1.The Educational Effects of the Experience of Nursing Students' Patients Role in the Simulation Practice Education for the Women's Health Nursing

PURPOSE: The purpose of this study is to identify the educational effect among nursing students who performed the patient role in women's health nursing simulations.METHODS: In this exploratory qualitative study, a sample of 31 third- and fourth-grade nursing students who participated in scenario-based patient roles in clinical performance examination of the women's health nursing simulation practice training. Data were collected through focus group interviews. Qualitative data were analyzed using qualitative content analysis.RESULTS: Three themes emerged from the data analysis. Participants experienced an enhancement of patient-centered nursing competence, deep learning immersion and display of self-regulated learning. The difficulty of performing the patient role contributed additional effects such as the difficulty to perform the patient role in the psychological training environment.CONCLUSION: It is recommended to utilize nursing students as patients in simulation practice training. On the other hand, the psychological training environment can cause difficulties in performing patient roles, a burden on the role of the patient, and involves the interruption of the role.

2.Influence of Hospital Ethical Climate and Nursing Professionalism on Patient Safety Management Activity by Nurses

PURPOSE: The purpose of this study was to investigate the influence of hospital ethical climate and nursing professionalism on patient safety management activity by nurses.METHODS: A cross-sectional correlation study design was used. Participants included 142 nurses from two tertiary hospitals in Seoul and Gyunggi-do, South Korea. Data were collected in April and May 2019 using self-report questionnaires and analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and multiple liner regression analysis with SPSS/WIN 21.0.RESULTS: The patient safety management activity correlated positively with hospital ethical climate (r=.32, p < .001) and nursing professionalism (r=.40, p < .001). The patient safety management activity was the factor that most influenced nursing professionalism and position, explaining 21% of the variance (F=12.06, p < .001).CONCLUSION: It is necessary to provide education on professionalism. It is necessary to continuously provide job education and training to nurture competence and quality in professional nurses.

3.Patient Safety Awareness and Emergency Response Ability Perceived by Nursing Homes and Home Visiting Caregivers.

PURPOSE: The purpose of this study was to identify patient safety awareness and emergency response ability and affecting factors perceived by nursing homes and home visiting caregivers. METHODS: This study was a descriptive study that conveniently extracts nursing caregivers who care for elderly patients in S and G provinces, Korea. Data collection was done by structural questionnaires from April to May 2018. A total of 204 responses consisting of 103 nursing homes and 101 home visiting caregivers were used for data analysis in SPSS Win 22.0. RESULTS: Patient safety awareness and emergency response ability of nursing homes caregivers with each 4.24±0.50, 74.26±09.57 was each higher than that of the home visiting caregivers with 3.68±0.49, 68.02±12.12 (p < .001). The affecting factors of the patient safety awareness were working place, safety education, and daily average working hours with 12 or more (F = 27.30, p < .001) and that of emergency response ability were number of patients per personnel with 9 or more and emergency situation experience (F=14.00, p < .001). CONCLUSION: These results suggest that it is necessary to develop a safety education program that can share indirectly experience emergency situations that occur on the job site.

4.Systematic Review of Data Mining Applications in Patient-Centered Mobile-Based Information Systems.

OBJECTIVES: Smartphones represent a promising technology for patient-centered healthcare. It is claimed that data mining techniques have improved mobile apps to address patients’ needs at subgroup and individual levels. This study reviewed the current literature regarding data mining applications in patient-centered mobile-based information systems. METHODS: We systematically searched PubMed, Scopus, and Web of Science for original studies reported from 2014 to 2016. After screening 226 records at the title/abstract level, the full texts of 92 relevant papers were retrieved and checked against inclusion criteria. Finally, 30 papers were included in this study and reviewed. RESULTS: Data mining techniques have been reported in development of mobile health apps for three main purposes: data analysis for follow-up and monitoring, early diagnosis and detection for screening purpose, classification/prediction of outcomes, and risk calculation (n = 27); data collection (n = 3); and provision of recommendations (n = 2). The most accurate and frequently applied data mining method was support vector machine; however, decision tree has shown superior performance to enhance mobile apps applied for patients’ self-management. CONCLUSIONS: Embedded data-mining-based feature in mobile apps, such as case detection, prediction/classification, risk estimation, or collection of patient data, particularly during self-management, would save, apply, and analyze patient data during and after care. More intelligent methods, such as artificial neural networks, fuzzy logic, and genetic algorithms, and even the hybrid methods may result in more patients-centered recommendations, providing education, guidance, alerts, and awareness of personalized output.

PURPOSE: The purpose of this study was to develop a smartphone app for use in patient controlled analgesia (PCA) education and to identify PCA knowledge and pain management following lower extremity orthopaedic surgery under spinal anesthesia in patients who received smartphone app education. METHODS: Participants were 150 patients in an orthopaedic hospital located in Busan. The measurement variables used in this study were PCA knowledge, pain management and pain level. 8.Effectiveness of a Stroke Risk Self-Management Intervention for Adults with Prehypertension.

PURPOSE: The aim of this study was to evaluate the effectiveness of a community-based intervention for prehypertensive adults, to enhance stroke risk awareness and to adopt a preventive lifestyle for primary stroke prevention. METHODS: This was a single-blinded, repeated measures quasi-experimental study with 47 participants (23 in the experimental group and 24 in the control group) recruited through convenience sampling from two urban areas. The stroke risk self-management intervention consisted of three weekly, 2-hour, face-to-face sessions and two booster telephone sessions, utilizing strategies to enhance motivation for behavioral changes based on the Self-Determination Theory. All participants completed a pretest, a 1- month and a 3-month post test of stroke risk awareness and preventive lifestyle including blood pressure self-monitoring, healthy diet, and regular physical activity. Data were analyzed using descriptive statistics, chi-square test, two sample t test, repeated measures analysis of variance, and Friedman test with PASW Statistics 18.0. RESULTS: After the intervention, significant improvements were found in the experimental group for stroke risk awareness, blood pressure self-monitoring and regular physical activity, and were sustained over time. CONCLUSIONS: Our preliminary results indicate that the stroke risk self-management intervention is feasible and associated with improvement in self-management of stroke risk factors for primary stroke prevention among a prehypertensive population.

9.A Prospective Analysis of Dynamic Loss of Breast Projection in Tissue Expander-Implant Reconstruction.

BACKGROUND: Breast projection is a critical element of breast reconstruction aesthetics, but little has been published regarding breast projection as the firm expander is changed to a softer implant. Quantitative data representing this loss in projection may enhance patient education and improve our management of patient expectations. METHODS: Female patients who were undergoing immediate tissue-expander breast reconstruction with the senior author were enrolled in this prospective study. Three-dimensional camera software was used for all patient photographs and data analysis. Projection was calculated as the distance between the chest wall and the point of maximal projection of the breast form. Values were calculated for final tissue expander expansion and at varying intervals 3, 6, and 12 months after implant placement. RESULTS: Fourteen breasts from 12 patients were included in the final analysis. Twelve of the 14 breasts had a loss of projection at three months following the implant placement or beyond. The percentage of projection lost in these 12 breasts ranged from 6.30% to 43.4%, with an average loss of projection of 21.05%. CONCLUSIONS: This study is the first prospective quantitative analysis of temporal changes in breast projection after expander-implant reconstruction. By prospectively capturing projection data with three-dimensional photographic software, we reveal a loss of projection in this population by three months post-implant exchange. These findings will not only aid in managing patient expectations, but our methodology provides a foundation for future objective studies of the breast form.

10.Assessment of Breast Cancer Patients' Knowledge and Decisional Conflict Regarding Tamoxifen Use.

Breast cancer is the most common type of female cancer. Tamoxifen, a selective estrogen receptor modulator, is widely used to decrease breast cancer recurrence and mortality among patients. However, it also increases the risk of endometrial cancer. This study aimed to assess knowledge and decisional conflict regarding tamoxifen use. Between June and October 2014, breast cancer patients using tamoxifen were consecutively screened and requested to complete a survey including the EQ-5D, Satisfaction with Decision Scale (SWD), Decisional Conflict Scale (DCS), and a self-developed, 15-item questionnaire measuring tamoxifen-related knowledge. The study sample comprised 299 patients. The mean total knowledge score was 63.4 of a possible 100.0 (range, 13.3-93.3). While 73.9% of the participants knew that tamoxifen reduces the risk of breast cancer recurrence, only 57.9% knew that the drug increases endometrial cancer risk. A higher education level (> or =college) was associated with a higher, total knowledge score (beta = 4.291; P = 0.017). A higher knowledge score was associated with a decreased DCS score (beta = -0.366; P < 0.001). A higher SWD score was also associated with decreased decisional conflict (beta = -0.178; P < 0.001). In conclusion, the breast cancer patients with higher levels of tamoxifen-related knowledge showed lower levels of decisional conflict regarding tamoxifen use. Clinicians should provide the exact information about tamoxifen treatment to patients, based on knowledge assessment results, so as to aid patients' decision-making with minimal conflict.
