1.Efficacy and safety of microneedle injection of extracellular vesicles from mesenchymal stem cells in treatment of melasma

Objective:To explore the effect and safety of mesenchymal stem cell exosome microneedle introduction in the treatment of melasma.Methods:Thirty cases of female patients with stable melasma in the Department of Dermatology, Changsha Meilai Medical Beauty Hospital, aged (36±5) years and with a disease duration of (42.4±20.7) months, from July 2021 to July 2022, were retrospectively included. According to Fitzpatrick skin typing, 23 cases of type Ⅲ and 7 cases of type Ⅳ were included. All patients were locally anesthetized with lidocaine cream for 30 min, and rolled with a 0.5 mm needle in a zigzag pattern with even force, in the order of the right cheek, the left cheek, the forehead, the nose, the mandible, and the upper lip. During the rolling process, 3 ml of MSC exosome medical liquid wound dressing was applied to the facial skin, and after it was fully absorbed, exosome was locally readministered in the area of melasma. Treatment ended with a slight redness at the site of application. 1 MSC exosome wound dressing was appllied as a cold compress for 15 min after treatment. Treatment was given once every 2 weeks for 6 consecutive sessions. All the patients were followed up at 4 and 12 weeks after the last session, and the area and severity index of melasma (MASI) were scored before and after the treatment, the clinical efficiency and patient satisfaction rate and the incidence of adverse reactions were also counted.Results:At 4 and 12 weeks after the end of treatment, the skin color of all 30 patients was brighter than that before treatment, and no recurrence of melasma symptoms seen. At 12 weeks after the end of treatment, the decrease rate of MASI score was 66.1%, among which the decrease rate of MASI score in patients with type Ⅲ melasma was 63.9%, and the decrease rate of MASI score in patients with type Ⅳ melasma was 63.9%. Among the 30 patients, 1 case was cured, 25 cases showed obvious improved, 4 cases were improved, and no cases were ineffective, with an effective rate of 86.7% (26/30). Five patients were very satisfied, 18 patients were satisfied, 6 patients were generally satisfied, and 1 patient was dissatisfied; the patient satisfaction rate was 76.7% (23/30). No serious adverse reactions occurred in all patients.Conclusions:MSC exosome microneedle introduction is safe and effective in the treatment of melasma without serious adverse reactions.
2.Application research on risk management of postoperative delirium in elderly patients with hip fragility fracture based on FMEA theory

Objective:To explore the effect of postoperative delirium risk management in elderly patients with hip fragility fracture based on failure mode and effect analysis (FMEA) theory, and to provide a basis for reducing the incidence of postoperative delirium.Methods:A total of 50 patients admitted to the First Affiliated Hospital of Sun Yat-sen University due to hip fragility fractures from January to December 2019 were selected as the control group, and 50 patients admitted to the First Affiliated Hospital of Sun Yat-sen University for hip fragility fractures from January to December 2020 were selected as the observation group. The control group received routine care, and the observation group implemented risk control intervention measures based on FMEA theory on the basis of the control group. The risk priority number (RPN) value, incidence of delirium, duration of delirium, pain score, satisfaction, and average length of hospital stay were compared between the two groups of patients in each link of failure risk.Results:The RPN values of each link failure risk of the observation group were 100.80 ± 13.39, 103.96 ± 9.96, 103.76 ± 8.04, delirium duration was (36.33 ± 9.07) min, pain scores were 1.86 ± 0.76, 4.16 ± 1.17, average length of stay was (8.98 ± 4.64) days, and incidence of delirium was 6.0% (3/50), the RPN values of each link failure risk of the control group were 274.10 ± 8.48, 291.00 ± 10.10, 287.78 ± 11.64, delirium duration (78.70 ± 20.10) min, pain scores 2.26 ± 1.02, 4.74 ± 1.19, average length of stay was (11.50 ± 7.66) days, and incidence of delirium was 22.0% (11/50). The differences between two groups showed significant differences ( t values were 1.99-93.24, χ2=4.07, P<0.05). The patient satisfaction score of the observation group was 99.36 ± 1.01, which was higher than that of the control group 89.63 ± 2.62, and the difference was statistically significant ( t=24.50, P<0.05). Conclusions:The perioperative implementation of postoperative delirium risk management model based on FMEA theory in elderly patients with hip fractures can reduce the incidence of postoperative delirium, relieve pain, shorten hospital stay, and improve satisfaction degree. It is worthy of clinical promotion.
3.Depressive symptoms detection among the urban elderly in Ya'an city and its influencing factors six years after Lushan earthquake

ObjectiveTo investigate the prevalence and characteristics of depressive symptoms among urban elderly six years after Lushan earthquake in Ya'an, so as to provide references for the mental health interventions for elderly following catastrophic stressful life events. MethodsFrom March to April 2019, a multi-stage stratified cluster random sampling method was adopted to enroll 885 urban elderly people aged 60 and above in Ya'an. A self-compiled questionnaire was used to collect the general demographic information, health-related and disaster-related information, meantime, the elderly was assessed using Geriatric Depression Scale (GDS-30). Thereafter, univariate and multivariate Logistic regression were applied to explore the influencing factors of depressive symptoms in urban elderly. ResultsA total of 783 valid questionnaires were collected, with a questionnaires response rate of 88.47%. Depressive symptoms were detected in 161 cases (20.56%). The prevalence of depression showed statistical differences among the elderly of different gender, age, marital status, family relationship, monthly per capita household income, physical exercise status, health status, self-care ability, sleep status and disaster-affected degree (P<0.05 or 0.01). Logistic regression analysis showed that the urban elderly of the female gender (OR=1.552, P=0.040), monthly per capita household income of 2000~3000 yuan (OR=6.982, P<0.01), monthly per capita household income≤2000 yuan (OR=6.857, P<0.01), lack of physical exercise (OR=1.693, P<0.01), being less capable of self-care (OR=3.838, P<0.01), being incapable of self-care (OR=8.547, P<0.01), complicating multiple curable diseases (OR=4.892, P<0.01) and complicating refractory chronic diseases (OR=5.657, P=0.031) were at high risk of depressive symptoms. The risk of depressive symptoms among the divorced or widowed elderly was greater than that among married elderly (OR=0.063, P<0.01). ConclusionThe prevalence of depressive symptoms is relatively high among the urban elderly six years after Lushan earthquake in Ya'an, moreover, female gender, low monthly per capita household income, lack of physical exercise, being incapable of self-care and poor health status are risk factors affecting the depressive symptom, while being married is a protective factor.
4. Keratinized tissue augmentation techniques around dental implants

Research have indicated that inadequate keratinized tissue has a negative effect on patient oral hygiene, resulting in peri-implant inflammation. It has been recommended that an apically repositioned flap alone or in combination with autogenous soft tissue grafts can increase the width of keratinized mucosa around dental implants, which promotes long term peri-implant health. This review summarized research progress on augmentation techniques of keratinized tissue arround implants in recent years, so as to provide reference for clinical practice and research design in the future.
5.Effect of lipopolysaccharide exposure on neuro-immune system of rat offspring with autism spectrum disorder at second trimester

Objective To establish the lipopolysaccharide(LPS)-induced autism spectrum disorder(ASD) models at second trimester in rat offspring,and to investigate the effect of LPS exposure on social interaction behavior and neuro-immune system in rat offspring.Methods Twelve-week-old pregnant specific pathogen free rats were randomly divided into non-exposed group(10 cases) and LPS-exposed group(10 cases).Rats in the exposure group were intraperitoneally injected with LPS 25 mg/kg at 12.5 d,while rats in the non-exposed group were intraperitoneally injected with 9 g/L saline.At the age of 28 days,10 rats were randomly selected from each group as the experimental group and the control group.The following tests were carried out in two groups of rats.Their offspring were randomly allocated to the control group (10 cases) and the experimental group (10 cases).The autism-like social behaviors were evaluated by the social interaction test.Density of dendritic spine was analyzed by Golgi staining.The expression of glial fibrillary acidic protein (GFAP) and ionized calcium-binding adapter molecule 1 (IBA1) were analyzed by immunohistochemisty.The levels of i nterleukin (IL)-1 β,IL-4,IL-6,IL-10,interferon-gamma (IFN-γ'),and transforming growth factor-beta(TGF-β) in rat offspring were detected by enzyme-linked immunosorbent assay.Results The score of social interaction test in the experimental group [(-197.81 ± 101.83) scores] was lower than that in the control group [(132.73 ± 114.63) scores],and the difference was statistically significant (t =5.453,P < 0.05).Density of dendritic spine in CA1 region of hippocampus was lower in the experimental group[(7.71 ± 1.33) numbers/10 μm] than that in the control group[(9.66 ± 1.76) numbers/10 μm] by Golgi staining,and the difference was statistically significant(t =2.775,P < 0.05).Expressions of GFAP and IBA1 in experimental group were higher than those in the control group by immunohistochemisty.Levels of IL-1 β,IL-6,IL-10 and IFN-γ in the experimental group were higher than those in the control group,while level of TGF-β was lower than that in the control group,and the differences were statistically significant(all P <0.05).The scores of social interaction test were correlated with the levels of IFN-γ(r =0.756,P < 0.05).Conclusion LPS exposure at early pregnancy may affect the neuro-immune system of rat offspring and result in autism-like behavior.
6.Pre-clinical application of self-developed computer assisted design/rapid prototyping and guidance system to assist precise acetabular component placement: A pilot study

Objective To introduce a self-developed computer-assisted design/rapid prototyping and guidance system used for precise placement of the acetabular component in total hip arthroplasty.Methods We collected the preoperative pelvic CT scanning data of 10 hips with aeetabular dysplasia that had undergone primary total hip arthroplasty from January 2016 to January 2017.The total time for import of radiographic images,model reconstruction,model segmentation,acetabular component position design and STL model export was calculated and compared between our self-designed software and Mimics vl 7.0.Three kinds of STL model from each case were imported into our self-developed 3D printing device,Stratasys Objet30 and Stratasys Demension SST1200es respectively for rapid prototyping.The printing efficiency and accuracy were compared among the 3 printers.The accuracy of placing acetabular component with guidance system was evaluated.Results The average time forpreoperative planningwas7.7±1.3 minbyourself-designedsoftware and 52.5 ± 15.9 min by Mimics v17.0,showing a significant difference (P < 0.001).In morphological point-based comparison for each case,the 3D models exported by the 2 different kinds of software showed an average difference of 0.072 1 ± 0.069 1 mm.The average durations for rapid prototyping by the 3 different printers were 5.3 ± 0.6 h,10.8 ± 0.5 h,and 9.3 ± 0.6 h,respectively,showing significant differences (P < 0.001).The guidance system resulted in precise placement.The locations of the acetabular component achieved by guide-assisted placement were not significantly different from the target ones (P > 0.05).Conelusion Our self-developed preoperative planning software,rapid prototyping device and guidance apparatus for acetabular component placement may lead to good accuracy and high efficiency.
7.Generation of GABAergic interneuron-specific PGC-1α knockout mice.

OBJECTIVETo generate mice which are specific for peroxisomproliferator-activated receptor-γ coactivator-1(PGC-1α) knockout in the GABAergic interneuron.
METHODSConditional mice specific for PGC-1αwere introduced from the Jackson Laboratory, USA and initially inbred to obtain homozygote PGC-1αmice. The PGC-1αconditional mice were further crossed with Dlx5/6-Cre-IRES-EGFP transgenic mice to achieve specific knockout of PGC-1α in the GABAergic interneuron.
RESULTSThe offspring with specific knockout PGC-1α gene were successful for the generation of GABAergic interneuron, with the resulting genotype being PGC-1α.
CONCLUSIONThe PGC-1αmice were obtained through a proper crossing strategy, which has provided a suitable platform for studying the function of PGC-1α in neuropsychiatric diseases.

Objective To investigate the effect of local delivery of delta 12-prostaglandinJ2-loaded poly(lactic-co-glycolic acid) (△ 12-PGJ22-NC) on growth factors expression and bone formation.Methods △ 12-PGJ2-NC was prepared by the emulsion solvent diffusion method.The physical and chemical properties of the nanoparticles were evaluated by particle size analysis,transmission electron microscopy,drug-loading ratio and the in vitro release study.Then standardized transcortical defect(5.0 mm × 1.5 mm) was conducted in the femur of 48 male Wistar rats which were randomly divided into four groups(n=12),S,K,F,and N.Thirty microliter of saline(S),unloaded nanoparticles(K),△ 12-PGJ2(F) and △ 12-PGJ2-NC(N) in a collagen vehicle were delivered inside a titanium chamber fixed over the defect.Then,four subgroups were randomly divided in each group named as D3,D7,D14,and D28(n=3) according to the days 3,7,14,and 28 after the surgery.At days 3,7,14,and 28,the mRNA expression of the bone morphogenetic protein-6(BMP-6),platelet-derived growth factor-B(PDGF-B) in defect aera was analyzed by real time quantitive-polymerase chain reaction(qRT-PCR).The protein expression of the BMP-6 and Ephrin-B2 was evaluated by Western blotting.HE staining was employed to reveal new bone formation in weeks 2 and 4.Results △ 12-PGJ2-NC appeared opalescent white and remained relatively stable,with an average particle size of (135.2±0.85) nm.The images from transmission electron microscopy showed that △ 12-PGJ2-NC was spherical in shape and homogeneously distributed.The encapsulation efficiency of △ 12-PGJ2 with the poly(lactic-co-glycolic acid) (PLGA) nanocapsules was about 92%.The in vitro release of △ 12-PGJ2-NC at 37 ℃ showed a sustained fashion and the average accumulated amount was 30%,52%,77%,91%,and 98% respectively,at 0.5,1,2,4 and 6 h.Compared with the animals treated with saline,after dose of 100 mg/L △ 12-PGJ2 and △ 12-PGJ2-NC apllication,the mRNA expression level of BMP-6,PDGF-B increased significantly(P<0.05,P< 0.001).The protein expression of BMP-6,Ephrin-B2 also was up-regulated.Histomorphometry revealed that new bone formation increased at the same dose of 100 mg/L.But the unloaded nanoparticles did not have the same effect(P>0.05).Conclusions A stable△ 12-PGJ2 loaded nanoparticle was successfully prepared.△ 12-PGJ2-NC may upregulate the expression of BMP-6,PDGF-B and Ephrin-B2,and promote new bone formation in bone defect area.
9.Gait parameters in lower extremities of patients in sequela period after unilateral total hip arthroplasty

BACKGROUND:Total hip arthroplasty is known to be a successful surgical procedure to al eviate hip pain and to regain gait ability, but there are many debates on whether patients’ gait could reach a normal level several years later. OBJECTIVE:To investigate spatiotemporal parameter characters of lower extremities in patients during walking in sequela period fol owing unilateral total hip arthroplasty, and to probe into patients’ walking abilities.
METHODS:In accordance with the method of Case Database Data Analysis detected by Motion Reconstruction Laboratory, 14 patients at 5-10 years after total hip arthroplasty served as the experimental group, and 14 matched healthy persons served as the control group. Gait parameters of lower extremities during walking were col ected by Vicon Nexus. Spatiotemporal parameter characters of lower extremities during gait cycle were analyzed by using Polygon.
RESULTS AND CONCLUSION:Compared with the control group, walking speed, cadence, stride length and step length decreased, but double support and opposite foot off increased on both affected and unaffected sides in the experimental group (P<0.05). Both stride time and step time of the operated lower extremities increased (P<0.05). Foot off of non-operated lower extremities increased (P<0.05). There was no significant difference in al spatiotemporal parameters between operated and non-operated lower extremities (P>0.05). The results suggested that gaits of patients showed good gait symmetry at 5-10 years after surgery. However, they do not reach the level that observed in healthy subjects, and walking ability was poorer than normal persons. Systematic rehabilitation training is needed to improve walking ability.
10.The effect of selectively upward placement of acetabular implants on limb length and post-operative function of develop-mental dysplasia of the hip patients with shortened legs

Objective To investigate the effect of selectively upward placement of acetabular implants on limb length and post?operative function of developmental dysplasia of the hip patients with shortened legs during total hip arthroplasty (THA). Methods Twenty?six cases of developmental dysplasia of the hip received THA between January 2008 and December 2013, in?cluding 12 cases of Crowe typeⅠ, 8 of Crowe typeⅡ, 6 of Crowe typeⅢ. There were 5 males and 21 females with an average age of 62.7 years (range, 36-80 years). The left hip was involved in 9 cases and the right hip in 17 cases. The preoperative mean Har?ris score was 42.30±12.84, and the preoperative mean WOMAC score was 59.08±13.84 at the last follow?up. The anteroposterior X?ray films and CT scan of the pelvis, anteroposterior and lateral X?ray films of the femur, and TraumaCad analysis were conducted routinely preoperation. More than 70%of the bone?implant interface was covered by appropriate upward distance of acetabular im?plant. Results The follow?up time ranged from 6 to 73 months (mean, 36 months). The Harris score improved to 91.18±7.09, and WOMAC score reduced to 9.85±3.75. According to postoperative measurement, affected limb had been lengthened by 0-5 mm in 8 cases, 6-10 mm in 5 cases, 11-15 mm in 5 cases,>15 mm in 7 cases, and shortening increased 1 mm in 1 case, but the average lengthening was 9.23±7.54 mm. The upward distance of acetabular implant was 0-5 mm in 10 cases, 6-10 mm in 7 cases and>10 mm in 9 cases. The average lengthening was 6.60±6.72 mm in patients having 0-5 mm upward distance, 11.90±5.64 mm in patients having 6-10 mm upward distance and 10.11 ± 9.35 mm in patients having>15 mm upward distance, showing no significant differ?ence. The leg length discrepancy was-3.70±6.43 mm in patients having 0-5 mm upward distance, 1.71±6.24 mm in patients having 6-10 mm upward distance and 0.56 ± 7.70 mm in patients having>15 mm upward distance, showing no significant difference. Con?clusion The limb length could be improved by selectively upward placement of acetabular implants in developmental dysplasia of the hip patients with anatomically abnormal acetabulum during THA, with reasonable preoperative design and corrective operation.
