1.Ulinastatin reduces ischemia-reperfusion injury mediated by NLRP3 in skeletal muscle

Objective To investigate the role of nuclear factor-κB(NF-κB)/NOD-like receptors family pyrin domain containing 3(NLRP3)pathway in limb ischemia-reperfusion injury(IRI)pretreated with ulinastatin(UTI).Methods Twenty-one SD rats were randomly divided into Sham group,IRI group,UTI group.In UTI group,ulinastatin was injected after anesthesia.After 10 minutes,the animal model was established by claming the femoral artery and ligaturing collateral circulation.After 3h of ischemia,the clamp and tourniquet were removed and the rats underwent 2h of reperfusion.In the other groups,the homologous saline was also injected in the same time.Plasma concentrations of lactate dehydrogenase(LDH)and creatine kinase(CK),interleukin(IL)-6,IL-18,tumor necrosis factor-α(TNF-α)were measured.The gastrocnemius muscle was harvested and immediately stored at-80℃.NF-κB and NLRP3 were detected by Western blot and PCR.The other section muscle was stored in triformol for HE staining.The wet/dry was also immediately detecting.Results The level of wet/dry,LDH,CK,IL-6,IL-18,TNF-α,NF-κB,NLRP3 IRI were higher than those in Sham group(P<0.05).The level of wet/dry,LDH,CK,IL-6,IL-18,TNF-α,NF-κB,NLRP3 IRI were significantly lower than those in IRI group(P<0.05).Conclusion Ulinastatin can reduce the cellular inflammatory response through the NF-κB/NLRP3 pathway,thereby achieving a protective effect on limb IRI in rats.

Objective:To investigate the clinical value of endometrial receptivity test(ERT)combined with per-sonalized embryo transfer in improving pregnancy outcomes in patients with repeated implantation failures.Meth-ods:The clinical data of patients diagnosed with repeated implantation failure at the Reproductive Medicine Cen-ter,The Sixth Affiliated Hospital of Sun Yat-Sen University from March 2019 to August 2022 were retrospectively analyzed,among which 428 patients who underwent endometrial receptivity test were included in the ERT group.396 patients who did not undergo ERT were selected by propensity score matching of SPSS 26.0 software as the control group.We compared the clinical characteristic and pregnancy outcomes of two groups and ana-lyzed the pregnancy outcomes of individualized embryo transfer in patients with normal,delayed by 1 day and de-layed by 2 days of endometrial window of implantation(WOI)after personalized embryo implantation of shifting WOI patients.Results:The clinical pregnancy rate(47.73%vs.12.79%)and the live birth rate(38.97%vs.9.76%)of ERT group were significantly higher than those in the control group(P<0.05).Moreover,endome-triosis and polycystic ovary syndrome(PCOS)accounted for a higher proportion of infertility factors in the ERT group compared to the control group(P<0.05).Among 428 patients who underwent ERT,there were 316 pa-tients with normal WOI(73.83%),112 patients with WOI displaced(26.17%).There was no statistical difference in the pregnancy outcome among the three groups of normal WOI delayed by 1 day and WOI delayed by 2 days(P>0.05).Conclusions:ERT technology combined with personalized embryo transfer protocol can improve the fertility outcomes of RIF patients,especially for those with WOI displacement.ERT technology has application prospect and clinical application value.

10.Risk analysis of perioperative outcomes of lung transplantation and the prediction of delayed extubation

Objective:To explore the risk factors of perioperative outcomes of lung transplantation and establish a predictive model for delayed extubation after lung transplantation.Methods:From January 1, 2020 to December 31, 2022, 104 lung transplantation recipients were retrospectively collected to identify the risk factors of early post-operative outcome.According to the timing of extubation post-lung transplantation, they were assigned into two groups of normal(77 cases)and delayed(27 cases). Baseline profiles, type of primary diagnosis, cold ischemic duration and lung transplantation approach were compared between two groups.The factors with significant difference were examined by univariate and multivariate Logistic regression.Furthermore, multivariate logistic model was visualized by a nomogram.Receiver operating characteristic(ROC)curve and decision curve analysis(DCA) were performed for evaluating the model's predictive performance and its value for clinical utilization.Results:The postoperative mortality rate was 9.6%.Delayed extubation was a strong predictor for postoperative mortality.Cold ischemic time outperformed others variates in terms of delayed extubation prediction.AUC of cold ischemic time and multivariate logistic model was 0.75(95% CI: 0.69-0.81)and 0.87(95% CI: 0.82-0.91). Conclusions:Delayed postoperative extubation is a key predictor of early post-lung transplantation mortality.The established predictive model may effectively identify high-risk patients for preventive intervention and survival improvement post-lung transplantation.
