Background: Stroke is the second leading cause of death and disability worldwide. According to the World Health Organization report (2018), Mongolia has one of the highest stroke rates in the Western Pacific region and ranks second globally in stroke mortality and first in stroke-related disability. However, studies investigating the prevalence of diabetes mellitus (DM) among stroke patients in Mongolia remain limited. Aim: To determine the prevalence of diabetes mellitus among patients hospitalized due to ischemic stroke and to analyse the distribution of stroke subtypes. Materials and Methods: This retrospective study included patients with ischemic stroke who were admitted to the First and Third Central Hospitals of Mongolia within 24 hours after stroke symptom onset between 2024 and 2025. Data from 292 patients were analyzed. Demographic characteristics, stroke severity, time intervals, and laboratory findings were collected and compared between patients with and without diabetes mellitus. Result: The mean age of the study population was 61.9±12.9 years, and the majority of participants were male. Diabetes mellitus was present in 25% of the patients. The median duration of diabetes was 16 years (range 0–23 years), and the mean HbA1c level was 8.81±2.46%. There were no significant differences between diabetic and non-diabetic patients in terms of age, sex distribution, stroke severity, length of hospital stay, or in-hospital mortality. However, body mass index and glycemic parameters were significantly higher in patients with diabetes (p<0.001). According to the TOAST classification, small vessel disease was the most common stroke subtype, accounting for 57.3% of cases in patients with diabetes and 44.2% in patients without diabetes. Conclusion 1. Diabetes mellitus was present in one out of every four patients hospitalized with ischemic stroke. 2. Atherosclerotic stroke was six times more frequent in patients with diabetes compared with those without diabetes, highlighting the importance of early detection and management of diabetes in stroke prevention.

Background: Diabetes mellitus and disturbances in glucose metabolism are associated with an increased risk of thrombosis through endothelial dysfunction, chronic low-grade inflammation, platelet hyperactivity, and alterations in coagulation factors. Aim: To investigate the association between fasting glucose levels and selected coagulation and platelet parameters. Materials and Methods: This analytical cross-sectional study included 172 participants. According to the exclusion criteria, individuals with previously diagnosed diabetes mellitus, anticoagulant use, or liver disease were excluded. Participants were categorized into two groups according to fasting glucose levels: <5.6 mmol/L and ≥5.6 mmol/L. Activated partial thromboplastin time (aPTT), prothrombin time (PT), international normalized ratio (INR), thrombin time (TT), fibrinogen, platelet count, mean platelet volume (MPV), platelet distribution width (PDW), and plateletcrit (PCT) were compared between the two groups. Between-group differences were assessed using the Mann–Whitney U test, and associations between continuous variables were evaluated using Spearman’s correlation analysis. Multivariable linear regression analysis was adjusted for age, sex, and body mass index (BMI). Result: Of the 172 participants, 145 (84.3%) had fasting glucose levels <5.6 mmol/L and 27 (15.7%) had fasting glucose levels ≥5.6 mmol/L. The median aPTT was significantly shorter in participants with elevated fasting glucose than in those with fasting glucose <5.6 mmol/L (25.4 [24.2–26.8] vs. 26.3 [24.9–27.95] seconds; p=0.029). No statistically significant between-group differences were observed in PT, INR, TT, fibrinogen, platelet count, MPV, PDW, or PCT. A weak inverse correlation was observed between fasting glucose and aPTT (ρ=−0.141). In multivariable regression analysis adjusted for age, sex, and BMI, each 1 mmol/L increase in fasting glucose was associated with a 0.242-second decrease in aPTT (B=−0.242; 95% CI, −0.465 to −0.020; p=0.033). Conclusion 1. The median aPTT was significantly shorter in participants with elevated fasting glucose than in those without elevated fasting glucose. 2. Each 1 mmol/L increase in fasting glucose was associated with a 0.242-second decrease in aPTT.

Background: High-resolution esophageal manometry (HREM) is the reference standard for the assessment of esophageal motor function and classification of esophageal motility disorders. HREM was introduced into clinical practice in Mongolia in 2022; however, the spectrum of manometric diagnoses and factors associated with major esophageal motility disorders in this population have not been systematically characterized. Aim: To characterize major esophageal motility disorders among adults undergoing HREM and to identify clinical factors independently associated with these disorders. Materials and Methods: This retrospective observational study included 1,033 adults who underwent HREM at Intermed Hospital between September 2022 and March 2026. Manometric studies were interpreted according to the Chicago Classification version 4.0 (CCv4.0). Major esophageal motility disorders were defined as achalasia, manometric esophagogastric junction outflow obstruction (EGJOO), distal esophageal spasm, hypercontractile esophagus, and absent contractility. Multivariable logistic regression was used to identify factors independently associated with major motility disorders. Result: Major esophageal motility disorders were identified in 112 patients (11.2%). Compared with patients with normal motility or ineffective esophageal motility, those with major motility disorders had a significantly lower mean body mass index (BMI) (24.7±4.9 vs. 27.5±4.2 kg/m; p<0.001) and a higher prevalence of dysphagia (50.5% vs. 21.3%; p<0.001) and referral for suspected esophageal motility disorders (70.5% vs. 25.2%; p<0.001). In multivariable analysis, older age per 10-year increment (adjusted odds ratio [aOR], 1.35; 95% confidence interval [CI], 1.12–1.61), lower BMI (aOR per 1-kg/m increase, 0.90; 95% CI, 0.85–0.95), dysphagia (aOR, 1.95; 95% CI, 1.20–3.15), and referral for suspected motility disorders (aOR, 5.47; 95% CI, 3.22–9.31) were independently associated with major esophageal motility disorders. Conclusion 1. Major esophageal motility disorders were identified in 11.2% of adults undergoing HREM in this clinical cohort. 2. Referral for suspected motility disorders was the strongest clinical correlate, while older age, lower BMI, and dysphagia were also independently associated with major motility disorders.

Background: Metabolic dysfunction–associated steatotic liver disease (MASLD) is a major cause of chronic liver disease and is increasingly linked to cardiovascular disease (CVD), the leading cause of mortality globally. Aim: To evaluate individuals participating in early screening using the Hepatic Steatosis Index (HSI) and examine its association with CVD risk levels. Materials and Methods: This population-based retrospective cross-sectional study included 47,542 adults aged 40–70 years who participated in a national screening program between 2023 and 2025. Hepatic steatosis was assessed using the Hepatic Steatosis Index (HSI) and categorized as <30 (no steatosis), 30–36 (indeterminate), and >36 (steatosis). Ten-year CVD risk was estimated using laboratory-based WHO cardiovascular disease risk charts for Central Asia. Associations were evaluated using logistic regression and expressed as odds ratios (ORs) with 95% confidence intervals (CIs). Result: The mean age of participants was 53.18±8.17 years, and 67.1% (31,886) were women. The prevalence of HSI <30, 30–36, and >36 was 8.9% (4,219), 32.1% (15,271), and 59.0% (28,052), respectively. The distribution of low-, moderate-, and high-risk CVD groups was 48.6% (23,115), 41.3% (19,637), and 10.1% (4,790), respectively. The proportion of individuals with high CVD risk was 5.5% in the HSI <30 group and 10.1% in the HSI >36 group. Compared with the reference group, the odds of high CVD risk were increased in the HSI 30–36 group (OR=1.333; 95% CI: 1.152–1.542) and the HSI >36 group (OR=2.401; 95% CI: 2.094–2.753). These associations remained statistically significant after adjustment for age, sex, systolic blood pressure, waist circumference, physical inactivity, and smoking. Conclusion The prevalence of hepatic steatosis in the study population was 59.0%. Individuals with hepatic steatosis had a 1.2-fold higher risk of cardiovascular disease compared with those without steatosis.

Background: Polycystic ovary syndrome (PCOS) is characterized by menstrual irregularities, hormonal imbalance, clinical or biochemical hyperandrogenism, and the presence of multiple ovarian follicles. It is one of the major causes of infertility, endometrial carcinoma, and pregnancy-related complications. PCOS is not limited to the reproductive system; it is a chronic disorder associated with metabolic abnormalities and increased cardiovascular risk, which can negatively affect women’s long-term quality of life.In recent years, growing attention has been given to the metabolic alterations seen in women with PCOS-particularly hyperuricemia-and to the emerging role of novel anthropometric indicators such as neck circumference. Given the limited data on these parameters among Mongolian women with PCOS, this study aimed to investigate the relationship between serum uric acid levels and selected anthropometric measurements. Aim: To examine the association between serum uric acid concentration and selected anthropometric parameters in women with polycystic ovary syndrome. Materials and Methods: This analytical cross-sectional study was conducted in a clinical setting and included 120 women aged 15–35 years. Participants completed a structured questionnaire that collected information on age, menstrual cycle characteristics, and medication use. Anthropometric measurements-including height, weight, neck, waist, and hip circumference—were recorded. PCOS was diagnosed based on the Rotterdam criteria. Serum uric acid levels were determined through biochemical testing. Statistical analyses were performed using the t-test and Pearson correlation test. Results: Of the 120 women enrolled, 45.8% (n=55) were diagnosed with PCOS according to the Rotterdam criteria, while 54.2% (n=65) were classified as controls. The mean neck circumference was 36.2±2.5 cm in the PCOS group and 31.5±1.8 cm in the control group (p<0.001). The mean waist circumference was 88.4±7.9 cm in the PCOS group and 75.8±6.7 cm in the control group (p<0.001). The mean hip circumference was 103.5±6.5 cm among women with PCOS and 94.7±5.5 cm in the control group (p<0.01). The waist-to-hip ratio was significantly higher in the PCOS group (0.85±0.07) compared to controls (0.80±0.05, p<0.05). Among women with PCOS, the mean serum uric acid concentration was 476±8.5 µmol/L, which-although not exceeding the upper reference limit for women (400 µmol/L) was slightly higher than the average value in healthy individuals. Conclusion Compared with healthy controls, women with PCOS demonstrated significantly higher anthropometric indices. Although serum uric acid did not surpass the reference maximum, elevated mean levels indicate a potential risk marker for metabolic disorders. The strongest correlation was observed between uric acid and neck circumference, suggesting this parameter may serve as a more sensitive anthropometric indicator than BMI or waist-to-hip rat

Background: The continuous annual increase in the prevalence of diabetes mellitus (DM) poses significant challenges not only within our nation but also globally in the control and management of tuberculosis. Aim: This study aimed to determine the incidence of pulmonary tuberculosis among individuals with type 2 diabetes mellitus (T2DM) residing in six central districts of Ulaanbaatar and to investigate associated factors. Materials and Methods: A cross-sectional study design was employed. Participants aged 18 years and older diagnosed with T2DM and receiving care at endocrinology clinics in six central districts of Ulaanbaatar were selected using systematic random sampling. Presumptive TB cases were identified through a structured questionnaire and chest X-ray. Sputum specimens were collected and subjected to smear microscopy and Xpert MTB/RIF assay for tuberculosis detection. Cases confirmed by laboratory diagnosis, currently undergoing tuberculosis treatment and previously treated cases as per questionnaire data were classified as tuberculosis cases, and prevalence was calculated. Results: A total of 1,644 individuals with T2DM were enrolled in the study, of whom 836 (50.9%) were female, with a mean age of 58 years (range 19–89). The overall prevalence of presumptive TB cases was 10.5% (n=172; 95% CI, 9.0–12.0). Among 112 suspected cases from whom sputum samples were obtained, 10 (8.9%; 95% CI, 4.9–15.7) were laboratory-confirmed for M.tuberculosis. Notably, 7.2% (6 cases; 95% CI, 3.4–14.9) of asymptomatic individuals with abnormal X-ray findings were diagnosed with tuberculosis. According to questionnaire responses, 9 participants (0.5%; 95% CI, 0.3–1.0) were undergoing tuberculosis treatment, and 53 (3.2%; 95% CI, 2.5–4.2) reported a previously treated TB cases. The overall prevalence of tuberculosis among individuals with diabetes was 4.4% (n=72; 95% CI, 3.5–5.5). Stratification by age and sex revealed a significantly higher prevalence among males (5.9%; n=48; 95% CI, 4.5–7.8) compared to females (2.9%; 95% CI, 1.2–4.2) (p=0.002), indicating a twofold increased risk of tuberculosis in males. Although no statistically significant differences in tuberculosis prevalence were observed across age groups (p>0.05), a declining trend in prevalence with older age was noted. Conclusion The prevalence of tuberculosis among individuals with type 2 diabetes was 4.4% (n=72; 95% CI, 3.5–5.5), with a significantly higher rate in males (p=0.002) and a decreasing trend with increasing age. Among asymptomatic individuals exhibiting radiographic abnormalities, 7.2% were confirmed to have tuberculosis via laboratory testing

Background: Studies on the risk of kidney stone disease by age and gender show that the prevalence varies by countries and regions, particularly influenced by the conutry development, population diet, etc. In our country, research on kidney stones has been scarce over the past two decades. Aim: To study the age and gender characteristics of patients undergoing surgical treatment for kidney stones. Materials and Methods: Data from a total of 551 individuals who underwent surgical treatment at the Urology Department of the First State Central Hospital over a six-month period (01.01.2024-06.01.2024) were analyzed. The age and gender characteristics of patients diagnosed with kidney stones were studied, and logistic regression was used to test the true probabilities. Statistical analysis was performed using SPSS 27 software, with a significance level set at 0.05. Results: Kidney stones were present in 12.5% of surgical patients (n=69), while other urological diseases accounted for 87.5% (n=482). The mean age of patients with kidney stones was 46.46±12.6 years, which is relatively younger compared to patients with other urological surgical conditions (mean age=55.77, SD=16.174). Regarding the prevalence of kidney stones by gender, females accounted for 56.5% and males 43.5%. Considering the gender distribution of all patients receiving urological surgical care, females constituted 19.9% compared to 8.5% for males, nearly double the incidence. According to logistic regression analysis, women had a 1.5-fold higher risk of developing kidney stones compared to men (OR=1.5, 95% CI: 1.5–4.2, p<0.01). The highest prevalence was observed in the 36–55-year age group, while individuals aged over 55 years had a significantly lower risk (OR=0.19, 95% CI: 0.10–0.39, p<0.001). Conclusion Kidney stones were identified in 12.5% of individuals who underwent urological surgery. The majority of hospitalized patients with kidney stones were aged 36–55 years, and women comprised 56.5% of these cases.

Background: The morphological characteristics of the alveolar bone thickness between the maxillary sinus and root ca nal tips of the premolars and molars is important for dental implant and orthodontic treatment. The hypotheses formulated were that there would be differences in the morphological characteristics of the alveolar bone at various tooth positions in the posterior maxilla and that age and sex would not influence the findings. This study may provide useful information for dental implant and mini-implant treatment, so as to help lead to successful treatment outcomes. Aim: To evaluate the vertical relationship between the maxillary sinus and molar teeth using cone beam computed tomography (CBCT). Materials and Methods: The study design was retrospective study. We selected 30 CBCT images that were taken in the Central Dental Hospital, Mongolian National University Medical Sciences (MNUMS), between 2021 and 2023. We collected all images according to the inclusion and exclusion criteria. We used Full CBCT (16cm*8cm) images using the target sampling method. The all CBCT images (85kW, 7mA) were obtained with HDX, WILL (DENTRI, Seoul, Korea) using OnDemand3D (CyberMed. Seoul. Korea). The vertical relationship between the maxillary sinus and the maxillary molars was classified into 5 categories according to Kwak’s classification. The study was approved by the Research Ethics Committee of MNUMS. Statistical analyses were done by IBM SPSS version 27 software. Results: A total of 202 maxillary premolars and molars were examined using CBCT images, and were taken in 30 patients. The patient group consisted of 12 men and 18 women with an average age of 26.87 years (range, 16–42 years). In the maxillary first premolars: Class I was 77.4%, Class II was 13.2%, Class V was 9.4%, Classes III and IV were not observed. In the maxillary second premolars: Class I was 44.8%, Class V was 37.9%, Class II was 17.2%. In the maxillary first molars: Class V was 62.3%, Class I was 24.4%, Class II was 6.7%, Class III was 4.4%, Class IV was 2.2%. In the maxillary second molars: Class V was 63.1%, Class I was 23.9%, Class II was 6.5%, Class III was 6.5%. When examining the distance from the apex of the root of the molars to the floor of the maxillary sinus, the distance between the root of the buccal side of the second molar was the smallest, 0.67±1.36mm, and the distance between the root of the first premolar was the furthest, 5.14±4.32mm (p<0.01). Conclusion Among the maxillary posterior teeth, the second molar was positioned closest to the floor of the maxillary sinus, whereas the first premolar was located at the greatest distance. Regarding the maxillary molars, Type V characterized by root protrusion into the floor of the maxillary sinus was the most frequently observed configuration. Conversely, for the premolars, Type I defined by a distinct separation from the sinus floor was the predominant anatomical relationship.

Background: In Mongolia, the prevalence of non-communicable diseases and their intermediate risk factors has continuously increased in recent years. From results of early detection and prevalence studies of non-communicable diseases in Mongolia, studies linking intermediate risk factors to insulin resistance are scarce. Aim: To identify the prevalence of insulin resistance among the population and study its connection with intermediate risk factors of non-communicable diseases. Materials and Methods: This study was approved by the MNUMS Ethics Committee on February 23, 2024 (2024/3- 02), and was conducted based on the data of participants who took part in the “Population-based Preventive and Early Detection Screening of Infectious and Non-Infectious Diseases” organized by the Mongolian government from 2022 to 2023. Insulin resistance was identified using the triglyceride-glucose index (TyG), calculated by the formula Ln [fasting triglycerides (mg/dL) × fasting glucose (mg/dL) / 2]. “Ln” refers to the natural logarithm, based on Euler’s number, approximately 2.71828. TyG levels were classified into low risk (<8.5), medium risk (8.5-9.0), and high risk (>9.0). Defined intermediate risk factors for non-communicable diseases according to stages of hypertension and diabetes. Results: The mean age of participants was 44.3±15.2 years, with 39.2% (n=49,270) male and 41.4% (n=49,749) residing in urban areas. Among the participants, 59.1% had overweight or obesity, 23.6% had diabetes or impaired fasting glucose, 61.4% had hypertension, and 19.7% had elevated triglycerides. Analyzing by levels of insulin resistance risk, 62.8% of the population had low risk, 22.5% medium risk, and 14.7% high risk. Comparing systolic blood pressure across levels of insulin resistance risk showed that even without central obesity or diabetes, as the level of insulin resistance increased, the level of systolic blood pressure also increased (low risk group: 117.0±11.7, medium risk group: 121.1±10.9, high risk group: 123.5±16.2 mmHg). Regression analysis of the risk of hypertension by insulin resistance risk level showed that the risk increased with higher levels of insulin resistance (medium risk group OR=1.35, p<0.0001; high risk group OR=1.63, p<0.0001). Conclusion 22.5% of the population is at medium risk and 14.7% at high risk of insulin resistance. The increase in hypertension risk with higher insulin resistance levels is statistically significant, independent of central obesity and diabetes stages.

Background: Complete blood count (CBC) is one of the most widely used clinical tests, offering a high-quality, inexpensive, and routine diagnostic tool for various diseases and for monitoring treatment outcomes. Due to modern technological advancements, blood cells are now measured in greater detail, with 36 parameters being evaluated. The prevalence of diabetes mellitus (DM) is rapidly increasing, not only globally but also in our country. This rise in prevalence leads to numerous adverse consequences, including delayed diagnosis, poor control, an increase in chronic complications, and treatment failure. Hyperglycemia is a predisposing factor for chronic inflammation, and a relatively new inflammatory marker, the neutrophil-to-lymphocyte ratio (NLR), may be useful for assessing diabetes control. In recent years, NLR has been studied as a composite biomarker that more effectively reflects systemic inflammation and is easier to detect compared to other inflammatory markers. An increase in neutrophil count and percentage indicates chronic, low-grade, toxic, and non-specific inflammation, while a decrease in lymphocyte count suggests insufficient immune regulation. Thus, an elevated NLR not only reflects the immune system’s functional state in chronic inflammation, but it is also studied as a reliable and selective marker of systemic inflammation in chronic diseases. NLR is considered more stable than the total leukocyte count, is less affected by physiological and pathological factors, is inexpensive, and can be incorporated into daily clinical practice. Aim: To study the relationship between the neutrophil-to-lymphocyte ratio in a complete blood count and diabetes control and complications Materials and Methods: A cross-sectional study was conducted involving 145 patients with type 2 diabetes mellitus, who were treated at the Endocrine Clinic of the Mongolian-Japanese Hospital, Mongolian National University of Medical Sciences. Data on the neutrophil-to-lymphocyte ratio (NLR), glycated hemoglobin (HbA1c), and diabetic complications, including retinopathy, neuropathy, and foot complications, were collected from the Carte-Hospital Management System. Based on the frequency distribution of the NLR parameter, the values were classified into three groups: low, medium, and high. These groups were subsequently compared with glycemic control and complications using correlation and linear regression analyses, with statistical significance set at p<0.05. Results: The mean age of the study participants (n=145) was 57.3±12.9 years, with 46.8% (n=131) being male. The mean duration of diabetes was 9.6 years (range: 1-31 years), and the mean HbA1c level was 8.6±2.47%. Among the participants, 59.3% (n=86) exhibited poor glycemic control. Regarding complications, 52.3% of participants experienced at least one diabetes-related complication. Of these, 25.2% had one complication, 13.3% had two, and 7.1% had three or more. The most common complications were retinopathy (25.7%), nephropathy (18.6%), and macrovascular complications (11.9%). A statistically significant increase in the mean HbA1c level was observed across groups stratified by NLR levels (p=0.003). Linear correlation analysis revealed a statistically significant positive correlation between HbA1c levels (r=0.194, p=0.001) and the number of chronic diabetes complications (r=0.162, p=0.002). Conclusion The NLR level is positively correlated with both diabetes control and the occurrence of chronic complications. As an inexpensive and easily accessible test, it can be used for daily monitoring and early detection of complications.
