1.Analysis of Genes Related to Platelet Activation in Essential Thrombocythemia Based on Transcriptomics

Objective:To analyze the genes related to platelet activation in essential thrombocythemia (ET)based on transcriptome sequencing technology (RNA-seq ),and to explore the potential targets related to ET thrombosis. Methods:Blood samples from ET patients and healthy individuals were collected for RNA-seq,and differentially expressed lncRNAs,miRNAs,and mRNAs were selected to construct a lncRNA-miRNA-mRNA regulatory network. Differential mRNAs in the regulatory network were enriched and analyzed using Gene Ontology (GO ) and Kyoto Encyclopedia of Genes and Genomes (KEGG).The real-time PCR method was applied to validate differential mRNAs on crucial signaling pathways.Results:A total of 32 lncRNAs (3 up-regulated,29 down-regulated),16 miRNAs (8 up-regulated,8 down-regulated),and 35 mRNAs (27 up-regulated,8 down-regulated)were identified as differentially expressed.Among them,5 lncRNAs,12 miRNAs,and 19 mRNAs constituted the regulatory network.KEGG enrichment analysis showed that the differential mRNAs were related to the platelet activation signaling pathway,and there were 6 differential mRNAs related to platelet activation,namely F2R,ITGA2B,ITGB1,ITGB3,PTGS1,and GP1 BB,which were all up-regulated in their expression.RT-PCR results showed that the expression of five mRNAs including F2R,ITGA2B,ITGB1,ITGB3,and GP1BB were upregulated in ET patients compared with healthy subjects,and consistent with RNA-seq results,while PTGS1 expression was not significantly different.Conclusion:Differential mRNAs in ET patients are related to the platelet activation pathway,and F2R,ITGA2B,ITGB1,ITGB3,and GP1BB mRNAs may serve as novel targets associated with platelet activation in ET.
3.Surveillance of cockroach population and evaluation of control effects in households from 2017 to 2019 in Jiading District of Shanghai

Objective To learn the population and infestation rates of cockroaches from 2017 to 2019 in Jiading District of Shanghai, to evaluate the effect of cockroach termination in household, and to provide information for cockroach control. Methods Cockroaches were controlled by dinotefuran baits and clean-up in households.Sticky trap and visual method were employed for density monitoring in farmers markets, supermarkets, hotels, restaurants, hospitals, and residential areas.Visual method was used in households before and after using the insecticide. Results Sticky trap result showed the room infestation rate was 3.24%, mean adhesion rate was 3.29%, the density was 0.06 per board, and the density peak appeared in May.Rate of invasion and density decreased year by year.
4.Meta analysis of effect of tracheostomy timing on prognosis of patients with cervical spinal cord injury

Objective To evaluate the effects of early tracheostomy and late tracheostomy on the prognosis of patients with cervical spinal cord injury,so as to provide evidence based guidance for the timing of tracheostomy.Methods Relevant literatures studying the timing of tracheostomy in patients with cervical spinal cord injury were searched in PubMed,Embase,Medline,Cochrane Library,Chinese Biological Medical Literature database (CBM),China National Knowledge Infrastructure database (CNKI),and VIP journal database with time range from journal establishment to March 2018.The retrieved articles were screened according to the inclusion and exclusion criteria.The article quality was rigorously evaluated according to the Newcastle-Ottawa scale (NOS).Meta analysis was conducted using Review Manager 5.3 software to compare the mechanical ventilation time,ICU stay,incidence of pneumonia,incidence of complications,and mortality between early and late tracheostomy in patients with cervical spinal cord injury.Results A total of eight articles of cohort study including 466 patients were included,with 241 patients in the early tracheostomy group and 225 patients in the late tracheostomy group.The eight articles were all determined as high quality studies according NOS.The results of Meta analysis showed that there were significant differences between the two groups in terms of the total mechanical ventilation time (MD =-12.28,95% CI-20.09--4.47,P < 0.01),post tracheostomy mechanical ventilation time (MD =-9.92,95% CI-14.27--5.57,P < 0.01),total ICU stay (MD =-10.30,95% CI-17.12--3.47,P < 0.01),post tracheostomy ICU stay (MD =-5.79,95% CI -6.53--5.05,P < 0.01),incidence of complications (RR =0.56,95% CI 0.38-0.83,P < 0.01),and mortality (RR =0.34,95% CI 0.15-0.78,P < 0.05).However,no significant differences were detected between the two groups in the incidence of total pneumonia (RR =0.77,95% CI 0.57-1.05,P >0.05) and the incidence of post tracheostomy pneumonia (RR =0.80,95% CI 0.51-1.26,P >0.05).Conclusion Early tracheostomy can shorten the mechanical ventilation time,ICU stay,incidence of complications,and mortality,but it cannot reduce the incidence of pneumonia.
5.Study on Therapeutic Regimen for Threatened Abortion with Low Estrogen

Objective To study the relationship between estradiol (E2) and embryonic development, and to explore the optimal therapeutic regimen for threatened abortion with low estrogen. Methods One hundred and sixty threatened abortion patients 14 days after frozen embryo transfer (FET) with E2 in the range of 100-200 pg/mL were randomized into Chinese medicine group 1, Chinese medicine group 2, Chinese and western medicine group, and control group, 40 cases in each group. Chinese medicine group 1 was treated orally with Shoutai Sijun Decoction, Chinese medicine group 2 was treated orally with Shoutai Sijun Decoction combined with Placenta Hominis Granules, Chinese and western medicine group was treated orally with Shoutai Sijun Decoction combined with Femonstone white tablets, and the control group was treated orally with Femonstone white tablets. Pregnancy rate and the changes in E2 level before and after treatment in the 4 groups were compared. The sensitivity and specificity of E2 level to the prognosis of pregnancy were also investigated. Results (1) The pregnancy rate of Chinese medicine group 1, Chinese medicine group 2, Chinese and western medicine group, and control group was 70.0%, 75.0%, 95.0%, 77.5% respectively, and Chinese and western medicine group had the highest pregnancy rate(P<0.05).(2) E2 level in the pregnant patients of 4 groups after treatment was increased, and the difference was significant compared with that on ET day 14 (P < 0.05 or P <0.01). (3) The results of prognosis of E2 for pregnancy were as follows: sensitivity was 92%, specificity was 80%, and the area of curve was 0.820 when threshold of E2 being 281 on ET day 22. Conclusion The low post-FET E2 level is correlated with disordered pregnancy, and the early monitoring of E2 and prompt intervention should be performed. Shoutai Sijun Decoction combined with Femonstone white tablets is effective on increasing E2 level rapidly and leading to a higher pregnancy rate, and the effect of Shoutai Sijun Decoction combined with Placenta Hominis Granules comes next.
6.Expressions and significance of peripheral galectin-3 and N-terminal B-type brain natriuretic peptide in patients with chronic heart failure

Objective To explore the change of peripheral galectin (Gal)-3 and N-terminal B-type brain natriuretic peptide (NT-proBNP) in patients with chronic heart failure.Methods A total of 89 patients with chronic heart failure (CHF) were enrolled.Peripheral Gal-3,NT-proBNP were detected by enzyme-linked immunosorbent assay (ELISA) or Electrochemical luminescence method.The expression levels of Gal-3,NT-proBNP in patients with different kinds of heart failure and cardiac functional grading were analyzed.Results There were significant differences in expression levels of Gal-3 and NT-proBNP between patients with HFPEF and patients with HFREF.There was significant differences of Gal-3 and NT-proBNP in patients with NYHA heart function class Ⅱ,Ⅲ and Ⅳ.Gal3 was positively correlated with NT-proBNP (r =0.230,P =0.030) and negatively correlated with LVEF (r =-0.533,P =0.000).NT-proBNP was negatively correlated with LVEF (r =-0.372,P =0.000).Conclusion Peripheral Gal-3,NT-proBNP could be considered as two important markers for judging severity of illness in patients with CHF.

9.Continuous Low-dose-rate Irradiation of Iodine-125 Seeds Inhibiting Perineural Invasion in Pancreatic Cancer.

BACKGROUNDPerineural invasion (PNI) is a histopathological characteristic of pancreatic cancer (PanCa). The aim of this study was to observe the treatment effect of continuous low-dose-rate (CLDR) irradiation to PNI and assess the PNI-related pain relief caused by iodine-125 ( 125 I) seed implantation.
METHODSThe in vitro PNI model established by co-culture with dorsal root ganglion (DRG) and cancer cells was interfered under 2 and 4 Gy of 125 I seeds CLDR irradiation. The orthotopic models of PNI were established, and 125 I seeds were implanted in tumor. The PNI-related molecules were analyzed. In 30 patients with panCa, the pain relief was assessed using a visual analog scale (VAS). Pain intensity was measured before and 1 week, 2 weeks, and 1, 3, and 6 months after 125 I seed implantation.
RESULTSThe co-culture of DRG and PanCa cells could promote the growth of PanCa cells and DRG neurites. In co-culture groups, the increased number of DRG neurites and pancreatic cells in radiation group was significantly less. In orthotopic models, the PNI-positive rate in radiation and control group was 3/11 and 7/11; meanwhile, the degrees of PNI between radiation and control groups was significant difference (P < 0.05). At week 2, the mean VAS pain score in patients decreased by 50% and significantly improved than the score at baseline (P < 0.05). The pain scores were lower in all patients, and the pain-relieving effect was retained about 3 months.
CONCLUSIONSThe CLDR irradiation could inhibit PNI of PanCa with the value of further study. The CLDR irradiation could do great favor in preventing local recurrence and alleviating pain.

10.Comprehensive Interventional Therapy with the Combination of Chinese and Western Medicine for Community Senior Patients with Diabetes:A Study of 44 Cases

OBJECTIVE To observe the clinical effects of integration of traditional Chinese medicine and western medicine in treating senile diabetes and explore proper community management for diabetes in our country and improve the therapeutic effect of diabetes treatment.METHODS All cases were randomly divided into 2 groups(control group and experiment group) after intervention.The control group (46 cases) were treated by comprehensive western treatment,while experiment group (44 cases) were given both comprehensive western treatment and Chinese patent.Changes of blood pressure,blood glucose, glycocylated hemoglobin and life quality score were observed in two groups.RESULTS After intervention,levels of fasting blood glucose in two groups both decreased compared to that of pre-intervention(P<0.05).Decreased levels of fasting blood glucose showed statistical difference between them(P<0.01).There was no difference in levels of postprandial blood glucose before and after treatment(P>0.05);both levels of glycocylated hemoglobin in two groups decreased (P<0.05),and the experiment group were much more obvious than that of control group(P<0.05).There was no statistical significance between two groups in differences of systolic and diastolic pressures before and after treatment(P>0.05).When it comes to life quali-ty,vitality(VT),mental health(MH)scores,the experiment group got improvement(P<0.05)and the psychological eval-uation score was higher after treatment (P<0.05).In control group,general health (GH),vitality (VT),mental health (MH),and psychological evaluation got improvement after treatment,which is significant statistically(P<0.05).While the difference between two groups didn't show any statistical significance(P>0.05).CONCLUSION Comprehensive intervention is successful in dealing with chronic diseases like diabetes and integration of traditional Chinese medicine and western medicine is effective in treating senile diabetes.
