1.Analysis of Medication Patterns for Ancient Epidemic Treatment Based on Data Mining

ObjectiveExploring the formula rules of commonly used traditional Chinese medicines(TCMs) for epidemic treatment from the Qin and Han dynasties to the Qing dynasty through data mining, providing reference for the prevention and control of contemporary epidemics. MethodsThe articles on epidemic treatment in the electronic database of Chinese Medical Code V5.0 were systematically searched, and the contents such as source, dynasty, author, diagnosis, formula name, therapeutic method and efficacy, and composition of medicines from each article that met the inclusion criteria were extracted. Then, an Excel standardized database was established, and Python programs were used for data mining to summarize the frequency of commonly used medicines and perform hierarchical cluster analysis, Pearson correlation analysis, and association rule analysis. ResultsA total of 1 595 formulas were included, involving 558 TCMs. The efficacy of these medicines could be classified into two categories, namely, expeling pathogenic factors and reinforcing healthy Qi. According to the frequency deconstruction analysis, high-frequency medicines were mainly detoxification, Fu-organ dredging, aromatization and promoting blood circulation, followed by the medicines with the effect of treating the lungs, such as clearing the lungs and resolving phlegm, clearing heat and purging the lungs, relieving cough and asthma, and purging the lungs and relieving asthma. And the proportions of acrid-warm herbs and acrid-cold herbs varied in different periods. Hierarchical clustering and correlation analysis both suggested TCMs for expeling pathogenic factors and reinforcing healthy Qi often formed stable combinations with high association degrees. Association rule analysis showed that the core acrid-warm herb was mainly Ephedrae Herba, and the core acrid-cold herb was mainly Forsythiae Fructus, resulting in the core formulas of Maxing Shigantang and Yinqiaosan. ConclusionThroughout history, the prevention and control of epidemics have been based on the principle of "preserving healthy Qi and avoiding toxic Qi", focusing on the treatment of the causes and characteristics of epidemics through detoxification, Fu-organ dredging, and aromatization, emphasizing the use of Rhei Radix et Rhizoma and other herbs to dredge Fu-organ, eliminate toxins and pathogens, and playing the role of actively intervene with symptomatic medication. And based on the external manifestations of the body's struggle between evil and righteousness, diagnose and treatment according to syndrome differentiation was performed.

3.Research progress on running-related joint injuries and rehabilitation treatment

Under the backdrop of the "Healthy China 2030" strategy, running has become the most common form of exercise. Fitness running is a kind of endurance aerobic exercise. Compared with swimming, aerobics, and other sports with high activity and intensity, the risk of lower limb fatigue injury during fitness running is higher. In this review paper, the risk factors for running-related injuries were summarized by consulting and analyzing the database and focuses on discussing and analyzing the impact of running on joints. The results showed that hip adduction, knee bending, and ankle joint abnormalities are the three joint-related factors that cause the main injuries during running. Four rehabilitation intervention methods for running-related injuries were proposed, that can guide patients to generate personalized rehabilitation treatment plans through training.

Positive pressure bio-protective equipment is a key protective equipment for high-level biosafety laboratories, which provides comprehensive protection for professionals working in pathogenic microorganisms or other biological contamination environments. This paper summarizes the development status of positive pressure bio-protective equipment at home and abroad, analyzes the existing problems of the current positive pressure bio-protective equipment, and puts forward suggestions, to provide reference for the development and application of positive pressure bio-protective equipment in our country.
5.Intervention effect of moderate-intensity aerobic exercise on Alzheimer's disease

Objective To investigate the effects of moderate-intensity aerobic exercise intervention and its influencing factors in patients with Alzheimer's disease(AD).Methods Ninety-eight AD patients were randomly assigned into exercise group(50 cases)and control group(48 cases).The exercise group received moderate-intensity aerobic training for 3 months.Cognitive function was assessed using the MMSE and the AD assessment scale-cognitive subscale(ADAS-cog)at baseline,3 months and 6 months.Daily living abilities was evaluated by the AD cooperative study-activities of daily living scale(ADCS-ADL),behavioral symptoms was assessed by the neuropsychiatric Inventory(NPI)and life quality was measured by quality of life in AD(QOL-AD).The difference between 3 months and baseline ADAS-cog(ADAS-cog3-0)score was used as the dependent variable,multiple-factor linear regression was performed to analyze whether baseline characteristics of patients would affect the efficacy of exercise.Patients with an improvement of ≥4 points in ADAS-cog3-0 were classified into good prognosis group,and the others were classified into non-good prognosis group.Variables with P<0.05 in the multiple-factor linear regression analysis were included in Logistic regression analysis to determine the best cutoff value through ROC curve.Results Compared with the control group,the exercise group showed a significant increase in MMSE and QOL-AD scores at 3 and 6 months,a significant decrease in ADAS-cog score at 3 and 6 months,a significant increase in ADCS-ADL score at 3 months,and a significant decrease in NPI score at 3 months(all P<0.05).However,there was no significant difference between the two groups in ADCS-ADL and NPI scores at 6 months.Regression analysis showed that baseline MMSE score had a negative impact on cognitive function improvement in the exercise group,while age,gender,education level,ischemic symptoms,baseline activities of daily living,behavioral and psychological symptoms,and quality of life had no impact.Sensitivity and specificity analysis showed that aerobic exercise was more effective in AD patients with an MMSE score>21.5 points,and aerobic exercise was still effective but had moderate effects in patients with an MMSE score below this cutoff.Conclusions Aerobic exercise has a significant improvement on cognitive function,activities of daily living,behavioral and psychological symptoms,and quality of life in AD patients.The better the cognitive level is preserved in AD patients,the better the short-term effects of aerobic exercise are.

The global COVID-19 coronavirus pandemic has infected over 109 million people, leading to over 2 million deaths up to date and still lacking of effective drugs for patient treatment. Here, we screened about 1.8 million small molecules against the main protease (Mpro) and papain like protease (PLpro), two major proteases in severe acute respiratory syndrome-coronavirus 2 genome, and identified 1851Mpro inhibitors and 205 PLpro inhibitors with low nmol/l activity of the best hits. Among these inhibitors, eight small molecules showed dual inhibition effects on both Mpro and PLpro, exhibiting potential as better candidates for COVID-19 treatment. The best inhibitors of each protease were tested in antiviral assay, with over 40% of Mpro inhibitors and over 20% of PLpro inhibitors showing high potency in viral inhibition with low cytotoxicity. The X-ray crystal structure of SARS-CoV-2 Mpro in complex with its potent inhibitor 4a was determined at 1.8 Å resolution. Together with docking assays, our results provide a comprehensive resource for future research on anti-SARS-CoV-2 drug development.

7.Research progress on the pathogenesis of Alzheimer disease and microbes

Alzheimer disease (AD) is a progressive neurodegenerative disease that often occurs in the elderly. The main symptoms of AD patients are cognitive decline, especially memory deficits, communication problems, impaired performance of activities of daily living, and other behavioral and psychological abnormalities. AD is generally divided into familial AD and sporadic AD. The signature features of AD are the presence of amyloid-beta proteins in extracellular plaques. Senile plaques formed by accumulation of Aβ insoluble fragments and intracellular accumulation of highly phosphorylated tau are neurofibrillary tangles. There is a lot of scientific evidence to confirm that the development of the two signature features of AD may be related to naturally occurring microbes in the human body. This paper reviews the mechanism of intestinal, oral and fungal microbiota involved in the development and progression of AD.
8.A two-dimensional video based quantification method and clinical application research of motion disorders.

The increasing prevalence of the aging population, and inadequate and uneven distribution of medical resources, have led to a growing demand for telemedicine services. Gait disturbance is a primary symptom of neurological disorders such as Parkinson's disease (PD). This study proposed a novel approach for the quantitative assessment and analysis of gait disturbance from two-dimensional (2D) videos captured using smartphones. The approach used a convolutional pose machine to extract human body joints and a gait phase segmentation algorithm based on node motion characteristics to identify the gait phase. Moreover, it extracted features of the upper and lower limbs. A height ratio-based spatial feature extraction method was proposed that effectively captures spatial information. The proposed method underwent validation via error analysis, correction compensation, and accuracy verification using the motion capture system. Specifically, the proposed method achieved an extracted step length error of less than 3 cm. The proposed method underwent clinical validation, recruiting 64 patients with Parkinson's disease and 46 healthy controls of the same age group. Various gait indicators were statistically analyzed using three classic classification methods, with the random forest method achieving a classification accuracy of 91%. This method provides an objective, convenient, and intelligent solution for telemedicine focused on movement disorders in neurological diseases.

9.Differences in expression of Wnt5a in prediabetes and diabetes and its correlation with immune inflammation in patients with prediabetes

Objective:To analyze the difference of expression of Wnt protein family member 5a(Wnt5a)in prediabetes and diabetes mellitus and its correlation with immune inflammation in patients with prediabetes.Methods:From November 2020 to Novem-ber 2021,64 patients with prediabetes who treated in Zaozhuang Municipal Hospital were selected as group A,51 patients with diabe-tes were selected as group B,and 50 healthy people who underwent physical examination were selected as group C.The baseline data of 3 groups were collected,level of Wnt5a was determined,expression difference of Wnt5a in prediabetes and diabetes was analyzed,the related indicators of immune inflammation were determined,and the correlation between Wnt5a and immune inflammation in patients with prediabetes was analyzed.Predictive value of Wnt5a for prediabetes and diabetes was analyzed by ROC curve,and its cutoff values were determined for clinical disease prediction.Results:Compared with group B,Wnt5a level,immune inflammatory indexes Th1,Th17,Th22,IFN-γ,IL-17A and IL-22 levels in group A were decreased(P<0.05).Logistic regression analysis showed that fasting blood glucose,Wnt5a,Th1,Th17,Th22,IFN-γ,IL-17A and IL-22 were independent risk factors for the occurrence of prediabetes(P<0.05).Wnt5a was positively correlated with immune inflammation-related indicators Th1,Th17,Th22,IFN-γ,IL-17A and IL-22 in patients with prediabetes(P<0.05).ROC curve showed that AUC values of Wnt5a in predicting prediabetes and diabetes were 0.921 and 0.897,respectively,and the AUC were all>0.7,which indicating that Wnt5a had a higher predictive value for predia-betes and diabetes(P<0.05).Conclusion:There is a certain difference in expression of Wnt5a in prediabetes and diabetes.When level of 42.38 pg/ml≤Wnt5a<62.50 pg/ml,it is prediabetes,and when level of Wnt5a≥62.50 pg/ml,it indicates that the transition from prediabetes to diabetes has occurred,and believed that Wnt5a is related to immune inflammation in prediabetic patients,which may increase the degree of insulin resistance by amplifying local immune inflammation in prediabetic patients and eventually lead to the occurrence of diabetes.
10.Cornelia de Lange syndrome with recurrent seizures and SMC1A gene c. 2923C > T mutation: a case report and literature review

Objective:To explore the characteristics, clinical manifestations and gene mutation types of Cornelia de Lange syndrome (CdLs), and to improve the understanding of the disease.Methods:Clinical data and gene test results of a pediatric case of CdLs diagnosed in the First Affiliated Hospital of Xinxiang Medical University in August 2019 were analyzed retrospectively.Results:A female patient with 2 years and 8 months old presented a special appearance with a low and flat nose, a wide eye distance, audition ears, a downward inclination of the mouth corner, a high arch of the jaw and a small jaw deformity, who had recurrent seizures, speech and mental retardation.The result of gene test showed the mutation of SMC1A gene c. 2923C > T, and thus the patient was diagnosed as type 2 CdLs. Conclusions:CdLs is a rare genetic metabolic disease with special facial features and physical signs.There is only one case of CdLs with gene mutation of SMC1A in China through literature review.The mutation of SMC1A gene c. 2923C>T in CdLs cases has not been reported at home and abroad, which expands the variation spectrum of the SMC1A gene.
