1.Feasibility of Multiparameter MRI-Guided Percutaneous Biopsy for Central Lung Lesions With Atelectasis

Objective:
To prospectively evaluate the feasibility, accuracy, and safety of multiparameter MRI-guided percutaneous biopsy using a 1T open MRI scanner for evaluating suspicious centrally located lung lesions with associated post-obstructive atelectasis.
Materials and Methods:
In this single-center study, MRI-guided percutaneous coaxial cutting biopsy was performed for 107 suspicious central lung lesions with associated post-obstructive atelectasis in 107 patients between July 2015 and December 2020. A fast T2-weighted imaging (T2WI)-turbo spin echo (TSE) sequence and an enhanced fast T1-weighted imaging (T1WI)-TSE sequence were used to identify, localize, and biopsy lung lesions, and diffusion-weighted imaging (DWI) was used as a supplementary sequence for identifying the lesion location. The final diagnosis was confirmed by surgical histopathology or clinical follow-up for a minimum of 24 months. The sensitivity, specificity, and accuracy for diagnosing lung malignancies were calculated, and the complications were recorded for each case.
Results:
Using multiparameter MRI, central lung lesions could be clearly distinguished from post-obstructive atelectasis in 96 patients (89.7%). The sensitivity, specificity, and accuracy of MRI-guided percutaneous biopsy for diagnosing lung malignancy was 97.0% (98/101), 100% (6/6), and 97.2% (104/107), respectively. Self-limited hemoptysis occurred in three patients. Pneumothorax occurred in five patients, of which none required pleural drainage. No serious procedure-related complications were observed.
Conclusion
As a technology that does not involve ionizing radiation, multiparameter MRI-guided percutaneous coaxial cutting biopsy is a safe and accurate diagnostic technique for evaluating centrally located lung lesions associated with post-obstructive atelectasis.
2.Application of a teaching model undertaking"learning,thinking,applying,integrating,and innovating"based on heritage and innovation in the teaching of Basic Theory of Traditional Chinese Medicine

Objective:To explore the application effect of the teaching model undertaking"learning,thinking,applying,integrating,and innovating"based on heritage and innovation in the teaching of Basic Theory of Traditional Chinese Medicine.Methods:Undergraduate students of two classes majoring in Traditional Chinese Medicine(grade 2024)from a university of Chinese Medicine were selected as our research subjects.A total of 55 students from class 7 were enrolled in the control group,while 55 students from class 8 were enrolled in the experimental group.The control group received the traditional lecture-based teaching model,whereas the experimental group adopted a teaching model undertaking"learning,thinking,applying,integrating,and innovating"based on heritage and innovation.The formative evaluation and the final exam scores of two groups were compared,and a questionnaire survey was conducted to evaluate teaching effectiveness.Results:The self-directed learning and final exam scores of the experimental group were significantly higher than those of the control group(P<0.01).The teaching effects such as learning participation and mastery of theoretical knowledge of the experimental group were significantly better than those of the control group(P<0.01).Conclusion:Application of the model of"learning,thinking,applying,integrating,and innovating"based on inheritance and innovation in the teaching of Basic Theory of Traditional Chinese Medicine can stimulate students'interest in learning,solve problems in teaching,improve teaching quality,and establish students'professional confidence in the application of Basic Theory of Traditional Chinese Medicine courses.
3.Application potential of multimodal CT imaging parameters in differentiation of TOAST classification for cerebral infarction

Objective To analyze the application value of multimodal CT imaging parameters in the differentiation of TOAST classification for cerebral infarction.Methods According to the TOAST classification,the selected 103 patients with cerebral infarction were categorized into large-artery atherosclerosis(LAA)group,cardioembolic(CE)group,small-artery occlusion(SAO)group,other determined etiologies(SOD)group,and undetermined etiologies(SUE)group.All patients underwent non-contrast CT scanning and CT angiography.Cerebral angiography was adopted as the gold standard.Results For the LAA,CE,SAO,SOD,and SUE groups,the diagnostic performance of multimodal CT imaging was as followed:true positive rates were 73.17%,45.45%,75.00%,40.00%,and 72.00%;sensitivities were 90.10%,85.71%,92.30%,85.71%,and 90.00%;specificities were 75.00%,75.00%,66.67%,66.67%,and 80.00%;miss rates were 9.09%,28.57%,7.69%,14.28%,and 10.00%;misdiagnosis rates were 25.00%,25.00%,33.33%,33.33%,and 20.00%;positive predictive rates were 93.75%,83.33%,92.31%,85.71%,and 94.74%;and negative predictive rates were 66.67%,60.00%,66.67%,66.67%,and 66.67%,respectively.Kappa test results showed that the multimodal CT imaging findings were highly consistent with the gold standard(Kappa>0.75,P<0.05).Conclusion Multimodal CT imaging technology can effectively assist in the differentiation of TOAST classification for cerebral infarction,thereby improving the accuracy of clinical diagnosis.
4.Effects of Advanced Glycation End Products and Their Receptor on Osteoporosis in Type 2 Diabetes Mellitus

Osteoporosis caused by type 2 diabetes mellitus(T2DM)increases the risk of fracture and post-fracture mortality.However,the pathogenesis of the disease remains unclear,resulting in a lack of effective strategies for its prevention and treatment.In this review,firstly,the effects of advanced glycation end products(AGEs)produced by non-enzymatic glycation on bone matrix composition,bone structure,and mechanical properties of T2DM are summarized.Then,the biological mechanism of AGEs and receptor for AGEs(RAGE)affecting bone degeneration in T2DM is clarified.Finally,antidiabetic and other drugs that are beneficial to bone anabolism are discussed.These drugs positively affect bone quality through inhibiting AGE/RAGE signaling pathway.Accordingly,it is expected to provide potential intervention targets and ideas for the prevention and treatment of T2DM-related osteoporosis.

7.Effect of life reconstruction training on anxiety and depression among individuals with chronic spinal cord injury and their caregivers

Objective To explore the effect of life reconstruction training on anxiety and depression among individuals with chronic spinal cord injury(SCI)and their caregivers,and to analyze the influencing factors.Methods A total of 216 SCI patients and 79 caregivers who participated in life reconstruction training were recruited from the Hope House for SCI individuals in Shanghai Yangzhi Rehabilitation Hospital from May,2017 to Octo-ber,2019.They were assessed with Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)be-fore and after intervention.Results After intervention,the scores of SAS and SDS decreased(P<0.05)in both SCI patients and their caregivers.SAS scores were more likely to reduce in SCI patients who suffered anxiety at admission(OR=2.315,95%CI 1.328 to 4.033,P=0.003)and their caregivers(OR=3.632,95%CI 1.270 to 10.390,P=0.016),as well as non-agricultural-registered SCI patients(OR=1.908,95%CI 1.074 to 3.391,P=0.028).Being unmarried,having an injury duration≥21 years,injury due to disease,and having depression at admission significantly promoted the reduction in SDS scores among SCI patients(P<0.05).Caregivers with a junior-high-school education or less were more likely to experience reductions in scores of SAS and SDS compared with those with higher education(P<0.05).Caregivers with five to 15 years of caregiving experience achieved more anxiety reduction(OR=7.155,95%CI 1.806 to 28.342,P=0.005).Conclusion Life reconstruction training is effective on anxiety and depression among individuals with chronic SCI and their caregivers.It is recommended to prioritize anxiety interventions for patients with rural household registra-tion in China,along with depression management for the married/divorced/widowed individuals,with<21 years since injury,and trauma-induced lesions.For caregivers,it is needed to focus to anxiety and depression in better-educated individuals,as well as anxiety in those with either<5 or≥15 years of caregiving experience.
8.Construction of perioperative pain nursing protocol for thoracoscopic surgery patients

Objective:To construct a perioperative pain nursing protocol for thoracoscopic surgery patients, providing a reference for clinical pain nursing practice.Methods:An evidence-based approach was used to search relevant guidelines and extract the best evidence. The initial draft was created through discussions among the research team, followed by two rounds of Delphi expert consultations. Based on the experts' suggestions, the protocol was revised and the best plan was finalized.Results:A total of 10 guidelines were included, and 22 experts participated in two rounds of consultations. The response rate for the consultation questionnaires was 100.00% (22/22) , with expert authority coefficients of 0.94 and 0.95 for the two rounds, respectively. The coefficient of variation for all indicators in the second round ranged from 0.04 to 0.24. The final pain nursing protocol included four primary indicators: personnel preparation, pain assessment, pain education, and pain intervention, with 10 secondary indicators and 27 tertiary indicators.Conclusions:The constructed perioperative pain nursing protocol for thoracoscopic surgery patients is significant, scientific, comprehensive, and targeted. It provides theoretical support and practical guidance for pain management, helping to reduce postoperative pain in patients.
