1.Different Effects of Fresh and Dried Dendrobium Huoshanense on Chronic Atrophic Gastritis

ObjectiveTo compare the protective effects of water extracts from fresh and dried Dendrobium huoshanense on gastric mucosa in chronic atrophic gastritis (CAG). MethodsMale SD rats (n=72) were randomly divided into 9 groups, with 8 rats in each group, which were normal group, model group, Yangwei Shu (4 g·kg-1) group, low-, medium-, and high-dose fresh D. huoshanense (3.5, 7, and 14 g·kg-1) groups, and low-, medium-, and high-dose dried D. huoshanense (0.7, 1.4, 2.8 g·kg-1) groups. The CAG rat model was successfully established by inducing with N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) and other factors for a total of 11 weeks. Then, the rats were intervened with fresh and dried D. huoshanense for 4 weeks. The serum and gastric tissues of the rats were collected. The changes in gastric juice secretion volume and gastric acid pH value in each group were observed. The gastric mucosal injury was observed by naked eyes and hematoxylin-eosin(HE) staining. The gastric mucus secretion level was determined by Alcian blue and periodic acid-Schiff staining(AB-PAS) staining. The expression levels of tight junction proteins Occludin and ZO-1 in gastric tissues were determined by immunofluorescence. The expression levels of serum pepsinogen Ⅰ (PG Ⅰ), pepsinogen Ⅱ (PG Ⅱ), gastrin 17 (G-17), interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) were determined by enzyme-linked immunosorbent assay (ELISA). The expression levels of aquaporin 1 (AQP1), aquaporin 3 (AQP3), and aquaporin 4 (AQP4) in gastric tissues were determined by Western blot. ResultsCompared with the normal group, the model group showed an obviously reduced gastric juice secretion volume (P0.05), significantly increased gastric acid pH value (P0.01), gastric mucosa with obvious atrophy, and a significantly reduced gastric mucus secretion volume (P0.01). The expression of Occludin and ZO-1 in the gastric mucosal barrier was significantly decreased (P0.01). The levels of PG Ⅰ and PG Ⅱ in the serum were obviously decreased (P0.05, P0.01), and the levels of G-17, IL-1β, IL-6, and TNF-α were significantly increased (P0.01). The expression level of AQP1 in the gastric tissue was significantly upregulated (P0.01), and the expression levels of AQP3 and AQP4 were significantly downregulated (P0.01). Compared with the model group, each drug administration group could improve the gastric mucosal atrophy of CAG model rats to varying degrees, obviously increase the gastric juice secretion volume of the model rats (P0.05, P0.01), significantly decrease the gastric acid pH value (P0.01), obviously increase the gastric mucus secretion volume (P0.05, P0.01), obviously decrease the expression levels of G-17, IL-6, IL-1β, and TNF-α (P0.05, P0.01), obviously increase the expression levels of Occludin, ZO-1, PG Ⅰ, and PG Ⅱ (P0.05, P0.01), obviously upregulate the expression levels of AQP3 and AQP4 (P0.05, P0.01), and obviously downregulate the expression level of AQP1 (P0.05, P0.01). ConclusionThe water extracts of fresh and dried D. huoshanense can exert therapeutic effects on CAG by improving gastric mucosal injury, reducing inflammation, and regulating water metabolism. Moreover, the dried D. huoshanense has a better effect.
2.Mechanism of action of Gegen QinLian Decoction in improving non-alcoholic fatty liver disease by inhibiting ferroptosis based on the Nrf2/SCLC7A11/GPX4 pathway

Objective To investigate the mechanism of action of the nuclear factor erythroid 2-related factor 2(Nrf2)/solute carrier family 7 member 11(SLC7A11)/glutathione peroxidase 4(GPX4)signaling pathway in non-alcoholic fatty liver disease(NAFLD),and to explore the mechanism of Gegen QinLian Decoction for the treatment of NAFLD,using in vivo and in vitro experiments.Methods Rats were fed with high-fat chow for 24 weeks to induce NAFLD,and were then divided randomly into normal(C),model(M),high-,medium-,and low-dose Gegen QinLian Decoction(GGQLT-H,GGQLT-M,GGQLT-L),and metformin(Met)groups.From week 25 onwards,the rats were administered the corresponding drugs by gavage for 2 weeks according to the grouping,until sampling.Levels of the oxidative stress markers malondialdehyde(MDA)and glutathione(GSH)in the liver tissues were measured in each group using biochemical kits and ferrous iron(Fe2+)in rat liver tissues was detected using a Fe2+kit.Nrf2,heme oxygenase-1(HO-1),SLC7A11,glutathione synthetase(GSS),GPX4,and acyl coenzyme A synthetase 4(ACSL4)mRNA levels in rat liver tissues were measured by reverse transcription quantitative polymerase chain reaction.For cellular experiments lipid acc umulation was induced in HepG2 hepatocellular carcinoma cells using 1 mmol/L free fatty acid,to mimic the NAFLD in vitro model.Different concentrations of Gegen QinLian Decoction and metformin-containing serum were added for treatment.Lipid accumulation was detected in the cells in each group by Oil red O staining.The MDA and GSH contents of HepG2 cells in the different groups were determined using appropriate kits,and the ferrous contents were detected using a cell-specific ferrous kit.Expression levels of Nrf2,HO-1,SLC7A11,GSS,GPX4,and ACSL4 mRNA was detected in each group of cells using reverse transcription quantitative polymerase chain reaction.Results In the animal experiments,MDA and Fe2+liver levels were significantly higher in the M group than in the C group,while GSH levels were significantly lower(P<0.01).GGQLT-H,GGQLT-M and Met groups showed significantly reduced MDA and Fe2+and elevated GSH levels compared with the M group(P<0.01,P<0.05).High-and medium-dose Gegen QinLian Decoction and metformin increased Nrf2,HO-1,GSS,and GPX4 mRNA and decreased ACSL4 mRNA expression levels(P<0.01,P<0.05).In cellular experiments,lipid droplets were significantly increased in the HepG2 cell M group compared with those in the C group,and lipid droplets were significantly reduced by Gegen QinLian Decoction and metformin.MDA and Fe2+levels were significantly increased and GSH levels were significantly decreased in the HepG2 M group compared with the levels in the C group(P<0.01),while all doses of Gegen QinLian Decoction and metformin significantly decreased MDA and Fe2+levels(P<0.01)and increased the GSH content(P<0.01,P<0.05).Nrf2,GSS,GPX4,and SLC7A11 mRNA expression levels in the GGQLT-H group,Nrf2,HO-1,and SLC7A11 in the GGQLT-L group,HO-1,SLC7A11,and GSS in the GGQLT-M group,and GSS,Nrf2,and HO-1 in the Met group were all significantly increased compared with the findings in the M group(P<0.01,P<0.05).ACSL4 mRNA expression levels were significantly decreased in the GGQLT-M and GGQLT-L groups and the Met group(P<0.01,P<0.05).Conclusions Gegen QinLian Decoction can improve NAFLD by inhibiting ferroptosis,and its mechanism may he related to regulation of the Nrf2/SLC7A 11/GPX4 signaling pathway.

Non-small cell lung cancer(NSCLC)is one of the most common and deadly malignant tumors worldwide,with surgical resection being the primary treatment for early-stage NSCLC.Tumor spread through air spaces(STAS)is a novel pattern of tumor dissemination into the air spaces in the lung.Its occurrence after sublobar resection is closely associated with recurrence and distant metastasis,making its con-sideration a vital factor in surgical strategy selection and prognostic evaluation.Patients with STAS-positive status exhibit significantly higher postoperative recurrence rates than do STAS-negative patients,with molecular mechanisms involving tumor microenvironment remodeling,specific genetic mutations,and epithelial-mesenchymal transition(EMT).Imaging techniques including computed tomography(CT)and positron emission tomography/CT have shown potential for preoperative STAS prediction,although their accuracy and practicality require improvement.This paper reviews the definition,pathological characteristics,and related mechanisms of STAS,with a focus on surgical ap-proach selection for STAS-positive patients and its role in cancer recurrence after sublobar resection of early-stage NSCLC.Future research directions include optimization of preoperative diagnostic methods for STAS,exploration of molecular targeted therapies,and development of imaging-based precision prediction models.
4.Effects of volatile oil from Acorus tatarinowii on CX3CL1/CX3CR1 signal axis and neuroinflammation in a rat model of tic disorders

AIM To investigate the effects of volatile oil from Acorus tatarinowii Schott(A.tatarinowii)on neuroinflammation in a rat model of tic disorders.METHODS The SD rats were randomly divided into the blank group(8 rats)and the model group(40 rats).The rat models of tic disorders established successfully by intraperitoneal injection of iminodiapropionitrile(IDPN)were further divided into the model group,the tiapride group and the high-dose,moderate-dose and low-dose A.tatarinowii volatile oil groups,with 8 rats in each group.The 4-week intragastric treatment of respective drug was initiated the next day after the completion of modeling,and normal saline was dosed upon the blank group and the model group,during which the rats' behavioral changes were assessed by stereotyped behavior and motor behavior score every week.After the administration,the rats had their morphological changes of striatal neurons observed by Nissl staining;their levels of TGF-β,IL-10,TNF-αand IL-1β in serum and striatum detected by ELISA;their striatal protein expressions of CX3CL1 and CX3CR1 detected by Western blot and immunohistochemistry;and their striatal expressions of M1,M2 microglia marker proteins CD86,CD206,SYN and PSD-95 detected by immunofluorescence co-staining.RESULTS Compared with the model group,the A.tatarinowii volatile oil groups demonstrated improved twitch-like behavior;decreased scores of motor behavior and rigid behavior(P<0.01);alleviated damage of Nissl bodies in neurons;increased serum and striatum levels of TGF-β and IL-10(P<0.05,P<0.01);decreased levels of TNF-α and IL-1β(P<0.01);decreased striatal protein expressions of CX3CL1 and CX3CR1(P<0.01);increased protein expressions of PSD95 and SYN(P<0.05,P<0.01);and decreased CD86/Iba1(P<0.01)and increased CD206/Iba1(P<0.01)in terms of the fluorescence intensity.CONCLUSION A.tatarinowii volatile oil contributes an anti-tic effect and improves the neuroinflammation in the brain of the rat model of tic disorders by promoting the transformation of microglia into M2 type via CX3CL1/CX3CR1 signal axis.
5.Benefit prediction and management of large medical equipment of daytime surgery center under DIP payment mode

Objective:To explore the effect of benefit prediction and management method of large medical equipment of daytime surgery center under the diagnosis-intervention packet(DIP)payment mode on improving the efficiency of resource utilization and the medical quality of hospital.Methods:According to the DIP payment mode,the benefit prediction and management of large medical equipment were carried out at different stages included pre-hospital,in-hospital and post-hospital.From April 2022 to April 2024,a total of 240 patients who received treatment in the daytime surgery center of Deyang People's Hospital were selected.According to the application of benefit prediction and management of large medical equipment of daytime surgery center under the DIP payment mode,120 patients were divided into control group and observation group,with 120 cases in each group.The medical quality,the efficiency of equipment utilization and the financial benefits of the patients of two groups,who received daytime surgery,under DIP payment mode were compared.A self-made questionnaire was adopted to investigate the overall satisfaction of the patients of two groups,who received daytime surgery,for daytime surgery by using the method of telephone communication.Results:The incidence of postoperative complications,and the rate of readmitted to hospital in the observation group were respectively 5.00%and 4.17%,which were lower than those(13.33 and 11.67%)in the control group,the success rate of surgery in the observation group was 95%,which was higher than that in the control group(86.67%),and the differences were statistically significant(x2=5.004,4.630,0.641,P<0.05).The average inpatient cost,total cost of the operation of medical equipment and DIP settlement fee of the patients of undergoing daytime surgery in the observation group were all lower than those in the control group,and differences were statistically significant(t=2.168,8.512,14.474,P<0.05).The frequency of using equipment and the average scheduling efficiency of the patients of undergoing day surgery in the observation group were higher than those in the control group,while the average failure rate of equipment and the frequency of maintenance for equipment were lower than those in the control group,and the differences were statistically significant(t=23.211,21.585,5.643,23.944,P<0.05).The average satisfaction of the patients of undergoing daytime surgery in the observation group was higher than that in the control group,and the difference was statistically significant(t=8.46,P<0.05).Conclusion:The benefit prediction and management method of large medical equipment in daytime surgery center under the DIP payment mode can improve the operation efficiency and medical quality of the daytime surgery center,and reduce the operation cost of hospital,and promote the optimization allocation of medical resources of hospital.
6.Establishment and application of ultra-fast real-time PCR for Brucella detection

This study was aimed at establishing a method of ultra-fast quantitative PCR for Brucella detection.We used an exogenous recombinant plasmid as the internal reference and targeted the T4SS secretion system,an important Brucella viru-lence factor,to design specific primers and probes.The sensitivity,specificity,and repeatability of this method were evaluated,and a standard curve was constructed.The coincidence rate of detection findings with this method versus quantitative PCR was determined.This method markedly decreased the detection time to only 10 minutes.The standard curve demonstrated a good linear relationship(Y=-3.410 7x+38.357,R2=0.998 5)with a low minimum detection limit of 10 copies/μL.The method exhibited good specificity and did not specifically amplify several common clinical bacteria other than Brucella.The de-tection of three concentrations of positive plasmids yielded coefficients of variation(CVs)of 0.20%to 0.91%,thus demonstra-ting the method's excellent repeatability.Furthermore,140 clinical samples were analyzed concurrently with the fluorescence PCR method,which yielded a 100%compliance rate and consistent results.Our findings indicated that the Brucella ultra-fast quantitative PCR was ultrafast;had high sensitivity,high specificity,and good specificity;and can be used for the clinical de-tection of Brucella and emergency investigation of epidemics.Therefore,this method is valuable for the early diagnosis of Bru-cella.
7.Design and Validation of Scoliosis Orthosis Based on Finite Element Model

Objective Based on the finite element simulation analysis of the patient's torso-spine model and combined with theoretical calculation data,an individualized scoliosis orthosis was designed,and the effectiveness of the orthosis was verified through three-dimensional(3D)printing.Methods A patient with idiopathic scoliosis was chosen as the research object.Reverse engineering technology and computer-aided technology were used to establish the torso-spine model of the patient.The finite element method was used to analyze the model,and the optimal position and magnitude of the corrective force were determined by combining literature theory calculation.Based on this,an orthosis was designed.To verify the orthopedic effect,the patient's X-rays before and after wearing the orthosis were compared and evaluated,and the patient was followed up 6 months later.Results The optimal position and magnitude of the initial corrective force were determined through theoretical calculations and finite element simulations.Specifically,a 62.95 N corrective force applied to the L3 vertebral body and the left posterior region corresponding to the upper and lower intervertebral discs in the patient's lateral curvature segment of the spine to achieve the optimal orthopedic effect.On this basis,the orthosis was designed,followed by relevant experimental tests before and after wearing the designed orthosis.By comparing X-ray images of the patient before and after wearing the orthosis and combining them with follow-up data six months later,the optimized design of the orthosis met the expected clinical requirements for orthopedic effects.Conclusions The design of orthosis needs to be personalized according to the specific situation of patients with scoliosis.This study takes a patient with idiopathic scoliosis as the research object,providing new ideas and methods for the design of orthosis for patients with idiopathic scoliosis.
8.Mesothelin-specific T-cell immune reconstitution after hematopoietic stem cell transplantation in acute myeloid leukemia

Objective:To investigate the pattern of Mesothelin(MSLN)-specific T-cell(CTLs)immune reconstitution after allogeneic hematopoietic stem cell transplantation.Methods:Two cases of Mesothelin-positive patients with acute myeloid leukemia(AML)were screened,and the number of MSLN-CTLs and its subpopulations,the expression of surface PD-1/CTLA-4/TIM-3 im-mune exhaustion molecules,and the functions of secreted cytokines TNF-α and IFN-γ were monitored by flow cytometry after trans-plantation.They were compared with WT1-CTLs and cytomegalovirus(CMV)CTLs.Meanwhile,the relationship between CTL recon-stitution and Minimal residual disease(MRD)and leukemia recurrence was analyzed.Results:After transplantation,MSLN-CTLs,WT1-CTLs and CMV-CTLs can be detected,and intracellular factors were secreted.The phenotypes of WT1 specific CD8+T cells,MSLN specific CD8+T cells and CMV specific CD8+T cells were mainly TEM subsets and TEMRA subsets,and the TEM subsets of CMV specific CD8+T cells were more obvious.Compared with CMV-CTLs,the proportions of T Naive,TCM and TEMRA subsets were relatively higher in MSLN-specific CD8+T cells and WT1-specific CD8+T cells,and the expression levels of PD-1,CTLA-4 and TIM-3 were higher in MSLN-CTLs and WT1-CTLs.The dynamic changes of MSLN-CTLs and WT1-CTLs after transplantation were related to leukemia load and the chimerism rate of donor and recipient.Conclusion:After allogeneic hematopoietic stem cell transplantation,im-mune recovery to MSLN is found,which is different from WT1-CTLs and CMV-CTLs.
9.Investigation on Risk Stratification of Atherosclerotic Cardiovascular Disease and Target Achievement of Lipid and Blood Pressure in Community-Based Hypertensive Patients

Objective:This study aimed to investigate the atherosclerotic cardiovascular disease(ASCVD)risk stratification and target achievement of lipid and blood pressure control among community-based hypertensive patients,with the goal of optimizing integrated management strategies.Methods:A total of 2832 hypertensive patients registered in 2021 at the Bicheng Community Health Service Center in Bishan District of Chongqing,were included.Baseline data were collected through retrospective analysis of health records.Non-high-density lipoprotein cholesterol(non-HDL-C)levels and estimated glomerular filtration rate(eGFR)were calculated.ASCVD risk stratification was performed,and target achievement for lipid and blood pressure control were analyzed,including comparisons among patients with different comorbidities.Results:Based on ASCVD risk stratification,patients were categorized as follows:ultra-high risk(22 cases,0.78%),very high risk(111 cases,3.92%),high risk(1324 cases,46.75%),moderate risk(997 cases,35.20%),and low risk(378 cases,13.35%).The LDL-C target achievement rate was 4.55%(1/22)in the ultra-high risk group and 15.32%(17/111)in the very high risk group,with blood pressure target achievement rate of 18.18%(4/22)and 11.71%(13/111),respectively.In the high-risk group,LDL-C and blood pressure target achievement rate were only 4.76%(63/1324)and 8.08%(107/1324),while moderate-risk groups showed 25.68%(256/997)and 26.18%(261/997),respectively.The low-risk group achieved 99.74%(377/378)LDL-C target achievement and 30.69%(116/378)blood pressure target achievement.Patients with ischemic stroke had a significantly higher lipid target achievement rate(13.73%,7/51)compared to non-ischemic stroke patients(6.40%,178/2781)(P<0.05).Similarly,those with coronary heart disease(12.65%,13/87)exhibited higher lipid target achievement than non-coronary heart disease patients(6.27%,172/2745)(P<0.05).However,no significant difference was observed between hypertensive patients with diabetes(8.04%,52/647)and non-diabetic patients(6.09%,133/2185)(P>0.05),or between those with chronic kidney disease(CKD)stages 3/4(6.72%,16/238)and non-CKD 3/4 patients(6.52%,169/2594)(P>0.05).Conclusion:Over half of the community-based hypertensive patients were classified as high-risk or above in ASCVD stratification,yet their lipid and blood pressure target achievement rates were markedly suboptimal.Hypertension patients with comorbidities,particularly diabetes or CKD stages 3/4,showed poor lipid target achievement.These findings underscore the necessity of incorporating ASCVD risk stratification into community management assessments for hypertensive patients,enhancing personalized management for high-risk populations,and prioritizing lipid target achievement in those with diabetes or CKD stages 3/4.

Thoracic anesthesia with spontaneous respiration represents a form of precision anesthesia meticulously customized to individual patients.Considering the more stringent requirements this anesthesia approach imposes on the regulation of respiratory function,the writing group of the"Consensus of Experts on the Management of Thoracic Anesthesia with Spontaneous Respiration"has formulated elaborate guidelines regarding indications and contraindications,preoperative evaluation,anesthesia implementation,common complications,and treatment strategies.This was accomplished by referencing relevant domestic and international literature and integrating it with actual clinical requirements.The objective is to standardize the rational application of this anesthesia method.
