1.The effect of exercise on knee function after anterior cruciate ligament reconstruction

Objective:To observe the effect of rehabilitative exercise on knee function after anterior cruciate ligament (ACL) reconstruction.Methods:Fifty-eight patients with a reconstructed ACL were divided at random into a control group of 28 and an experimental group of 30. In addition to conventional basic treatment, the control group received routine orthopedic rehabilitation training, while the experimental group underwent exercise-based rehabilitation training 6 days a week for 6 weeks. Before and after the treatment, the efficacy in both groups was evaluated using Lysholm knee scoring (LKSS), numerical pain scoring (NRS), maximum knee flexion angle, and a thigh muscle atrophy index.Results:Both groups had significantly higher LKSS scores, lower NRS scores, larger maximum knee flexion angles, and increased thigh muscle atrophy indices, on average, after their treatments. Compared with the control group, the experimental group tended to have significantly higher LKSS scores, larger maximum knee flexion angles, and lower thigh muscle atrophy indices after the treatment. There was, however, no significant difference between the groups in their average NRS scores.Conclusions:Exercise-based rehabilitation training significantly improves the knee function of patients after ACL reconstruction, and its efficacy is superior to conventional orthopedic rehabilitation training.
2.Protective mechanism of nitrooleic acid on submandibular gland cell radiation injury in rats

BACKGROUND:Recent studies have found that Nrf2/ARE signaling pathway activators have the characteristics of low toxicity and control,and have a protective effect against radiation tissue damage.OBJECTIVE:To investigate whether nitrooleic acid can protect submandibular gland epithelial cells from radiation injury by regulating the Nrf2/ARE signaling pathway.METHODS:Rat submandibular gland epithelial cells were cultured in vitro and CCK-8 assay was used to screen the optimal concentration and time of nitrooleic acid administration.Submandibular gland epithelial cells were divided into non-radiation group,radiation control group,nitrooleic acid group,nitrooleic acid+ML385(Nrf2/ARE signaling pathway specific inhibitor)group,and ML385 group.Submandibular gland cells were pretreated with nitrooleic acid and ML385 for 24 hous according to the experimental groups,and then irradiated with 5 Gy radiation to establish the models.At 48 hours after irradiation,CCK-8 assay was used to detect the cell proliferation rate.Real-time quantitative PCR was used to detect the expression of Nrf2,HO-1,and NQO1 mRNA in the cells.Real-time quantitative PCR and enzyme-linked immunosorbent assay were used to detect the cell secretion function and the expression of inflammatory factors.DCFH-DA fluorescent probe kit was used to detect the level of intracellular reactive oxygen species.RESULTS AND CONCLUSION:(1)Compared with the radiation control group,the proliferation rate of submandibular gland epithelial cells and the expression levels of secretion function related factors aquaporin 5 and α-amylase in the nitrooleic acid group of rats increased(P<0.05),and the expression levels of Nrf2,HO-1,and NQO1 mRNA increased(P<0.05),while the expression levels of inflammatory factors interleukin-1β,interleukin-6,and tumor necrosis factor-αdecreased(P<0.05),and reactive oxygen species generation reduced(P<0.01).(2)Compared with the nitrooleic acid group,the addition of nitrooleic acid and ML385 group resulted in a decrease in cell proliferation rate and expression levels of secretion function related factors aquaporin 5 and α-amylase(P<0.05),and mRNA expressions of Nrf2,HO-1,and NQO1 were all decreased(P<0.05),while the expression levels of inflammatory factors interleukin-1β,interleukin-6,and tumor necrosis factor-α increased(P<0.05),and generation of reactive oxygen species increased(P<0.05).(3)Results indicated that in the radiation environment,nitrooleic acid has a certain protective effect on the proliferation ability and secretion function of rat submandibular gland epithelial cells,reduces the expression of inflammatory factors,lowers intracellular reactive oxygen species levels,and alleviates the damage of rat submandibular gland epithelial cells caused by radiation.This function may be related to the activation of Nrf2/ARE signaling pathway.
3.Study on quality evaluation of Alpiniae Katsumadai Semen based on HPLC specific chromatogram and chemometricsy

Objective:To determine the contents of four flavonoids by establishing HPLC specific chromatogram for Alpiniae Katsumadai Semen; To evaluate the differences of Alpiniae Katsumadai Semen from different producing areas.Methods:The specific chromatogram was developed on a column of Thermo Acclaim C18 with acetonitrile-0.1% phosphoric acid solution as the mobile phase by gradient elution at a flow rate of 1.0 ml/min. The detective wavelength was 260 nm, and the column temperature was 30 ℃. Similarity evaluation, PCA analysis, and OPLS-DA analysis were conducted. The contents of Alpinetin, Pinocembrin, Cardamonin, Alnustone in 16 batches of Alpiniae Katsumadai Semen.Results:There were 9 characteristic peaks in the specific chromatogram of Alpiniae Katsumadai Semen. Except the sample of S2 (Hainan producing area), the similarity of Alpiniae Katsumadai Semen in different producing areas was greater than 0.90; PCA analysis divided 16 batches of Alpiniae Katsumadai Semen into 2 categories, and OPLS-DA analysis identified 4 differential biomarkers, with the order of impact being peak 3>peak 5>Alpinetin>Cardamonin. Among them, the quality of Alpiniae Katsumadai Semen from Guangdong producing area was generally stable. Moreover, there were significant differences in the contents of Alpinetin and Cardamonin among the indicator components of Alpiniae Katsumadai Semen from different producing areas.Conclusion:This method can effectively analyze the differences in the quality of Alpiniae Katsumadai Semen from different producing areas, providing reference for the quality evaluation of Alpiniae Katsumadai Semen.
4.Association Between Low Birth Weight and Dementia Risk:A Large-scale Prospective Study

Objective To investigate the association between birth weight and dementia risk and the mediating roles of chronic diseases,and to assess potential biological pathways underlying the birth weight-associated dementia risk based on large-scale proteomics.Methods We used data from 279 743 participants aged 40 to 69 years enrolled in the UK Biobank.Birth weight was categorized into low birth weight(≤2 500 g),normal birth weight(2 500-3 999 g),and macrosomia(≥4 000 g).Multivariable Cox proportional hazards regression models were used to assess the associations between birth weight categories and all-cause dementia and its subtypes(Alzheimer's disease and vascular dementia).Proteomics analyses were conducted to identify proteins and the potential pathways involved.Results Low birth weight was associated with higher risks for all-cause dementia and its subtypes.The hazard ratios were 1.18(95%CI,1.08-1.30)for all-cause dementia,1.14(95%CI,1.00-1.31)for Alzheimer's disease,and 1.22(95%CI,1.01-1.48)for vascular dementia.A non-linear relationship was observed between birth weight and dementia risk(P for nonlinearity<0.001).Certain cardiometabolic diseases in middle-aged adults,such as diabetes,stroke,hypertension,and dyslipidemia,played a significant mediating role in the relationship between low birth weight and dementia risk,with the mediation proportion being 6.3%to 15.8%.Proteomic analyses identified 21 proteins linked to both low birth weight and all-cause dementia risk,which were significantly enriched in the pathways for viral protein interaction with cytokines and cytokine receptors,adipocytokine signaling,and cytokine-cytokine receptor interaction.Conclusion Low birth weight is positively associated with dementia risk.Cardiometabolic diseases in middle-aged adults may mediate the relationship between low birth weight and dementia risk.A number of proteins and the associated pathways underscore the relationship between low birth weight and dementia risk.

7.Analysis of pediatric pre-prescription review orders based on PCNE classification system

OBJECTIVE To provide reference for improving the pre-prescription review system and reducing the occurrence of medication error by analyzing the drug-related problems(DRPs)in the pre-prescription review orders of pediatric outpatient clinics using the Pharmaceutical Care Network Europe(PCNE)classification system.METHODS The data of pre-prescription review orders were retrospectively collected from outpatient department of Shanghai Children's Medical Center,Shanghai Jiao Tong University School of Medicine from July 2022 to June 2023;DRPs in the pre-prescription review orders were classified and summarized by using the PCNE classification system(version 9.1),and then analyzed in terms of types and causes of issues,and the acceptance of interventions.RESULTS A total of 66 017 DRPs orders were included,involving 41 165 patients.The proportion of DRPs orders in children aged ≤5 years old was the highest(58.25%),followed by children aged 6-12 years old(33.52%);the department with the highest proportion of DRPs was internal medicine of pediatrics department(71.41%);the department with the highest incidence of DRPs was thoracic surgery department(9.73%);top three drug categories of DRPs orders were systemic anti-infective drugs(25.26%),Chinese patent medicines(24.74%)and respiratory drugs(22.38%).Referring to PCNE classification system,the types of DRPs mainly focused on treatment safety(64.86%);the reasons of DRPs orders mainly focused on dose selection(82.09%),of which 41.26%were due to excessive drug dosage;92.13%of interventions could be accepted and fully executed by doctors.CONCLUSIONS DRPs orders identified by the pre-prescription review system can be effectively analyzed by using PCNE classification system.Pharmacists should focus on medication use in children aged<5 years old,update and develop personalized prescription review rules timely,and meet the rational needs of clinical medication for children.
8.Role of exosomes in the development, progression, diagnosis, and treatment of liver fibrosis

An increasing number of studies have demonstrated that exosomes are closely associated with liver fibrosis and mediate the process of liver fibrosis by participating in cytokine secretion, macrophage activation, extracellular matrix remodeling, and hepatic stellate cell activation. This article summarizes that the resolution of liver fibrosis requires the reduction of pro-inflammatory and fibrotic cytokines, the reduction of extracellular matrix protein production, the increase of collagenase activity, and finally the loss of activated myofibroblasts. It is believed that exosomes play an important role in the treatment of liver fibrosis and are potential markers for diagnosis and treatment, and in future studies, it is necessary to improve exosome extraction techniques and standardization of treatment quantification.
9.Development and evaluation of ultrasound contrast agents with AS1411-conjugated nanoparticles with liquid core.

To prepare AS1411 targeted nano-ultrasonic contrast agent with liquid core, and to evaluate its ability for ultrasonic contrast enhancement and targeting MCF-7 cell in vitro.
Methods: The modified solvent evaporation, self-synthesized membrane material and perfluorobrominane (PFOB) was used to form nano-ultrasonic contrast agent with PFOB core (nanoparticles, NP); then N-(3-dimethylaminopropyl)-N'-ethylcarbodiimide hydrochloride and N-hydroxysuccinimide (EDC/NHS) catalysis was used to connect AS1411 to the surface of NP to prepare NP-AS1411. The transmission electron microscopy was chosen to check the morphology of NP-AS1411. The size, surface charge, encapsulation efficiency, biocompatibility, the contrast grey value and the stability of NP-AS1411 and NP were compared. Whether AS411 was attached to the surface of NP was checked by gel electrophoresis. Fluorescence microscopy and flow cytometry were performed to examine the targeting ability of AS1411.
Results: NP-AS1411 was a shell-nuclear structure under the electron microscope. Its size was at (245.4±16.5) nm, which was larger than that of NP (P=0.05). There was no significant difference in surface charge and encapsulation efficiency between NP-AS1411 and NP (P>0.05). In the MTT experiment, the cell viability decreased significantly at high concentration of NP-AS411 (25 mg/mL) after incubation for 24 h compared with the control group (0 mg/mL ) (P<0.05). The contrast gray value of NP-AS1411 was at 86.1+ 6.7, which was significantly higher than that of deionized water (P<0.05), and equivalent to that of NP (P>0.05). The contrast grey value of AS1411-NP was 80.1±9.2 after keeping at room temperature for 24 h, which showed no obviously change comparing with that before the treatment (P>0.05). The size of NP-AS1411didn't change too (P>0.05). The results of gel electrophoresis demonstrated that the AS1411 connecting to the surface of NP was the most when the molar ratio of NP:AS1411 was at 40:1. Flow cytometry analysis confirmed that NP and NP-AS1411 were combined with MCF-7 cells separately but the fluorescence produced by the combination of NP-AS1411 and MCF-7 was more intense.
Conclusion: The modified solvent evaporation and EDC/NHS catalysis could successfully prepare ultrasound contrast agents with aptamer-conjugated nanoparticles with liquid core. The targeted ultrasonic contrast agents with liquid core possess good ultrasonic contrast enhancement ability in vitro, stability and specificity as well.

10.The expression of SOX-2 and β-catenin in gastric cancer and the relationship with recurrence and metastasis after operation

Background and purpose:The recurrence and metastasis of gastric cancer seriously affect survival in patients.SOX gene as a regulatory factor of the classical Wnt pathway, may play an important role in the process. This study was to explore the expression of stem cell marker SOX-2 and β-catenin in gastric cancer and to analyze the relationship with recurrence and metastasis after operation.Methods:Immunohistochemistry was used to detect the expression of SOX-2 and β-catenin in 71 tumor samples from 71 cases after surgery for gastric cancer. The correlation between SOX-2 and β-catenin expression and the clinicopathological characteristics of gastric cancer and disease-free survival was analyzed.Results:The SOX-2 protein expression was associated with metastasis, lymph node inifltration or differentiation (P=0.011,P=0.036,P=0.034) in the 71 gastric cancer, but not with gender, age or T stage. β-catenin expression was correlated with metastasis, lymph node invasion or T stage (P=0.025,P=0.014,P=0.026), but was not related to differentiation, gender or age. The survival analysis showed that SOX-2 and β-catenin expression was closely associated with prognosis of patients, and metastatic rate in positive expression was higher than that in negative expression.Conclusion:The expression of SOX-2 and β-catenin is associated with the development, recurrence, metastasis of gastric cancer and may be used as a useful prognostic parameter to predict overall survival.
