1.Status quo and identification efficiency of metabolic-associated fatty liver disease in patients with chronic hepatitis C

Objective To analyze the status quo and identification efficiency of metabolic-associated fatty liver disease (MAFLD) in patients with chronic hepatitis C (CHC). Methods A total of 407 patients with CHC who were admitted to the hospital between January 2022 and January 2025 were collected as study subjects. According to whether MAFLD was newly diagnosed during the study period, all enrolled patients were classified into two groups, including MAFLD group (CHC with MAFLD, n=143) and CHC group (CHC, n=264). The general data [gender, age, place of residence, body mass index (BMI), systolic blood pressure, diastolic blood pressure, course of chronic hepatitis C (CHC), history of diabetes mellitus], liver function indicators [alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), γ-glutamyl transferase (GGT)], glucose metabolism indicators [fasting blood glucose (FBG), fasting insulin (FINS), hemoglobin A1c (HbA1c), homeostasis model assessment of insulin resistance (HOMA-IR)], lipid metabolism indicators [total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C) and low-density lipoprotein cholesterol (LDL-C)], serum specific protein markers {serum 25-hydroxyvitamin D [25(OH)D], adipocyte fatty acid binding protein (A-FABP), retinol binding protein 4 (RBP4), sterol regulatory element binding protein-1c (SREBP-1c)}were compared between both groups. Binary logistic regression analysis was performed for multivariate regression analysis. ROC curve was utilized to evaluate the identification value. Results The levels of ALT, AST, ALP, GGT, FBG, FINS, HbA1c, TC, TG, LDL-C, A-FABP, RBP4 and SREBP-1c in the MAFLD group were higher than those in the CHC group (P<0.05) while the levels of HDL-C and 25(OH)D were lower compared to the CHC group (P<0.05). Multivariate logistic regression analysis results suggested that TG, 25(OH)D, RBP4, HbA1c and HOMA-IR were independent related factors influencing MAFLD occurrence (P<0.05). ROC curve analysis results revealed that the identification efficiency of combined prediction (AUC=0.854, 95% CI : 0.812-0.887)was better than that of TG (AUC=0.653, 95% CI: 0.607-0.720), 25(OH)D (AUC=0.685, 95% CI: 0.607-0.715), RBP4 (AUC=0.649, 95%CI: 0.633-0.741), HbA1c (AUC=0.718, 95% CI: 0.657-0.755) or HOMA-IR (AUC=0.726, 95% CI: 0.654-0.757), and combined prediction had the best identification efficiency (P<0.05). Conclusion CHC patients with MAFLD have significant metabolic disorders and changes in specific protein markers. TG, 25(OH)D, RBP4, HbA1c and HOMA-IR are independent related factors, and combined prediction model of multiple indicators exhibits good identification efficiency.

Objective:To summarize the patient profiles and therapeutic efficacies of ABO-incompatible living-related kidney transplantations at 19 domestic transplant centers and provide rationales for clinical application of ABOi-KT.Methods:Clinical cases of ABO-incompatible/compatible kidney transplantation (ABOi-KT/ABOc-KT) from December 2006 to December 2009 were collected. Then, statistical analyses were conducted from the aspects of tissue matching, perioperative managements, complications and survival rates of renal allograft or recipients.Results:Clinical data of 342 ABOi-KT and 779 ABOc-KT indicated that (1) no inter-group differences existed in age, body mass index (BMI), donor-recipient relationship or waiting time of pre-operative dialysis; (2) ABO blood type: blood type O recipients had the longest waiting list and transplantations from blood type A to blood type O accounted for the largest proportion; (3) HLA matching: no statistical significance existed in mismatch rate or positive rate of PRA I/II between two types of surgery; (4) CD20 should be properly used on the basis of different phrases; (5) hemorrhage was a common complication during an early postoperative period and microthrombosis appeared later; (6) no difference existed in postoperative incidence of complications or survival rate of renal allograft and recipients at 1/3/5/10 years between ABOi-KT and ABOc-KT. The acute rejection rate and serum creatinine levels of ABOi-KT recipients were comparable to those of ABOc-KT recipients within 1 year.Conclusions:ABOi-KT is both safe and effective so that it may be applied at all transplant centers as needed.
3.The clinic significance of intracranial pressure monitor for operation in treatment of hypertensive cerebral hemorrhage

Objective To investigate the clinic significance of intracranial pressure (ICP)monitor for operation of hy-pertensive cerebral hemorrhage. Methods Fifty-eight patients suffering from hypertensive cerebral hemorrhage from Jan 2011 to Nov 2012 were included,and were divided into two groups,ICP-monitor group(A group) and non-ICP-moni-tor group(B group). The clinic data of all patients were statistically analyzed and the prognosis was evaluated with six-month follow up period after operation. Results Compared with patients who had ICP monitor post operation, the inci-dence of distance of water and electrolyte reduced significantly in A group; Moreover,the scores of GOS were signifi-cant higher in A group. Nevertheless,there was no significant difference on operative mortality between two groups af-ter operation 14 days. Conclusion The study demonstrates that ICP monitor can guide the clinical therapy,decrease the incidence of complication and improve the prognosis for patients suffered from operation of hypertensive cerebral hem-orrhage.
4.Novel method for dynamic monitoring and early-warning on wild resources of traditional Chinese medicines based on price ratio between drug and foodstuff.

OBJECTIVETo probe into the new idea along with establishment of a novel method for dynamic monitoring and early-warning on the wild resources of traditional Chinese medicines (TCMs).
METHODThe alterations of wild traditional Chinese medicinal resources were assessed through the price ratio between drug and foodstuff (PRDF) indicating the balance between supply and demand of the specific TCMs, referred to the price ration between pork to foodstuff which is used in national monitoring to the balance between pork supply and demand.
RESULTSince the price of rice was tightly controlled by government, it was selected as a relatively stable reference to build the PRDF in order to take away the non-marketing influence to TCMs price such as CPI and inflation rate. The modified relative alteration trend of TCMs price had been researched through comparing different formulae to build PRDF, including absolute average month price of TCMs, month average price ratio of TCMs to foodstuff (rice) , month-on-month change of TCMs to rice, year-on-year change of TCMs to rice, and difference in value of period-on-period change (DVPPC). In the research, Cordyceps, Glycyrrhiza and totally five herbs were selected as model drugs and the price data were collected from 2002 to 2008. The results showed that DVPPC calculated of relative long time window was more sensitive and stable to reflect the relative alteration trend of TCMs price. For instance, the DVPPC of Ligustici showed continuously increase trend in recent years. This suggested appearance of unbalance between supply and demand of Ligustici, and forced policy intervention to maintain reasonable and continuable utilization of Ligustici resource.
CONCLUSIONThe proposed method and the formula of DVPPC revealed some useful guidance for dynamic monitoring the wild resources of TCMs.

METHODUltrasonic extraction, HPLC analysis.
RESULTThe content of magnoflorine of Epimedium leaves range between 0.0029% and 1.688%.
CONCLUSIONThe content of magnoflorine of Epimedium show large differences between species but relatively stable within the species, E. koreanum Nakai is the highest one and E. brevicornu is the lowest.

6.Development trend of traditional Chinese medicine resources.

The paper analyzed the current status and existing problems of traditional Chinese medicine resources. Blind introduction phenomenon, slow progress of variety breeding, low utilization rate of medicinal parts and herb residue seriously affected the sustainable development of traditional Chinese medicine resources. This paper raised suggestions according to these problems: to develop directional cultivation based on genuine drug, to develop directive breeding based on improvement the content of effective components, to expanding use the medicinal parts based on diversification and to comprehensive use the herb residue.

7.Determination of saponins in Dipsaci Radix by HILIC-HPLC.

OBJECTIVETo develop an HILIC-HPLC method for simultaneous analysis of three main active triterpenoid saponins including dipsacus asponin VI, dipsacus asponin X and dipsacus asponin XII. To evaluate the medical material from various habitats in China, different commercial grades or parts of plant.
METHODThe HPLC was conducted on Venusil HILIC column (4.6 mm x 250 mm, 5 microm) with acetonitrile and water as the mobile phase at 25 degrees C, which was in gradient elution. The flow rate was 1 mL x min(-1); and the detection wavelength was 203 nm.
RESULTThe contents of dipsacus asponin VI, X and XII were 0.77%-14.31%, 0.39%-3.19% and 0.41%-1.49% respectively in different provinces of China, that were related to locations. The contents of saponins from Yunnan and Sichuan provinces were higer than those of Hubei and Guizhou. Thick roots, which were high-class products, contained less saponins than thin ones. In addition, the contents of stem, residual parts of stem and fibrous roots were fewer than main roots.
CONCLUSIONThis method is simple, sensitive and accurate. It could be used to determine the contents of dipsacus asponin VI, X, XII and evaluate the quality of dipsacus asperoides.

8.Chemical constituents of Iris dichotoma.

OBJECTIVETo study the chemical constituents in the rhizoma of Iris dichotoma.
METHODThe chemical constituents were isolated by various column chromatographic methods. The structures of the compounds were elucidated on the basis of physiochemical properties and spectral analysis.
RESULTEleven compounds, hispidulin (1), rhamnocitrin (2), iristectorigenin A (3), 4', 5, 7, 8-tetrahydroxy-6-methoxy isoflavone (4), 6-hydroxybiochanin A (5), iristectorin B (6), iristectorigenin A (7), kaempferol-7-methyl ether (8), tamarixetin-7-glucoside (9), iristectorin A (10), 3', 3, 5-trihydroxy-4', 7-dimethoxy-flavone-3-O-beta-D-galactopyranoside (11) were isolated and identified.
CONCLUSIONCompounds 1-11 were obtained from this plant for the first time.

9.Study on antioxidant effect of Herba Lycopi and screening of active fractions.

OBJECTIVETo Study the antioxidant activity of Herba Lycopi, and screen the main active fractions.
METHODThe different polar fractions were isolated from the ethanol extract of Herba Lycopi. The scavenging activity to DPPH and superoxide anions, reducing effect on Fe3+, and inhibiting activity of lipid peroxidation of the fractions were studied in vitro by high-throughput screening technique.
RESULTThe ethanol extract, ethyl acetate fraction (LLE-A. E, MCCM2921) and n-buthanol fraction (LLE-A. B, MCCM2922) of Herba Lycopi showed the scavenging activity to DPPH and superoxide anions, reducing effect on Fe3+, and inhibiting activity of lipid peroxidation.
CONCLUSIONThe ethanol extract and different polar fractions showed antioxidant capacity in vitro. That may be explain that the effect of activating blood and dissolving stasis and anti-fibrosis of Herba Lycopi in traditional application. The results may be helpful for further development of Herba Lycopi as a low-toxic antioxidant drug.
