1.Effect of PTEN mRNA expression level in granulosa cells on follicular fluid hormone secretion in infertile patients with polycystic ovary syndrome

Objective:To detect the levels of sex hormones and insulin in follicular fluid(FF)and the expression level of phosphatase and tensin homolog deleted on chromosome ten(PTEN)in granulosa cells in the infertile patients with polycystic ovary syndrome(PCOS),and to preliminarily explain the correlations between the insulin level and the expression level of PTEN mRNA.Methods:Seventy infertile patients were selected as the subjects and divided into PCOS group and control group(tubal obstruction or infertility due to male factors)according to infertility factors.All patients received in vitro fertilization-embryo transfer(IVF-ET)treatment.FF and ovarian granulosa cells were collected on the day of ovulation.The expression levels of PTEN mRNA in ovarian granulosa cells of the patients in two groups were detected by real-time fluorescence quantitative PCR(RT-qPCR)method.The levels of sex hormone and insulin in FF were measured by electrochemiluminescence.The correlations of the PTEN mRNA expression level in ovarian granulosa cells and testosterone(T)in FF with the level of insulin in FF were analyzed by Spearman correlation analysis method.Results:There were no significant differences in age,infertility years,body mass index(BMI),basic sex hormone,total dose of gonadotropin(Gn)and days of ovulation induction in two groups(P>0.05).Compared with control group,the anti-Mullerian hormone(AMH)and antral follicle counting(AFC)of the patients in PCOS group were significantly increased(P<0.05).The RT-qPCR results showed that the PTEN mRNA expression level in ovarian granulosa cells of the patients in the PCOS group was higher than that in control group(P<0.001).The electrochemiluminescence results showed that the levels of T and insulin in FF of the patients in PCOS group were higher than those in control group(P<0.05),whereas the estrogen and progesterone levels were lower than those in control group(P<0.05).The Spearman correlation analysis showed that that T level in FF was positively correlated with the insulin level of the patients in PCOS group(r=0.577,P<0.001),and the PTEN mRNA expression level in ovarian granulosa cells was positively correlated with the insulin levels in FF(r=0.616,P<0.001);in control group,there was no correlation between T level and insulin level in FF(r=0.266,P=0.123),and there was no correlation between the expression level of PTEN mRNA in granulosa cells and the insulin level in FF in control group(r=-0.214,P=0.216).Conclusion:The high expression of PTEN in granulosa cells of the infertile patients with PCOS may be related to the local hyperinsulin level in the ovary,and PTEN participates in the occurrence and development of PCOS.
2.Prevalence of loss of activities of daily living and influencing factors in elderly population in China

Objective To investigate the prevalence of the loss of basic activities of daily living (ADL) and instrumental ADL (IADL) influencing factors in the elderly population in China and provide evidence for the effective keeping and improvement of the elderly daily living.Methods The information about demographic characteristics and activities of daily living of elderly Chinese aged ≥ 60 years were collected by using the data of 2013 China Health and Retirement Longitudinal Study.The elderly's ADL of taking bath,eating,getting in and out of bed,dressing,toilet use,and defecating and the IADL of doing housework,cooking,making phone call,taking medicine,shopping and money managing were evaluated.The differences in loss of ADL among different populations were compared by x2 test and logistic regression analysis was conducted to identify influencing factors for the loss of ADL and IADL.Results The overall ADL loss rate was 23.8% and the overall IADL loss rate was 35.4% in elderly Chinese.The proportion of having trouble in toilet use was highest among all ADL items,followed by bathing and getting in and out of bed.The proportion of having trouble in making phone call was highest among all IADL items,followed by doing housework and money managing.Female,older age,low educational level,living in central and western China,chronic diseases and disability were factors associated with ADL loss and IADL loss;the divorced or separated,widowed were more likely to have ADL loss;people living in rural area were more likely to have IADL loss.Conclusion It is necessary to take comprehensive measures to delay and reduce the ADL and IADL loss in elderly Chinese and provide timely and appropriate health care for the elderly with ADL or IADL loss.
3.Maintenance effects of acupoint catgut embedding at early time on gastrointestinal function in patients with craniocerebral injury.

OBJECTIVETo explore the maintenance effects of acupoint catgut embedding at early time on gastrointestinal function in patients with craniocerebral injury.
METHODSSixty craniocerebral injury patients with 5 to 12 points of Glasgow coma scale (GCS), according to treatment order, were alternately divided into an observation group and a control group, 30 cases in each one. Patients in the control group were treated with regular treatment and nursing care. Based on this, patients in the observation group, according to different pathogenesis and symptoms presented within 24 h into hospitalization, were additionally treated with acupoint catgut embedding. The recovery time of borborygmus, time of first anal aerofluxus, time of first defecation, abdominal pressure at different time points, the occurrence rate of complications (upper gastrointestinal hemorrhage, diarrhea, vomiting), time of enteral nutrition tolerance rate reaching 30 kcal/kg x d were observed and recorded.
RESULTSThe recovery time of borborygmus, time of first anal aerofluxus, time of first defecation and time of enteral nutrition tolerance rate reaching 30 kcal/kg x d in the observation group were all earlier to those in the control group (all P<0.01). At 48 h, 4 d and 7 d into hospitalization, the abdominal pressures in the observation group were all lower than those in the control group [(11.10 +/- 1.47) mmHg vs. (13.50 +/- 1.43) mmHg, (8.40 +/- 1.25) mmHg vs. (11.90 +/- 1.56) mmHg, (6.73 +/- 0.74) mmHg vs. (10.80 +/- 1.30) mmHg, all P<0.01]. There were 8 cases with complications of gastrointestinal hemorrhage, diarrhea and vomiting in the observation group with the occurrence rate o 27% (8/30), which was lower than those in the control group (70.0% (21/30), P<0.01.
CONCLUSIONThe acupoint catgut embedding at early time in craniocerebral injury patients could improve the recovery of gastrointestinal function, reduce intolerance of enteral nutrition and occurrence rate of various complications.
