1.Epidemiological characteristics and influencing factors of the cluster outbreaks of influenza-like illness in Yangpu District, Shanghai, 2012‒2022

ObjectiveTo analyze the epidemiological characteristics and influencing factors of clustered influenza-like illness (ILI) outbreaks in Yangpu District, Shanghai from 2012 to 2022, and to provide a scientific basis for formulating specific prevention and control measures of influenza in this district. MethodsData on clustered ILI outbreaks from 2012 to 2022 were collected, and the epidemiological pattern of the cluster outbreaks of influenza and its influencing factors of prevalence and duration were explored using statistical methods such as descriptive epidemiology, χ² test and logistic regression analysis. ResultsA total of 60 clustered ILI outbreaks were reported in Yangpu District, Shanghai from 2012 to 2022, with a cumulative number of 543 cases and an annual average incidence rate of 16.96%. The male-to-female ratio was1.06∶1. The largest number of ILI were reported in 2019, mainly from November to January of the next year, with December being the peak of outbreaks. The Streets of Changhai Rd. in Yangpu District had the largest number of reported outbreaks, accounting for 30.00% of the total outbreaks. Influenza cluster outbreaks occurred in schools, with the majority of outbreaks and cases reported in primary schools (accounting for 65.00% and 62.06% of the total, respectively). The outbreaks were mainly caused by influenza B virus, accounting for 50% of the total number of outbreaks. The prevalence rate of influenza cluster outbreaks varied among different schools in terms of their affiliated streets, school attributes and school types, with those in Kongjiang Rd. Subdistrict, private schools, and childcare institutions being relatively high. The duration of outbreaks reported for >2 days was longer than those reported for ≤2 days (OR=8.604, 95%CI: 1.509–49.067). ConclusionThe ILI outbreaks in Yangpu District present a seasonal pattern, with primary schools and Changhai Rd. Street as the targeted places for implementing prevention and control measures. Timely detection and reporting and active disposal can prevent a further spread of the outbreaks.
2.Mechanism of Exogenous Melatonin in Inhibiting Early Bolting in Angelica sinensis

ObjectiveThis study aims to investigate the effects of different sizes of seedlings and melatonin treatment on physiological and biochemical indicators and bolting-related gene expression in Angelica sinensis, find substances related to early bolting, and elucidate the inhibitory mechanism of melatonin on bolting. MethodsSpectrophotometry was used to detect the related enzyme activities of A. sinensis leaves. The contents of endogenous hormones and polyamines were detected using ultra-high performance liquid chromatography-tandem mass spectrometry. Real-time polymerase chain reaction (Real-time PCR) was used to detect the expression levels of bolting-related genes. Inter-group differential indicator analysis, orthogonal partial least squares discriminant analysis, and principal component analysis were comprehensively applied to identify factors related to early bolting. ResultsEndogenous jasmonic acid and melatonin were identified as the most important factors affecting early bolting. Secondly, the activity of antioxidant enzymes, abscisic acid content, gibberellin content, and the expression levels of CO3, HD3A, and FD genes had important effects on the bolting process. Compared with small seedlings, exogenous melatonin treatment mainly inhibited early bolting by increasing endogenous melatonin content, reducing gibberellin content, and decreasing the expression levels of SOC1 and FD genes. ConclusionExogenous melatonin can inhibit early bolting in A. sinensis by regulating its physiological, biochemical, and gene expression levels.

4.Bacterial Diversity in Phyllosphere and Rhizosphere Soil of Angelica sinensis in Continuous Cropping and Effects of Different Disease Control Measures

ObjectiveTo investigate the bacterial diversity in the rhizosphere soil and phyllosphere of Angelica sinensis and examine the effects of foliar applications of a composite bacterial agent,salicylic acid,and coronatine on the bacterial diversity,disease incidence,and plant yield,thus providing a theoretical basis and guidance for the artificial construction of functional minimal communities and the regulation of rhizosphere through foliar treatments. MethodsUnder continuous cropping conditions in the field,foliar applications of a composite bacterial agent,salicylic acid,coronatine,and sterile water were conducted. The 100-plant weight was measured via the conventional method,and the incidence of diseases was recorded. The microbial community composition,diversity,and inter-group differences in the phyllosphere and rhizosphere soil of A. sinensis were analyzed by 16S high-throughput sequencing,and the potential microbial functions were predicted. ResultsCompared with the blank control,foliar applications of salicylic acid and coronatine both significantly reduced the yield and root rot incidence of A. sinensis. The foliar application of salicylic acid decreased the content of ferulic acid and increased that of ligustilide. The foliar application of coronatine increased the content of both ferulic acid and ligustilide. The microbial communities and functions in the phyllosphere and rhizosphere soil were significantly different. The phyllosphere had lower microbial diversity,with all bacteria being Gram-negative,mainly Cyanobacteria and Proteobacteria with limited functions. The rhizosphere soil had higher microbial diversity,harboring dominant phyla including Proteobacteria,Actinobacteria,Acidobacteria,and Bacteroidetes with rich functions. All foliar treatments regulated the microbial community in the rhizosphere soil,with a more significant effect on the microbial community in the rhizosphere soil than that in the phyllosphere. The coronatine treatment significantly reduced the abundance of Proteobacteria and nitrate-reducing and aromatic compound-degrading microorganisms in the rhizosphere soil,thus affecting nutrient cycling and autotoxic substance degradation and leading to a yield reduction. Compared with the salicylic acid treatment,the coronatine treatment significantly increased the abundance of Bacillus and Streptomyces in the rhizosphere soil,demonstrating enhanced disease control efficacy. ConclusionFoliar application of coronatine and salicylic acid can significantly regulate the composition and function of bacterial communities in the rhizosphere soil,thereby reducing the disease incidence and the plant yield.
5.Orthopedic manipulation combined with percutaneous reduction and Kirschner wire internal fixation for Sanders typeⅡand Ⅲ calcaneal fractures.

OBJECTIVE:
To explore clinical effects of bone setting manipulation combined with pry reduction and Kirschner needle internal fixation in treating SandersⅡ-Ⅲ calcaneal fracture.
METHODS:
Clinical data of 52 patients with types Sanders Ⅱand Ⅲ calcaneal fracture (foot) treated with bone-setting manipulation combined with pry reduction and Kirscher needle internal fixation from July 2017 to July 2019 were retrospectively analyzed, including 43 males and 9 females, aged from 31 to 72 years old with an average of (50.83±10.48) years old; 15 patients with Sanders typeⅡ and 37 patients with Sanders type Ⅲ. The changes of Bühler angle, Gissane angle, calcaneus width and calcaneus height before operation and 24 months after operation were compared, and Maryland foot function score was performed to evaluate clinical effects.
RESULTS:
All patients were followed up from 24 to 60 months with an average of (41.50±9.86)months. The fracture healed normally and the healing time was (11.00±0.95) weeks. Bühler angle, Gissane angle, calcaneal bone width and calcaneal bone height were increased from (16.37±8.36)°, (96.27±9.62)°, (46.82±4.67) mm, (38.41±3.58) mm before operation to (31.48±8.24)°, (111.62±8.69)°, (42.06±4.83) mm, (44.21±3.82) mm at 24 months after operation, and the difference were statistically significant (P<0.01). Postoperative Maryland score at 24 months was (93.04±8.83), 40 patients got excellent result, 7 good and 5 fair.
CONCLUSION
Orthopedic manipulation combined with percutaneous reduction and Kirschner wire internal fixation could significantly improve Bühler angle, Gissane angle, width, and height of Sanders typeⅡ and Ⅲ calcaneal fractures, and the curative effect is satisfactory.

6.Multicenter randomized controlled trial of Yiqi Huoxue formula() for the treatment of ruptured lumbar disc herniation.

OBJECTIVE:
To observe the clinical symptoms and MRI outcomes of patients with ruptured lumbar disc herniation(LDH) through a multicenter randomized controlled study, and to evaluate the clinical efficacy and safety of Yiqi Huoxue formula() in the treatment of this disease.
METHODS:
A total of 160 outpatients and inpatients with ruptured LDH admitted to 4 medical centers from January 2023 to June 2023 were selected and randomly divided into the Yiqi Huoxue formula group and the control group, with 80 patients in each group. In the Yiqi Huoxue formula group, there were 43 males and 37 females, with an age of (41.03±9.56) years and a disease duration of (10.45±25.37) days, and the patients were treated with Yiqi Huoxue formula. In the control group, there were 34 males and 46 females, with an age of (42.14±8.73) years and a disease duration of (11.31±21.14) days;during the acute phase, patients in this group could take celecoxib capsules orally, and methylcobalamin orally at the same time. The Japanese Orthopaedic Association (JOA) score, Oswestry disability index (ODI), changes in the volume of herniated disc tissue on MRI, herniation rate, and absorption rate were recorded at the time of enrollment and during follow-ups at the 3rd, 6th, and 12th month after treatment.
RESULTS:
A total of 156 patients completed the clinical follow-up, and 4 patients withdrew midway. The clinical symptoms of all patients who completed the study were relieved to varying degrees, and reabsorption of herniated disc tissue was observed in all patients in the Yiqi Huoxue formula group after treatment. For the JOA score:in the Yiqi Huoxue formula group, it was (10.73±2.76) points before treatment and (24.65±2.19) points at the 12th month after treatment;in the control group, it was (11.01±1.20) points before treatment and (17.07±3.26) points at the 12th month after treatment. For the ODI score:in the Yiqi Huoxue formula group, it was (26.21±3.55) points before treatment and (5.65±2.19) points at the 12th month after treatment;in the control group, it was (27.92±2.51) points before treatment and (9.09±2.15) points at the 12th month after treatment. At the 12th month after treatment, the JOA and ODI scores of both groups were better than those before treatment, and the scores of the Yiqi Huoxue formula group were better than those of the control group, with statistically significant differences (P<0.05). In terms of the herniated disc volume and herniation rate on MRI, the Yiqi Huoxue formula group was superior to the control group, with statistically significant differences(P<0.05). Reabsorption occurred in 56.96%(45/79) of patients in the Yiqi Huoxue formula group, which was significantly higher than the 37.66%(29/77) in the control group.
CONCLUSION
After treatment with Yiqi Huoxue formula, patients with ruptured LDH show significant improvement in clinical symptoms and a marked reduction in the volume of herniated discs. During the follow-up period, no obvious adverse drug reactions are observed in patients, and no recurrence of symptoms is found at the last follow-up, indicating that the formula has safe and reliable efficacy.

7.BRD4 regulates m6A of ESPL1 mRNA via interaction with ALKBH5 to modulate breast cancer progression.

The interaction between m6A-methylated RNA and chromatin modification remains largely unknown. We found that targeted inhibition of bromodomain-containing protein 4 (BRD4) by siRNA or its inhibitor (JQ1) significantly decreases mRNA m6A levels and suppresses the malignancy of breast cancer (BC) cells via increased expression of demethylase AlkB homolog 5 (ALKBH5). Mechanistically, inhibition of BRD4 increases the mRNA stability of ALKBH5 via enhanced binding between its 3' untranslated regions (3'UTRs) with RNA-binding protein RALY. Further, BRD4 serves as a scaffold for ubiquitin enzymes tripartite motif containing-21 (TRIM21) and ALKBH5, resulting in the ubiquitination and degradation of ALKBH5 protein. JQ1-increased ALKBH5 then demethylates mRNA of extra spindle pole bodies like 1 (ESPL1) and reduces binding between ESPL1 mRNA and m6A reader insulin like growth factor 2 mRNA binding protein 3 (IGF2BP3), leading to decay of ESPL1 mRNA. Animal and clinical studies confirm a critical role of BRD4/ALKBH5/ESPL1 pathway in BC progression. Further, our study sheds light on the crosstalks between histone modification and RNA methylation.
8.Three-dimensional kinematic analysis can improve the efficacy of acupoint selection for post-stroke patients with upper limb spastic paresis: A randomized controlled trial.

BACKGROUND:
China is seeing a growing demand for rehabilitation treatments for post-stroke upper limb spastic paresis (PSSP-UL). Although acupuncture is known to be effective for PSSP-UL, there is room to enhance its efficacy.
OBJECTIVE:
This study explored a semi-personalized acupuncture approach for PSSP-UL that used three-dimensional kinematic analysis (3DKA) results to select additional acupoints, and investigated the feasibility, efficacy and safety of this approach.
DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS:
This single-blind, single-center, randomized, controlled trial involved 74 participants who experienced a first-ever ischemic or hemorrhagic stroke with spastic upper limb paresis. The participants were then randomly assigned to the intervention group or the control group in a 1:1 ratio. Both groups received conventional treatments and acupuncture treatment 5 days a week for 4 weeks. The main acupoints in both groups were the same, while participants in the intervention group received additional acupoints selected on the basis of 3DKA results. Follow-up assessments were conducted for 8 weeks after the treatment.
MAIN OUTCOME MEASURES:
The primary outcome was the Fugl-Meyer Assessment for Upper Extremity (FMA-UE) response rate (≥ 6-point change) at week 4. Secondary outcomes included changes in motor function (FMA-UE), Brunnstrom recovery stage (BRS), manual muscle test (MMT), spasticity (Modified Ashworth Scale, MAS), and activities of daily life (Modified Barthel Index, MBI) at week 4 and week 12.
RESULTS:
Sixty-four participants completed the trial and underwent analyses. Compared with control group, the intervention group exhibited a significantly higher FMA-UE response rate at week 4 (χ2 = 5.479, P = 0.019) and greater improvements in FMA-UE at both week 4 and week 12 (both P < 0.001). The intervention group also showed bigger improvements from baseline in the MMT grades for shoulder adduction and elbow flexion at weeks 4 and 12 as well as thumb adduction at week 4 (P = 0.007, P = 0.049, P = 0.019, P = 0.008, P = 0.029, respectively). The intervention group showed a better change in the MBI at both week 4 and week 12 (P = 0.004 and P = 0.010, respectively). Although the intervention group had a higher BRS for the hand at week 12 (P = 0.041), no intergroup differences were observed at week 4 (all P > 0.05). The two groups showed no differences in MAS grades as well as in BRS for the arm at weeks 4 and 12 (all P > 0.05).
CONCLUSION:
Semi-personalized acupuncture prescription based on 3DKA results significantly improved motor function, muscle strength, and activities of daily living in patients with PSSP-UL.

9.Study on rapid identification of Cremastrae Pseudobulbus Pleiones Pseudobulbus and its adulterants with Fourier exchange infrared spectroscopy

Objective:To establish a Fourier transform infrared spectroscopy (FT-IR) method for rapid identification of Cremastrae Pseudobulbus Pleiones Pseudobulbus and its adulterants.Methods:The Fourier transform infrared spectra of Cremastrae Pseudobulbus Pleiones Pseudobulbus and its adulterants were established, and the second derivative spectral analysis, clustering analysis, principal component analysis, opls-da and cluster independent soft mode classification model were analyzed to explore the difference characteristic peaks of Cremastrae Pseudobulbus Pleiones Pseudobulbus and its adulterants.Results:The first order infrared spectrum showed that the peak shape and peak intensity of Cremastrae Pseudobulbus Pleiones Pseudobulbus and its adulterants were different. Clustering analysis, principal component analysis and OPLS-DA results showed that Cremastrae Pseudobulbus Pleiones Pseudobulbus and its adulterants showed good clustering characteristics. SIMCA method was used to construct the model, and the accuracy of the training set and the verification set were 100%, which further verified the feasibility of this method in identifying the authenticity of Cremastrae Pseudobulbus Pleiones Pseudobulbus.Conclusions:The second-order infrared spectroscopy can accurately distinguish the differences between Cremastrae Pseudobulbus Pleiones Pseudobulbus and its adulterants. The method is fast and accurate, and can be used for the authenticity identification of Cremastrae Pseudobulbus Pleiones Pseudobulbus.
10.Comparison of functional end-to-end esophagojejunostomy and side-to-side esophagojejunostomy with cis-peristalsis in laparoscopic radical resection for esophageal cancer

Objective To compare the effects of functional end-to-end esophagojejunostomy(FETE method)and side-to-side esophagojejunostomy with cis-peristalsis(overlap method)on post-operative rehabilitation,anastomotic leakage,and inflammatory-oxidative stress factors in patients un-dergoing laparoscopic radical resection for esophageal cancer.Methods A total of 115 patients with esophageal cancer were selected as study subjects,and were randomly divided into overlap group(n=57)and FETE group(n=58)using random number table method,and both groups underwent lapa-roscopic radical resection for esophageal cancer.The overlap group received the overlap method,and the FETE group received the FETE method.The surgical-related indicators,postoperative recovery indicators,incidence of anastomotic leakage,inflammatory factors[interleukin-10(IL-10),interleu-kin-6(IL-6),tumor necrosis factor-α(TNF-α)],oxidative stress factors[malondialdehyde(MDA),superoxide dismutase(SOD)],pulmonary function indicators[forced vital capacity(FVC),forced expiratory volume in one second(FEV1),FEV1/FVC],and the scores of the esophageal cancer-specific quality of life questionnaire(QLQ-OES18)were compared between the two groups.Results There was no statistically significant difference in intraoperative blood loss between the two groups(P>0.05).The FETE group had shorter operative time and intraoperative anastomosis time,and a lar-ger number of lymph node dissection compared with the overlap group,with statistically significant differences(P<0.05).The FETE group had earlier postoperative first flatus time,first oral intake time,and drainage tube removal time compared with the overlap group,with statistically significant differences(P<0.05).The incidence of anastomotic leakage was 1.72%in the FETE group and 7.02%in the overlap group,with no statistically significant difference(P>0.05).One week after surgery,the serum levels of IL-10,IL-6,TNF-α and MDA in the FETE group were lower than those in the overlap group,while the serum SOD level was higher,with statistically significant differences(P<0.05).One week after surgery,the FVC,FEV,and FEV1/FVC in the FETE group were higher than those in the overlap group,with statistically significant differences(P<0.05).Three months after surgery,the QLQ-OES18 functional domain scores in the FETE group were higher than those in the overlap group,while the symptom domain and single symptom domain scores were low-er,with statistically significant differences(P<0.05).Conclusion Both the FETE method and the overlap method can reduce intraoperative blood loss and the incidence of anastomotic leakage when applied in laparoscopic radical resection for esophageal cancer.However,FETE method has shorter operative time,larger number of intraoperative lymph node dissections,faster postoperative recovery,and patients have less inflammatory-oxidative stress response and pulmonary function im-pairment,as well as higher quality of life after surgery,showing greater advantages compared with the overlap method.
