1.Effect of Runmu Dihuang Decoction on Perimenopausal Dry Eye in Rats with Liver-kidney Yin Deficiency Syndrome Based on SIRT3/HIF-1α/NF-κB Signaling Pathway

ObjectiveTo investigate the mechanisms of Runmu Dihuang decoction (RMDHD) in treating perimenopausal dry eye with liver-kidney Yin deficiency syndrome based on the silent information regulator 3 (SIRT3)/hypoxia-inducible factor-1α (HIF-1α)/nuclear factor-κB (NF-κB) signaling pathway. MethodsSixty female Sprague-Dawley rats were randomly divided into six groups (n=10 per group): Sham operation group, model group, sodium hyaluronate eye drop group, and low-, medium-, and high-dose RMDHD groups (5.625, 11.25, 22.50 g·kg-1). Except for the sham operation group, all rats underwent bilateral ovariectomy and were administered 0.1% benzalkonium chloride eye drops combined with long-term chronic irritation to establish a perimenopausal dry eye model with liver-kidney Yin deficiency syndrome. Drug administration began in the 11th week after modeling and continued for 21 days. General conditions, screen-grip test scores, tear secretion volume, tear film breakup time (TFBUT), and corneal fluorescein staining were recorded. Serum levels of reactive oxygen species (ROS), follicle-stimulating hormone (FSH), estradiol (E2), and progesterone (PROG) were measured by enzyme-linked immunosorbent assay (ELISA). Pathological changes in the lacrimal glands, corneas, and uteri were observed using hematoxylin-eosin (HE) staining. Protein expression levels of SIRT3, HIF-1α, phosphorylated NF-κB p65 (p-NF-κB p65), and total NF-κB p65 in the lacrimal glands were detected by Western blot. The expression of inflammatory cytokines interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) in the lacrimal glands was assessed by immunohistochemistry (IHC). ResultsAfter model establishment, no significant differences were observed among the groups except the sham operation group. Compared with the sham operation group, the other groups exhibited slowed movement, dull responses, increased irritability, reduced body weight, elevated rectal temperature, decreased screen-grip test scores, reduced tear secretion, and significantly shortened TFBUT (P<0.05). After treatment, compared with the model group, the sodium hyaluronate eye drop group and all RMDHD groups showed improved general conditions, significantly increased tear secretion (P<0.05), prolonged TFBUT (P<0.05), and elevated screen-grip test scores (P<0.05). Serum ROS and FSH levels were significantly decreased, while E2 and PROG levels were significantly increased (P<0.05). Pathological damage to the cornea, lacrimal glands, and uterus was ameliorated. In addition, protein expression levels of SIRT3 and HIF-1α in the lacrimal glands were significantly upregulated (P<0.05), whereas the expression of p-NF-κB p65, IL-1β, and TNF-α was significantly downregulated (P<0.05). ConclusionRMDHD increases tear secretion and TFBUT, improves lacrimal gland and corneal injury, and alleviates dry eye symptoms in a perimenopausal dry eye rat model with liver-kidney Yin deficiency syndrome. The underlying mechanism may be related to regulation of the SIRT3/HIF-1α/NF-κB signaling pathway, inhibition of oxidative stress and inflammatory responses, and reduction of ocular surface tissue damage.
2.Analysis of related factors for the comorbidity of allergic rhinitis and obesity among primary and secondary school students in Inner Mongolia

Objective:
To investigate the factors influencing the co-prevalence of allergic rhinitis and obesity among primary and secondary school students in Inner Mongolia, so as to provide a data foundation and theoretical basis for developing targeted intervention measures.
Methods:
In September and October 2024, a stratified cluster random sampling method was employed to select 139 102 students from 539 schools across 12 leagues/cities and 103 banners/counties in Inner Mongolia Autonomous Region. Participants who were diagnosed with allergic rhinitis by a doctor at least once within one year and had a body mass index ≥ 28 kg/m 2 were considered to have comorbid conditions.
Results:
The coprevalence rate of allergic rhinitis and obesity among primary and secondary school students in Inner Mongolia was 6.4% (8 931 cases). Lasso-Logistic regression revealed that nonboarding status, higher maternal education, consuming high protein foods ≥1 time daily, occasionally or never eating breakfast, engaging in moderate to vigorous physical activity for ≥60 minutes on fewer than half of holidays, and having been exposed to second hand smoke in person within the past seven days were associated with higher odds ratios for co-prevalence of allergic rhinitis and obesity( OR = 1.23 , 1.22-1.63, 1.20, 1.19, 1.38, 1.35); being female, higher grade level, residence in flag/county/district areas, non only child status, never having consumed a full glass of alcohol, non hypertensive status, and households without pets were associated with lower co-prevalence risks ( OR =0.65, 0.67-0.77, 0.81, 0.87, 0.73, 0.41, 0.68) (all P <0.05). The ROC curve indicated an area under the curve of 0.64 for the predictive model, demonstrating satisfactory discriminatory ability. The calibration curve showed consistency between predicted and actual occurrence probabilities.
Conclusions
The co-prevalence of allergic rhinitis and obesity among primary and secondary school students in Inner Mongolia is closely associated with demographic characteristics, dietary behaviours, and lifestyle habits. Future prevention and control strategies should prioritize these factors to implement targeted interventions.
3.Surgical treatment of hepatic alveolar echinococcosis: challenges and innovations

Hepatic alveolar echinococcosis is a highly invasive zoonotic parasitic disease with poor prognosis. Surgical intervention serves as the pivotal approach to achieve radical cure and improve the prognosis of hepatic alveolar echinococcosis patients. In recent years, with the popularization of the concept of precision surgery and the development of the multidisciplinary diagnosis and treatment model, the surgical treatment strategies for hepatic alveolar echinococcosis have been continuously enriched, and the selection of surgical procedures has become increasingly diversified. Although key surgical techniques such as radical hepatectomy, autologous liver transplantation and allogeneic liver transplantation have achieved remarkable progress in clinical application, many insurmountable challenges still remain. Therefore, by sorting out the latest evidence-based advances in the field of surgical treatment for hepatic alveolar echinococcosis, this article focuses on discussing the application status and bottlenecks of radical hepatectomy, autologous liver transplantation and allogeneic liver transplantation in hepatic alveolar echinococcosis, aiming to provide a reference for the clinical treatment of hepatic alveolar echinococcosis.
4.Traditional Chinese medicine improves synaptic plasticity in Alzheimer's disease: A review of experimental studies

Abnormal synaptic plasticity is an early pathological feature of Alzheimer disease (AD). Synaptic damage and dysfunction initiate neuronal degeneration and death, ultimately leading to cognitive impairment. Traditional Chinese medicine (TCM) can effectively ameliorate cognitive dysfunction through multitarget regulation of synaptic plasticity. This review summarizes the mechanisms by which TCM, including active components, single herbs, and classical formulas, modulates synaptic plasticity, offering new insights for future research and clinical applications. Relevant experimental studies published between 2020 and 2024 were retrieved from major databases, including China National Knowledge Infrastructure, the National Science and Technology Library, Wanfang Data, Elsevier, ScienceDirect, PubMed, SpringerLink, and Web of Science. Network pharmacology and bioinformatics approaches were used to predict the therapeutic effects and mechanisms of TCM on AD-related synaptic plasticity. In total, 15 TCM single herbs and 11 TCM formulas were identified as enhancing AD-related synaptic plasticity. Additionally, 15 active ingredients targeting synaptic plasticity in AD were retrieved from TCM databases over the past decade. This review provides novel perspectives and strategic directions for future AD research and therapeutic development.
5.Research advances in robot-assisted partial nephrectomy for anterior and posterior lip tumors of the kidney:focusing on postoperative complications

Renal hilar tumors pose significant surgical challenges due to their proximity to major renal vessels and the collecting system. In robot-assisted partial nephrectomy, although both anterior and posterior lip tumors are considered renal hilar tumors, their operative difficulty and complication profiles differ markedly owing to distinct anatomical relationships and vascular adjacency. Anterior lip tumors, often involving tributaries of the renal vein, carry a relatively higher risk of intraoperative and postoperative bleeding. In contrast, posterior lip tumors, closer to the renal artery, are more likely to prolong warm ischemia time, thereby increasing the risk of postoperative renal impairment. Regarding urine leakage, although anterior lip tumors are theoretically closer to the collecting system and may pose a slightly elevated risk, precision reconstruction strategies—such as lip reconstruction based on sinus space anatomy—can effectively mitigate this risk. However, the specific difference in urine leakage risk between anterior and posterior lip tumors warrants further investigation. Both anterior and posterior lip tumors of the kidney can achieve low positive surgical margin rates in experienced centers. Complication risks are affected by multiple factors, including tumor anatomical complexity, warm ischemia time, and surgeon experience. Notably, R.E.N.A.L. score>8 is associated with significantly increased complication risks. Preoperative meticulous imaging evaluation, selection of an appropriate surgical approach based on tumor location, controlled warm ischemia time, and application of delicate suturing and reconstruction techniques are essential strategies to reduce complications.
6.A clinical study on the prognostic value of preoperative prognostic nutritional index in patients with renal cell carcinoma undergoing partial nephrectomy

Objective To investigate the clinical prognostic value of the prognostic nutritional index (PNI) in patients with renal cell carcinoma (RCC) undergoing partial nephrectomy.Methods A retrospective analysis was conducted on the clinical data of RCC patients who underwent partial nephrectomy at our hospital during 2020 and 2024.The optimal cutoff value of PNI was determined based on the maximum Youden index from the receiver operating characteristic (ROC) curve.The clinicopathological characteristics, overall survival (OS), and progression-free survival (PFS) between patients with different PNI levels were compared.Cox regression analysis was used to explore the independent prognostic factors for RCC patients. Results Based on the ROC curve, the optimal cutoff value corresponding to the maximum Youden index was calculated to be 46.675, at which point the sensitivity was 65.3% and the specificity was 62.2%.Of the 815 patients included in the study, 293 (36.0%) patients had a PNI<46.675.Cox regression analysis confirmed that preoperative PNI, tumor diameter, ISUP classification, and ECOG score were independent risk factors for postoperative OS in patients with RCC who underwent PN (P<0.05).In addition, preoperative PNI, tumor diameter, ISUP classification and ECOG score were independent risk factors for postoperative PFS (P<0.05).After covariates in the multivariable Cox regression model were adjusted, patients in the low PNI group demonstrated significantly shorter overall survival (HR=2.392, P<0.001) and significantly poorer progression-free survival (HR=2.076, P<0.001) compared to those in the high PNI group.Conclusion Preoperative PNI has significant clinical value in evaluating the prognosis of RCC patients undergoing partial nephrectomy.An elevated preoperative PNI is associated with a better prognosis.
7.Effects of Total Intravenous Anesthesia and Inhalational Anesthesia on Postoperative Recovery in Patients Undergoing Transsphenoidal Pituitary Surgery:A Systematic Review.

Objective To systematically evaluate the effects of total intravenous anesthesia and inhalational anesthesia on postoperative recovery in patients undergoing transsphenoidal pituitary tumor resection.Methods A comprehensive search was conducted in international biomedical databases including Ovid Medline,Embase,CINAHL(EBSCO),Cochrane Library,and Web of Science,from inception to July 4,2023.Additionally,ClinicalTrials.gov and the World Health Organization International Clinical Trials Registry Platform were searched for ongoing and completed trials.The randomized controlled trials(RCT)comparing total intravenous anesthesia and inhalational anesthesia in patients undergoing transsphenoidal surgery for pituitary tumors were included.The methodological quality of the included studies was evaluated by the Cochrane Collaboration tool.Relevant data were extracted and synthesized for analysis.Results A total of 327 records were identified,of which eight RCTs met the inclusion criteria.Four studies showed that the patients receiving desflurane or sevoflurane anesthesia experienced faster emergence from anesthesia than those receiving propofol.Two studies indicated that patients in the propofol group had lower levels of emergence agitation and a lower incidence of early postoperative nausea and vomiting.The results on postoperative cognitive function were inconsistent across studies.No differences were found between the groups in terms of postoperative complications or overall recovery quality during hospitalization.Conclusions Inhalational anesthesia appears to provide an advantage in promoting faster emergence following transsphenoidal pituitary surgery,whereas total intravenous anesthesia may contribute to smoother and more stable recovery.Further high-quality studies are needed to clarify the effects of different anesthetic techniques on both short- and long-term postoperative recovery.

8.Relationship between serum Arg-1,ADAMTS-9 and Ectodysplasin A levels and cardiac function after PCI in patients with acute myocardial infarction and their predictive value for MACE

Objective To investigate the relationship between serum arginase-1(Arg-1),thrombomodulin motif 9(ADAMTS-9),Ectodysplasin A and cardiac function after percutaneous coronary intervention(PCI)in patients with acute myocardial infarction(AMI),and their predictive value for major adverse cardiovascular events(MACE).Methods A total of 102 AMI patients who underwent PCI in the Third Medical Center of People's Liberation Army General Hospital from January 2021 to December 2023 were selected as the AMI group,and 110 healthy people were recruited as the control group.The serum levels of Arg-1,ADAMTS-9 and Ectodysplasin A were detected by enzyme-linked immunosorbent assay.Echocardiography was performed by color Doppler ultrasound,and left ventricular mass index(LVMI)was calculated.Patients were divided into MACE group(n=32)and non-MACE group(n=70)according to the occurrence of MACE.Pearson correla-tion analysis was used to analyze the correlation between serum Arg-1,ADAMTS-9,Ectodysplasin A levels and LVMI after PCI in AMI patients.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum Arg-1,ADAMTS-9 and Ectodysplasin A levels for MACE in AMI patients after PCI.Z test was used to compare the area under the curve(AUC).Multivariate Logistic regression analysis was used to analyze the influencing factors of MACE in patients with AMI.Results Compared with the con-trol group,the AMI group had significantly higher serum levels of Arg-1,ADAMTS-9 and Ectodysplasin A(P<0.05).The serum levels of Arg-1,ADAMTS-9 and Ectodysplasin A in patients with AMI were positively correlated with LVMI(P<0.05).There were no significant differences in gender,body mass index,systolic blood pressure,heart rate,history of diabetes,diastolic blood pressure,smoking history,time from onset to treatment,drinking history,age,course of disease,and LVMI between MACE group and non-MACE group(P>0.05).The serum levels of Arg-1,ADAMTS-9 and Ectodysplasin A in the MACE group were higher than those in the non-MACE group(P<0.05).Arg-1,ADAMTS-9 and Ectodysplasin A were independent risk factors for MACE in AMI patients(P<0.05).The AUC of serum Arg-1,ADAMTS-9 and Ectodysplasin A levels in predicting MACE in AMI patients were 0.821,0.779,0.818 and 0.950,respectively.The combined prediction value of the three was higher than that of individual prediction(Z=3.137,3.702,2.699,P=0.002,<0.001,0.007).Conclusion The serum levels of Arg-1,ADAMTS-9 and Ectodysplasin A are increased in AMI patients,and they have certain predictive value for the occurrence of MACE in AMI patients after PCI.
9.Attention impairment in patients with temporal lobe epilepsy

Attention impairment in temporal lobe epilepsy (TLE) leads to other advanced cognitive impairments, such as memory and emotional recognition. Moreover, the severity of attention impairment determine the differences in strategies for preventing sudden unexpected death in epilepsy (SUDEP). Factors such as abnormal brain electrical discharges, sleep, and antiepileptic drugs are all associated with attention impairment. Neuroelectrophysiological observations have shown that abnormal electrical discharges interfere with the synchronization of cortical oscillations, and preventing the spread of abnormal epileptic discharges to subcortical activation structures helps reduce attention disorders. Neuroimaging reveals damage to the gray matter and white matter of the brain related to attention and poor connectivity of the whole-brain network. Therefore, early identification of attention impairment in TLE patients has become even more important. Treatment of attention impairment includes supplementing attention-related neurotransmitters, cognitive-behavioral therapy, surgical procedures, and neuromodulation techniques. This article mainly reviews the characteristics of attention impairment in TLE patients from the perspectives of clinical behavior, pathophysiology, and treatment, aiming to provide new insights for the early identification and management of attention impairment in TLE patients.
10.Comparison of efficacy of different biological scaffolds for pulp regeneration therapy in immature permanent teeth:a Bayesian network meta-analysis

OBJECTIVE:There are many kinds of biological scaffolds used for pulp revascularization in clinical practice,and the difference of efficacy between different scaffolds is controversial.The efficacy of nine kinds of biological scaffolds in endodontic revascularization was evaluated by Bayesian network meta-analysis.METHODS:The computer was used to search the literature in CNKI,VIP,WanFang,China Biomedical Literature Service System,PubMed,Cochrane Library,Web of Science,Embase,and Scopus databases.Randomized controlled trials of different biological scaffolds for the treatment of pulp revascularization in young permanent teeth meeting inclusion criteria were collected from each database up to April 1,2024.Two researchers sifted through the literature,data collection,sorting and extraction were completed independently,and the quality of the included literature was assessed for risk of bias.A network meta-analysis was performed using BUGSnet1.1.1 package of R4.2.0 software.RESULTS:A total of 22 studies with 926 affected teeth and 9 different interventions were included in this study.The results of network meta-analysis showed that:(1)Clinical success rate(primary goal):platelet-rich fibrin was superior to blood clot[OR=1.45,95%CI(0.32,2.69)],and the top three ranking results were:concentrated growth factor(82.77%)>platelet-rich fibrin(75.38%)>modified platelet-rich fibrin(62.39%).(2)Increased root length(secondary goal):There was no difference among the 7 biological scaffolds at 1-6 months of follow-up(P>0.05),the top 3 ranking results of rank probability were:concentrated growth factor(86.25%)>platelet-rich plasma(53.76%)>platelet-rich fibrin(51.11%).When followed up for>6 months and<12 months,concentrated growth factor was superior to blood clot[MD=9.59,95%CI(0.52,18.40)],the top 3 ranking results of rank probability were:concentrated growth factor(92.42%)>platelet-rich plasma(56.03%)>platelet-rich fibrin(55.76%).When followed up for more than 12 months,concentrated growth factor was superior to modified platelet-rich fibrin[MD=11.01,95%CI(0.02,22.72)],the top 3 ranking results of rank probability were:concentrated growth factor(86.95%)>platelet-rich fibrin(68.61%)>blood clot combined with collagen(52.5%).(3)Increased root wall thickness(secondary goal):at 1-6 months of follow-up,platelet-rich fibrin was superior to blood clot[MD=11.37,95%CI(4.74,17.71)],the top 3 ranking results of rank probability were:platelet-rich fibrin(93.66%)>concentrated growth factor(63.11%)>modified platelet-rich fibrin(50.48%).At>6 months and<12 months of follow-up,there was no difference among the 6 biological scaffolds(P>0.05),the top 3 ranking results of rank probability were:modified platelet-rich fibrin(73.63%)>platelet-rich fibrin(62.36%)>concentrated growth factor(56.25%).When followed up for more than 12 months,there was no difference among the 9 biological scaffolds(P>0.05),and the top 3 ranking results of rank probability were:blood clot combined with collagen(81.9%)>platelet-rich plasma(62.67%)>modified platelet-rich fibrin(59.49%).(4)Pulp vitality(third-level goal):there was no difference among the 6 biological scaffolds(P>0.05),and the top 3 ranking results of rank probability were:blood clot combined with collagen(84.22%)>concentrated growth factor(67.71%)>platelet-rich fibrin(48.79%).CONCLUSION:Existing evidence shows that the clinical success rate of different scaffolds is higher in pulp revascularization,among which platelet-rich fibrin is better than blood clots.In terms of comprehensive comparison of root length and root wall thickness increase,concentrated growth factor performs best in the follow-up period of 1-6 months and>6 months and<12 months,while blood clot combined with collagen performs best after follow-up of more than 12 months;concentrated growth factor performs outstandingly in all levels of goals,and may be more conducive to the continued development of the tooth root than other scaffolds,and has great potential in pulp regeneration treatment.Limited by the quality and quantity of literature,the above conclusions still need to be verified by more high-quality studies.
