1.Association of increased greater tubercle angle and critical shoulder angle with rota-tor cuff tears

Objective:The greater tuberosity angle(GTA)and critical shoulder angle(CSA)are commonly referred to as radiographic markers which were used to described morphology of the greater tu-berosity and acromion respectively.At present,most international studies focus on the correlation be-tween the above two parameters and rotator cuff tears(RCTs),and their diagnostic value and risk assess-ment.This study attempts to find out the trend of GTA and CSA changes and risk threshold of RCTs,as well as the protective factors and risk factors.Methods:In this study,130 individuals from May 2019 to December 2020 were recruited.According to Southern California Orthopedic Institute(SCOI)classifica-tion,the individuals were divided into four groups retrospectively:Group A,negative control group;Group B,partial tears(articular side);Group C,partial tears(bursal side);Group D,full-thickness tears.GTA and CSA were measured respectively on true anteroposterior position X-ray of shoulder with arm in neutral rotation and performed by the same trained technician team in single-blind.The correla-tions between RCTs and relevant factors were analyzed.Results:According to the area under the receiver operating characteristic curve(AUC),GTA and CSA of RCTs(Groups B,C and D)were 0.736 and 0.673 with 95%confidence interval(CI),the cut-off value of GTA and CSA of RCTs were 70.5° and 39.5° respectively.Comparing with the control group,RCTs groups had significant statistical differences in age and body mass index(BMI)(P<0.05),especially the full-thickness RCTs(Group D),which was older than Groups A,B and C(P<0.05,cut-off value:56.5 years old)and shorter than Groups A and B(P<0.05,cut-off value:1.58 m).Analyzed from scatter plot and regression analysis,there was no linear correlation between GTA and CSA.There were no significant differences in gender,dominant shoulders and smoking between the RCTs groups and the control group(P>0.05).Conclusion:Larger GTA(>70.5°)and CSA(>39.5°)would be highly predictive in diagnosing RCTs without linear cor-relation,and GTA has a higher diagnostic value in contrast.Subacromial impingement and shoulder de-generation occurred before RCTs.Patients with age>56.5 years and height<1.58 m were more likely to develop disease of full-thickness RCTs and no statistic differences in weight and BMI.Gender,domi-nant shoulder and smoking were neither risk factors nor protective factors.
2.Supplementing repeated low-frequency repetitive transcranial magnetic stimulation with visual deprivation better improves the lower limb motor function of stroke survivors

Objective:To observe any effect of combining visual deprivation training (VDT) with repeated low-frequency transcranial magnetic stimulation (rTMS) in the treatment of lower limb motor dysfunction among stroke survivors.Methods:Fifty stroke survivors were randomly assigned to either a control group or an experimental group ( n=25 each). In addition to routine rehabilitation treatment, the control group received rTMS targeting the primary motor cortex (M1, lower limb representation area) of the unaffected hemisphere, while the experimental group also received VDT. Before and after 4 weeks of the treatments, everyone′s lower limb motor function, balance, gait and ability in the activities of daily living were evaluated using the Fugl-Meyer lower extremity assessment (FMA-LE), the root mean square (RMS) values generated through surface electromyography of the affected tibialis anterior muscle, the Berg Balance Scale (BBS), the Timed Up and Go Test (TUGT), the Tinetti Gait Assessment (TGA), and the modified Barthel Index (MBI). Results:After the treatment, both groups showed significant improvements in their average FMA-LE, BBS, TUGT, TGA and MBI results, as well as in the RMS values of the affected tibialis anterior muscle. The experimental group showed significantly greater improvement compared to the control group.Conclusions:The combination of VDT and low-frequency rTMS can effectively improve the lower limb motor function, balance and gait of stroke survivors. It is more effective than rTMS alone. The combined therapy is worthy of clinical application and promotion.
3.Early prediction and warning of MODS following major trauma via identification of cytokine storm: A prospective cohort study.

PURPOSE:
Early mortality in major trauma has decreased, but MODS remains a leading cause of poor outcomes, driven by trauma-induced cytokine storms that exacerbate injuries and organ damage.
METHODS:
This prospective cohort study included 79 major trauma patients (ISS >15) treated in the National Center for Trauma Medicine, Peking University People's Hospital, from September 1, 2021, to July 31, 2023. Patients (1) with ISS >15 (according to AIS 2015), (2) aged 15-80 years, (3) admitted within 6 h of injury, (4) having no prior treatment before admission, were included. Exclusion criteria were (1) GCS score <9 or AIS score ≥3 for TBI, (2) confirmed infection, infectious disease, or high infection risk, (3) pregnancy, (4) severe primary diseases affecting survival, (5) recent use of immunosuppressive or cytotoxic drugs within the past 6 months, (6) psychiatric patients, (7) participation in other clinical trials within the past 30 days, (8) patients with incomplete data or missing blood samples. Admission serum inflammatory cytokines and pathophysiological data were analyzed to develop machine learning models predicting MODS within 7 days. LR, DR, RF, SVM, NB, and XGBoost were evaluated based on the area under the AUROC. The SHAP method was used to interpret results.
RESULTS:
This study enrolled 79 patients with major trauma, and the median (Q1, Q3) age was 51 (35, 59) years (52 males, 65.8%). The inflammatory cytokine data were collected for all participants. Among these patients, 35 (44.3%) developed MODS, and 44 (55.7%) did not. Additionally, 2 patients (2.5%) from the MODS group succumbed. The logistic regression model showed strong performance in predicting MODS. Ten key cytokines, IL-18, Eotaxin, MCP-4, IP-10, CXCL12, MIP-3α, MCP-1, IL-1RA, Cystatin C, and MRP8/14 were identified as critical to the trauma-induced cytokine storm and MODS development. Early elevation of these cytokines achieved high predictive accuracy, with an AUROC of 0.887 (95% CI 0.813-0.976).
CONCLUSION
Trauma-induced cytokine storms are strongly associated with MODS. Early identification of inflammatory cytokine changes enables better prediction and timely interventions to improve outcomes.

4.Application of superficial ultrasonography in diagnosing and guiding management of a refractory scalp wound complicated by epidural abscess.

A middle-aged patient presented with persistent purulent discharge from a scalp incision five years after undergoing craniotomy with artificial dura mater implantation. The wound showed no significant improvement despite a month of systemic antibiotic therapy and local debridement. Subsequent superficial ultrasonography revealed complete separation of the artificial dura mater implant area from the surrounding flap tissue, with a loss of local blood supply. Based on these findings, the artificial dura mater was surgically removed, and a free skin flap transplantation was performed to successfully cover the wound. The wound was well-healed at the 10-month postoperative follow-up.

6.The effect of soluble receptor for advanced glycation end products on protein changes and function in cardiac ischemia-reperfusion based on proteomics

Objective To investigate the effects of soluble receptor for advanced glycation end products(sRAGE)on the changes and functions of myocardial tissue proteins in mice during myocardial ischemia/reperfusion(I/R).Methods Establish the myocardial I/R model using cardiac-specific sRAGE transgenic mice,and apply proteomic methods to detect the types and levels of protein expression in myocardial tissue.Results Compared with the I/R+sRAGE KIfl/fl group,the I/R+sRAGE CKI group had 59 upregulated proteins and 42 downregulated proteins in myocardial tissue.The volcano plot showed that the significantly upregulated proteins were lghg1,lgh2b,Mcm7,and Nifk,and the significantly downregulated proteins were Abca7,Colla2,Ablim1,Crebrf,and Kcp,respectively.The subcellular localization results showed that the proteins with significant changes were mainly distributed in the nucleus,cytoplasm,extracellular space,plasma membrane,endoplasmic reticulum,cytoskeleton,and others.The results of functional enrichment showed that the significantly changed proteins were mainly involved in regulating signal transduction,cell motility,metabolism,infectious diseases,tumors and the immune system.Conclusion sRAGE can inhibit myocardial I/R injury by increasing or decreasing target proteins involved in regulating intracellular and extracellular signal transduction processes following myocardial I/R.
7.Role of the Nrf2/HO-1 pathway in cypermethrin-induced oxidative injury of mice hippocampal neurons.

OBJECTIVES:
To explore whether the antioxidant axis Nrf2/HO-1 is involved in the regulation of hippocampus injury induced by cypermethrin and its underlying mechanism.
METHODS:
Ten-week-old C57BL/6 mice were randomly divided into control group and cypermethrin exposure groups with low, medium, and high exposure levels. After 21 days of oral gavage of corn oil (control) or cypermethrin, the levels of MDA, T-SOD, GSH-Px and CAT in the hippocampus of the mice were examined to evaluate the oxidative stress levels. HE staining was used to observe morphological changes of the hippocampal neurons. Western blotting, immunofluorescence staining and RT-qPCR were employed to detect the protein expressions and mRNA expression of Nrf2 and HO-1 and HO-1.
RESULTS:
Subacute oral exposure to cypermethrin significantly increased MDA level, decreased the activities of antioxidant enzymes T-SOD, GSH-Px and CAT, and induced neuronal damage in the CA1 and CA3 regions in the hippocampus of C57BL/6 mice. Cypermethrin exposure also caused Nrf2 protein translocation from the cytoplasm to the nucleus, accompanied by upregulated expression levels of the key antioxidant factor Nrf2 and its downstream target kinase HO-1.
CONCLUSIONS
Cypermethrin exposure dose-dependently causes oxidative damage in the hippocampus of C57BL/6 mice, which is regulated by the Nrf2/HO-1 antioxidant pathway.

8.Expression of I kappa B kinase-interacting protein in cervical cancer tissue and its effect on proliferation,migration and invasion of cervical cancer cells

Objective:To explore the correlation between IκB kinase-interacting protein(IKBIP)expression in tumor cells within cervical cancer tissues(TCCCT)and the clinical pathological characteristics and prognosis of patients,as well as its impact on the biological behaviors of cervical cancer(CC)cells HeLa and SiHa,and to elucidate its potential mechanism.Methods:GENT2 and Kaplan-Meier plotter databases were utilized to analyze the differential expression of IKBIP mRNA in CC and normal cervical tissues,as well as its correlation with the clinical prognosis of CC patients.The Hallmark reference gene set was chosen for pathway enrichment analysis using the Gene Set Enrichment Analysis(GSEA)software.Immunohistochemistry method was used to detect the IKBIP protein expression in TCCCT and epithelial cells in normal cervical tissue(ECNCT),and analyze the correlations between its expression level and clinicopathological characteristics and prognosis of CC patients.Furthermore,univariate and multivariate Cox regression analyses were performed to identify the risk factors impacting the prognosis of CC patients.The stably transfected cells of CC(HeLa cells and SiHa cells)with IKBIP knockdown were established for the experiment,which were divided into sh-NC group and sh-IKBIP group.The expression of IKBIP protein in various groups was assessed using Western blotting method.The cell proliferation activity and the percentage of 5-ethynyl-2'-deoxyuridine(EdU)positive cells were measured using cell counting kit-8(CCK-8)and EdU methods,while Transwell chamber assay was employed to determine the numbers of migration and invasion cells in various groups.Additionally,the expression levels of E-cadherin,N-cadherin,and Snail proteins in the cells in various groups were analyzed by Western blotting method.Results:The GENT2 database analysis revealed that the expression level of IKBIP mRNA in CC tissue was higher than that in normal cervical tissue(P<0.001).The GSEA enrichment analysis identified the epithelial-mesenchymal transition(EMT)pathway as the top-ranked pathway in IKBIP high-expression group.The immunohistochemistry results indicated the positive expression rate of IKBIP protein in TCCCT was higher than that in ECNCT(50.5%vs 8.0%),and its over-expression was associated with FIGO stage(2018)and differentiation grade of tumor(P<0.05).The univariate and multivariate Cox regression analyses suggested that lymph node metastasis(LNM)and high expression of IKBIP were the risk factors affecting the overall survival(OS)and progression-free survival(PFS)of CC patients(P<0.05).The Western blotting method results showed that compared with sh-NC group,the expression level of IKBIP protein in the cells in sh-IKBIP group was decreased(P<0.05).The CCK-8 and EdU assay results showed that compared with sh-NC group,the proliferation activity and the percentage of EdU positive cells in sh-IKBIP group were decreased(P<0.05).The Transwell chamber assay results showed that compared with sh-NC group,the numbers of migration and invasion cells in sh-IKBIP group were significantly decreased(P<0.05).Additionally,the Western blotting method results indicated that compared with sh-NC group,the expression level of E-cadherin protein in sh-IKBIP group in CC cells was increased(P<0.05),while the expression levels of N-cadherin and Snail protein were decreased(P<0.05).Conclusion:The expression of IKBIP protein is significantly up-regulated in HeLa and SiHa cells derived from CC,and it is closely correlated with poor prognosis in CC patients.Suppression of IKBIP protein expression can effectively inhibit the proliferation,invasion and migration capabilities as well as EMT process of CC cells.
9.Analysis of factors influencing the deterioration of nutritional status after radiotherapy/radiochemotherapy for esophageal cancer

Objective To analyze the factors influencing the deterioration of nutritional status after radiotherapy/radiochemotherapy for esophageal cancer,so as to provide reference for nutritional management during antitumor therapy.Methods A total of 106 patients with esophageal cancer who received radiotherapy or radiochemotherapy at Anqing First People's Hospital of Anhui Medical University from Dec.2017 to Dec.2023 were enrolled.Patients'gender,age,surgical history,timing of radiotherapy intervention,synchronous chemoradiotherapy,radiotherapy dose,clinical stage,initial nutritional status,and performance status score were collected.The patient generated subjective global assessment scale(PG-SGA)scores were monitored before and after antitumor treatment.According to the nutritional status at the beginning of enrollment and at the end of radiotherapy,the patients were assigned to deterioration group or non-deterioration(stable or improved)group.The clinical characteristics of the 2 groups were compared.The factors influencing the deterioration of nutritional status were screened by logistic regression analysis.The correlation between nutritional status deterioration and adverse reactions(radiation esophagitis,pulmonary infection,neutropenia,thrombocytopenia,and elevated aminotransferase)was analyzed by Spearman correlation analysis.Results There were no significant differences in gender,radiotherapy dose,initial nutritional status,or performance status score between the 2 groups for the deterioration of nutritional status after radiotherapy(all P>0.05).The proportions of patients with previous surgical history of esophageal cancer,synchronous chemoradiotherapy,initiation of radiotherapy at less than 90%of target calorie requirement,and clinical stage Ⅳ were significantly higher in the deterioration group than those in the non-deterioration group(all P<0.05).Logistic regression analysis showed that clinical stage Ⅳ(odds ratio[OR]=4.684,95%confidence interval[CI]1.252-17.519,P=0.022)and previous surgical history of esophageal cancer(OR=7.338,95%CI 1.878-28.666,P=0.004)were the independent adverse risk factors for the deterioration of nutritional status after radiotherapy/radiochemotherapy.The timing of radiotherapy intervention was also an independent risk factor for the deterioration of nutritional status,and taking the tolerance of 70%-90%target energy as the reference level,starting radiotherapy when the tolerance of 90%-100%target energy had the optimal protection of nutritional status(OR=0.166,95%CI 0.050-0.551,P=0.003).Spearman correlation analysis showed that the deterioration of nutritional status was positively correlated with elevated transaminases after radiotherapy(rs=0.283,P=0.003),while it was not correlated with the other adverse reactions(all P>0.05).Conclusion Under the standard nutritional intervention model,patients with previous surgery and recurrent metastatic esophageal cancer who receive radiotherapy/chemoradiotherapy are still at risk of nutritional status deterioration.Tolerance to 90%-100%target energy requirement may be a more appropriate timing for radiotherapy intervention.When the nutritional status deteriorates during treatment,it is necessary to be alert to the elevated transaminases.
10.Combination of ulinastatin and somatostatin attenuates intestinal injury in rats with acute pancreatitis

Objective To investigate the effects of ulinastatin(UTI)combined with somatostatin(SOM)on intestinal damage in acute pancreatitis(AP)rats and the possible mechanisms of action.Methods The rats were randomly divided into sham-operated(sham)group,AP group,UTI group,UIT+SOM group and UIT+SOM+JAK2 activator(CA1)group,15 rats/group,respectively.Twelve hours after administration,the intestinal permeability,serum biochemical indicators,and inflammatory factor of rats were detected.HE staining was applied to observe the pathological changes in pancreatic and intestinal tissues.TUNEL method was applied to detect apoptosis in intestinal tissue.Western blot was applied to detect the expression of JAK2/STAT3 signaling pathway proteins.Results Compared with the sham group,the pancreatic tissue of rats in the AP group showed obvious inflammatory cell infiltration,edema and bleeding,while the intestinal mucosa showed inflammatory cell infiltration,irregular villi,shedding and necrosis of intestinal epithelial cells in the intestinal tissue.The intestinal permeability,serum amylase(AMY),lipase(LIPA)activities,diamine oxidase(DAO)activities,tumor necrosis factor α(TNF-α),interleukin-6(IL?6)level,pancreatic and intestinal histopathological scores,intestinal tissue cell apoptosis rate,and p?JAK2/JAK2,p?STAT3/STAT3 ratio all significantly increased(P<0.05).Compared with the AP group,the pancreatic and intestinal tissue injury of rats in the UTI group and UIT+SOM group was reduced,and inflammatory cell infil?tration was reduced.The intestinal permeability,serum AMY,LIPA activities,DAO activities,TNF?α,IL?6 level,pancreatic and intestinal histopathological scores,intestinal tissue cell apoptosis rate and p?JAK2/JAK2,p?STAT3/STAT3 ratio were all decreased(P<0.05).The pancreatic and intestinal tissue injury of rats in the UIT+SOM+CA1 group were more severe,and the trends of the above indicators were opposite to those found in UIT+SOM group(P<0.05).Conclusions The combination of UTI and SOM attenuated intestinal injury in AP rats,and potential mechanism may involve in the inhibition of the JAK2/STAT3 signaling pathway.
