1.Evaluation of high-risk HPV genotyping detection in cervical cancer screening based on a prospective cohort study

Objective:To evaluate the clinical performance of high-risk human papillomavirus (HR-HPV) genotyping in cervical cancer screening.Methods:Between June and July 2017, a prospective cervical cancer screening cohort was established in Xiaye Town, Jiyuan City, Henan Province, China by recruiting 3 254 women aged 21 to 64 years. At baseline screening, cervical exfoliated cell specimens were collected for HR-HPV genotyping and liquid-based cytology testing. Follow-ups were conducted over a 3-year period, with cytology testing in the first and second years and both HR-HPV genotyping and cytology testing in the third year. Women meeting the referral criteria were referred for colposcopy, with cervical biopsy and histopathological diagnosis performed as necessary. The endpoint was defined as cervical intraepithelial neoplasia grade 2 (CIN2) or higher confirmed by histopathological diagnosis. The sensitivity and specificity for detecting CIN2 or higher lesions of HR-HPV genotyping were calculated, as well as the cumulative risk of developing CIN2 or higher lesions over the 4-year study period in women with different baseline HR-HPV genotyping results.Results:A total of 2 741 women were included in the statistical analysis. Baseline HR-HPV genotyping detected 453 HR-HPV positive cases (16.53%), including 98 HPV 16/18 positive cases (3.58%) and 355 other HR-HPV positive cases (12.95%). During the 4-year period, 83 cases of CIN2 or higher were diagnosed. The sensitivity and specificity of baseline HR-HPV positivity for CIN2 or higher were 89.16% (95% CI: 80.66%-94.19%) and 85.74% (95% CI: 84.36%-87.02%), respectively. The corresponding rates for HPV 16/18 positivity were 43.37% (95% CI: 33.24%-54.09%) and 97.67% (95% CI: 97.02%-98.18%). The 4-year cumulative absolute risk of CIN2 or higher was highest in the HPV 16/18 positive group (36.73%, 95% CI: 27.85%-46.62%), followed by other HR-HPV positive groups (10.70%, 95% CI: 7.87%-14.38%), and the HR-HPV negative group was the lowest (0.39%, 95% CI: 0.19%-0.76%). Conclusions:HR-HPV genotyping testing exhibits high sensitivity and specificity for detecting CIN2 or higher lesions in cervical cancer screening. It also provides a scientific basis for stratifying the individual risk of developing CIN2 or higher lesions to guide subsequent management. Therefore, the HR-HPV genotyping testing can be considered as an effective method for cervical cancer screening.

Objective:To analyze the clinical characteristics of a patient with Short rib-polydactyly syndrome type 6 (SRTD6) with long-term misdiagnosis, and improve its clinical recognition by reviewing the relevant literature.Methods:A patient presented at the Children′s Hospital Affiliated to Zhejiang University School of Medicine on August 19, 2024 for the discovery of liver dysfunction for 13 years and vision loss for 9 years was selected as the study subject. Her medical history, clinical data, laboratory findings and results of imaging examination were collected. High-throughput sequencing was carried out, and candidate variants were verified by Sanger sequencing. This study was approved by the Ethics Committee of the Hospital (Ethics No.: 2021-IRB-292).Results:The patient had long-term unexplained liver dysfunction, vision loss, and growth delay. Blood acylcarnitine and urinary organic acid analysis have failed to found any abnormality. Previous genetic testing revealed a homozygous c. 203A>C (p.Glu68Ala) missense variant in the ETFDH gene, leading to a misdiagnosis of various acyl-CoA dehydrogenase deficiencies. However, treatment with high-dose vitamin B2 showed a poor effect. Physical examination revealed small hands, short and stubby fingers, and a narrow chest. Medical imaging showed shortened bilateral ribs, a narrowed chest, and short, thick metacarpals. High-throughput sequencing has detected a pathogenic homozygous c. 1957C>T (p.R653*) nonsense variant in the NEK1 gene, confirming the diagnosis of SRTD6. Conclusion:SRTD6 is characterized by rib and sternum dysplasia as the primary skeletal deformities, which is often accompanied by multi-organ impairment. Genetic testing can facilitate the precise diagnosis.

Intraoperative neural monitoring (IONM) combines electrophysiology with anatomy to monitor nerve function during thyroid surgery,and has become an important auxiliary technology for neuroprotection. After more than ten years of development,the technology has been widely applied and popularized in China,promoting the development of a number of new technologies in the industry. Combined with the questionnaire survey data of the Chinese Neural Monitoring Study Group,this paper aims to summarize the clinical effect of IONM technology, the application status and existing problems at home and abroad, and propose the possible future development direction.

5.Clinical Efficacy of Shufei Huatan Decoction in the Treatment of Children with Chronic Cough of Phlegm-Damp Accumulating in Lung Type and Its Inflammatory Regulatory Effect

Objective To investigate the clinical efficacy of Shufei Huatan Decoction in the treatment of children with chronic cough of phlegm-dampness accumulating in lung type and to observe its inflammatory regulatory effect.Methods A total of 116 children with chronic cough of phlegm-damp accumulating in lung type who were treated in the outpatient or inpatient departments of Dongguan Hospital,Guangzhou University of Chinese Medicine from February 2022 to February 2024 were randomly divided into the observation group and the control group,with 58 cases in each group.The control group was treated with conventional western medicine of antibiotics and antihistamines,while the observation group was treated with Shufei Huatan Decoction,a self-formulated Chinese medicine prescription.The course of treatment covered two weeks and then the patients were followed up for four weeks.Before and after treatment,the changes of scores of cough,expectoration,and pulmonary auscultation findings,levels of type 2 inflammatory factors of interleukin 4(IL-4)and total immunoglobulin E(IgE),and indicators of airway inflammation such as Eotaxin,fractional exhaled nitric oxide(FeNO)of the two groups were observed.After treatment,the clinical efficacy,the incidence of adverse reactions and the recurrence rate of the children in the two groups were compared.Results(1)After two weeks of treatment,the total effective rate of the observation group was 94.83%(55/58)and that of the control group was 86.21%(50/58),and the intergroup comparison(tested by chi-square test)showed that the therapeutic efficacy of the observation group was significantly superior to that of the control group(P<0.05).(2)After treatment,the scores of cough,expectoration,and pulmonary auscultation findings as well as their total scores in the two groups were decreased compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the control group(P<0.05).(3)After treatment,the levels of inflammatory factors of FeNO value and serum Eotaxin,IL-4,and total IgE of the two groups were decreased compared with those before treatment(P<0.05),and the decrease in the observation group was significantly superior to that in the control group(P<0.05).(4)The incidence of adverse reactions in the observation group was 8.62%(5/58)and that in the control group was 5.17%(3/58),and the intergroup comparison showed that the difference was not statistically significant(P>0.05).(5)The four-week follow-up showed that the recurrence rate in the observation group was 3.64%(2/55)and that in the control group was 10.00%(5/50),and the difference was not statistically significant between the two groups(P>0.05).Conclusion Shufei Huatan Decoction exerts certain efficacy in treating children with chronic cough of phlegm-damp accumulating in lung type,which is effective and safe on significantly improving children's clinical symptoms,reducing the release of inflammatory factors,and inhibiting the inflammatory reactions.
6.Influencing factors and coping strategies for mental health promotion activities among junior and senior high schools from the perspective of teachers

Objective:
To explore the facilitating and hindering factors of mental health promotion activities in junior and senior high schools from teachers perspectives, as well as coping strategies, so as to provide evidence for implementing teacher led mental health promotion programs.
Methods:
From September 2023 to September 2024, by using purposive sampling method, 5 junior high schools, 5 regular high schools, 2 vocational high schools in four provinces and municipalities (Tianjin, Shanxi, Shandong, and Jiangxi) were selected. A total of 92 teachers (78 homeroom teachers and 14 full time or part time psychological counselors) were interviewed using semi structured focus group discussions (one session per school, totaling 12 sessions). Thematic analysis was applied to code and analyze the interview transcripts.
Results:
The implementation of mental health promotion activities in middle schools was influenced by three levels: teachers, schools and society. Specifically, teachers exhibited a high support low capability phenomenon (81 participants supported conducting such activities, but 71 felt lacking in professional capacity); activity effectiveness and support systems were imbalanced (42 mentioned significant effects, while 78 reported insufficient support); there was a mismatch between student demand and activity content (9 mentioned students had psychological needs, but 11 indicated existing activities failed to meet these needs); administrative support and sustainability showed disparities (14 believed sufficient administrative support existed, while 37 noted sustainability issues); parental awareness and participation remained inadequate (11 highlighted parents need for mental health knowledge, and 37 perceived insufficient understanding of psychological issues among parents). Effective strategies included enhancing teachers effectiveness, providing essential skill training, focusing on the needs of teenagers,enhancing program sustainability, and eliminating misconceptions about mental health among parents.
Conclusion
Mental health promotion activities in junior and senior high schools should focus on enhancing teachers skills, improving activity sustainability, reducing stigma among parents, and establishing a collaborative network for school based mental health promotion.
7.Construction of China’s bio-health narrative system: enhancement of narrative ecology and national bio-health narrative consciousness

Narrative is the cornerstone of interpersonal relationships and life safety. However, its important value in daily life, school education, health management, personal happiness, career development, and other aspects has been ignored. The narrative ecology of families, schools, hospitals, workplaces, and elderly care institutions is worrying, the narrative connection between parent-child and intergenerational is broken, the narrative nature of adolescents is ignored, the narrative demands of patients are neglected, narrative relationships in the workplace are indifferent, and the narrative capital of the elderly is idle. These issues have resulted in serious social problems, such as depression and suicide among adolescents, conflicts between doctors and patients, workplace and life burnout among middle-aged people, and the inability of the elderly to achieve healthy aging, which have become a “stumbling block” to the realization of holistic health. Advocating the construction of narrative ecology and interpersonal narrative connections is an important measure of achieving holistic health. Taking the “narrative concept” as the overall framework, and based on the research, education, and practice carried out by the Alliance of Narrative Medicine in Higher Education Institutions, this paper proposed that actively build China’s narrative system of life and health, to enable narrative play an active and dynamic role in the construction of narrative ecology in different spaces, such as the families, the schools, the hospitals, the workplaces, and the elderly care institutions, as well as practically improve the quality of life of the people.
8.Research progress on new drugs for the treatment of chronic hepatitis B virus infection

Hepatitis B virus (HBV) infection is a major global public health problem. There will be about 257 million chronic HBV-infected patients worldwide, according to the World Health Organization's estimation by 2025. Currently, the main drugs for the treatment of chronic HBV infection are nucleos(t)ide analogues and pegylated interferon (PEG-IFN). However, previous research results show that whether it is nucleos(t)ide analogues and PEG-IFN-α monotherapy or combination therapy, or sequential combination therapy, the rate of hepatitis B surface antigen seroconversion in patients is low, and there is still a high risk of disease progression after a certain course of antiviral treatment. Therefore, to achieve the ambitious target of "eliminating viral hepatitis by 2030" set by the World Health Organization and help more hepatitis B patients achieve functional cure, a large number of new drugs have been developed and entered clinical trials. This paper summarizes and reviews the types, safety, and efficacy of new drugs to further promote advancements in the field of treatment of chronic HBV infection.
9.Advances in innovative applications of artificial intelligence combined with intraoperative neuromonitoring technology in thyroid surgery

Intraoperative neural monitoring (IONM) is a critical technique for the protection of recurrent laryngeal nerve (RLN) function during thyroid surgery. In recent years, continuous innovations in IONM have established it as a cornerstone of modern thyroid surgery. However, current methodologies still exhibit limitations in the accurate assessment of neural function, and guidelines have yet to provide clear strategies for managing patients with intraoperative signal loss who subsequently develop postoperative voice disorders. This article reviews representative advances in the application of deep learning to bioelectrophysiological signals and systematically summarizes the use of deep learning in the field of voice medicine, thereby exploring the feasibility of integrating IONM with deep learning technologies.
10.Analysis of current mental health status and stressors of new professional master's degree graduates in clinical medicine in China

Objective:To investigate the current situation and correlation between mental health and stressors of new professional master's degree graduates in clinical medicine, and to provide empirical support for improving student mental health.Methods:An online questionnaire survey was conducted among 3 587 fresh graduate students with master's degree in clinical medicine from 65 training institutions in China. SPSS 26.0 was used to compare and analyze the measurement data. Descriptive statistics were used to analyze the mental health using Symptom Checklist-90 (SCL-90) and the current status of stressors. The influence factors and correlation between mental health and stressors were analyzed by means of differentiation analysis and correlation analysis.Results:The SCL-90 overall mean score as well as the scores of obsessive-compulsive symptoms, interpersonal sensitivity, depression, terror, anxious, psychoticism, and hostility of new professional master's degree graduates in clinical medicine were all higher than the national norm ( P<0.05). Among the participants, 58.21% (2 088/3 587) exhibited varying degrees of symptoms listed in the SCL-90. Students from families with low per capita income and low parental education level have more prominent psychological problems. The top three stressors perceived were job seeking, research tasks, and self-ability. Students with positive SCL-90 symptoms reported significantly higher perceived stress in all three stressor dimensions compared to their symptom-negative counterparts ( P<0.001), indicating a significant correlation between mental health status and stressors. Conclusions:The mental health status of new professional master's degree graduates in clinical medicine is concerning and warrants focused attention from educational authorities, academic institutions, and families.
