1.Application research of ultrasound oblique axis plane guidance technique in PICC puncture

Objective To explore the application efficacy and safety of oblique ultrasound-guided techniques in PICC puncture,in order to provide guidance and references for clinical application.Methods Through convenient sampling,654 patients from a tertiary A hospital in Zhejiang Province from March to December 2022 were selected as the study subjects.The random numbers were generated through Excel table functions and they were randomly grouped into 3 groups:A,B,and C.The ultrasound short axis method,long axis method,and oblique axis method were employed to guide PICC puncture catheterization,respectively.The success rate of PICC puncture,the number of subcutaneous adjustments of the puncture needle,puncture time,and the occurrence of puncture complications(such as hematoma,puncture of the posterior wall of blood vessels,accidental injury to arteries,and accidental injury to nerves)were recorded during the catheterization process in 3 groups.Results A total of 654 patients completed the study,including 215 in group A,219 in group B,and 220 in group C.The success rate of first-time puncture in the group C(86.36%)was higher than that in group A(73.95%)and group B(63.93%),and there was a statistically significant difference among 3 groups(P<0.001).The subcutaneous adjustment frequency of the puncture needle was 1(1,1)in group C,1(1,2)in group A,and 1(1,2)in group B.The difference between 3 groups was statistically significant(P<0.001);the puncture time of group C was shorter than that of group A and group B,and the difference was statistically significant(P<0.001).There was a statistically significant difference in the puncture time between 3 groups(P<0.017);the pairwise comparison of the number of subcutaneous needle adjustments and the success rate of a puncture between 3 groups showed that there was a statistical difference between group C and group A,and between group C and group B(P<0.017),while there was no statistical difference between group A and group B(P>0.017).There was statistical significance(P<0.05)among 3 groups in terms of complications such as accidental nerve injury and puncture of the contralateral vascular wall by puncture needle,but there was no statistical significance in terms of accidental arterial injury and hematoma occurrence among 3 groups.Conclusion Compared with the short axis approach and the long axis approach,the ultrasound oblique axis approach guided PICC puncture has statistical differences in the success rate of a puncture and the incidence of puncture complications,etc.It is recommended to use the ultrasound oblique axis approach during PICC puncture.
2.An investigation of literature reading report among professional postgraduate students in pediatrics

Objective:To investigate the current status of literature reading report and clinical research ability among professional postgraduate students in pediatrics, and to provide a reference for cultivating high-quality pediatric talents.Methods:A total of 91 professional postgraduate students majoring in pediatrics were selected as research subjects from five affiliated hospitals of Zhengzhou University, and according to whether they participated in literature reading report, they were divided into participating group with 38 students and non-participating group with 53 students. The method of questionnaire combined with interview was used to investigate the implementation of literature reading report, the willingness to participate in literature reading report, and the current status of clinical research ability. SPSS 25.0 was used to perform the chi-square test and the Wilcoxon rank-sum test.Results:In the participating group, 60.53% (23/38) of the students thought that there was a significant improvement in clinical research ability after literature reading report, and in the non-participating group, 94.12% (48/51) of the students wanted to carry out literature reading report. Compared with the non-participating group, the participating group had significantly higher publication rate, project participation rate, academic exchange rate, and literature reading quantity ( P<0.05) and significantly better abilities of literature search and reading, professional English, and PPT presentation ( P<0.05), while there were no significant differences between the two groups in the scores of statistical analysis ability, project design ability, clinical thinking ability, and evidence-based medicine ability ( P>0.05). Conclusions:Literature reading report can promote the improvement in clinical research ability among professional postgraduate students in pediatrics, and it should be carried out regularly for a long time with adherence to the principle of clinical orientation and emphasis on the reading of methodology and statistics in literature.
3.The correlations of serum Aβ1-42,P-Tau181,and Hcy levels with sleep disorders in Parkinson's disease patients

Objective To investigate the correlations of serum Aβ1-42,P-Tau181,and Hcy levels with sleep disorders in patients with Parkinson's disease(PD).Methods A total of 80 PD patients were divided into a normal sleep group(<7 points)and a sleep disorder group(≥7 points)based on the evaluations with Pittsburgh Sleep Quality Index(PSQI).Univariate and multivariate logistic regression analyses were performed to investigate the correlations between the levels of serum A β1-42,P-Tau181,and Hcy levels and sleep disorders.Receiver operating characteristic(ROC)curves were used to analyze the efficacy parameters of different indicators in pre-dicting sleep disorders in those patients.Results Univariate and multivariate logistic regression analyses showed that serum Aβ1-42 and Hcy were protective and independent risk factors for sleep disorders in the PD patients,respectively(P<0.05).The predictive AUCs of serum Aβ1-42 and Hcy levels for sleep disorders in the PD patients were 0.757(95%CI:0.652~0.861)and 0.796(95%CI:0.688~0.905),respectively.Conclusion The levels of serum Aβ1-42 and Hcy in PD patients are closely related to sleep disorders.Therefore,they can be used as predic-tive factors for clinical diagnosis of PD with sleep disorders.

5.A Case Report of Homozygous Familial Hypercholesterolemia Liver Transplantation

Homozygous familial hypercholesterolemia (HoFH) is a rare and serious autosomal genetic metabolic disease. Patients without intervention often die younger than 30 years old from early atherosclerotic cardiovascular disease (ASCVD)incurred by extremely high levels of low-density lipoprotein cholesterol (LDL-C). We present a case of HoFH, a child with compound heterozygous mutation in this study. The effect of conventional lipid-lowering therapy through diet control and lipid-lowering drugs was unsatisfactory. The blood-lipid purification proves effective but has poor compliance and difficult to maintain for a longer time. The patient received orthotopic liver transplantation and had been followed for 2 years, with the patient shows normal LDL-C, well growth and development. We hope the case will provide the clinician with better understanding of the diagnosis and treatment of the rare disease of HoFH.
6.Clinicopathological features and prognosis of 128 children with idiopathic membranous nephropathy

Objective:To analyze the clinicopathological features and prognosis of idiopathic membranous nephropathy (IMN) in children, and to investigate the factors influencing their prognosis.Methods:The clinical and pathological data of 128 children with IMN hospitalized in the First Affiliated Hospital of Zhengzhou University from January 2012 to December 2019 were retrospectively analyzed.They were divided into 2 groups according to the pathological manifestations: group A[typical membranous nephropathy(MN) group] and group B (atypical MN group), and the clinicopathological characteristics of the 2 groups were compared.Different treatment regimens and their efficacy were summarized, and the prognosis and its influencing factors were analyzed.The primary endpoint event at follow-up was the occurrence of end stage renal disease (ESRD), and the secondary endpoint event was the occurrence of renal insufficiency.Children with IMN were further divided into endpoint event group and non-endpoint event group according to the presence or absence of endpoint events at the last follow-up.Survival analysis was performed using the Kaplan-Meier survival curve method.The Cox proportional risk model method was used to analyze the factors influencing the prognosis of poor kidney outcomes in children with IMN. Results:(1)A total of 128 children were included, with the male-to-female ratio of 1.13∶1.00.The median age of onset and peak age of onset were 13.0 (10.3, 15.0) years, and 12-16 years (68.8%), respectively.Massive proteinuria was detected in 119 cases (93.0%), including 103 cases (80.5%) with massive proteinuria and hematuria, 4 cases(3.1%) with simple hematuria, and 5 cases (3.9%) with non-renal proteinuria.There were 29 cases (22.7%) in group A and 99 cases (77.3%) in group B. (2)Blood triacylglycerol level was significantly higher in group B than that of group A[2.1 (1.5, 3.0) mmol/L vs.1.7(1.1, 2.5) mmol/L], while high-density lipoprotein[1.5(1.1, 1.8) mmol/L vs.1.8(1.4, 2.1) mmol/L], serum albumin[22.0(17.0, 27.3) g/L vs.25.5 (21.0, 32.5) g/L] and complement C3[(1.1±0.2) g/L vs.(1.2±0.2) g/L] were significantly lower in group B than those of group A (all P<0.05). (3)Complete clinical data during hospitalization and follow-up data were obtained from 91 children with IMN, with a median follow-up time of 87.0 (49.0, 104.5) months.Among them, 5 cases (5.5%) progressed to ESRD, involving 3 cases received renal transplantation, and 9 cases (9.9%) had secondary endpoints.Cumulative renal survival rate for ESRD at 5 and 10 years were 96.2% and 92.9%, respectively, which, for the secondary endpoints at 5 and 10 years were 95.2% and 84.8%, respectively.(4)Kaplan-Meier survival analysis showed no significant difference in the cumulative renal survival between group A and group B ( P>0.05). Multifactorial Cox regression analysis showed that tubular atrophy/interstitial fibrosis was an independent risk factor for renal insufficiency in children with IMN ( HR=0.102, 95% CI: 0.011-0.940, P<0.05). Conclusions:Massive proteinuria combined with hematuria is the major clinical manifestation of IMN in children, and atypical MN is the major pathological manifestation.Tubular atrophy/interstitial fibrosis is an independent risk factor for renal insufficiency in children with IMN.
7.Diagnostic value of triglyceride/cystatin C ratio combined with three indicators in patients with diabetic kidney disease

Objective:To investigate the diagnostic value of triglyceride/cystatin C (TG/Cys-C) ratio combined with diabetic retinopathy, diabetes course and systolic pressure in patients with diabetic kidney disease (DKD).Methods:Patients with type 2 diabetes and renal insufficiency, who underwent renal biopsy in four Grade A tertiary hospitals in Xuzhou from January 2013 to February 2021 were included in this retrospective study. Patients were divided into DKD group ( n=51) and non-DKD (NDKD) group ( n=49) based on renal biopsy results. Another 50 patients with type 2 diabetes mellitus without renal dysfunction were selected as control group. The first admission information and blood biochemical indexes were collected, and the TG/Cys-C ratio was calculated. The differences of clinical indexes between the DKD and NDKD groups were compared. The correlative factors of DKD in type 2 diabetes patients with renal dysfunction were analyzed by logistic regression. The ROC curve was used to evaluate the diagnostic value of TG/Cys-C ratio, diabetic retinopathy, duration of diabetes, systolic blood pressure and combined detection for DKD in patients with type 2 diabetes and renal dysfunction. Results:Among patients with type 2 diabetes with renal dysfunction, renal biopsy results showed that membranous nephropathy was the most common pathological type in NDKD group, accounting for 63.3% (31/49) of all NDKD patients. Compared with Type 2 diabetes mellitus without renal injury, type 2 diabetic patients with kidney injury had higher systolic blood pressure, higher total cholesterol, higher low-density lipoprotein, higher cystatin, higher creatinine, higher uric acid, higher fibrinogen level, more hypertension, more changes of urinary active sediment, lower hemoglobin, lower albumin, and the lower the glomerular filtration rate (all P<0.05). Compared with NDKD group, patients in DKD group had higher systolic and diastolic blood pressure, higher cystatin C and creatinine, more hypertension, more diabetic retinopathy, longer course of diabetes, lower concentration of hemoglobin, lower glomerular filtration rate, lower triglyceride and lower TG/Cys-C ratio (all P<0.05). Multivariate logistic regression analysis showed that TG/Cys-C ratio was associated with DKD in patients with type 2 diabetes mellitus and renal impairment ( OR=0.298, P=0.007), diabetic retinopathy ( OR=12.209, P=0.005), duration of diabetes ( OR=1.016, P=0.034) and systolic blood pressure ( OR=1.049, P=0.006) were independent risk factors for DKD in type 2 diabetic patients with renal dysfunction. The ROC curve showed that AUC of the TG/Cys-C ratio was 0.866 (the cot-off point was 2.06), and the sensitivity was 88.2% and the specificity was 71.4% for the diagnosis of DKD, and AUC of the TG/Cys-C ratio in combination with diabetic retinopathy, diabetes duration and systolic blood pressure was 0.952, and the sensitivity was 92.2% and the specificity was 89.8% for the diagnosis of DKD. Conclusion:TG/Cys-C ratio has significant clinical value in the diagnosis of DKD. Combined detection of TG/Cys-C ratio with diabetic retinopathy, course of diabetes and systolic blood pressure could further improve the diagnostic efficacy of DKD.

Objective:To summarize the clinical characteristics of patients with coronavirus disease 2019 (COVID-19) infected with Delta variant, so as to provide further references for clinical diagnosis and treatment.Methods:A real-world study was conducted to analyze the characteristics of 166 COVID-19 patients infected with Delta variant at Guangzhou Eighth People’s Hospital, Guangzhou Medical University.Results:The study enrolled 5 asymptomatic cases, 123 non-severe cases (mild and moderate type), and 38 severe cases (severe and critical type). Among these patients, 69 (41.6%) were male and 97 (58.4%) were female, with a mean age of 47.0±23.5 years. Thirty-nine cases (23.5%) had received 1 or 2 doses of inactivated vaccine. The incidence of severe COVID-19 cases was 7.7% in 2-doses vaccinated patients, which was lower than that of 11.5% in 1-dose and 26.8% in unvaccinated patients. The proportion of severe cases in 2 dose-vaccinated patients was 7.7%, which was lower than that of 11.5% in 1-dose vaccinated patients and 26.8% in unvaccinated patients, but the difference was not significant ( P>0.05). The most common clinical symptom was fever (134 cases, 83.2%), and 39.1% of cases presented with high-grade fever (≥39 °C); other symptoms were cough, sputum, fatigue, and xerostomia. The proportion of fever in severe cases was significantly higher than that of non-severe cases (97.4% vs. 76.4%, P<0.01). Similarly, the proportion of severe cases with high peak temperature (≥39 ℃) () was also higher than that of non-severe cases (65.8% vs. 30.9%, P<0.01). The median minimal Cycle threshold (Ct) values of viral nucleic acid N gene and ORFlab gene were 20.3 and 21.5, respectively, and the minimum Ct values were 11.9 and 13.5, respectively. Within 48 h of admission, 9.0% of cases presented with decreased white blood cell counts, and 52.4% with decreased lymphocyte counts. The proportions of increased C-reactive protein, serum amyloid A, interleukin 6, and interleukin 10 were 32.5%, 57.4%, 65.3%, and 35.7%, respectively. The proportions of elevated C-reactive protein, serum amyloid A and interleukin-6 in severe cases were significantly higher than those in non-severe cases ( P<0.01). Logistic regression analysis showed that older age and higher peak temperature were associated with a higher likelihood of severe cases ( OR>3, 95% CI: 2-7, P<0.01). In terms of treatment, traditional Chinese medicine (TCM) was used in 97.6% of non-severe cases and 100% in severe cases. Other treatments included respiratory and nutritional support, immunotherapy (such as neutralizing antibodies and plasma of recovered patients). The median times from admission to progression to severe cases, of fever clearance, and of nucleic acid conversion were 5 days, 6 days and 19 days, respectively. No deaths were reported within 28 days. Conclusions:The symptoms of Delta variant infection in Guangzhou are characterized by a high proportion of fever, high peak temperature, long duration of fever, high viral load, a long time to nucleic acid conversion, and a high incidence of severe cases. The severe cases exhibit a higher percentage of elderly patients, a longer duration of fever and have a higher fever rate and a higher hyperthermia rate than non-severe cases. Age and hyperthermia are independent risk factors for progression to severe disease. The combination of TCM and Western medicine can control the progression of the disease effectively.
9. A qualitative study on the construction of clinical problems based on the guidelines of primary percutaneous coronary intervention nursing practice

Objective:
To understand the working categories of primary percutaneous coronary intervention (PCI) nursing and its existing problems, and to provide a realistic basis for the construction of primary PCI in clinical nursing.
Methods:
Qualitative interviews were used to carry out semi-structured and personal in-depth interviews among 5 doctors and 27 nurses in 5 hospitals. 7 step analysis of Colaizzi was used to analyze the data.
Results:
The working categories of primary PCI nursing involves professional team management, early identification, preoperative preparation, evaluation and predictive nursing, disease observation, safe transfer, psychological nursing and health guidance.
Conclusion
Primary PCI nursing is still in the stage of continuous optimization, but some parts of the process are not standardized and the nursing behavior is inconsistent. Primary PCI nursing needs to form nursing behavior norms under the guidelines of evidence-based medicine.
10. Prediction of white matter hyperintensities progression based on radiomics of whole-brain MRI: a study of risk factors

Objective:
To explore the risk factors of predicting white matter hyperintensities progression based on radiomics of MRI of whole-brain white matter.
Methods:
The imaging and clinical data of 152 patients with white matter hyperintensities admitted to Zhejiang People′s Hospital from March 2014 to October 2018 were retrospectively analyzed. The whole brain white matter on baseline T1WI images of each patient were segmented by SPM12 software package, and images of white matter were imported into AK software for texture feature extraction and dimensionality reduction. At last, least absolute shrinkage and selection operator(LASSO) was used to calculate the score of radiomics signature of each patient. According to the improved Fazekas scale, patients with WMH progression were divided into three groups: any white matter hyperintensities (AWMH), periventricular white matter hyperintensities (PWMH) and deep white matter hyperintensities (DWMH).
