1.Association of serum ITLN—1,NT—proBNP and Lp—PLA2 levels with coronary artery disease severi-ty in patients with acute myocardial infarction

Objective:To investigate the association of serum levels of intelectin-1(ITLN-1),N-terminal pro brain natriuretic peptide(NT-proBNP),lipoprotein-associated phospholipase A2(Lp-PLA2)with coronary ar-tery disease severity in patients with acute myocardial infarction(AMI).Methods:A total of 269 patients suspected of AMI admitted in Yulin Xingyuan Hospital between January 2021 and January 2024 were retrospectively enrolled.According to coronary angiography combined with examination of clinical symptoms,137 patients were diagnosed with AMI(AMI group)and the remaining 132 cases were included in control group.Serum ITLN-1,NT-proB-NP,and Lp-PLA2 levels were compared between AMI group and control group;Logistic regression was used to ex-plore the factors associated with AMI.A nomogram model was constructed based on the factors of AMI.Serum ITLN-1,NT-proBNP and Lp-PLA2 levels were compared among AMI patients with different coronary artery disease severity,and their association with coronary artery disease severity was analyzed by Pearson correlation coef-ficient.Results:Serum ITLN-1 level was significantly lower,and serum levels of NT-proBNP and Lp-PLA2 were significantly higher in AMI group compared to control group(P<0.001 all).Multivariate Logistic regression indicated that ITLN-1(OR=0.971,95%CI 0.945~0.998,P=0.032)was independent protective factor for AMI,while NT-proBNP(OR=1.017,95%CI 1.007~1.028,P<0.001)andLp-PLA2(OR=1.039,95%CI 1.004~1.075,P=0.030)were independent risk factors.A nomogram model was constructed according to these factors.The calibration curves and receiver operating characteristic(ROC)curves showed good predictive value,and decision curve analysis(DCA)also indicated a significant net benefit.As coronary artery disease aggravated,se-rum ITLN-1 level significantly reduced and serum NT-proBNP and Lp-PLA2 levels significantly increased(P<0.05 or<0.01).Pearson correlation analysis showed that ITLN-1 was inversely correlated with coronary artery disease severity(r=-0.906,P<0.001),while NT-proBNP and Lp-PLA2 were positively correlated with it(r=0.860,0.885,P<0.001 both).Conclusion:ITLN-1 is an independent protective factor for AMI,while NT-proBNP and Lp-PLA2 are the independent risk factors.All of above-mentioned indexes are significantly correla-ted with coronary artery disease severity,and the combination of them had good diagnostic value for AMI.
2.A comparative study on the efficacy of two types of rotary nickel-titanium files in removing root canal fillings

Objective·To explore the efficacy of the XP-Endo Shaper(XPS)and Reciproc(RC)engine-driven nickel-titanium file systems in removing root canal fillings,and to provide evidence for clinical application.Methods·The maxillary first permanent molars were obtained from patients who visited the Department of General Dentistry,Shanghai Ninth People's Hospital,Shanghai Jiao Tong University School of Medicine,between March and September 2022,which required tooth extraction due to periodontal disease.A total of 30 mesiobuccal or distobuccal root canals with moderate curvature were selected and randomly divided into the RC file group(control group,n=15)and the XPS file group(experimental group,n=15).All root canals were first negotiated and shaped using M3-PRO nickel-titanium files,followed by root canal obturation with bioceramic paste using the single-cone technique.Then,RC files(R25)were used in the control group,while XPS files were used in the experimental group to remove the root canal obturation.Cone Beam Computed Tomography(CBCT)scans were performed before and after the removal procedures.The time required for obturation removal,the percentage of residual obturation volume,the amount of apical debris extrusion,and the degree of instrument deformation were recorded.Independent-sample t-tests and chi-square test were used to compare the differences between the two groups.Results·The XPS group required slightly more time(118.87±18.58)s than the RC group(107.93±14.79)s,but the difference between two groups was not statistically significant.The XPS group had a lower percentage of residual obturation volume(7.51±8.06)%compared to the RC group(15.02±14.63)%,but the difference was not statistically significant.The XPS group had significantly less apical debris extrusion(6.15±1.42)mg than the RC group(8.29±2.01)mg,with a statistically significant difference(P=0.002).Both groups exhibited varying degrees of instrument deformation,but yet no instrument separation occurred in either group.Conclusion·The RC engine-driven nickel-titanium files were slightly more time-efficient in removing root canal fillings,whereas the XPS files achieved a higher canal cleanliness and produced less apical debris extrusion.As there was no significant difference in removal time,and considering the superior cleaning performance and reduced risk of postoperative complications,the XPS file system appears more suitable for removing root canal fillings in moderately curved root canals.

4.Therapeutic effect of FDP combined with levocarnitine on patients with viral myocarditis and its effect on serum AST,LDH,cTnI

Objective:To investigate the clinical efficacy of levocarnitine combined with fructose 1,6-diphosphate(FDP)on patients with viral myocarditis(VMC)and its effect on serum levels of aspartate aminotransferase(AST),lactate dehydrogenase(LDH)and cardiac troponin I(cTnI).Methods:This randomized controlled study enrolled 126 patients with VMC admitted to Xingyuan Hospital of Yulin between May 2020 and August 2021.Pa-tients were divided into control group(n=63)and intervention group(n=63).Patients in the control group re-ceived routine treatment comparing to those in the intervention group receiving additional FDP combined with levoc-arnitine,both groups were treated for 4 weeks.Left ventricular ejection fraction(LVEF),levels of AST,LDH,cTnI,creatine kinase(CK),creatine kinase isoenzyme MB(CK-MB)and therapeutic effective rate were com-pared between the two groups.Pearson correlation analysis was used to analyze the association of AST,LDH,cTnI levels with LVEF.Results:Compared with patients in control group after 4-week treatment,those in the inter-vention group had significant higher LVEF[(73.17±6.39)%vs.(63.30±6.04)%]and therapeutic effective rate(96.67%vs.80.00%)(P<0.001 all),and significant lower levels of AST[(32.35±7.34)U/L vs.(48.22±8.40)U/L],LDH[(154.50±28.10)U/Lvs.(183.77±29.60)U/L],cTnI[(77.47±12.04)ng/ml vs.(96.12±15.33)ng/ml],CK[(181.47±8.93)U/L vs.(192.33±8.85)U/L],CK-MB[(24.25±2.19)U/L vs.(28.17±2.01)U/L](P<0.001 all).Pearson correlation analysis indicated that AST,LDH and cTnI levels were significant negatively correlated with LVEF(r=-0.404,-0.231,-0.339,P<0.05 or<0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P=0.543).Conclusion:FDP combined with levocarnitine could significantly improve cardiac function,reduce the levels of AST,LDH and cTnI,and effectively improve therapeutic effective rate with good safety in patients with viral myocarditis.

Objective:To develop a treatment-related quality of life scale for Chinese patients with multiple myeloma (MM) and to test its reliability and validity.Methods:The initial scale was constructed through a literature search, Delphi expert correspondence, and cognitive testing. This study conducted a preliminary survey of 379 patients with MM and a formal survey of 865 patients from the hematology departments of 155 hospitals nationwide from February 2024 to March 2024. The final scale was obtained after conducting item analysis and reliability and validity tests on the initial scale.Results:The constructed scale contains 36 items covering six domains: physiological, psychological, social, treatment side effects, general health, and others. In the preliminary survey, the Cronbach’s alpha coefficient of each item ranged from 0.597 to 0.939, and the test-retest reliability was 0.747 ( P<0.001). Exploratory factor analysis extracted eight common factors with a cumulative variance contribution of 60.058%. In the formal survey, the Cronbach’s alpha coefficient of each item ranged from 0.484 to 0.930, and the test-retest reliability was 0.835 ( P<0.001). Confirmatory factor analysis revealed a comparative fit index of 0.750, a root-mean-square error of approximation of 0.090, and a root-mean-square residual of 0.067. Conclusion:The treatment-related quality of life scale for Chinese patients with MM designed in this study exhibited good reliability and validity, reflecting the impact of treatment on the quality of life of patients. This scale can provide a reference to clinicians for assessing the disease status of patients.

10.Tibetan medicine Twelve Flavors Yishou San alleviated lipopolysaccharide-induced acute lung injury through inhibiting neutrophil infiltration

Objective To explore the immune mechanism of Tibetan medicine Twelve Flavors Yishou San in intervening with acute lung injury.Methods Construct a lipopolysaccharide(LPS)-induced acute lung injury rat model and treated it with Twelve Flavors Yishou San.The intervention effect of Twelve Flavors Yishou San on LPS induced acute lung injury was determined through lung pathological sections and blood gas analysis.Enzyme linked immunosorbent assay(ELISA)was used to detect the expression of pro-inflammatory cytokine tumor necrosis factor-α(TNF-α)and anti-inflammatory cytokine interleukin(IL)-10 in rat's bronchoalveolar lavage fluid.Flow cytometry was used to detect changes in immune cells in rat's bronchoalveolar lavage fluid,lungs,spleen and peripheral blood.Results LPS successfully induced an acute lung injury model in rats,and Twelve Flavors Yishou San could alleviate the exudation of inflammatory cells,elevated oxygen partial pressure in LPS induced acute lung injury to varying degrees.The ELISA results of rat bronchoalveolar lavage fluid showed that compared with normal control group,the TNF-α level in the bronchoalveolar lavage fluid of rats with acute lung injury was increased and the IL-10 level was decreased,while the Twelve Flavors Yishou San could alleviate this effect.The results of flow cytometry showed that Twelve Flavors Yishou San could significantly reduce neutrophil infiltration in rat bronchoalveolar lavage fluid,but had no significant effect on macrophages and adaptive immune response.Conclusion Twelve Flavors Yishou San can alleviate LPS induced acute lung injury by inhibiting neutrophil infiltration,providing an important experimental basis for fully understanding the therapeutic mechanism of Twelve Flavors Yishou San.
