the risk factors of cfDNA testing failure and the effect of body mass index (BMI) on the success rate, and evaluate the feasibility of re-sampling and the factors affecting the unsuccessful testing of a second sample. Results:The failure rate of first cfDNA testing was 1.7% (88/5 195). Successful cfDNA testing was achieved in 74 (87.1%, 74/85) of 85 re-sampling cases, while results of the other 11 cases (12.9%, 11/85) remained invalid. Thus, the final failure rate was 0.2% (11/5 195). Multivariate logistic regression revealed that increased maternal age ( OR=1.086, 95% CI: 1.023-1.152, P=0.006), BMI ( OR=1.083, 95% CI: 1.021-1.149, P=0.008) and twin pregnancies ( OR=3.093, 95% CI: 1.715-5.577, P<0.001) were the risk factors of cfDNA testing failure, while increased cell-free fetal DNA (cffDNA) concentration ( OR=0.758, 95% CI: 0.720-0.761, P<0.001) was a protective factor. The overweight (BMI: 25-29.9 kg/m 2) and obese (BMI≥30 kg/m 2) women were 3.626 ( OR=3.626, 95% CI: 2.298-5.724, P<0.001) and 4.064 ( OR=4.064, 95% CI: 1.779-9.284, P=0.001) times more likely to have failed cfDNA testing than those with normal weight (BMI: 18.5-24.9 kg/m 2), respectively. The success rate of re-testing decreased as the maternal BMI increased, regardless of the time interval between the two samplings ( OR=0.840, 95% CI: 0.699-1.245, P=0.065). Seven out of the 74 cases with successful results in re-testing were at high risk, including one 45,X and one 47,XXY, confirmed by karyotyping amniocentesis. Among the 11 pregnant women with a failed testing after second sampling, eight underwent prenatal diagnosis with normal fetal chromosome karyotypes, and the other three cases without prenatal diagnosis all gave birth to neonates with normal phenotype. There was no statistical difference in the incidence of pregnancy loss between the failure and success group [9.1% (8/88) vs 2.5% (128/5 107), P=0.090]. Conclusions:Pregnant women with advanced age and higher BMI, lower cffDNA fraction and twin pregnancies are more likely to fail in cfDNA testing. For obese women, blood sampling can be postponed to a larger gestational age to reduce the failure rate. For pregnant women with failed testing in first sampling, a re-sampling is recommended, moreover, prenatal diagnosis is necessary for those had high-risk results or failed in re-testing.
6.Transoral Orvil EEA stapler (OrVil) in laparoscopy-assisted total gastrectomy for cardiac carcinoma

Objective To evaluate transoral Orvil EEA stapler (OrVil) procedure in laparoscopic total gastrectomy for cardiac carcinoma compared with conventional anvil head method (purse-string suture).Methods From May 2014 to December 2014 20 cases were included into OrVil group,and 25 cases into purse-string suture group.Results The two groups had similar mean numbers of dissected lymph nodes [(25 ± 3) vs.(24 ± 4),t =1.067,P =0.292],the mean time of operation,intraoperative blood loss,and postoperative complications (5 vs.6,P =0.938).The length of incision was significantly shorter [(5 ±1) cm vs.(11 ± 2) cm,t =-10.724,P < 0.0l] and the esophagojejunostomy time was significantly less [(28 ± 4) min vs.(39 ± 5) min,t =-7.996,P < 0.01] with the use of OrVil.The time to first flatus and postoperative hospital stay were (3.7 ± 0.9) d vs.(4.4 ± 1.0) d,t =-2.485,P =0.017 and (13 ± 5) d vs.(16 ±4) d,t =-2.184,P =0.035.Conclusions OrVil is a technically safe and feasible surgical procedure for esophagojejunostomy in laparoscopy assisted total gastrectomy in the treatment of cardiac carcinoma.
7.Expression and clinical significance of ubiquitin-specific protease 9X in pancreatic carcinoma of elderly patients

Objective There is close relationship between ubiquitin-specific protease 9X(USP9X) and the biological behav-ior of some tumor.The aim of this study is to investigate the expression and clinical significance of USP 9X in pancreatic carcinoma of elderly patients. Methods The expression of USP9X was detected in 30 pieces of surgically resected primary pancreatic carcinoma tissue and adjacent nontumorous pancreatic tissue of elderly patients by streptavidin -perosidase immunohistochemical method . Results The rate of USP9X positive expression was 56.7%, there was not positive expression in adjacent nontumorous pancreatic tissue .There was no relation between the expression of USP 9X with gender, age, the tumor positin, the tumor size and degree of differentiation (P>0.05), while it was significantly correlated with lymph node metastasis and TNM stages (P<0.05).By using Cox proportional haz-ards analysis, multivariable analysis revealed that TNM stages , lymph node metastasis and USP9X expression were independent risk factor(P<0.05). Conclusion The results indicated that USP9X may play a role in the pathogenesis and prognosis of pancreatic cancer of elderly patients .
8.The current status and correlation analysis on knowledge, attitude and practice of fast-track surgery in medical staff

Objective To investigate the current status about the knowledge,attitude and practice of medical staff for fast-track surgery (FTS) in third-grade class-A general hospital in Xuzhou,and provide reference for the popularization of the FTS concept.Methods The self-administered questionnaires were employed to survey 130 medical staff in the general surgical department of one third-grade class-A general hospital in Xuzhou.Results The FTS knowledge score was (20±8).The differences of knowledge scores were statistically significant because of different age,degree of education and positional title.The FTS attitude score was (66±20).And the differences of attitude scores were statistically significant because of different age and degree of education.The FTS practice score was (64.50±13.25).There was no statistical significance on the practice scores between the medical staff with different characteristics.Attitude was correlated with knowledge and practice while knowledge was not correlated with practice.Conclusions The FTS knowledge of medical staff was not rich.The FTS attitude was not so positive,and there were also few clinical practice.Medical staff should study the FTS concept and its contents actively.They also should apply FTS in clinic scientifically in order to better control the iatrogenic injury and promote the rehabilitation of patients effectively.
