1.Observation on Clinical Efficacy of Shengdi Xiexin Decoction in the Treatment of Upper Gastrointestinal Bleeding with Stomach-Heat Congestion Syndrome

Objective To observe the clinical efficacy of Shengdi Xiexin Decoction combined with proton pump inhibitors(PPIs)in the treatment of acute non-variceal upper gastrointestinal bleeding(ANVUGIB)with stomach-heat congestion syndrome.Methods Sixty-two patients with ANVUGIB of stomach-heat congestion syndrome admitted to Dongguan Hospital of Guangzhou University of Chinese Medicine between January and December 2024 were randomly divided into a treatment group(n=31)and a control group(n=31)using a random number table.The control group received conventional western therapy(e.g.,Omeprazole Sodium),while the treatment group additionally received Shengdi Xiexin Decoction.Both groups were treated for 7 days,followed by 8 weeks of follow-up.The changes in traditional Chinese medicine(TCM)syndrome scores,hematological parameters[hemoglobin(Hb),red blood cells(RBC),hematocrit(Hct),albumin(ALB)],tissue perfusion and metabolic markers[blood urea nitrogen/creatinine ratio(BUN/Cr),lactate(Lac)levels],hemostasis time,time to resume oral diet,hospital stay duration,hospitalization costs,and rebleeding rates were observed to evaluate the clinical efficacy and drug safety.Results(1)After 7 days of treatment,the total effective rate was significantly higher in the treatment group(90.32%,28/31)than that in the control group(74.19%,23/31).The intergroup comparison(by chi-square test)showed that the treatment group,efficacy was superior to that of the control group(P<0.05).(2)The treatment group showed significantly short hemostasis time,time to resume oral diet,and hospital stay duration(P<0.05),as well as significantly low hospitalization costs(P<0.05)compared to the control group.(3)Both groups exhibited significant reductions in the scores of epigastric burning pain,abdominal distension,acid regurgitation,restlessness,excessive thirst,scanty dark uring and dry stools,and total TCM syndrome scores(P<0.05).The treatment group demonstrated significantly greater improvements in the scores of abdominal distension,acid regurgitation,restlessness,dark uring and dry stools,and total TCM syndrome scores(P<0.05),while no significant differences were observed in epigastric burning pain and excessive thirst(P>0.05).(4)Hb and Hct levels increased significantly in both groups(P<0.05),with a greater increase in the treatment group(P<0.05).Although RBC and ALB levels showed an upward trend,no significant differences were observed between groups(P>0.05).Following treatment,intergroup comparisons revealed that the treatment group demonstrated significantly greater increases in Hb and Hct levels than the control group(P<0.05).In contrast,the two groups showed comparable effects in elevating RBC and ALB levels,with no statistically significant differences(P>0.05).(5)BUN/Cr ratio and Lac levels decreased significantly in both groups(P<0.05),with a more pronounced reduction in the treatment group(P<0.05).(6)No significant differences were found in early or delayed rebleeding rates between groups(P>0.05).(7)Neither group experienced significant adverse events.Conclusion Shengdi Xiexin Decoction combined with PPIs significantly improves clinical outcomes in ANVUGIB patients with stomach-heat congestion syndrome.It is effective on enhancing hematological parameters,correcting tissue hypoperfusion,alleviating symptoms,shortening hemostasis time and hospitalization durations,and reducing medical costs,demonstrating excellent safety.
2.A fusion model of manually extracted visual features and deep learning features for rebleeding risk stratification in peptic ulcers.

OBJECTIVES:
We propose a multi-feature fusion model based on manually extracted features and deep learning features from endoscopic images for grading rebleeding risk of peptic ulcers.
METHODS:
Based on the endoscopic appearance of peptic ulcers, color features were extracted to distinguish active bleeding (Forrest I) from non-bleeding ulcers (Forrest II and III). The edge and texture features were used to describe the morphology and appearance of the ulcers in different grades. By integrating deep features extracted from a deep learning network with manually extracted visual features, a multi-feature representation of endoscopic images was created to predict the risk of rebleeding of peptic ulcers.
RESULTS:
In a dataset consisting of 3573 images from 708 patients with Forrest classification, the proposed multi-feature fusion model achieved an accuracy of 74.94% in the 6-level rebleeding risk classification task, outperforming the experienced physicians who had a classification accuracy of 59.9% (P<0.05). The F1 scores of the model for identifying Forrest Ib, IIa, and III ulcers were 90.16%, 75.44%, and 77.13%, respectively, demonstrating particularly good performance of the model for Forrest Ib ulcers. Compared with the first model for peptic ulcer rebleeding classification, the proposed model had improved F1 scores by 5.8%. In the simplified 3-level risk (high-risk, low-risk, and non-endoscopic treatment) classification task, the model achieved F1 scores of 93.74%, 81.30%, and 73.59%, respectively.
CONCLUSIONS
The proposed multi-feature fusion model integrating deep features from CNNs with manually extracted visual features effectively improves the accuracy of rebleeding risk classification for peptic ulcers, thus providing an efficient diagnostic tool for clinical assessment of rebleeding risks of peptic ulcers.

3.Association Between the Coexistence of Chronic Non-communicable Diseases and Quality of Life in Middle-aged and Elderly People Living with HIV/AIDS

[Objective]To investigate the prevalence rate of chronic non-communicable diseases(NCDs)and the association with quality of life in middle-aged and elderly patients with HIV/AIDS.[Methods]This cross-sectional study surveyed 432 patients with HIV/AIDS(aged≥45 years)in the Infectious Disease Center in Guangzhou Eighth People's Hospital of Guangzhou Medical University,and 366 participants were included in the analysis after quality control.A questionnaire and the EuroQol 5-Dimensional 3-level version(EQ-5D-3L)were used to investigate NCDs and quality of life and Tobit regression model was used to estimate the association between chronic diseases and quality of life.[Results]Among the 366 participants,29(7.9%)had cardiovascular disease,45(12.3%)had hypertension,122(33.3%)had hyperglycemia,151(41.3%)had hyperlipidemia,7(1.9%)had cancer,17(4.6%)had chronic kidney disease,38(10.4%)had chronic liver disease,21(5.7%)had musculoskeletal disorders,and 253(69.1%)suffered from at least one type of chronic diseases.The median(lower and upper quartiles)of EQ-5D utility index was 1.000(0.964~1.000).Multivariate Tobit regression results of the total population showed that cancer[ba=-0.08,95%CI(-0.15,-0.01),P=0.036],chronic kidney disease[ba=-0.07,95%CI(-0.12,-0.02),P=0.006],musculoskeletal disease[ba=-0.09,95%CI(-0.13,-0.05),P<0.001],and≥3 types of chronic diseases[ba=-0.05,95%CI(-0.08,-0.01),P=0.013]were negatively correlated with EQ-5D utility index.The stratified analysis results of different CD4+T cell levels showed that hypertension[ba=-0.07,95%CI(-0.12,-0.02),P=0.007],chronic kidney disease[ba=-0.10,95%CI(-0.18,-0.03),P=0.006],musculoskeletal disease[ba=-0.15,95%CI(-0.22,-0.07),P<0.001]and≥3 types of chronic diseases[ba=-0.09,95%CI(-0.09,-0.01),P<0.001]were negatively correlated with EQ-5D utility index in the group with CD4≤500(cells/μL),whereas cancer[ba=-0.11,95%CI(-0.20,-0.01),P=0.031]was negatively correlated with EQ-5D utility index in the group with CD4>500(cells/μL).[Conclusions]The prevalence rate of chronic non-communicable diseases in middle-aged and elderly patients with HIV/AIDS is relatively high.The classification of NCDs such as cancer or chronic kidney disease or other chronic diseases and the numbers of NCDs categories are negatively correlated with quality of life.However,this association varies among patients with HIV/AIDS of different CD4+T cell levels.It is suggested that we should try to prevent and identify NCDs at an early stage,strengthen linkages and integration of health services for AIDS and chronic NCDs,and jointly manage and control AIDS with chronic diseases to improve the quality of life among people living with HIV/AIDS.
4.Effect of Various Factors on Non-suicidal Self-injury in Adolescent Depression

[Objective]To investigate the non-suicidal self-injury(NSSI)behaviors in adolescents with depressive disorder,analyze related influencing factors,and provide theoretical basis and reference for the prevention and treatment of NSSI.[Methods]According to DSM-5 criteria,95 depressive adolescents were divided into two groups:one with NSSI(NSSI group)and one without NSSI(nNSSI group).All patients were assessed with Adolescent Non-suicidal Self-injury Assessment Questionnaire(ANSAQ),Self-Rating Depression Scale(SDS),Self-Rating Anxiety Scale(SAS),Simplified Coping Style Questionnaire(SCSQ),Experiences in Close Relationships-Relationship Structures Scale(ECR-RS),and Childhood Trauma Questionnaire-Short Form(CTQ-SF).The inter-group differences were compared.The influencing factors of NSSI were analyzed by using binary logistic regression.[Results]Of the 95 depressive adolescents,59 cases of NSSI were identified,with a detection rate of 62.11%.NSSI group had higher scores than nNSSI group on SDS,SAS,negative coping style,paternal attachment anxiety,maternal attachment anxiety and avoidance,CTQ-SF total score,emotional neglect,physical neglect,emotional abuse,and sexual abuse(all P<0.05).Binary logistic regression analysis showed that anxiety,negative coping style,maternal attachment avoidance and emotional abuse increased the risk of NSSI among adolescents with depressive disorders(all P<0.05).[Conclusions]Adolescents with depression have a high incidence of NSSI behaviors,which is related to anxiety,negative coping style,maternal attachment avoidance and emotional abuse.In addition to improving patients' depression and anxiety in clinical setting,attention should also be paid to patients' coping styles,parent-child relationship and childhood trauma to reduce the occurrence of NSSI behaviors.
5.Predictive value of dual-modality ultrasound combined with S-Detect for cervical lymph node metastasis in papillary thyroid carcinoma

Objective To evaluate the predictive value of dual-modality ultrasound,incorporating conventional ultrasound and ultrasound elastography,in combination with S-Detect for cervical lymph node metastasis(CLNM)in patients with papillary thyroid carcinoma(PTC).Methods A retrospective analysis was conducted on the clinical data of 135 patients diagnosed with PTC who received treatment at the First Affiliated Hospital of Shihezi University between November 2023 and August 2024.For all patients,clinical baseline characteristics,conventional ultrasound findings,ultrasound elastography results,and S-Detect analysis data were collected.Independent predictors of CLNM in PTC were identified,and predictive models were developed.Receiver operating characteristic(ROC)curves were generated to compare the area under the curve(AUC)of the models.The most effective predictive model was selected to construct a risk probability nomogram,and the predictive performance and clinical applicability of this nomogram were subsequently evaluated.Results Age,maximum nodule diameter,boundary characteristics,capsular invasion,transverse-sectional morphological findings assessed by S-Detect,and ECI-based elasticity grading were identified as independent predictors of CLNM in PTC(all P<0.05).The AUC of the predictive model constructed using these six variables was 0.890(95%CI:0.835~0.945).The calibration curve demonstrated strong agreement between predicted and observed outcomes,and decision curve analysis indicated that the nomogram provided a favorable net clinical benefit within a threshold probability range of 2%to 91.5%.Conclusions Age,maximum nodule diameter,boundary characteristics,capsular invasion,sonographic features assessed by S-Detect in the transverse plane,and ECI-based elasticity grading are independent predictors of CLNM in PTC.A nomogram model incorporating these parameters demonstrates effective performance in predicting the likelihood of CLNM.
6.Analysis of potential profiles and influencing factors of body image disorder in patients with polycystic ovary syndrome

Objective:To explore the potential types of body image disorders in patients with polycystic ovary syndrome (PCOS) and to analyze the influencing factors of each category, so as to provide a basis for clinical formulation of targeted intervention programs.Methods:A total of 275 patients with PCOS who visited the outpatient clinic of the Center for Reproductive Medicine of Fenyang Hospital of Shanxi Province from April 2023 to April 2024 were selected by convenience sampling, and the general information questionnaires, Body Image Scale, Sociocultural Attitudes Towards Appearance Questionnaire-3, and Adaptation, Partnership, Growth, Affection and Resolve were used to conduct a cross-sectional survey. The potential profile analysis of body image disorders in PCOS patients was carried out, and the influencing factors of different categories were explored by multivariate Logistic regression analysis.Results:A total of 260 patients with PCOS were surveyed in this study, and 113 patients (43.5%) were aged 25-35 years old. The score of the Body Image Scale for PCOS patients was 11.00 (7.25, 14.00) points, the score of the Sociocultural Attitudes Towards Appearance Questionnaire-3 was (44.80 ± 4.65) points, and the score of Adaptation, Partnership, Growth, Affection and Resolve was (5.80 ± 1.43) points. PCOS patients were divided into three potential categories: low body image impairment group (37.3%, 97/260), intermediate body image level-high cognitive impairment group (38.1%, 99/260), and high body image impairment group (24.6%, 64/260). Body mass index, acne, alopecia, reproductive history, course of disease, whether medical staff have carried out detailed education, social media attention and internalization, and family care were the influencing factors of potential types of body image disorders in PCOS patients ( OR values were 0.339-19.099, all P<0.05). Conclusions:There is obvious group heterogeneity in patients with PCOS body image disorders, and medical staff should formulate targeted interventions according to the characteristics and influencing factors of each category to improve patients′ lifestyle, reduce the level of body image disorders.
