1.Supplementing repeated low-frequency repetitive transcranial magnetic stimulation with visual deprivation better improves the lower limb motor function of stroke survivors

Objective:To observe any effect of combining visual deprivation training (VDT) with repeated low-frequency transcranial magnetic stimulation (rTMS) in the treatment of lower limb motor dysfunction among stroke survivors.Methods:Fifty stroke survivors were randomly assigned to either a control group or an experimental group ( n=25 each). In addition to routine rehabilitation treatment, the control group received rTMS targeting the primary motor cortex (M1, lower limb representation area) of the unaffected hemisphere, while the experimental group also received VDT. Before and after 4 weeks of the treatments, everyone′s lower limb motor function, balance, gait and ability in the activities of daily living were evaluated using the Fugl-Meyer lower extremity assessment (FMA-LE), the root mean square (RMS) values generated through surface electromyography of the affected tibialis anterior muscle, the Berg Balance Scale (BBS), the Timed Up and Go Test (TUGT), the Tinetti Gait Assessment (TGA), and the modified Barthel Index (MBI). Results:After the treatment, both groups showed significant improvements in their average FMA-LE, BBS, TUGT, TGA and MBI results, as well as in the RMS values of the affected tibialis anterior muscle. The experimental group showed significantly greater improvement compared to the control group.Conclusions:The combination of VDT and low-frequency rTMS can effectively improve the lower limb motor function, balance and gait of stroke survivors. It is more effective than rTMS alone. The combined therapy is worthy of clinical application and promotion.
2.Effect of visual deprivation training combined with proprioceptive training on balance in hemiplegic patients af-ter stroke

Objective To explore the effect of visual deprivation training combined with proprioceptive training on balance function of hemiplegic patients after stroke.Methods A total of 80 stroke patients with hemiplegia in Jiangsu Shengze Hospital were selected from May,2022 to March,2024,and randomly divided into control group(n=20),proprioceptive training group(n=20),visual de-privation group(n=20)and combined group(n=20).All the groups received routine rehabilitation training,while the proprioceptive training group added proprioceptive training,the visual deprivation group added balance training under visual deprivation,and the combined group added visual deprivation training and proprioceptive training,for four weeks.They were assessed with ProKin Balance Test and Training System,Berg Balance Scale(BBS),10-metre walking test(10MWT),Fugl-Meyer Assessment-Lower Extremities(FMA-LE)and Functional Gait Assessment(FGA)before and after treatment.Results The intra-group effect(F>96.618,P<0.001)and interaction effect(F>5.444,P<0.01)were significant in mean longitudinal velocity and mean horizontal velocity.The intra-group effect(F>177.671,P<0.001),inter-group effect(F>3.761,P<0.05)and interaction effect(F>7.555,P<0.001)were significant in movement el-lipse area and movement length both with eyes open and closed.The intra-group effect(F>221.902,P<0.001)and interaction effect(F>7.586,P<0.001)were significant in the time of 10MWT,and the scores of BBS,FMA-LE and FGA;and the inter-group effect were significant in FGA score(F=5.258,P<0.01).Post Hoc test showed that all the indicators were better in the combined group and the visual deprivation group than in the con-trol group(P<0.05);as well as in the proprioceptive training group than in the control group(P<0.05)except mean longitudinal velocity with eyes open,mean horizontal velocity with eyes closed,and movement length with eyes open;while all the indicators were better in the combined group than both in the visual deprivation group and the proprioceptive training group(P<0.05);there was no significant difference between the visual depriva-tion group and the proprioceptive training group for all the indicators(P>0.05).Conclusion Both visual deprivation training and proprioceptive training can improve balance,lower limb motor function and walking of hemiplegic stroke patients,and the combination is more effective.

4.Effects of Modified Guomin Decoction (加味过敏煎) on Traditional Chinese Medicine Syndromes and Quality of Life in Patients with Mild to Moderate Atopic Dermatitis of Heart Fire and Spleen Deficiency Pattern:A Randomized,Double-Blind,Placebo-Controlled Trial

ObjectiveTo observe the clinical efficacy and safety of Modified Guomin Decoction (加味过敏煎, MGD) in patients with mild to moderate atopic dermatitis (AD) of the traditional Chinese medicine (TCM) pattern of heart fire and spleen deficiency, and to explore its possible mechanisms. MethodsIn this randomized, double-blind, placebo-controlled study, 72 patients with mild to moderate AD and the TCM pattern of heart fire and spleen deficiency were randomly divided into a treatment group and a control group, with 36 cases in each group. The treatment group received oral MGD granules combined with topical vitamin E emulsion, while the control group received oral placebo granules combined with topical vitamin E treatment. Both groups were treated twice daily for 4 weeks. Clinical efficacy, TCM syndrome scores, Visual Analogue Scale (VAS) for pruritus, Dermatology Life Quality Index (DLQI) scores, Scoring Atopic Dermatitis (SCORAD) and serum biomarkers, including interleukin-33 (IL-33), interleukin-1β (IL-1β), immunoglobulin E (IgE), and tumor necrosis factor-α (TNF-α) were compared before and after treatment. Safety indexes was also assessed. ResultsThe total clinical effective rates were 77.78% (28/36) in the treatment group and 38.89% (14/36) in the control group, with cure rates of 19.44% (7/36) and 2.78% (1/36), respectively. The treatment group showed significantly better clinical outcomes compared to the control group (P<0.05). The treatment group exhibited significant reductions in total TCM syndrome scores, including erythema, edema, papules, scaling, lichenification, pruritus, irritability, insomnia, abdominal distension, and fatigue scores, as well as reductions in VAS, DLQI, SCORAD, and serum IgE and IL-33 levels (P<0.05 or P<0.01). Compared to the control group, the treatment group had significantly better improvements in all indicators except for insomnia (P<0.05). No adverse events occurred in either group. ConclusionMGD is effective and safe in treating mild to moderate AD patients with heart fire and spleen deficiency pattern. It significantly alleviates pruritus, improves TCM syndromes and quality of life, and enhances clinical efficacy, possibly through modulation of immune responses.

6.Effect of peroxisome proliferator-activated receptor γ on skin physiological and pathological processes

PPARγ agonists can inhibit skin inflammatory response, protect epidermal barrier function, and repair skin injury. This review summarizes various roles of PPARγ in skin biology, and discusses its function in skin diseases, such as psoriasis and skin tumors.
7.Effect of Aqueous Extract of Sophorae Tonkinensis Radix et Rhizoma on Bone Destruction and PI3K/Akt Signaling Pathway in Rheumatoid Arthritis

ObjectiveTo observe the effect of aqueous extract of Sophorae Tonkinensis Radix et Rhizoma (STRR) on rheumatoid arthritis (RA) and to explore the anti-bone destruction mechanism based on phosphatidylinositol 3-kinase (PI3K)/serine/threonine-protein kinase (Akt) pathway. MethodHigh-performance liquid chromatography (HPLC) was used to determine the content of main active components in aqueous extract of STRR, and type Ⅱ collagen to induce RA (CIA) in mice. The blank group, model group, methotrexate (MTX) group (0.5 mg·kg-1), and low-dose (100 mg·kg-1) and high-dose (200 mg·kg-1) STRR aqueous extract groups were designed. Joint swelling was observed and clinical scores of CIA mice were calculated. Pathological changes of mouse joints were observed based on hematoxylin and eosin (HE) staining, and Micro-CT was performed to monitor joint destruction. TRAP staining was used to observe osteoclast formation in mouse joint, and Western blot to detect the expression of key proteins in PI3K/Akt signaling pathway in mouse joint tissue. ResultThe model group demonstrated higher degree of joint swelling, clinical scores of CIA, and degrees of synovial hyperplasia, inflammatory cell infiltration (P<0.01), and joint destruction, more osteoclasts, and higher levels of matrix metallopeptidase-9 (MMP-9), cathepsin K (CTSK), nuclear factor of activated T-cells cytoplasmic 1 (NFATc1), PI3K, and phosphorylated-Akt (p-Akt) proteins than the blank group (P<0.01). Compared with the model group, the low-dose and high-dose aqueous extract of STRR alleviated joint swelling, reduced the clinical scores of CIA mice (P<0.05, P<0.01), relieved the pathological changes of joint tissue (P<0.01) and joint destruction, decreased osteoclasts (P<0.05, P<0.01), and lowered the levels of PI3K/Akt signaling pathway-related proteins in joint tissue of mice (P<0.01). ConclusionThe aqueous extract of STRR can significantly delay the inflammatory response of RA and especially inhibit bone destruction by down-regulating the PI3K/Akt signaling pathway.
8.Intervention Effect and Mechanism of Qingmei Compound on Acute Gouty Arthritis Based on NLRP3 Pathway

ObjectiveTo investigate the therapeutic effect of Qingmei compound on acute gouty arthritis (AGA) in rats and preliminarily clarify its mechanism. MethodForty male SD rats were randomly divided into a blank group, a model group, a colchicine group (0.3 mg·kg-1), and low- and high-dose Qingmei compound groups (200 and 400 mg·kg-1), with eight rats in each group. The AGA model was induced by injecting 50 g·L-1 monosodium urate (MSU) into the ankle joint of the rats except those in the blank group. The ankle swelling index was measured before and 6, 24, and 48 h after modeling. The pathological changes in the joint tissues of AGA rats were observed by hematoxylin-eosin (HE) staining. The expression of tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in the joint tissues of rats was detected by immunohistochemistry. The protein expression of NOD-like receptor protein 3 (NLRP3) pathway and key proteins in the joint tissues of rats was detected by Western blot. ResultCompared with the blank group, the model group showed increased ankle swelling index, synovial hyperplasia, and inflammatory infiltration, and up-regulated expression of IL-1β, TNF-α, and NLRP3 proteins in the ankle joint and the ratio of Caspase-1 shear body to Caspase-1 precursor protein (Caspase-1 p20/Caspase-1) (P<0.01). Compared with the model group, the Qingmei compound groups showed reduced ankle swelling index of AGA rats, especially the low-dose Qingmei compound group (P<0.01). Meanwhile, Qingmei compound inhibited synovial hyperplasia and inflammatory infiltration (P<0.01) and reduced the levels of IL-1β, TNF-α, and NLRP3 proteins and Caspase-1 p20/Caspase-1 in joint tissues (P<0.01). ConclusionQingmei Compound can significantly alleviate the joint swelling and inflammatory infiltration of AGA, and its mechanism may be related to the inhibition of the NLRP3 signaling pathway.
9.Molecular Mechanism of Bailing Capsules in Treatment of Chronic Obstructive Pulmonary Disease Based on Network Pharmacology

ObjectiveThis study aimed to predict the pharmacodynamic material basis and core targets of Bailing capsules in the treatment of chronic obstructive pulmonary disease (COPD) based on network pharmacology and molecular docking, which were further verified by cell experiments to explore the mechanism. MethodThe main active ingredients and related targets of Bailing capsules were screened in Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) and SwissTargetPrediction. The main COPD targets were searched from GeneCards, DrugBank, Online Mendelian Inheritance in Man (OMIM) and Therapeutic Target Database (TTD). The protein-protein interaction (PPI) network was constructed by STRING and Cytoscape 3.6.1. Gene Ontology (GO) function annotation and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis were performed by the Database for Annotation, Visualization and Integrated Discovery (DAVID). Molecular docking verification was carried out using AutoDock Vina. The cell viability was detected by 3-(4,5-dimethylthiazol-2-yl)-5-(3-carboxymethoxyphenyl)-2-(4-sulfophenyl)-2H-tetrazolium (MTS) assay, and the mRNA level of the targets was detected by real-time polymerase chain reaction (Real-time PCR). ResultA total of 11 active ingredients of Bailing capsules such as cerevisterol, 270 related drug targets, and 1 020 COPD target proteins were obtained, with 74 intersection targets. The visualization analysis of the PPI network showed that the core targets of Bailing capsules in the treatment of COPD were tumor protein P53 (TP53), catenin beta 1 (CTNNB1), tumor necrosis factor (TNF), interleukin-6 (IL-6) and insulin (INS). Further, 20 signaling pathways were screened by KEGG enrichment analysis as the main pathways for Bailing capsules to treat COPD, involving phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt), cyclic adenosine monophosphate (cAMP), forkhead box O (FoxO), TNF, and hypoxia inducible factor-1 (HIF-1) signaling pathways. Molecular docking validation demonstrated that four active ingredients had stable binding to IL-6, with the lowest energy. Bailing capsules could reduce the mRNA level of IL-6 in RAW264.7 cells induced by lipopolysaccharide (LPS) (P<0.01) compared with the control group. ConclusionThe pharmacological mechanism of Bailing capsules in the treatment of COPD might be that its main active ingredients improved the inflammatory response by acting on TP53, CTNNB1, TNF, IL-6 and other targets and regulating PI3K/Akt, cAMP and other signaling pathways, thereby ameliorating COPD symptoms. This study provided experimental basis for subsequent in-depth research, and provided a diagnosis and treatment direction for disease-related clinical treatment.
10.Hepatic metastases after cervical cancer surgery: clinical analysis of 13 cases

Objective:To investigate the clinical characteristics of patients with hepatic metastasis after cervical cancer operation, and analyze the risk factors and prognostic factors of hepatic metastasis.Methods:A total of 1 312 patients with stage Ⅰa2-Ⅱb2 cervical cancer received radical surgery from January 2013 to January 2016 in the First Affiliated Hospital of Zhengzhou University were collected, of which 13 cases (0.99%, 13/1 312) had hepatic metastasis after operation. A retrospective analysis was conducted on clinical features of patients with hepatic metastasis after cervical cancer operation. T-test, chi-square test, rank sum test and logistic regression were used to analyze the risk factors of hepatic metastasis after surgery of cervical cancer operation. Kaplan-Meier method was used for survival analysis. Log-rank test was used for screening of prognostic factors in patients with postoperative hepatic metastasis.Results:(1) Clinical features: there were 3 cases of simple hepatic metastasis without obvious clinical symptoms, 2 patients with perihepatic lymph node metastasis showed only low back pain, 8 patients with multiple extrahepatic metastases, and their clinical symptoms were related to the site of metastasis. Five cases out of 9 (5/9) with liver metastasis had abnormal tumor marker results. The abnormal kinds of tumor markers were mainly carcinoembryonic antigen (CEA), CA 125, CA 199, and CA 72-4. The interval time of hepatic metastasis after operation was 2-22 months. (2) Analysis of risk factors for hepatic metastasis: univariate analysis showed that lymph node metastasis, histological type, infiltration depth, and lymph-vascular space invasionwere associated with hepatic metastasis after cervical cancer surgery ( P<0.05).Multivariate analysis showed that lymph node metastasis and small cell carcinoma were independent risk factors for postoperative hepatic metastasis ( P<0.05). (3) Prognostic factors in patients with hepatic metastasis:among 13 patients with postoperative hepatic metastases from cervical cancer, 9 died during the follow-up period and 4 survived. The median total survival time after hepatic metastases was 7 months (range 3-32 months). Univariate analysis showed that multiple extrahepatic metastases and treatment after hepatic metastasis had significant effects on the prognosis of patients with hepatic metastasis after cervical cancer operation ( P<0.05). Conclusions:The interval of hepatic metastasis after surgery for cervical cancer operation is within 2 years. Patients with lymph node metastasis and small cell carcinoma are more prone to postoperative hepatic metastasis. The prognosis of patients with extrahepatic multiple metastases is poor, and individualized treatment should be carried out after comprehensive analysis for patients with hepatic metastasis after cervical cancer operation.
