1.Exploration on the Mechanism of Sanzi Sijun Formula in Non-alcoholic Fatty Liver Disease Based on Network Pharmacology and Experimental Validation

Objective To explore the effects and mechanisms of Sanzi Sijun Formula(SSF)in non-alcoholic fatty liver disease(NAFLD)through network pharmacology,molecular docking and molecular dynamics simulation;To carry out experimental validation in vivo and in vitro.Methods The active components and target genes of SSF were screened using TCMSP,TCMIP and TCMIO databases.NAFLD-related targets were screened using the GeneCards database,and the intersection targets were obtained to construct a protein-protein interaction network and screen for core targets.The intersection targets were imported into the DAVID database for GO and KEGG enrichment analysis.Molecular docking was performed using AutoDock Vina software between the key active components of SSF and core targets,and molecular dynamics simulations were conducted using Gromacs 2022 for 100 ns.C57BL/6J mice NAFLD model was established by diet induction.SSF was administered by gavage for 8 weeks.Liver histopathological changes and the levels of non-esterified fatty acids(NEFA)were detected.In vitro NAFLD model was established by inducing AML12 cells with palmitic acid(PA)for 24 hours.SSF-containing serum was added to incubate simultaneously.The lipid accumulation and cell viability were detected.The core targets of SSF intervention in the in vitro and in vivo NAFLD models were verified by RT-qPCR and Western blot.Results Network pharmacological analysis identified 75 active components in SSF and revealed 179 shared targets between these components and NAFLD.Ten main active components including arachidonate,12-senecioyl-2E,8E,10E-atractylodin,cerebrosterol,glycyrrhizol B and sinapic acid,etc.as well as 8 core targets were identified.GO enrichment analysis of targets mainly involved protein phosphorylation,inflammatory response,and apoptosis,while the KEGG enrichment analysis mainly included AGE-RAGE,TNF,AMPK,PPAR and NF-κB signaling pathways.Molecular docking demonstrated that the major active components of SSF exhibited favorable binding affinity and stability with the core targets.Molecular dynamics simulation confirmed the stability of the complex of glyasperin B with AKT1,SIRT1,STAT3,PPARG,and TNF.SSF alleviated the pathological damage of liver tissues in mice NAFLD model,reduced NAS score and NEFA levels in liver tissues(P<0.05).Additionally,SSF reversed lipid accumulation and decreased cell viability of PA-induced AML12 cells(P<0.01).Further in vivo and in vitro experiments demonstrated that SSF significantly reversed the elevated mRNA levels of TNF-α,IL-6,IL-1β and PPARγ and protein expression of STAT3(P<0.05,P<0.01)in NAFLD models,up-regulated the protein levels of SIRT1 and p-Akt/Akt(P<0.05,P<0.01).Conclusion SSF can improve NAFLD of both in vitro and in vivo models.The regulation of multiple targets,such as AKT,SIRT1,STAT3 and PPARG,by its multiple active components,and adjustment of multiple approaches,such as lipid metabolism disorder,inflammatory responses,are involved in the potential underlying mechanisms.

Objective To explore the effect of the pneumatic and electric hand training system with multiple sensory stimulation mo-dalities combined with low-frequency repetitive transcranial magnetic stimulation on hand movement and tactile pressure sensation in patients with stroke.Methods From May,2023 to October,2024,62 stroke patients from the First Affiliated Hospital of University of South China were randomly divided into control group(n=31)and experimental group(n=31).Both groups received conventional rehabilitation,and 1 Hz low-frequency repetitive transcranial magnetic stimulation in the primary cerebral motor cortex on the unaffected side,while the experimental group received the pneumatic and electric hand training system with multiple sensory stimulation modalities in addition,for four weeks.They were as-sessed with Fugl-Meyer Assessment-Upper Extremities(FMA-UE),Motor Assessment Scale(MAS),Semmes Weinstein Monofilament Test(SWMT)and modified Barthel index(MBI)before and after treatment.Results One case dropped out in control group and one in the experimental group.After treatment,the scores of FMA-UE,MAS,SWMT and MBI increased in both groups(|t|>5.464,P<0.001),expect the score of SWMT in the control group(P>0.05),and all scores were better in the experimental group than in the control group(|t|>2.195,P<0.05).Conclusion The pneumatic and electric hand training system with multiple sensory stimulation modalities combined with low-frequency repetitive transcranial magnetic stimulation can improve the motor and tactile pressure func-tion of stroke patients,and improve their activity of daily living.

5.Anti-fatigue effect of Wujia Shengmai Yin in mice and its mechanism

Objective:To discuss the anti-fatigue effect of Wujia Shengmai Yin,and to clarify its mechanism.Methods:Thirty-six male ICR mice were randomly devided into control group(equivalent volume of distilled water),Shengmai Yin group(500 mg·kg?1 of Shengmai Yin),and Wujia Shengmai Yin group(600 mg·kg?1 of Wujia Shengmai Yin).The body weights of the mice in various groups were detected every 7 d,and the mental states were observed.The rotating rod test and exhaustive swimming test were used to detect the duration on the rod and the swimming time to exhaustion of the mice in various groups,respectively;the levels of urea nitrogen(BUN)and lactate(LA),and the activities of lactate dehydrogenase(LDH)in serum,the levels of liver glycogen(LG),muscle glycogen(MG),and malondialdehyde(MDA),and activities of glutathione peroxidase(GSH-Px)and superoxide dismutase(SOD)in muscle tissue of the mice in various groups were detected by kits;the expression levels of glucose metabolism-related proteins in liver tissue of the mice in various groups were detected by Western blotting method.Results:Compared with before experiment,the body weights after experiment of the mice in various groups showed a increasing trend but the differences were not statistically significant(P>0.05).The rotating rod test results showed that compared with control group,the duration on the rod of the mice in Wujia Shengmai Yin group was significantly increased(P<0.01).The exhaustive swimming test results showed that compared with control group,the swimming time to exhaustion of the mice in Shengmai Yin group and Wujia Shengmai Yin group was significantly increased(P<0.01).Compared with control group,the levels of BUN in serum of the mice in Shengmai Yin group and Wujia Shengmai Yin group were significantly decreased(P<0.01),and the activities of LDH were significantly increased(P<0.01);the level of LA of the mice in Wujia Shengmai Yin group was significantly decreased(P<0.01).Compared with Shengmai Yin group,the levels of BUN and LA in the serum and LDH activity of the mice in Wujia Shengmai Yin group were significantly decreased(P<0.01).Compared with control group,the levels of LG in liver tissue and the levels of MG in muscle tissue of the mice in Shengmai Yin group and Wujia Shengmai Yin group were significantly increased(P<0.01);compared with Shengmai Yin group,the level of LG in liver tissue and the level of MG in muscle tissue of the mice in Wujia Shengmai Yin group were increased(P<0.01).Compared with control group,the activities of GSH-Px and SOD in muscle tissue of the mice in Shengmai Yin group and Wujia Shengmai Yin group were significantly increased,and the levels of MDA in Shengmai Yin group and Wujia Shengmai Yin group was significantly decreased(P<0.01);compared with Shengmai Yin group,the activities of GSH-Px and SOD in muscle tissue of the mice in Wujia Shengmai Yin group were significantly increased and the level of MDA was decreased(P<0.01).The Western blotting results showed that compared with control group,the expression levels of phosphorylated phosphoinositide 3-kinase(p-PI3K),phosphorylated protein kinase B(p-AKT),phosphorylated glycogen synthase kinase 3 beta(p-GSK3β),and glycogen synthase(GS)proteins in liver tissue of the mice in Shengmai Yin group and Wujia Shengmai Yin group were significantly increased(P<0.05 or P<0.01);compared with Shengmai Yin group,the expression levels of p-PI3K,p-AKT,p-GSK3β,and GS proteins in liver tissue of the mice in Wujia Shengmai Yin group were significantly increased(P<0.01).Conclusion:Wujia Shengmai Yin enhances the anti-fatigue effect by activating the phosphoinositide 3-kinase(PI3K)/protein kinase B(AKT)/glycogen synthase kinase 3 beta(GSK3β)signaling pathways,and improves the body's antioxidant capacity,and increases the glycogen synthesis.
6.Expression of MicroRNA-146b in the Serum of Patients with Viral Myocarditis and Its Clinical Significance

Objective:To detect the serum level of microRNA-146b (miR-146b)in patients with viral myocarditis,and prelimi-narily explore its clinical significance.Methods:Forty patients with viral myocarditis and forty healthy controls were enrolled in the study.The serum level of miR-146b was detected with reverse transcription-polymerase chain reaction (RT-PCR).The level of cardiac troponin T (cTnT)in the serum of patients with viral myocarditis was detected with electrochemical lumines-cence method.The relationship between miR-146b and cTnT level was statistically analyzed.Results:Compared with the ser-um level of miR-146b (1 .35±0.42)in healthy controls,the serum level of miR-146b(1 .85 ±0.68)in the patients with viral myocarditis increased significantly(t=3.957,P <0.01).The serum level of miR-146b was positively correlated with the level of cTnT in the patients with viral myocarditis (r=0.635,P <0.05).Conclusions:The serum level of miR-146b in the patients with viral myocarditis increased,and it was correlated with the severity of myocardial injury.It indicates that miR-146b may be involved in the pathogenesis of viral myocarditis.
7.The effects of gossypol acetate on the morphological features and the gene expression in the bone tissues of the femoral head of Sprague-Dawley rat in vivo after treated with dexamethasone

Objective To explore the effects of gossypol acetate on the morphological features and the gene expression in the femoral head of Sprague-Dawley rat in vivo after treated with dexamethasone .Methods Dexamethasone(Dex) was injected into the abdominal cavity of SD rats at an dose of 10 mg/kg ,twice a week ,and feed gossypol acetate 5 mg · kg -1 · d-1 .The controls re-ceived saline 2 mL injection .The treatment lasted for 12 and 20 weeks .The slices of the femoral head were made for HE and immu-nohistochemical study .The total mRNA was extracted for RT-PCR assessment .Results The cancellous bone trabecular became sparse ,trabecular bone area ratio decreased ,bone marrow fat tissue increased .These changes were fitted for pathological character of bone necrosis .The gossypol acetate could not affect the pathological changes .The proportion of the positive stained osteoblasts increased ,adipocytes decreased .PPARγ,C/EBPα,11β-HSD1 expression enhanced ,Runx2 down regulated in the treatment groups and GAA group .Conclusion Dex can induce evident pathological changes conform to the characters of femoral head necrosis .They may have closed correlation between 11β-HSD1 and the gene expression .But GAA could not affected the pathological changes and abnormality of the gene expression .
8.The expressive level and clinical significance of tumor necrosis factor ligands related molecule 1A in peripheral blood of patients with viral myocarditis

Objective To explore the expressive level and clinical significance of tumor necrosis factor ligands related molecule 1A (TL1A) in peripheral blood of patients with viral myocarditis.Methods In 70 patients with viral myocarditis (viral myocarditis group) and 70 normal controls (control group),the plasma level of TL1A was detected by enzyme linked immunosorbent assay and the mRNA level of TL1A was detected by real-time quantitative reverse transcription polymerase chain reaction.Results The plasma level of TL1A in viral myocarditis group was significantly higher than that in control group [(1.37 ± 0.41) μg/L vs.(0.85 ± 0.22) μg/L](P=0.000).The level of peripheral blood TL1A mRNA in viral myocarditis group was significantly higher than that in control group (0.39 ±0.17 vs.0.31 ±0.11,P =0.001).Conclusion The level of TL1A in patients with viral myocarditis is increased,and TL1A may participate in the occurrence of viral myocarditis.

10.Effect of Caregiver Education on Health of Stroke Patients and Their Caregivers

Objective To observe the effect of the health education and skills training for caregivers on health status of stroke patients and their family caregivers. Methods Hospitalized stroke patients were divided into the intervention group (n=38) and the control group (n= 36). Both groups received rehabilitation training for 1 month in hospital. The family caregivers of intervention group received Timing It Right education and skills training in addition. All the patients were assessed with the European Stroke Scale (ESS), Mini-Mental State Examination (MMSE), simple Fugl-Meyer Assessment (FMA), modified Barthel Index (MBI), and the caregivers were assessed with the General Health Questionnaire (GHQ-12) before and 3 months after intervention. Results The scores of ESS, FMA, MBI improved more in intervention group than in the control group after intervention (P<0.05), as well as their caregivers in the items of playing a useful part, under stress, overcoming difficulties, enjoying normal activities, facing up to problems, feeling unhappy and depressed, lossing confidence, thinking of self as worth of GHQ-12 (P<0.05). Conclusion Health education and skills training for family caregivers can improve the health of caregivers, and further improve the function outcome of stroke patients.
