Glaucoma is a multifactorial degenerative optic neuropathy, and its irreversible and blinding pathological characteristics mainly come from the damage to the optic nerve, namely glaucomatous optic neuropathy(GON). The difficulty in the treatment of GON lies in the early intervention, and currently there is no optic neuroprotective drug for the treatment of all types of GON. The death of retinal ganglion cells(RGCs)is the core pathological change caused by various pathogenic mechanisms of GON. Recent studies have found that the widespread second messenger cyclic adenosine 3', 5' -monophosphate(cAMP)and its downstream effector protein kinase A(PKA)signal cascade play an important role in the pathogenesis of GON. It can also inhibit the apoptosis of RGCs and play a protective and therapeutic role in glaucoma. Therefore, this article reviews the role of cAMP/PKA pathway in the pathophysiological development of GON, focusing on its effects on glaucoma intraocular pressure regulation, oxidative stress, neuroinflammation and optic nerve degeneration, in order to find a common central regulatory target for the optic nerve damage caused by different pathological mechanisms of GON and promote the further understanding and clinical treatment of this disease.

Objective:To evaluate the effects of different exercises on patients with chronic kidney disease based on network Meta-analysis so to provide a basis for medical and nursing staff to develop exercise programs. Methods:Randomized controlled trials on the effects of different exercises on patients with chronic kidney disease were electronically searched in databases such as Web of Science, PubMed, Embase, Cochrane Library, China Biology Medicine disc, WanFang Data, and China National Knowledge Infrastructure. The search period was from January 1, 2000, to March 1, 2023. Two researchers conducted literature screening, data extraction, and literature quality evaluation independently. The data was analyzed by using R software in conjunction with the gemtc package and plotted by using Stata software. Results:A total of 52 articles were included, with a cumulative sample size of 3 127 cases, involving three types of exercise (aerobic, resistance, and combined). The network Meta-analysis showed statistically significant differences between the three exercise methods and routine nursing in delaying renal function progression and improving cardiovascular endurance ( P<0.05). In controlling blood pressure and improving walking ability, the effects of three exercise methods were all superior to routine nursing, and the differences were statistically significant ( P<0.05). The area under the cumulative ranking probability curve showed that aerobic exercise>resistance exercise>combined exercise in terms of delaying renal function progression. In controlling blood pressure, resistance exercise>aerobic exercise>combined exercise. In terms of improving cardiovascular endurance, aerobic exercise>resistance exercise>combined exercise; In terms of improving walking ability, resistance exercise>combined exercise>aerobic exercise. Subgroup analysis showed that ≥ 24 weeks was the optimal intervention period for aerobic exercise. Resistance exercise intervention for ≥ 12 weeks could lower blood pressure, and intervention for ≤12 weeks could improve walking ability. However, when the intervention time was longer than 24 weeks, combined exercise had an excellent effect. Conclusions:Aerobic exercise is an excellent way to delay renal function progression and improve cardiovascular endurance. Resistance exercise is the optimal exercise method for controlling blood pressure and improving walking ability. Medical and nursing staff should choose appropriate types of exercise based on the patient's medical needs.

Soluble programmed cell death 1 ligand 1(PD-L1)in the serum includes exosomal,mi-crovesical and secreted forms of PD-L1.Previous studies have shown that the level of exosomal PD-L1 in the serum significantly correlated with the prognosis of various cancers. However, current analysis detects all forms of PD-L1 in the serum as a whole, without distinguishing exosomal PD-L1 from other forms. In this study,a specific detection method for exosomal PD-L1 was established based on surface plasmon res-onance.This method first captures PD-L1 by antibody recognition and immobilizes it on the surface of the detection chip.Then,α-hemolysin was recruited to form multiple oligomers on the exosomal membrane. This method quantifies the content of exosomal PD-L1 by monitoring the signal change during the binding process of α-hemolysin,effectively reducing background noises and amplifying the signal. The linear range before signal amplification with α-hemolysin was 0.035-2.208 pg/mL,and after signal amplifica-tion,it was 0.004-0.552 pg/mL.Methodological validation showed that this method has good specifici-ty,sensitivity,and repeatability, and has certain clinical application prospects.

To investigate the recent prevalence and molecular epidemiological characteristics of por-cine diarrhea viruses in Sichuan Province, this study used fluorescence quantitative PCR to detect porcine diarrhea samples from multiple regions in Sichuan Province from 2021 to 2023.RT-PCR was used to identify the genotypes of PEDV,PoRVA,PDCoV,and PTV,and their genetic variabil-ity,evolutionary characteristics, and recombination events were analyzed. The results showed that PEDV,PoRVA,PDCoV,and PTV were still prevalent in Sichuan region, with overall positive rates of 14.2%(40/281),13.2%(37/281),15.6%(44/281),and 12.5%(35/281),respectively.PEDV mixed infection with other pathogens was the most common. This study obtained a total of six strains of G2b PEDV,three strains of G3 PDCoV,three strains of G9P[13]PoRVA,one strain of G3P[13]PoRVA,three strains of Type 5 PTV,and one strain of Type 9 PTV.Compared to the seven vaccine strains including CV777,DR13,KPEDV-9,Chinju99,KNU-0801,AJ1102,and LW/L,the 6 PEDV strains showed multiple amino acid mutation sites in the COE region and S1D epitope region.Among them, the strains PSCLZ01 and PSCMY04 formed a separate branch in the phylogenetic tree. The three PDCoV strains have a closer genetic evolution distance to the previ-ously prevalent strains in Sichuan, but they also have 6-48 amino acid mutations compared to them. The four PoRVA strains have 104-108 amino acid variations in the VP4 gene compared to the early vaccine strain LLR,and they have 25 common amino acid variations in the VP7 gene. From the phylogenetic tree, the VP7 gene of RSCMY01/G3P[13]belongs to the same branch as the Heilongjiang strain LNCY,but its VP4 gene clusters with the Sichuan strain SCYA-C7,indica-ting that this PoRVA strain may have undergone genetic reassortment during inter-provincial transmission between different genotypes. It is worth noting that in the detected samples of PTV-5 and PTV-9,other diarrheal viruses tested negative, indicating that these two genotypes of PTV may be important pathogens causing porcine diarrhea.Additionally, the S gene of PEDV PSCLZ01 strain and PDCoV PCSCMY02 strain have undergone recombination events, and their parental strains come from different regions, both domestic and international. These findings reveal the main types of porcine diarrheal viruses,as well as their genetic diversity and variations in Sichuan Province in recent years. This study enriches the molecular epidemiological data of porcine diarrhe-al pathogens in the region and provides an important theoretical basis for the prevention, control, and purification of porcine diarrhea in the local area.
