Body weight abnormalities, including overweight, obesity, and underweight, have become a dual public health challenge in Chinese adults: overweight and obesity lead to a variety of chronic complications, while underweight increases the risks of malnutrition, sarcopenia, and organ dysfunction. To systematically address these issues, multidisciplinary experts in endocrinology, sports science, nutrition, and psychiatry from various regions have held multiple weight management seminars. Based on the latest epidemiological data and clinical evidence, they expanded the guideline to include assessment and intervention strategies for underweight, in addition to the core content of obesity management. This guideline outlines the etiological mechanisms, evaluation methods, and multidimensional management strategies for overweight and obesity, covering key areas such as diagnosis and assessment, medical nutrition therapy, exercise prescription, pharmacological intervention, and psychological support. It is intended to provide a scientific and standardized approach to weight management across the adult population, aiming to curb the rising prevalence of obesity, mitigate complications associated with abnormal body weight, and improve nutritional status and overall quality of life.

4.Screening of Illicit Oxyphenisatin Substances in Weight-Loss Dietary Supplements by High Performance Liquid Chromatography-in-Source-Fragmentation-Quadrupole Time-of-Flight Mass Spectrometry

By summarizing the mass spectrometric fragmentation patterns of oxyphenisatin substances,an analytical method was established for screening of illegally added oxyphenisatin compounds in weight-loss health foods using high performance liquid chromatography-in-source-fragmentation-quadrupole time-of-flight mass spectrometry(HPLC-ISF-QTOF-MS),along with a quantitative method for 11 kinds of oxyphenisatin compounds.Based on the developed screening method,an oxyphenisatin derivative was discovered in the reference standards,which was tentatively identified as 4-(3-(4-hydroxyphenyl)-2-oxoindolin-3-yl)phenyl acetate and confirmed by MS/MS analysis.The results showed that all 11 kinds of oxyphenisatin compounds had correlation coefficients greater than 0.9971,with limits of detection(LODs)ranging from 0.12 to 0.68 μg/L and limits of quantification(LOQ)from 0.21 to 2.29 μg/L.The LODs for 11 kinds of characteristic ions ranged from 0.45 to 9.11 μg/kg.At spiking levels of 25,50 and 100 μg/kg,the recoveries ranged from 78.9%to 117.3%.The instrumental precision,intra-day method precision and inter-day method precision were 0.23%?1.70%,0.7%?2.4%,and 1.1%?3.3%,respectively.The developed targeted and non-targeted detection method demonstrated high sensitivity,strong stability,rapid analysis,and an expanded screening range for oxyphenisatin substances,and provided robust technical support for regulatory authorities in combating illegal adulteration.
5.Effect of positive ruminative thinking training on negative emotions in depression patients during modified electroconvulsive therapy

Objective To explore the effect of positive ruminative thinking training(PRT)on negative emotions during modified electroconvulsive therapy(MECT)in depression patients.Method A total of 80 patients with depression who received MECT at Ganzhou Third People's Hospital from February 2024 to February 2025 were selected as subjects.Using a random number table method,they were divided into control group(n=40)and observation group(n=40).The control group received standard interventions,while the observation group received PRT in addition to control group's interventions.Both groups continued treatment until the completion of MECT,with all patients receiving 8-12 sessions of MECT.Negative emotions,self-efficacy,and the positive and negative ruminative thinking(PNR)scale were compared between two groups.Before and after intervention,adverse reactions were evaluated in both groups post-treatment.Results After intervention,the scores of anxiety,depression scale,and negative factor rumination thinking in observation group were lower than those in control group.The scores of general self-efficacy scale and positive factor rumination thinking were higher than those in control group,and the differences were statistically significant(P<0.05).The total incidence of adverse reactions in observation group was lower than that in control group(P<0.05).Conclusion PRT can reduce negative emotions and negative rumination thinking in depression patients during MECT,improve positive rumination thinking and self-efficacy.

Ischemic heart disease(IHD)is one of the major threats to global health,characterized by complex and incompletely elucidated pathogenesis.Recently,with the continuous advancement of epige-netic research,lactylation(Kla),a newly discovered type of protein post-translational modification,has gradually attracted attention.Kla significantly influences the pathophysiological processes and cellular molecular functions of IHD by directly affecting gene transcription,signal transduction,and metabolic pathways.Kla extensively occurs in both histones and non-histone proteins and participates in regulating protein functions involved in various pathological processes.By modulating enzymatic activities and signal transduction pathways,Kla affects multiple processes in cardiomyocytes,including energy metabolism,inflammatory response,angiogenesis,lipid metabolic disorders,apoptosis,fibrosis,and myocardial re-pair.Although current studies on specific mechanisms and therapeutic targets of Kla in IHD remain limit-ed,its potential therapeutic value cannot be overlooked.This review summarizes the mechanisms and re-search progress of Kla in critical pathological stages of IHD,such as myocardial infarction,myocardial is-chemia-reperfusion injury,heart failure,and cardiac hypertrophy.Furthermore,we discuss the potential therapeutic targets and application prospects of Kla,aiming to provide insights and directions for identif-ying effective intervention strategies and opening new avenues for the prevention and treatment of IHD.

Objective:To report the nationwide surveillance results of pathogenic profiles and antimicrobial resistance patterns of Gram-positive bloodstream infections in China in 2023.Methods:The clinical isolates of Gram-posttive bacteria from blood cultures were collected in member hospitals of National Bloodstream Infection Bacterial Resistant Investigation Collaborative System(BRICS)during January to December 2023. Antimicrobial susceptibility testing was performed using the dilution method recommended by the Clinical and Laboratory Standards Institute(CLSI). Statistical analyses were conducted using WHONET 5.6 and SPSS 25.0 software.Results:A total of 4 385 Gram-positive bacterial isolates were obtained from 60 participating center. The top five pathogens were Staphylococcus aureus( n=1 544,35.2%),coagulase-negative Staphylococci( n=1 441,32.9%), Enterococcus faecium( n=574,13.1%), Enterococcus faecalis( n=385,8.8%),and α-hemolytic Streptococci( n=187,4.3%). The prevalence of methicillin-resistant Staphylococcus aureus(MRSA)and methicillin-resistant coagulase-negative Staphylococci(MRCNS)was 26.2%(405/1 544)and 69.8%(1 006/1 441),respectively. Notably,all Staphylococci remained susceptible to glycopeptide or daptomycin. Staphylococcus aureus demonstrated excellent susceptibility(>97.0%)to cephalobiol,rifampicin,trimethoprim-sulfamethoxazole,linezolid,minocycline,tigecycline,and eravacycline. No Enterococcus exhibiting resistance to linezolid were detected. Glycopeptide resistance was uncommon but more frequent in Enterococcus faecium(resistance to vancomycin and teicoplanin:both 1.7%)compared to Enterococcus faecalis(both 0.3%). The detection rates of MRSA and MRCNS exhibited significant regional variations across the country( χ2=17.674 and 148.650,respectively,both P<0.001). No vancomycin-resistant Enterococci were detected in central China. Institutional comparison demonstrated higher prevalence of MRSA( χ2=14.111, P<0.001)and MRCNS( χ2=4.828, P=0.028)in provincial hospitals than that in municipal hospitals. Socioeconomic analysis identified elevated detection rates of both MRSA( χ2=18.986, P<0.001)and MRCNS( χ2=4.477, P=0.034)in less developed regions(per capita GDP

Objective:To report the results of bacterial resistant investigation collaborative system(BRICS)on the distribution and antimicrobial resistance profile of clinical Gram-negative bacteria isolates from bloodstream infections in China in 2023,and provide reference for clinical tretment of bloodstream infections and prevention and control of bacterial resistance.Methods:The clinical isolates of Gram-negative bacteria from blood cultures in member hospitals of BRICS were collected during January 2023 to December 2023. Antibiotic susceptibility tests were conducted by agar dilution or broth dilution methods recommended by Clinical and Laboratory Standards Institute(CLSI). WHONET 5.6 and SPSS 25.0 were used to analyze the data.Results:During the study period,11 492 strains of Gram-negative bacteria were collected from 60 hospitals,of which 10 098(87.9%)were Enterobacterales and 1 394(12.1%)were non-fermentative bacteria. The top 5 bacterial species were Escherichia coli(50.0%), Klebsiella pneumoniae(26.1%), Pseudomonas aeruginosa(5.1%), Acinetobacter baumannii complex(5.0%)and Enterobacter cloacae complex(4.1%). The ESBL-producing rates in Escherichia coli, Klebsiella pneumoniae and Proteus mirablilis were 46.8%(2 685/5 741),18.3%(549/2 999)and 44.0%(77/175),respectively. The prevalence of carbapenem-resistant Escherichia coli(CREC)and carbapenem-resistant Klebsiella pneumoniae(CRKP)were 1.3%(76/5 741)and 15.0%(450/2 999);32.9%(25/76)and 78.0%(351/450)of CREC and CRKP were sensitive to ceftazidime/avibactam combination,respectively. 94.7%(72/76)and 90.2%(406/450)of CREC and CRKP were sensitive to aztreonam/avibactam combination. Furthermore,57.9%(44/76)and 79.1%(356/450)were sensitive to imipenem/relebactam combination. The prevalence of carbapenem-resistant Acinetobacter baumannii(CRAB)complex was 64.6%(370/573),while more than 80.0% of CRAB complex was sensitive to tigecycline,eravacycline and polymyxin B. The prevalence of carbapenem-resistant Pseudomonas aeruginosa(CRPA)was 17.0%(99/581). There were differences in the composition ratio of Gram-negative bacteria in bloodstream infections and the prevalence of important Gram-negative bacteria resistance among different regions in China,with statistically significant differences in the prevalence of CREC,CRKP,CRPA and CRAB complex( χ2=10.6,28.6,10.8 and 19.3, P<0.05). The prevalence of ESBL-producing Escherichia coli, CREC,CRAB complex and CRKP were higher in provincial hospitals than those in municipal hospitals( χ2=12.5,9.8,12.7 and 57.8,all P<0.01). Conclusions:Gram-negative bacteria are the main pathogens causing bloodstream infections in China,and Escherichia coli is ranked in the top,while the trend of Klebsiella pneumoniae increases continuously with time. CRKP infection shows a slow upward trend,CREC infecton maintains a low prevalence level,and CRAB complex infection continues to exhibit a high prevalence rate. The composition and resistance patterns of pathogens causing bloodstream infections vary to some extent across different regions and levels of hospitals in China.
9.Outcomes of transcatheter transseptal mitral valve-in-valve replacement using Edward's SAPIEN 3 in high surgical risk patients-a multicenter study in China

Objective To evaluate the safety and efficacy of valve-in-valve transcatheter mitral valve replacement(ViV-TMVR)in patients with bioprosthetic valve degeneration who are at high surgical risk.Methods This study is a multi-center,retrospective cohort analysis of 20 consecutive patients who underwent transseptal ViV-TMVR using the Edwards SAPIEN 3 transcatheter heart valve(THV).The primary endpoints include technical success and procedural success,both defined according to the Mitral Valve Academic Research Consortium(MVARC)criteria,as well as mortality and functional change assessed based on New York Heart Association(NYHA)classification at 30-days and six months post-procedure.Clinical follow-up assessments are conducted at 30-days and six months.Results From February 2021 to October 2022,a total of 20 patients with symptoms of bioprosthetic valve degeneration were enrolled across nine sites in China.The patients had a mean age of(73.5±5.5)years,with 85.0%being females and 70.0%classified as NYHA class Ⅲ/Ⅳ.The study achieved a 100.0%technical success rate and a 90.0%procedural success rate finally.All patients remained alive during the 30-day follow-up period.However,six months post-intervention,two patients(10.0%)were re-hospitalized due to heart failure,and sadly,one of them(5.0%)died.None of the patients reported any adverse events related to ViV-TMVR during the follow-up period.Notably,there was a significant improvement in NYHA class compared to baseline(P=0.0004)at six-month follow-ups.Conclusions The transseptal ViV-TMVR technique proved to be highly successful and was associated with significant improvement in NYHA class function.These findings strongly suggest that it serves as a safe and efficient treatment alternative for high-risk patients suffering from bioprosthetic valve degeneration.
10.Operational management survey and countermeasure research of tertiary maternal and child health hospitals in guizhou province

Objective To investigate the current status of operational management in tertiary maternal and child health(MCH)hospitals in Guizhou Province and propose recommendations for refining their operational management practices.Meth-ods A self-designed electronic questionnaire was distributed to seven tertiary MCH hospitals in Guizhou Province.The survey fo-cused on fundamental dimensions of operational management,particularly the implementation of refined management practices,with results analyzed using descriptive statistics.Results Among the seven hospitals,five had independently established opera-tional management departments,with three staffing 3-4 personnel.Sixty percent of the hospitals engaged in data statistics,strate-gic management,and process optimization.Only two hospitals had implemented operational management information systems(HRP),but these systems featured incomplete modules and lacked interconnectivity between subsystems,failing to meet opera-tional needs.Additionally,while total expenditures increased year-over-year across all hospitals,insufficient government compen-sation has heightened operational pressures.Conclusions Operational management in Guizhou's tertiary MCH hospitals faces multiple challenges,including incomplete organizational structures and systems,inadequate execution of core functions,subopti-mal information systems for refined management,and limited fiscal support.Recommendations include establishing a multi-tiered operational management framework,implementing comprehensive long-term safeguards,and developing an integrated data man-agement platform.Simultaneously,improving government compensation mechanisms through increased fiscal investment and sup-portive policies is critical to enhancing operational capabilities and achieving high-quality development.
