1.Diagnostic value of 99mTc-MDP three-phase bone scintigraphy combined with C-reaction protein for periprosthetic joint infection.

OBJECTIVE:
To investigate the diagnostic efficacy of 99mTc-MDP three-phase bone scintigraphy (TPBS) combined with C-reactive protein (CRP) for periprosthetic joint infection (PJI).
METHODS:
The clinical data of 198 patients who underwent revision surgery of artificial joint between January 2017 and January 2024 and received TPBS examination before surgery were retrospectively analyzed. There were 77 males and 121 females with an average age of 63.74 years ranging from 24 to 92 years. There were 90 cases of hip arthroplasty and 108 cases of knee arthroplasty. PJI was diagnosed according to the 2013 American Musculoskeletal Infection Society (MSIS) standard diagnostic criteria. The sensitivity, specificity, accuracy, negative predictive value (NPV), and positive predict value (PPV) were calculated. The receiver operating characteristic (ROC) curve was used to compare the diagnostic performance of the three methods, and the area under curve (AUC) was used to evaluate the diagnostic performance.
RESULTS:
According to the 2013 MSIS criteria, 116 cases were diagnosed as PJI, and the remaining 82 cases were aseptic loosening. The cases of PJI diagnosed by TPBS, CRP, and TPBS-CRP were 125, 109, and 137 respectively, and the cases of aseptic loosening were 73, 89, and 61 respectively. The sensitivity, accuracy, NPV, and PPV of TPBS-CRP combination in the diagnosis of PJI were higher than those of TPBS and CRP, but the specificity was lower than that of TPBS and CRP. ROC curve analysis further showed that the AUC value of TPBS-CRP combination was better than that of TPBS and CRP. The severity of bone defect and the duration of symptoms in patients with false positive TPBS diagnosis were worse than those in patients with true negative TPBS diagnosis (P<0.05), but there was no significant difference in the survival time of prosthesis between the two groups (P>0.05). Among the patients diagnosed with PJI by TPBS, CRP, and TPBS-CRP, 49, 35, and 54 patients had received antibiotic treatment 2 weeks before diagnosis, respectively. There was no significant difference in the diagnostic accuracy of TPBS and TPBS-CRP before diagnosis between patients treated with and without antibiotics and those not treated (P>0.05). The diagnostic accuracy of antibiotic therapy before CRP diagnosis was significantly lower than that of untreated patients (P<0.05).
CONCLUSION
TPBS and CRP have limited specificity in differentiating PJI from aseptic loosening. The TPBS-CRP combination diagnostic method can synergize the local bone metabolic characteristics and systemic inflammatory response to achieve higher diagnostic accuracy, but caution should be exercised in patients with severe bone defects and longer symptom duration.

2.Study design and rationale of the TXL-CAP trial: a randomized, double-blind, placebo-controlled, multicenter clinical trial assessing the effect of Tongxinluo capsules on the stability of coronary atherosclerotic plaques.

Recent clinical trials have demonstrated a protective effect in using traditional Chinese medicine Tongxinluo (TXL) capsule to treat atherosclerosis. However, clinical evidence of the effects of TXL treatment on coronary plaque vulnerability is unavailable. In response, we developed this study to investigate the hypothesis that on the basis of statin therapy, treatment with TXL capsule may stabilize coronary lesions in patients with acute coronary syndrome (ACS). The TXL-CAP study was an investigator-initiated, randomized, double-blind clinical trial conducted across 18 medical centers in China. Patients with ACS aging from 18 to 80 years old who had a non-intervened coronary target lesion with a fibrous cap thickness (FCT) < 100 μm and lipid arc > 90° as defined by optical coherence tomography (OCT) were recruited. A total of 220 patients who met the selection criteria but did not meet the exclusion criteria will be finally recruited and randomized to receive treatment with TXL (n = 110) or placebo (n = 110) for a duration of 12 months. The primary endpoint was the difference in the minimum FCT of the coronary target lesion between TXL and placebo groups at the end of the 12-month follow-up. Secondary endpoints included: (1) changes of the maximum lipid arc and length of the target plaque, and the percentage of lipid, fibrous, and calcified plaques at the end of the 12-month period; (2) the incidence of composite cardiovascular events and coronary revascularization within the 12 months; (3) changes in the grade and scores of the angina pectoris as assessed using the Canadian Cardiovascular Society (CCS) grading system and Seattle angina questionnaire (SAQ) score, respectively; and (4) changes in hs-CRP serum levels. The results of the TXL-CAP trial will provide additional clinical data for revealing whether TXL capsules stabilizes coronary vulnerable plaques in Chinese ACS patients.
3.Effects of Fear of Movement on Lower Limb Muscle Activation in Patients with Knee Osteoarthritis

Objective To analyze muscle activation intensity in patients with knee osteoarthritis(KOA)during physical activity by applying surface electromyography(sEMG),and investigate the impact of fear of movement on lower limb muscle function in patients with KOA.Methods A total of 30 patients with KOA were selected and categorized into a high-fear group(n=16)and a low-fear group(n=14)based on the TSK-17 scale(Tampa Scale for Kinesiophobia)scores,with 14 healthy individuals serving as the control group.A wireless sEMG system was used to record sEMG signals of different muscles.Test activities included walking at normal speed,stair ascent,and stair descent.Muscle activation was normalized to maximum voluntary contraction(MVC)by calculating the root mean square(RMS)of the EMG signals to compare muscle activation under different conditions.Correlation analyses were conducted to evaluate the relationship between muscle activation and TSK-17 scores.Results During normal walking,the healthy limb side of the high-fear group showed significantly greater activation in the rectus femoris(P<0.05),vastus medialis(P<0.01),and vastus lateralis(P<0.01)compared to the affected limb side.Muscle activation of the healthy limb side was also elevated during stair ascent and stair descent in the high-fear group,with significant differences in the rectus femoris(P<0.01),vastus medialis(P<0.01),and vastus lateralis(P<0.01).In the high-fear group,the affected/undominant and healthy/dominant limb side exhibited significantly increased activation in the rectus femoris(P<0.05)during normal walking compared with the low-fear group and control group.The affected/undominant and healthy/dominant side also exhibited increased activation in the rectus femoris(P<0.05)during stair ascent compared to the low-fear group and control group.There was a significant positive association between TSK-17 scores and rectus femoris activation on the healthy limb side during stair descent(r=0.95,P<0.01).Conclusions Fear of movement has a significant impact on muscle activation patterns in patients with,particularly during stair descent,which places a greater functional demand on the lower limbs.Therefore,great attention should be paid to the intervention of psychological factors in KOA treatment,so as to comprehensively improve both motor function and life quality of the patients.
4.Analysis of the genotype distribution and changes in norovirus in Asia and Russia in GenBank from 1995 to 2023

An analysis of 24 144 norovirus sequences from Asia and Russia deposited in GenBank between 1995 and 2023 was conducted,to understand the temporal and spatial variations in norovirus genotypes in these regions.Norovirus sequences from Asia and Russia were downloaded in FASTA format from GenBank for the years 1995-2023,and analyzed in Excel,R language,and GraphPad Prism for data visualization.The number of norovirus sequences submitted to GenBank increased annually from 2004 and peaked in 2015.Notably,China and Japan contributed 62.3%of all submitted norovirus sequences.These sequences encompassed 31 capsid genotypes(C-type),with GⅠ accounting for 9%and GⅡ accounting for 90%.Additionally,49 polymerase types(P-type)were identified,along with 68 combinations of CP types;among the analyzed recombinant sequences(4 460 entries in total),approxi-mately 41%belonged to three predominant recombinant strains:GⅡ.2[P16],GⅡ.4[P31],and GⅡ.4[P16].This analysis provides valuable insights into the distribution characteristics of norovirus genotypes across Asia and Russia over time,thereby supporting vac-cine design and evaluation efforts.
5.Effects of rice wine type and wine processing method on chemical constituents and anti-coagulation effect of Angelicae sinensis Radix

AIM To investigate the effects of rice wine type and wine processing method on chemical constituents and anti-coagulation effect of Angelicae sinensis Radix.METHODS Wine-washed products and wine-stir-fried products were prepared by different types and ages of rice wine,respectively,after which HPLC was adopted in the content determination of tryptophan,chlorogenic acid,vanillic acid,phthalic acid,ferulic acid,senkyunolide I,senkyunolide H,coniferyl ferulate and ligustilide,and PT,APTT,TT were detected in rabbit plasma.RESULTS Phenolic acids and volatile constituents demonstrated lower contents in the wine-stir-fried products than those in the raw product(P<0.05),while those in the wine-washed products displayed no obvious changes(except for senkyunolide I)(P>0.05).The contents of volatile constituents in the wine-washed products were higher than those in the wine-stir-fried products(P<0.05).After being processed with dry rice wine,various constituents exhibited increased contents as compared with those after being processed with sweet rice wine(P<0.05).Compared with the raw product,prolonged PT,APTT and TT were observable in the processed products prepared by 3-year semi-dry rice wine(P<0.05).CONCLUSION The optimal rice wine type is determined to be 3-year semi-dry.Wine-washed Angelicae sinensis Radix shows high contents of ferulic acid and volatile constituents,whose activating blood and resolving stasis effect may be stronger.
6.Clinical Efficacy of Tangning Tongluo Tablets for Nonproliferative Diabetic Retinopathy

ObjectiveTo observe the clinical efficacy and safety of Tangning Tongluo tablets in the treatment of nonproliferative diabetic retinopathy (DR). MethodsFourteen research centers participated in this study, which spanned a time interval from September 2021 to May 2023. A total of 240 patients with nonproliferative DR were included and randomly assigned into an observation group (120 cases) and a control group (120 cases). The observation group was treated with Tangning Tongluo tablets, and the control group with calcium dobesilate capsules. Both groups were treated for 24 consecutive weeks. The vision, DR progression rate, retinal microhemangioma, hemorrhage area, exudation area, glycosylated hemoglobin (HbA1c) level, and TCM syndrome score were assessed before and after treatment, and the safety was observed. ResultsThe vision changed in both groups after treatment (P<0.05), and the observation group showed higher best corrected visual acuity (BCVA) than the control group (P<0.05). The DR progression was slow with similar rates in the two groups. The fundus hemorrhage area and exudation area did not change significantly after treatment in both groups, while the observation group outperformed the control group in reducing the fundus hemorrhage area and exudation area. There was no significant difference in the number of microhemangiomas between the two groups before treatment. After treatment, the number of microhemangiomas decreased in both the observation group (Z=-1.437, P<0.05) and the control group (Z=-2.238, P<0.05), and it showed no significant difference between the two groups. As the treatment time prolonged, the number of microhemangiomas gradually decreased in both groups. There was no significant difference in the HbA1c level between the two groups before treatment. After treatment, the decline in the HbA1c level showed no significant difference between the two groups. The TCM syndrome score did not have a statistically significant difference between the two groups before treatment. After treatment, neither the TCM syndrome score nor the response rate had significant difference between the two groups. With the extension of the treatment time, both groups showed amelioration of TCM syndrome compared with the baseline. ConclusionTangning Tongluo tablets are safe and effective in the treatment of nonproliferative DR, being capable of improving vision and reducing hemorrhage and exudation in the fundus.
7.Epidemic analyses of brucellosis in humans in Tangshan City, Hebei Province from 2016 to 2023

ObjectiveTo analyze the epidemiological trends and characteristics of brucellosis in humans (hereinafter referred to as brucellosis) in Tangshan City, Hebei Province from 2016 to 2023, and to provide a scientific basis for formulating brucellosis prevention and control strategies in the region. MethodsThe incidence data of human brucellosis in Tangshan City from 2016 to 2023 were collected from the China Disease Prevention and Control Information System. The diagnosis time, infection route, and clinical characteristics of the cases were obtained from the case investigation reports. Descriptive epidemiological methods were used to analyze the temporal, spatial, demographic distributions, and clinical characteristics of human brucellosis. Brucella species were identified using agglutination tests with bacterial suspension and A/M antigen-positive serum. ResultsA total of 2 193 cases of human brucellosis were confirmed and clinically diagnosed in Tangshan City from 2016 to 2023, with the peak incidence occured from March to August, and which exhibited distinct geographic distribution patterns. The highest incidence rate was found in people aged 60‒<70 years. The occupation of cases were primarily farmers. The incidence rate in males (528/100 000) was higher than that in females (184/100 000). All cases had confirmed exposure to infected animals or contaminated animal products. ConclusionThe epidemic of human brucellosis in Tangshan exhibited an overall steady downward trend from 2016 to 2023, except for a slight increase in 2016 and 2021, with the incidence rate controlled at 289/100 000‒335/100 000. The prevention and control situation of human brucellosis still remains severe, with the highest incidence rate in the eastern region of Tangshan, which are characterized by the breeding, slaughtering, and processing of cattle and sheep. Therefore, it it is necessary to enhance the prevention and control of human brucellosis among the personnel engaged in these industries in the eastern areas.

9.Infection Mechanism and Detection of Occult Hepatitis B Virus in Voluntary Blood Donors

The risk of post-transfusion hepatitis(PTH)primarily stems from blood transfusion containing HBV-infected blood that is HBsAg-negative,specifically during the serological window period(WP)and occult HBV infection(OBI).OBI is a low viral replication form of HBV,making it a challenge in hepatitis B virus infection prevention and control,and a significant transmission source of post-transfusion hepatitis.Although nucleic acid testing(NAT)can reduce the occurrence of post-transfusion hepatitis to some extent,some OBI-infected individuals exist among blood donors,representing a potential risk of HBV transmission through blood.This article primarily summarizes an overview of OBI's concept,serological characteristics,genomic features,pathogenesis mechanisms,epidemiological characteristics,and advances in detection strategies,to provide theoretical basis for the collection,quality control and safe use of blood products for the OBI population.
10.Study on medical choice preferences of middle-aged and older chronic disease patients based on latent class analysis

Objective To study the latent classification of medical service utilization behaviors among middle-aged and older chronic disease patients,providing a theoretical basis for the allocation of medical resources.Methods Using data from the 2018 China Health and Retirement Longitudinal Study(CHARLS)database,latent class analysis was employed to conduct subgroup analysis on the medical choice preferences of middle-aged and older chronic disease patients.Logistic regression was used to analyze the influence of various factors on medical preference subgroups.Results The medical choice preferences of middle-aged and older chronic disease patients were classified into three latent subgroups:public hospital treatment preference type,non-public hospital treatment preference type,and public hospital consultation preference type.Compared to the public hospital treatment preference type,patients under 60 years old(OR=0.456,P=0.034),those living in rural areas(OR=0.256,P=0.006),and those with smoking habits(OR=3.11,P<0.001)were more inclined to choose non-public hospitals for treatment.In contrast,patients over 60 years old(OR=1.916,P=0.015),those with an education level of primary school or above(OR=2.595,P=0.004),and those with smoking habits(OR=2.591,P=0.044)tended to choose public hospitals for consultation or physical examinations.Conclusion Currently,private hospitals play an increasingly prominent role in chronic disease treatment.However,the utilization rate of traditional Chinese medicine among middle-aged and older chronic disease pa-tients is relatively low,and health awareness remains weak.It is recommended to strengthen the regulation of private hospitals,enhance the credibility and accessibility of traditional Chinese medicine,and intensify health education in communities to meet the growing health demands of middle-aged and older chronic disease patients.
